Puji syukur penulis ucapkan kehadirat Tuhan Yang Maha Esa, karena
berkat rahmat dan karunianya penulis dapat menyelesaikan literature review yang
berjudul “Gambaran Gaya Kepemimpinan Kepala Ruangan”.
Literature review ini disusun dalam rangka pengganti skripsi karena
pandemic Covid-19 untuk memperoleh gelar Sarjana Keperawatan pada Program
Studi Keperawatan Program Sarjana Sekolah Tinggi Ilmu Kesehatan Wira Medika
Bali.
Dalam penyusunan literature review ini, penulis banyak mendapat bantuan
sejak awal sampai terselesainya literature review ini, untuk itu dengan segala

ABSTRAK
Latar Belakang : Kepemimpinan adalah kemampuan untuk memberikan
pengaruh yang konstruktif kepada orang lain untuk melakukan satu usaha
kooperatif mencapai tujuan yang sudah direncanakan. Ini menujukkan bahwa
gaya kepemimpinan yang berbeda akan memberikan dampak tersendiri terhadap
kinerja pendokumentasian keperawatan. Tujuan : Tujuan dari literature review
adalah untuk mengetahui jenis gaya kepemimpinan pada pimpinan keperawatan.
Metode : Penulisan literature review ini dilakukan dengan Metode yang
digunakan dalam penulisan literature review ini dengan pencarian artikel baik
internasional maupun nasional, dengan penelusuran internet dari database Google
Scholar dan Pubmed melalui advanced search. Hasil : Berdasarkan pada 7 artikel
yang telah dipublikasikan pada umumnya menunjukkan hasil penelitian
mendapatkan pimpinan keperawatan menerapkan gaya kepemimpinan demokratis,
otoriter dan transformasional untuk mencapai tujuan organisasi. Gaya
kepemimpinan demokratik lebih dominan atau sering digunakan. Kesimpulan :
Gaya kepemimpinan demokratik lebih dominan atau sering digunakan oleh
manajer/pimpinan pada keperawatan dibandingkan dengan gaya kepemimpinan
otoriter dan bebas tindak.

ABSTRACT
Background: Leadership is the ability to exert constructive influence on others to
make a cooperative effort to achieve the planned goals. This shows that different
leadership styles will have their own impact on the performance of nursing
documentation. Purpose: The purpose of the literature review is to determine the
type of leadership style in nursing leaders. Method: Writing this literature review
is done by the method used in writing this review literature by searching articles
both internationally and nationally, by searching the internet from Google
Scholar and Pubmed databases through advanced search. Results: Based on 7
articles that have been published generally show the results of research getting
nursing leaders to apply democratic, authoritarian and transformational
leadership styles to achieve organizational goals. Democratic leadership style is
more dominant or often used. Conclusion: Democratic leadership style is more
dominant or often used by managers / leaders in nursing compared to
authoritarian leadership style and free of action.

PENDAHULUAN
1. Latar Belakang
Keberhasilan suatu organisasi di dalam sebuah Rumah Sakit dari beberapa
faktor dengan salah satunya yaitu tingkat sumber daya manusia termasuk
seoarang perawat, selain itu pemimpin juga merupakan suatu faktor yang
sangat berpengaruh terhadap aktivitas bawahannya untuk melakukan suatu
pekerjaan dengan suatu pencapaian yang di tuju.
Kepemimpinan adalah bentuk dari dominasi yang didasari atas
kemampuan pribadi yang sanggup mendorong atau mengajak orang lain untuk
melakukan sesuatu yang berdasarkan penerimaan dari kelompoknya, dan
memiliki keahlian khusus yang tepat bagi situasi yang khusus. Gaya
kepemimpinan seorang kepala ruangan ada 4 jenis, dimana setiap ruangan
berbeda kondisi akan berbeda pula gaya kepemimpinan. Gaya kepemimpinan
otoriter lebih banyak terdapat pada ruangan khusus, seperti di UGD, ICU
ataupun perawatan khusus lainnya. Sedangkan, gaya kepemimpinan
demokratis sering terdapat di ruangan–ruangan rawat inap seperti di penyakit
dalam maupun bedah. Gaya kepemimpinan kepala ruangan tersebut akan
mempengaruhi kinerja perawat pelaksana di setiap ruangannya.
Kepala ruangan adalah manajer tingkat pemula yang fokus utama
kegiatannya berada di unit kerja. Kepala ruangan, dalam melakukan
kegiatannya dibantu oleh orang-orang yang bekerja di tingkat manajer pemula
antara lain wakil kepala ruangan dan ketua tim serta perawat pelaksana.
Kurniadi (2013) mendefinisikan kepala ruangan adalah seorang tenaga
keperawatan yang diberi tanggung jawab dan wewenang dalam mengatur dan
mengendalikan kegiatan pelayanan keperawatan di ruang rawat.
Seorang pemimpin mempunyai peranan yang strategis yaitu sebagai
manajer, semestinya memiliki kompetensi sesuai dengan tuntutan
perkembangan dan pelayanan yang prima pada masa sekarang dan yang akan
datang agar dapat lebih memahami tugas dan fungsi pokok sebagai pemimpin.
Salah satunya adalah kemampuan seorang pimpinan dalam membina dan
mengayomi serta mempengaruhi motivasi bawahan agar ikut serta mencapai
tujuan organisasi.
Gaya kepemimpinan kepala ruang sedikit tidaknya dapat mempengaruhi
kinerja perawat, motivasi kerja dan kepuasan kerja. Sebuah penelitian di
RSUD dr. Rasidin Padang didapatkan 28,6% mengatakan kurang termotivasi
dengan gaya kepemimpinan demokratis, 33,3% kurang termotivasi dengan
gaya partisipatif, dan 50% kurang termotivasi dengan gaya kepemimpinan
otoriter. (Kontesa, M., 2014). Namun pada penelitian berbeda oleh Zebua pada
tahun 2014 menunjukkan bahwa tidak ada hubungan antara gaya
kepemimpinan transformasional kepala ruangan dengan kinerja perawat
(Zebua,O., 2014). Ini menujukkan bahwa gaya kepemimpinan yang berbeda
akan memberikan dampak tersendiri terhadap kinerja pendokumentasian
keperawatan.
Menurut (Zulfikhar,2016) yang menyebutkan bahwa sebagian besar gaya
kepemimpinan yang diterapakan oleh kepala ruang merupakan gaya

kepemimpinan demokratik (87,5%). Hasil penelitian ini hampir sama dengan
penelitian yang dilakukan oleh Hutahaen (2009) diperoleh gaya
kepemimpinan demokratis (49%) dan penelitian yang dilakukan oleh Kontesa
(2014) didapatkan hasil penelitian bahwa sebanyak (44,7%) perawat di Ruang
Rawat Inap Rumah Sakit Umum Daerah dr. Rasidin Padang berpendapat
bahwa gaya kepemimpinan kepala ruangan adalah demokratis. Sedangkan
hasil yang berbeda ditunjukkan pada penelitian Rohani (2018) yang
menyebutkan Hasil penelitian terlihat bahwa sebagian besar perawat
pelaksana mempersepsikan bahwa gaya kepemimpinan 20 kepala
ruangan termasuk tipe otoriter/autokratis. Hal ini terlihat dari 53 orang
perawat pelaksana yang berhasil didapatkan 34 orang perawat pelaksana
(64,2%) berpendapat gaya kepemimpinan kepala ruangan adalah
otoriter/Autokratis, sedangkan 17 orang perawat (32,1%) mempersepsikan
gaya kepemimpinan kepala ruangan adalah demokratis dan sisanya sekitar 2
orang perawat pelaksana (3,8%) berpendapat bahwa gaya kepemimpinan
kepala ruangan termasuk Laissez faire/liberal.
Kepala ruang sebagai pemimpin perlu melakukan pembinaan atau
pengarahan kepada perawat pelaksana dan pengembangan motivasi, inisiatif
dan keterampilan agar dapat melaksanakan tugasnya dengan baik, dalam
hal ini pemimpin harus mampu memberitahu, menjelaskan, bekerja sama
dan memonitor perilaku perawat sesuai dengan situasi yang ada untuk
dapat meningkatkan motivasi kerja perawat sehingga dapat melaksanakan
tugasnya dengan baik dan sesuai dengan tujuan yang telah disepakati.
Terdapatt 3 gaya kepemimpinan diantaranya gaya kepempinan otokratik, gaya
kepemimpinan demokratis/partisipatif, gaya kepemimpinan laissez-faire atau
kendali bebas. Gaya kepemimpinan yang diterapkan dalam suatu organisasi
dapat membantu menciptakan efektivitas kerja yang positif bagi anggota.
Dengan adanya gaya kepemimpinan dengan situasi dan kondisi suatu
organisasi maka anggota akan lebih bersemangat dalam menjalankan tugas
dan kewajiban untuk mencapai tujuan bersama. Selain itu, adapun dampak
negatif dari gaya kepemimpinan, salah satunya stres kerja pada anggota
organisasi. Penelitian tersebut diambil untuk dilakukan analisis melalui
literature review.
2. Tujuan
Tujuan dari literature review ini adalah untuk menganalisa hasil penelitian
yang berfokus pada gaya kepemimpinan kepala ruangan.

2. Pembahasan
Gaya kepemimpinan merupakan suatu karakter seorang pemimpin dalam
mempengaruhi bawahannya dalam memcapai tujuan atau sasaran dalam
sebuah organisasi. Gaya kepemimpinan juga dapat diartikan sebagai pola
periaku atau strategi yang diterapkan seorang pemimpin dalam menjalankan
organisasi yang dinaunginya. Gaya kepemimpinan yang menunjukkan secara
langsung maupun tidak langsung terhadap kepercayaan seorang pemimpin
terhadap kemampuan bawahannya dalam mengerjakan suatu pekerjaan. Hal
ini dapat memdefinisikan gaya kepemimpinan adalah perilaku dan strategi
sebagai hasil kombinasi dari falsafah, keterampilan, sifat dan sikap seorang
pemimpin yang diterapkan dalam mempengaruhi kinerja bawahannya.
Berdasarkan jurnal acuan, diketahui bahwa pimpinan pada keperawatan
menerapkan tiga jenis gaya kepemimpinan, yaitu jenis demokratis, otoriter
serta transformasional (Rivai, 2014).
Kepala ruangan atau manajer perawat pemula adalah seorang perawat
yang bertugas sebagai kepala di unit pelayanan perawatan terdepan yang
langsung berhadapan dengan pasien. Kepala ruangan dalam menjalankan n
tugasnya menggunakan gaya kepemimpinan dalam menjalankan fungsi
manajemen keperawatan untuk meningkatkan hasil mutu pelayanan
keperawatan (Kurniadi, 2013).
Efektifitas kepeminpinan dapat dinilai melalui keterampilan managerial
dalam pelaksanaan kerjasama. Seorang pemimpin harus memiliki kecakapan
teknis dan juga managerial yang profesional. Kecakapan teknis seorang
pemimpin disesuaikan dengan bidang yang dipimpinnya, sedangkan
kecakapan managerial seorang pemimpin disesuaikan dengan perannya dalam
mempengaruhi atau memimpin orang lain. Ketrampilan seorang pemimpin
dapat kita lihat melalui tindakannya dalam menyeleksi, mendidik
bawahannya, memnerikan motivasi, mengembangkan dan memutuskan suatu
hubungan kerja. Tantangan untuk seorang pemimpin yaitu kepemimpinan dan
penyesuaian terhadap suatu perubahan yang terjadi. Seorang pemimpinan
menggunakann gaya kepemimpinan yang efektif dalam mempengaruhi
persepsi bawahan terhadap tujuan kinerja dan cara untuk memcapainya. Peran
seorang pemimpin dalam hubungan kerja antar manusia berkaitan dengan
gaya kepemimpinan yang diterapkan oleh seorang pemimpin.
Kepemimpinan yang efektif dapat mempengaruhi perubahan kinerja
bawahan. Perubahan sorang pemimpin disebuah organisasi tergantung pada
pemilihan gaya kepemimpinan yang tepat dan sesuai untuk diterapkan pada
organisasi yang dipimpinnya. Seorang pemimpin diharapkan mampu
menerapkan gaya kepemimpinan sesuai dengan situasi dan kondisi yang
dihadapi serta bawahan yang dipimpinnya. seorang pemimpin dapat
menggunakan gaya kepemimpinan lebih dari satu gaya untuk mendapatan
hasil yang efektif dalam memimpin suatu organisasi. Seorang pemimpin juga
dapat memberikan inspirasi dalam keadaan sulit sehingga dapat meningkatkan
kepercayaan seorang pemimpin dari bawahannya.
Penelitian yang dilakukan Deniati (2019) menyebutkan bahwa banyak
perawat pelaksana yang kinerjanya baik dengan gaya kepemimpinan

demokratis yang diterapkan oleh kepala ruangan, karena gaya kepemimpinan
demokratis ini kepala ruangan dapat memberikan kesempatan kepada perawat
pelaksana untuk memberikan pendapatnya dan mendiskusikan suatu masalah
dengan bersama – sama. Perawat pelaksana akan merasa dihargai karena
pendapatnya di terima dan didengarkan oleh kepala ruangan. Hal ini juga
sesuai dengan penelitian yang dilakukan oleh Hutahaen (2009), yang
mengidentifikasi gaya kepemimpinan kepala ruangan dan pengaruh semangat
kerja perawat pelaksana di Rumah Sakit Umum Pusat Adam Malik Medan.
Hasil yang diperoleh dalam penelitian tersebut bahwa gaya kepemimpinan
yang sering dipakai kepala ruangan adalah demokrasi. Semangat kerja perawat
pelaksana tinggi dengan gaya kepemimpinan demokrasi.
Hasil serupa ditunjukkan oleh Zulfikhar (2016) yang menyebutkan bahwa
sebagian besar gaya kepemimpinan yang diterapkan oleh kepala ruang
merupakan gaya kepemimpinan demokratik (87,5%). Hasil penelitian ini
hampir sama dengan penelitian yang dilakukan oleh Hutahaen (2009)
diperoleh gaya kepemimpinan demokratis (49%) dan penelitian yang
dilakukan oleh Kontesa (2014) didapatkan hasil penelitian bahwa sebanyak
(44,7%) perawat di Ruang Rawat Inap Rumah Sakit Umum Daerah dr.
Rasidin Padang berpendapat bahwa gaya kepemimpinan kepala ruangan
adalah demokratis. Sedangkan hasil yang berbeda ditunjukkan pada penelitian
Rohani (2018) yang menyebutkan Hasil penelitian terlihat bahwa sebagian
besar perawat pelaksana mempersepsikan bahwa gaya kepemimpinan 20
kepala ruangan termasuk tipe otoriter/autokratis. Hal ini terlihat dari 53 orang
perawat pelaksana yang berhasil didapatkan 34 orang perawat pelaksana
(64,2%) berpendapat gaya kepemimpinan kepala ruangan adalah
otoriter/autokratis, sedangkan 17 orang perawat (32,1%) mempersepsikan
gaya kepemimpinan kepala ruangan adalah demokratis dan sisanya sekitar 2
orang perawat pelaksana (3,8%) berpendapat bahwa gaya kepemimpinan
kepala ruangan termasuk laissez faire/liberal.
Hasil yang didapat oleh Manning (2016) yang menyebutkan gaya
kepemimpinan transaksional dan transformasional dalam manajer perawat
dipengaruhi secara positif keterlibatan kerja perawat staf. Gaya kepemimpinan
penghindaran pasif dalam manajer perawat dipengaruhi secara negatif oleh
keterlibatan kerja staf perawat sedangkan hasil yang didapat oleh Dian,dkk
(2015) hasil penelitian dan pembahasan dapat diambil ada hubungan
signifikan antara gaya kepemimpinan, motivasi, dan stres kerja terhadap
kinerja perawat.
Hasil yang didapat oleh Kraaij (2018) Tiga tema utama mengenai peran
kepemimpinan praktisi perawat saat ini muncul dan mereka tema tampaknya
mempengaruhi lingkup praktik sehari-hari dari praktisi perawat. Perawatan
pasien secara langsung yang ditekankan dan kepemimpinan pada tingkat lain
tampaknya kurang dimanfaatkan. Kebanyakan perawat praktisi ingin
membentuk kembali profesi mereka. Namun penggunaan keterampilan
kepemimpinan mereka yang tidak menguntungkan terutama pada tingkat
profesional dan organisasi dan kurangnya faktor pendukung sepertinya

menghambat mereka terkait dengan penentuan posisi perawat praktisi yang
berhasil. Semua
Menurut Thoha (2013) Gaya Kepemimpinan Otokratik kata otokratik
diartikan sebagai tindakan menurut kemauan sendiri, setiap produk pemikiran
dipandang benar, keras kepala, atau rasa aku yang keberterimaannya pada
khalayak bersifat dipaksakan. Kepemimpinan otokratik disebut juga
kepemimpinan otoriter. Kepemimpinan otokratis sebagai gaya yang
didasarkan atas kekuatan posisi dan penggunaan otoritas. Gaya kepemimpinan
otokratik dapat didefinisikan suatu kepemimpinan yang dilakukan oleh
seorang pemimpin yang memiliki sikap ingin menang sendiri, tidak menerima
saran maupun pendapat dari orang lain serta memiliki idealism yang tinggi.
Ketika seorang kepala ruangan yang memadukan gaya kepemimpinan
transaksional yang pasif, maka akan berdampak negative dalam keterlibatan
kerja. Perilaku pasif termasuk menunda umpan balik dan komunikasi terbatas
oleh manajer keperawatan. Temuan penelitian untuk gaya kepemimpinan pasif
pada kepala ruangan mengungkapkan dampak signifikan dan negatif terhadap
keterlibatan kerja perawat, ketika kepala ruanganan sering memberikan umpan
balik, hasilnya meningkatnya keterlibatan kerja perawat. Kurangnya
komunikasi dan umpan balik dari kepala ruangan menghasilkan penurunan
keterlibatan kerja perawat. Komunikasi adalah ciri khas gaya kepemimpinan
transformasional, dan gaya kepemimpinan pada kepala ruangan memiliki
potensi positif mempengaruhi keberhasilan organisasi melalui keterlibatan
kerja perawat. Gaya kepemimpinan transformasional harus berfungsi sebagai
panduan untuk pengembangan kepemimpinan karena itu memberdayakan,
memotivasi, dan mendorong akuntabilitas di antara pengikut dan pada
akhirnya meningkatkan kepuasan, yang meningkatkan keberhasilan organisasi
(Thoha,2013).
Gaya kepemimpinan demokratis menyangkal asumsi bahwa dengan
kekuatan kelompok dapat mencapai tujuan-tujuan yang bermutu. Gaya
kepemimpinan demokratis dikaitkan dengan kekuatan personal dan keputusan
bersama dalam proses pemecahan masalah dan pengambilan suatu keputusan.
Gaya kepemimpinan Laisez Faire adalah tipe kepemimpinan ini merupakan
kebalikan dari tipe kepemimpinan otokratik. Kepemimpinan tipe ini seorang
pemimpin biasanya menunjukkan perilaku yang pasif dan seringkali
menghindar diri dari tanggung jawab. Seorang pemimpin yang kendali bebas
cenderung memilih peran yang pasif dan membiarkan organisasi berjalan
menurut temponya sendiri. Seorang pemimpin mempunyai keyakinan bebas
dengan memberikan kebebasan yang seluas-luasnya terhadap bawahan maka
semua usahanya akan cepat berhasil.
Menurut pendapat penulis gaya kepemimpinan yang efektif diterapkan
oleh kepala ruangan yaitu gaya kepemimpinan demokratis karena gaya
kepemimpinan demokratis melakukan pendekatan dimana kepala ruangan
memberikan kesempatan dan keterlibatan kepada perawat pelaksana untuk
berdiskusi untuk memecahkan suatu masalah bersama-sama maka diperlukan
peran aktif dari perawat pelaksana untuk memberikan sebanyak mungkin ide
dan masukan kepada pemimpinnya untuk mencapai tujuan bersama maka

perawat akan merasa dihargai dan pendapatnya diterima dan didengarkan oleh
pemimpinnya.

SIMPULAN DAN SARAN
1. Simpulan
Gaya kepemimpinan demokratik lebih dominan atau sering digunakan
oleh manajer/pimpinan pada keperawatan dibandingkan dengan gaya
kepemimpinan otoriter dan bebas tindak. Gaya kepemimpinan demokrasi
merupakan gaya dimana pemimpin selalu melibatkan bawahan dalam
pengambilan keputusan, dan membutuhkan waktu yang lebih untuk berkonsultasi
dengan banyak orang sehingga akan menyebabkan proses pengambilan keputusan
menjadi lambat dan dapat menimbulkan frustasi bagi perawat yang menginginkan
pengambilan keputusan yang cepat. Penerapan gaya kepemimpinan otoriter
dilakukan untuk mengatasi kondisi yang membutuhkan kondisi darurat yang
membutuhkan keputusan segera. Sedangkan gaya kepemimpinan transformasional
dilakukan untuk meningkatkan kinerja perawat dengan menekankan komunikasi
yang baik.
2. Saran
1. Bagi mahasiswa
Hasil literature review ini diharapkan bias menjadi motivasi bagi para
pembaca khususnya untuk mahasiswa yang tertarik untuk meneliti tentang
gaya kepemimpinan dan meneliti gaya kepemimpinan lebih dalam.
2. Bagi peneliti selanjutnya
Hasil literature review ini dapat di jadikan bahan kajian atau tinjauan
pustaka untuk peneliti selanjutnya yang akan melakukan penelitian terkait
Leadership.

Abstrak
Kepemimpinan adalah kemampuan untuk memberikan pengaruh yang
konstruktif kepada orang lain untuk melakukan satu usaha kooperatif mencapai
tujuan yang sudah direncanakan. Motivasi adalah sesuatu yang berupa kekuatan,
tenaga atau daya, atau suatu keadaan yang kompleks dan kesiapsediaan
merupakan dorongan dalam diri individu untuk bergerak kearah tujuan tertentu.
Motivasi seorang perawat pun dapat mengalami perubahan yang disebabkan oleh
faktor-faktor yang mempengaruhinya. Tujuan penelitian ini adalah mengetahui
hubungan gaya kepemimpinan kepala ruang dengan motivasi kerja perawat.
Penelitian ini adalah penelitian kuantitatif dan metode penelitian yang digunakan
pendekatan korelasional. Populasi dalam penelitian ini sebesar 232 orang perawat
pelaksana. sampel penelitian berjumlah 56 responden dengan teknik pengambilan
sampel menggunakan Simple random sampling. Pengukuran gaya kepemimpinan
pada kepala ruang dan motivasi perawat pelaksana menggunakan kuesioner dan
analisa data menggunakan uji Chi Square . Hasil penelitian diperolah data 5
orang kepala ruang (8,9%) menerapkan gaya kepemimpinan secara otokratik, 49
orang kepala ruang (87,5%) menerapkan gaya kepemimpinan secara Demokratis,
2 orang kepala ruang (3,6%) menerapkan gaya kepemimpinan secara Bebas
Tindak. 4 responden (7,1%) mempunyai motivasi kerja yang cukup, 52 responden
(92,9%) mempunyai motivasi kerja yang baik. Hasil uji statistic Chi Square
diperoleh nilai signifikansi p = 0,024. Hasil uji tersebut disimpulkan ada
hubungan antara gaya kepemimpinan kepala ruang dengan motivasi kerja perawat
di Rumah Sakit Islam Surakarta.

Abstracts
Leadership is the ability to provide a constructive influence others to do the
business of the cooperative achieve the planned objectives. Motivation is form of
strength, force or power, or a state of readiness is complex and encouragement
within the individual to move towards a certain goal. Motivation was a nurse may
change due to factors that influence it. the objective was aim know correlation
between leadership style of head room with work motivation of nurses. This
research is a quantitative and research method used correlation approach.
population are 232 nursing staff. sample are 56 nurse practitioner with taking
sample was using simple random sampling technique. instrument research of
leadership style and motivation nursing staff using questionnaire. data analysis
using Chi Square test. the results obtained data. 5 respondents (8.9%) to
implement autocratic leadership style, 49 respondents (87.5%) apply democratic
leadership style, 2 respondents head room (3.6%) to implement free act leadership
style. 4 respondents (63.2%) have medium motivation to work, 52 respondents
(92.9%) had high motivation. Chi Square statistic test results with a significance
of p = 0.024. the results concluded there was correlation between leadership style
of head room with work motivation of nurses in Surakarta Islamic Hospital.

1. PENDAHULUAN
Dalam era globalisasi seperti saat ini banyak persaingan dalam pelayanan
kesehatan. Oleh karena itu perlu adanya peningkatan mutu pelayanan sehingga
menghasilkan pelayanan kesehatan yang maksimal. Mutu pelayanan kesehatan
dipengaruhi beberapa faktor, faktor yang paling menonjol adalah sumber daya
manusia.
Menurut Ilyas (2004) mengemukakan bahwa sumber daya manusia adalah
kunci dari keberhasilan dan kesuksesan organisasi. Sumber daya manusia yang
berhubungan dengan pemberian pelayanan kesehatan di rumah sakit adalah tenaga
perawat. Untuk meningkatkan kualitas perawat dalam pemberian asuhan
keperawatan kepada klien membutuhkan peran kepemimpinan dalam
mempengaruhi dan menggerakkan perawat
Kepemimpinan adalah kemampuan untuk memberikan pengaruh kepada
orang lain untuk melakukan satu usaha kooperatif dalam pencapaian tujuan
yang sudah direncanakan, sehingga pemimpin itu harus bisa melaksanakan
kepemimpinannya, jika dia ingin sukses dalam melakukan tugas-tugasnya
(Kartono, 2011).
Kepala ruang sebagai pemimpin perlu melakukan pembinaan atau pengarahan
kepada perawat pelaksana dan pengembangan motivasi, inisiatif dan
keterampilan agar dapat melaksanakan tugasnya dengan baik, dalam hal ini
pemimpin harus mampu memberitahu, menjelaskan, bekerja sama dan
memonitor perilaku perawat sesuai dengan situasi yang ada untuk dapat
meningkatkan motivasi kerja perawat sehingga dapat melaksanakan tugasnya
dengan baik dan sesuai dengan tujuan yang telah disepakati.
Dari wawancara yang telah dilakukan dengan bagian bidang keperawatan
Rumah Sakit Islam Surakarta mengatakan bahwa dalam menentukan kepala ruang
berdasarkan pengalaman kerja di Rumah Sakit Islam Surakarta minimal 4 tahun,
memiliki sertifikat manajemen bangsal, memiliki pendidikan minimal D3 dan
memiliki kompetensi. Wawancara yang telah dilakukan dengan perawat pelaksana
bahwa sebagai perawat pelaksana harus bisa menyelesaikan dokumentasi selama
24 jam dan dengan sebaik mungkin walaupun dengan jumlah pasien yang
berbanding terbalik dengan jumlah perawat pelaksana terutama pada saat berjaga
pada shift siang dan malam hari. Sedangkan salah satu kepala ruang menyatakan
bahwa melakukan rapat dan bertatap muka dengan perawat pelaksana selama
sebulan sekali dalam membahas sistem,operan dan shift. Sedangkan melakukan
motivasi kerja pada perawat pelaksana hanya dilakukan apabila diperlukan dan
dalam kondisi tertentu saja
Disamping itu terdapat fenomena yang menarik ketika dilakukan pengamatan
saat survey pendahuluan. Terdapat beberapa jumlah perawat pelaksana yang
berbeda – beda dengan pasien yang relatif sama di dalam suatu ruangan yang

bekerja. Tetapi walaupun terdapat perbedaan itu di setiap ruangan selalu tercipta
suasana ruang begitu senang tidak terlihat adanya beban kerja sebagai perawat
pelaksana, tidak terlihat perselisihan dalam bertugas antar perawat pelaksana.
Disamping itu terlihat kepala ruang juga mau untuk bertindak langsung ke pasien
walaupun terdapat perawat pelaksana di ruang tersebut. Kepala ruang juga terlihat
begitu akrab dan mengedepankan hubungan yang baik dengan perawat pelaksana
sehingga tidak terlihat adanya perbedaan jabatan.
Berdasarkan uraian di atas penulis tertarik untuk melakukan penelitian tentang
“Hubungan antara gaya kepemimpinan kepala ruang dengan motivasi kerja
perawat pelaksana di Rumah Sakit Islam Surakarta”
2. METODE
2.1 Jenis Penelitian
Dalam penelitian ini menggunakan jenis penelitian kuantitatif
dengan menggunakan pendekatan korelasional. Metode kuantitatif
merupakan pendekatan ilmiah yang memandang suatu realitas itu dapat
diklasifikasikan, konkrit, teramati dan terukur,hubungan variabelnya
bersifat sebab akibat dimana data penelitiannya berupa angka-angka dan
analisisnya menggunakan statistik (Sugiyono, 2008). Selain itu metode
kuantitatif memiliki sifat mengukur tingkat kejadian, membuktikan sesuatu
(apakah ada hubungan, pengaruh, dan perbedaan faktor yang dominan),
tindakan atau eksperimen serta memprediksi sesuatu variabel berdasarkan
variabel lain.
2.2 Populasi dan Sample
Populasi dari penelitian ini yaitu seluruh perawat yang berada di
bangsal rawat inap. Jumlah perawat pelaksana menurut data bidang
keperawatan sekitar 232 orang. Dalam penelitian ini menggunakan teknik
pengambilan sampel random sampling. Jadi sampel yang digunakan
sebanyak 56 orang. Teknik pengambilan sampel setiap ruangan akan
menggunakan teknik proporsional sampling.
2.3 Instrumen Penelitian
Penelitian ini dalam melakukan pengambilan data menggunakan
instrumen penelitian kuesioner atau angket. Dalam penelitian ini terdapat 2
jenis kuesioner yaitu kuesioner untuk menilai gaya kepemimpinan kepala
ruang dan kuesioner untuk mengukur motivasi kerja perawat pelaksana.
Dalam pengukuran gaya kepemimpinan kepala ruang dan motivasi kerja
perawat pelaksana masing-masing menggunakan kuesioner yang berjumlah
18 pernyataan.

2.4 Analisa Data
Untuk mengetahui apakah terdapat hubungan antara
kepemimimpinan kepala ruang dengan motivasi kerja perawat pelaksana
maka digunakan analisis korelatif. Teknik analisis korelatif yang dipakai di
penelitian ini adalah Analisis Chi Square.
3. HASIL DAN PEMBAHASAN
3.1 Karakteristik Responden

4. PEMBAHASAN
4.1 Karakteristik Responden
4.1.1 Karakteristik Umur Responden
Data karakteristik responden menurut umur diperoleh hasil
sebagian besar responden berumur 23-30 tahun sebanyak 34 responden
(60,7%), sedangkan untuk responden yang beumur 31- 39 tahun sebanyak
15 responden (26,8%) dan sebanyak 7 responden (12,5%) berumur lebih
dari 40 tahun. Hal ini sejalan dengan penelitian yang dilakukan Farida
(2011) yaitu bahwa umur ≤ 40 tahun merupakan usia produktif dalam
bekerja, memiliki semangat cukup tinggi untuk berkompetensi dan bekerja
lebih baik dibandingkan dengan orang lain, sehingga mereka cenderung
lebih inovatif dan kreatif dalam melakukan pekerjaan.
4.1.2 Karakteristik Jenis Kelamin Responden
Distribusi karakteristik responden berdasarkan jenis kelamin
menunjukkan bahwa sebagian besar responden memiliki jenis kelamin
perempuan sebanyak 47 responden (83,9%). Hal ini sesuai dengan
kenyataan pada umumnya bahwa profesi keperawatan didominasi oleh
perempuan. Hal ini mungkin dikarenakan pekerjaan sebagai seorang
perawat yang mengedepankan pelayanan lebih cocok dilakukan oleh
perempuan dibandingkan dengan laki-laki. Dibandingkan dengan laki-laki,
perempuan secara tabiat lebih intuitif (lebih peka), sehingga sebagai suatu
pekerjaan yang didasarkan atas naluri keperawatan banyak dilakukan dan
diminati oleh perempuan (Asmadi, 2008).
4.1.3 Karakteristik Tingkat Pendidikan Responden
Hasil distribusi frekuensi berdasarkan tingkat pendidikan
menunjukkan bahwa sebagian besar perawat memiliki tingkat pendidikan
D3 keperawatan sebanyak 47 orang (83,9%). Pendidikan adalah salah satu
karakteristik yang harus dipertimbangkan karena dapat mempengaruhi
persepsi seseorang dalam memilih tindakan ketika terjadi sesuatu di
lingkungannya. Pendidikan dapat mempengaruhi proses belajar mengajar,
semakin tinggi pendidikan seseorang maka semakin mudah orang tersebut
untuk menerima informasi. Semakin tinggi tingkat pendidikan seseorang
idealnya akan memiliki pengetahuan yang lebih baik terhadap suatu hal
(Astuty, 2011)
4.1.4 Karakteristik Lama Bekerja Responden
Distribusi responden berdasarkan lama bekerja menunjukkan bahwa
sebagian besar responden bekerja selama 2–10 tahun sebanyak 44 orang
(78,16%). Masa kerja yang lebih lama akan mengkondisikan seseorang

dalam beradaptasi dengan kondisi kerjanya. Kinerja masa lalu sering
dikaitkan dengan produktivitas kerja yang lebih baik, namun sesungguhnya
lama bekerja tidak bisa dijadikan acuhan seseorang untuk memiliki
produktivitas kerja yang lebih baik dibandingkan dengan seseorang yang
belum lama bekerja (Robbins dan Judge, 2008).
4.2 Karakteristik Berdasarkan Variabel
4.2.1 Distribusi Berdasarkan Gaya Kepemimpinan
Berdasarkan hasil penelitian diperoleh data bahwa sebagian besar
gaya kepemimpinan yang diterapakan oleh kepala ruang merupakan gaya
kepemimpinan demokratik (87,5%). Hasil penelitian ini hampir sama
dengan penelitian yang dilakukan oleh Hutahaen (2009) diperoleh gaya
kepemimpinan demokratis (49%) dan penelitian yang dilakukan oleh
Kontesa (2014) didapatkan hasil penelitian bahwa sebanyak (44,7%)
perawat di Ruang Rawat Inap Rumah Sakit Umum Daerah dr. Rasidin
Padang berpendapat bahwa gaya kepemimpinan kepala ruangan adalah
demokratis.
4.2.2 Distribusi Berdasarkan Motivasi Kerja
Berdasarkan hasil penelitian yang dilakukan kepada 56 perawat
pelaksana diperoleh data bahwa sebagian besar motivasi kerja yang
dimiliki perawat pelaksana baik (92,9%). Penelitian ini bertentangan
dengan penelitian yang dilakukan oleh Putra (2012) yang berjudul
Hubungan Gaya Kepemimpinan Kepala Ruang Dengan Motivasi Kerja
Perawat Dalam Melaksanakan Asuhan Keperawatan Di RSUD Dr. Raden
Soedjati Grobogan dengan penelitian yang dilakukan kepada 68 perawat
didapatkan hasil bahwa motivasi kerja perawat masih kurang (63,2%).
Sedangkan penelitian yang dilakukan oleh Muhammad, Herman dan
Hendry (2013) yang berjudul Hubungan Kepemimpinan Kepala Ruangan
Menurut Persepsi Perawat Terhadap Motivasi Kerja Perawat Pelaksana Di
Ruang Instalasi Rawat Inap F Blu Rsup Prof. Dr. R.D. Kandou Manado
menunjukkan bahwa terdapat hubungan antara gaya kepemimpinan kepala
ruang menurut persepsi perawat dengan motivasi kerja perawat dan ini
sejalan dengan hasil penelitian yang dilakukan oleh peneliti. Terdapat
beberapa faktor yang dapat memotivasi kerja perawat pelaksana sehingga
memiliki motivasi kerja yang baik meliputi karakteristik dan kondisi kerja
perawat, diantaranya merupakan situasi dalam pekerjaan, gaji ataupun
insentif, teman kerja dan promosi dalam hal kenaikan jabatan. Hal ini
membuktikan teori Motivasi Herzberg bahwa motivasi dipengaruhi oleh

faktor instrinsik dan faktor ekstrinsik. Sedangkan gaya kepemimpinan
merupakan salah satu faktor esktrinsik.
4.3 Hubungan antara Gaya Kepemimpinan dengan Motivasi Kerja
Perawat Pelaksana
Berdasarkan hasil penelitian yang dilakukan pada 56 perawat
pelaksana menunjukkan bahwa terdapat responden yang menilai kepala
ruang menggunakan gaya kepemimpinan otokratik yaitu 2 perawat
pelaksana dengan motivasi kerja cukup (20%) dan 3 perawat pelaksana
dengan motivasi kerja baik (80%). Gaya Kepemimpinan otokratik ternyata
tidak menonjol dibandingkan dengan gaya kepemimpinan demokratik.
Gaya kepemimimpinan otokratik merupakan kepemimpinan yang befokus
pada tugas atau pekerjaan. Pemimpin menentukan semua tujuan yang akan
dicapai dalam pengambilan keputusan. Hasil penelitian ini sejalan dengan
peneitian yang dilakukan Khuong dan Dang (2015) yang berjudul The
Effects of Leadership Styles on Employee Motivation in Auditing
Companies in Ho Chi Minh City, Vietnam dengan hasil bahwa gaya
kepemimpinan otokratik memiliki hubungan yang positif dengan motivasi
kerja. Hal ini bisa dijelaskan dengan baik ketika dalam situasi luar biasa
mendesak untuk menyelesaikan tugasnya maka tidak perlu
mempertimbangkan pendapat staf sehingga keputusan dapat dibuat secara
cepat. Saat semacam ini diperlukan kepemimpinan yang kuat sehingga
mendapatkan sesuatu yang dicapai secara cepat dan efisien.
Dalam penelitian ini terdapat responden yang menilai kepala ruang
menggunakan gaya kepemimpinan demokratik dengan terdapat responden
memiliki motivasi kerja yang cukup (4,1%) dan responden yang memiliki
motivasi kerja tinggi (95,9%). Gaya kepemimpinan demokratik dalam
penelitian ini merupakan gaya kepemimpinan yang dominan dibandingkan
gaya kepemimpinan yang lainnya. Hal ini sejalan dengan penelitian yang
dilakukan oleh Al-Ababneh (2010) dengan judul The Influence Of
Managerial Leadership Style On Employee Job Satisfaction In Jordanian
Resort Hotels yang mendapatkan hasil bahwa gaya kepemimpinan
demokratik lebih dominan atau sering digunakan oleh manajer
dibandingkan dengan gaya kepemimpinan otoriter dan bebas tindak. Gaya
kepemimpinan demokrasi merupakan gaya dimana pemimpin selalu
melibatkan bawahan dalam pengambilan keputusan, dan membutuhkan
waktu yang lebih untuk berkonsultasi dengan banyak orang sehingga akan
menyebabkan proses pengambilan keputusan menjadi lambat dan dapat
menimbulkan frustasi bagi perawat yang menginginkan pengambilan
keputusan yang cepat (Marquis dan Huston, 2010).

Hasil lain dari penelitian ini terdapat responden yang menilai
kepala ruang menggunakan gaya kepemimpinan bebas tindak dan masing-
masing memiliki motivasi kerja cukup (50%) dan tinggi (50%). %). Gaya
kepemimpinan bebas tindak merupakan kepemimpinan dimana anggota
menentukan kegiatannya tanpa adanya pengarahan, supervisi dan
koordinasi. Hasil penelitian ini sejalan dengan penelitian yang dilakukan
Gopal dan Rima (2014) yang berjudul Leadership Styles And Employee
Motivation: An Empirical Investigation In A Leading Oil Company In
India dengan hasil bahwa gaya kepemimpinan bebas tindak memiliki
hubungan yang negatif dengan motivasi kerja. Ini berarti bahwa karyawan
tidak puas apabila di bawah kepemimpinan bebas tindak.
Dari seluruh responden yang memiliki motivasi kerja baik (95,9%)
dipengaruhi oleh gaya kepemimpinan demokratik. Dari tiga gaya yang
diidentifikasi, gaya demokratik paling dominan dibanding dengan gaya
yang lainnya. Seperti yang disebut dalam literatur bahwa gaya demokratik
dipengaruhi oleh faktor pemimpin dan juga dari bawahannya. Bawahan
yang “mampu” dan “mau” bekerja serta berpartisipasi dalam pelayanan
dan asuhan keperawatan. Sehingga dalam mengatur kinerja perawat lebih
tepat menggunakan gaya kepemimpinan demokratik. Menurut peneliti hal
ini bisa terjadi dikarenakan gaya kepemimpinan demokratik merupakan
gaya dengan melakukan pendekatan dimana kepala ruang memberikan
kesempatan dan keterlibatan dari perawat pelaksana. Dimana menentukan
tujuan diperlukan peran aktif dari perawat pelaksana untuk memberikan
sebanyak mungkin ide dan masukannya.
Sedangkan dari statistik juga memperlihatkan bahwa diantara
kedua variabel tersebut memiliki nilai p=0.024, dan ini menunjukkan nilai
p < 0, 05. Sehingga H0 ditolak, yang berarti ada hubungan antara gaya
kepemimpinan kepala ruang dengan motivasi kerja perawat pelaksana di
Rumah Sakit Islam Surakarta.
5. KESIMPULAN DAN SARAN
5.1 Kesimpulan 5.1.1 Sebagian besar perawat pelaksana berpersepsi kepala ruang
menerapkan gaya kepemimpinan demokratis (87,5%), sedangkan gaya
kepemimpinan otokratik (8,9%) dan gaya kepemimpinan bebas tindak
(3,6%)
5.1.2 Sebagian besar perawat pelaksana memiliki motivasi kerja yang
tinggi (92,9%)
5.1.3 Berdasarkan persepsi perawat pelaksana gaya kepemimpinan
demokratis memiliki motivasi kerja tinggi (95,9%) dibandingkan dengan

gaya kepemimpinan otoriter (80%) dan gaya kepemimpinan bebas tindak
(50%)
5.1.4 Terdapat hubungan antara gaya kepemimpinan kepala ruang
dengan motivasi kerja perawat pelaksana di Rumah Sakit Islam Surakarta
5.2 Saran 5.2.1 Bagi Institusi
Diharapkan dengan adanya penelitian ini pimpinan rumah sakit
dapat mempertahankan dan meningkatkan gaya kepemimpinan yang
demokratis terutama dalam hal melibatkan diri dalam interaksi bersahabat,
tetapi terus berusaha memastikan bahwa semua anggota menyadari
tanggung jawab dan standar pelayanan serta bersama-sama terlibat dalam
pemecahan masalah.
5.2.2 Bagi Peneliti Lain
Untuk peneliti lain dengan adanya penelitian ini dapat dijadikan
sebagai referensi serta dapat dikembangkan untuk menambah variabel lain
yang sekiranya dapat menjadi faktor yang mempengaruhi motivasi kerja
perawat pelaksana

HUBUNGAN GAYA KEPEMIMPINAN KEPALA RUANGAN DENGAN
KEPATUHAN KEBERSIHAN CUCI TANGAN BERDASARKAN PERSEPSI PERAWAT PELAKSANA

ABSTRAK
Kejadian infeksi nosokomial belum diimbangi dengan pemahaman tentang pencegahan dan cara-cara penyebaran
infeksi yang terjadi di rumah sakit. Penerapan program inovasi Global patient safety challenge dengan metode my
five moments for hand hygiene sebagai upaya menurunkan infeksi nosokomial belum diimplementasikan dengan
baik. Penelit tertarik untuk mengetahui hubungan Persepsi Gaya Kepemimpinan Kepala Ruangan dengan
Kepatuhan kebersihan cuci tangan perawat pelaksana di Ruang Inap Gedung C3, D3, dan E2 RSUD Cibabat
Cimahi. Rancangan penelitian yang digunakan Survei deskriptif dengan pendekatan cross sectional, sampel
penelitian sebanyak 53 perawat pelaksana, pengumpulan data dengan kuesioner dan observasi. Hasil penelitian 45
orang (84,9%) kepatuhan kebersihan cuci tangan perawat pelaksana belum sesuai SOP dan 8 orang perawat (15,1)
sesuai SOP. Sebanyak 34 orang perawat pelaksana (64,2%) mempersepsikan gaya kepemimpinan kepala ruangan
adalah otoriter/Autokratis, sedangkan 17 orang perawat (32,1%) mempersepsikan gaya kepemimpinan kepala
ruangan adalah demokratis dan 2 orang perawat pelaksana (3,8%) berpendapat bahwa gaya kepemimpinan kepala
ruangan termasuk laissez faire/liberal. Hasil uji statistik didapatkan nilai p = 0,087, berarti pada alpha 0,05 dapat
disimpulkan tidak ada perbedaan yang signifikan antara persepsi perawat pelaksana gaya kepemimpinan kepala
ruangan dengan kinerja perawat pelaksana. Akan tetapi pada alpha 0,1 ada hubungan yang signifikan antara
persepsi perawat pelaksana tentang gaya kepemimpinan kepala ruangan dengan kinerja perawat pelaksana.
Diharapkan kepala ruangan menerapkan gaya kepemimpinan situasional, mengiktui pelatihan leadershif dan
menilai kinerja secara berkala. Meneliti faktor-faktor yang mempengaruhi kepatuhan kebersihan cuci tangan.

ABSTRACT
The incidence of nosocomial infections has not been matched with the knowledge about
preventing and its spreading in hospital. Global Patient Safety Challenge as new program with My
Five Moments for Hand Hygiene method to reduce nosocomial infections has not been
applied effectively. Therefore, the researcher examines further the relationship between the
Perception of Leadership Style on Head of the Inpatient’s room and the Implementation of Hand
Hygiene Method by Nurses within inpatient room of C3, D3 and E2 building in RSUD Cibabat,
Cimahi. This study used descriptive survey design with cross sectional approach. The samples were
53 nurses and data were collected by questionnaire and observation. The result shows 45 nurses
(84.9%) did not apply hand hygiene method based on Standard Operating Procedure (SOP) while
this metod has been properly applied by 8 nurses (15,1%). Furthermore, 34 nurses (64,2%)
perceived the leadership style of the head of inpatient’s room is authoritative, 17 nurses (32,1%)
argued their head’s leadership style is democratic while 2 nurses (3,8%) choosed laissez faire/
liberal. Statistical test result obtained p value = 0,087, as it shows in alpha 0.05 there is no
significant differences between nurses’ perceptions of head’s leadership style and the performance of
the nurses while these significant relationship depicted in alpha 0,1. As a result, the head of
inpatient room is expected to have situational leadership style, involved in leadership training,
evaluates his staff’s performances regularly and examines the factors affecting to the implementation
of hand hygiene method.

Hasil penelitian didapatkan bahwa Hasil uji statistik didapatkan nilai p =

dan sisanya didapatkan dari 2 perawat kepemimpinan otoriter berpeluang untuk
pelaksana yang mempersepsikan gaya memberikan kinerja yang lebih baik sebesar
kepemimpinan kepala ruangan 2,091 (0,119 – 36,64) kali dibandingkan
Liberal/Laissez faire didapatkan hasil 1 yang mempunyai persepsi gaya
orang perawat pelaksana (50 %) yang kepemimpinan liberal persepsi gaya
berkinerja baik. Dari hasil presentase kepemimpinan demokratis berpeluang
tersebut perawat pelaksana yang untuk kinerja yang lebih baik sebesar 0,545
mempersepsikan gaya kepemimpinan (0,029 – 10,37) kali dibandingkan yang

PEMBAHASAN
Mencuci tangan memegang peranan
penting dalam upaya meningkatkan keamanan
(safety) karena mencuci tangan merupakan
upaya memutus rantai transmisi infeksi yang
dapat menyebabkan terjadinya infeksi
nosocomial. Pelaksanaan cuci tangan akan
efektif apabila perawat memahami dan
menyadari moment cuci tangan yang harus
dilakukan oleh perawat dan melaksanakan
cuci tangan sesuai dengan SOP.
Meningkatkan kepatuhan cuci tangan
pada petugas pelayanan kesehatan saat ini
merupakan intervensi yang paling efektif
untuk mengurangi risiko infeksi yang didapat
di rumah sakit. Kepatuhan merupakan tingkat
dimana perilaku seseorang sesuai dengan
praktisinya suatu aturan dan perilaku (Tjandra,
2007). Menurut Rosita Saragih dan Natalina
Rumapea (2010), dalam penelitiannya
karakteristik perawat dengan tingkat
kepatuhan perawat melakukan cuci tangan di
Rumah Sakit columbia asia Medan. Hasil
penelitian menunjukkan bahwa tingkat
kepatuhan perawat dalam melakukan cuci
tangan di Rumah Sakit Columbia Asia Medan
adalah 72,61% bila disesuaikan dengan
kategori tingkat kepatuhan perawat melakukan
cuci tangan maka termasuk kategori tingkat
kepatuhan minimal (<75%) dan yang tidak
patuh 27,38%.
Perawat sebagai tenaga kesehatan pada
dasarnya mempunyai tugas dan tanggung
jawab dalam memberikan pelayanan
kesehatan, agar selama proses pelayanan
kesehatan dapat mencapai tujuan keperawatan
secara efektif dan efisien. Untuk itu
diperlukan peran pimpinan untuk melakukan
penilaian kinerja sebagai proses formal yang
dilakukan untuk mengevaluasi tingkat
pelaksanaan pekerjaan seorang personel
termasuk perawat dan memberikan umpan
balik untuk kesesuaian tingkat kerja.
Menurut Rivai (2009) Kepemimpinan
Diperlukan sebagai kekuatan untuk
menggerakan dan mempengaruhi orang.
Kepemimpinan sebagai alat, sarana atau
proses untuk membujuk orang agar bersedia
melakukan sesuatu secara sukarela/sukacita.

(2009) gaya kepemimpinan adalah
sekumpulan ciri yang digunakan pimpinan
untuk mempengaruhi bawahan agar sasaran
organisasi tercapai.
Sebagian besar perawat pelaksana
mempersepsikan gaya kepemimpinan kepala
ruangan di instalasi rawat inap penyakit dalam
adalah otoriter/autokratis berdasarkan hasil
penelitian dalam hal pengambilan keputusan
wewenang dan perencanaan selalu diatur oleh
pimpinan, hubungan antara atasan dengan
bawahan bersifat satu arah, bawahan
dipandang belum matang/dewasa, kebutuhan
bawahan kurang diperhatikan dan pelaksanaan
tugas didasarkan adanya ketakutan, ancaman
dan sanksi hal tersebut sesuai dengan teori
yang dikemukakan oleh Nursalam (2014)
menjelaskan ciri-ciri dari kepemimpinan tipe
otoriter ini, yaitu : pemimpin akan mengambil
keputusan sendiri tanpa melibatkan
bawahannya, hubungan antara atasan dan
bawahan menggunakan pendekatan formal,
memandang dan memperlakukan bawahannya
sebagai orang-orang yang tingkat
kedewasaannya lebih rendah daripada dirinya,
cenderung berpendapat bahwa apabila kepada
para bawahan telah diberikan upah atau gaji
kewajibannya kepada para bawahan telah
ditunaikannya, dan biasanya lebih
mengutamakan orientasi penyelesaian tugas
yang menjadi tanggung jawab para
bawahannya dan kurang memberikan
perhatian pada hubungan yang intim dengan
bawahannya.

Gaya kepemimpinan tokratik bukanlah kepemimpinan yang efektif adalah sesuai
gaya yang didambakan oleh para bawahan dengan situasi dan kondisi dimana pemimpin
dalam mengelola suatu organisasi karena pada harus mampu menganalisa terhadap situasi
gaya kepemimpinan otoriter/autokratis yang dihadapi pada suatu saat tertentu dan
kekuasaan sepenuhnya pada pimpinan mengidentifikasi kondisi anggota atau
sehingga bawahan tidak ada kebebasan untuk bawahannya. Kondisi bawahan merupakan
mengeksplorasi kemampuan yang dimilikinya faktor yang penting pada kepemimpinan
yang akan menyebabkan adanya stress kerja situasional karena bawahan selain sebagai

ABSTRACT: STYLE OF LEADERSHIP TO THE PERFORMANCE OF NURSES IN THE INPATIEN WARD OF THE GENERAL HOSPITAL IN THE CITY OF BEKASI
Backround: Leadership is the ability to influence people by setting goals for direction/command and motivation to work. The activities of leaders will show the pattern of leadership style applied in an organization can create a positive work effectiveness for members. Various kinds of leadership style of the head of the room is devided into 3 the leadership, authoritarian, democracy and laisez faire, the style of leadership applied will affect the performance of nurses. Methods: This research uses analytic survey research design using cressectional approach. Population of 67 repondents with a sample of 60 respondents. Sampling technique using simple random sampling. Univariate and bivariate data analysis. Result: Univariate based research result, showed that leadership style of the majority of the head of democratic leadership style as much as 47 (78,3%) respondents. INTISARI: HUBUNGAN GAYA KEPEMIMPINAN KEPALA RUANGAN TERHADAP KINERJA PERAWAT PELAKSANA DI RUANG RAWAT INAP RUMAH SAKIT UMUM DAERAH KOTA BEKASI
Pendahuluan: Kepemimpinan adalah kemampuan mempengaruhi orang-orang dengan menetapkan tujuan memberi pengarahan /perintah dan motivasi untuk bekerja. Aktivitas pemimpin akan menunjukan pola gaya kepemimpinan yang diterapkan dalam suatu organisasi dapat menciptakan efektivitas kerja yang positif bagi anggota. Macam-macam gaya kepemimpinan kepala ruangan terbagi menjadi 3 yaitu kepemimpinan otoriter, demokratis dan laisez faire (bebas tindak), gaya kepemimpinam yang diterapkan akan berpengaruh terhadap kinerja perawat. Metode: Penelitian ini menggunakan desain penelitian Survey Analitik dengan menggunakan pendekatan crosectional. Populasi sebanyak 67 responden dengan sampel 60 responden. Teknik sampling menggunakan Simple Random Sampling. Analisa data Univariat dan Bivariat.

Hasil: Hasil penelitian berdasarkan univariat, menunujukan bahwa Gaya Kepemimpinan Kepala ruangan mayoritas bergaya kepemimpinan demokratis sebanyak 47 (78,3%). Kinerja perawat pelaksana mayoritas berkinerja baik sebanyak 43 (71.7%). Analisa bivariat menggunakan chi square. Hasil penelitian menunjukan (p value =0.000 α=0.05). Kesimpulan: Terdapat hubungan yang signifikan antara gaya kepemimpinan dengan kinerja perawat

PENDAHULUAN
Keberhasilan suatu organisasi di dalam sebuah Rumah Sakit dari beberapa faktor dengan salah satunya yaitu tingkat sumber daya manusia termasuk seoarang perawat, selain itu pemimpin juga merupakan suatu faktor yang sangat berpengaruh terhadap aktivitas bawahannya untuk melakukan suatu pekerjaan dengan suatu pencapaian yang di tuju (Mulyana, 2013; Tribhuwana & Malang, 2018).
Berdasarkan hasil data sistem informasi organisasi profesi perawat yaitu PPNI jumlah perawat di indosnesia yang mendaftar sampai bulan april 2017 adalah 359.339 orang perawat. Berdasarkan rekapitulasi BPPSDMK per Desember 2016 adalah 296.876 orang perawat, data tersebut berdasarkan rekapitulasi akhir tahun 2016 melalui sistem informasi BPPSDMK yang terkumpul di berbagai fasilitas pelayanan kesehatan diantara nya Puskesmas, Rumah Sakit pemerintah , dan beberapa Rumah Sakit Swasta (Infodatin, 2017).
Kepemimpinan adalah suatu proses mempengaruhi orang lain dalam mencapai tujuan. Kepemimpinan adalah kemampuan mempengaruhi orang-orang dengan menetapkan tujuan, memberi pengarahan/perintah dan motivasi sehingga secara operasiona tujuan tercapai dan meningkatkan keberadaan organisasi, hasil pengaruhnya akan ditandai adanya kemauan staf yang bekerja dengan
sukarela (Noyita, 2013; Anwar, 2016) .
Aktivitas pemimpin akan menunjukanpola gaya kepemimpinan yang di terapkan dalam suatu organisasi yang ia bawahi. Gaya kepemimpinan adalah pola tingkah laku yang di rancang untuk mengintegrasikan tujuan organisasi dengan tujuan individu, untuk mencapai suatu tujuan dan gaya tersebut dapat digunakan oleh pemimpin untuk menilai staf atau bawahannya. Masing-masing gaya kepemimpinan memiliki keunggulan dan kelemahan. Seorang pemimpin akan menggunakan gaya kepemimpinan sesuai kemampuan dan kepribadiannya (Ariesta & Rahardjo; 2014; Syaifudin dkk, 2017).
Macam gaya kepemimpinan yang diterapkan dalam suatu organisasi dapat membantu menciptakan efektivitas kerja yang positif bagi anggota. Gaya kepemimpinan yang sesuai dengan situasi dan kondisi organisasi maka anggota akan lebih semangat dalam menjalankan tugas dan kewajibanya (Reza & Dirgantara, 2010; Sari & Zainaro, 2018).
Macam-macam gaya kepemimpinan kepala ruangan menurut Gillies (1996) yaitu, gaya otoriter, demoktratis, partisipatif dan laiseez faire (bebas tindak), dari gaya kepemimpinan yang diterapkan oleh pemimpin berpengaruh terhadap kinerja perawat. Kinerja perawat

merupakan aktivitas perawat dalam mengimplementasikan sebaik-baiknya suatu wewenang tugas dan tanggung jawab dalam rangka pencapaian tujuan tugas dan sasaran suatu organisasi dalam memberikan asuhan keperawatan. Keberhasilan dalam pelayanan keperawatan sangat ditentukan oleh kinerja seoarang perawat (Hesselbein & Goldsmith; 2013; Sari & Zainaro, 2018).
Kinerja dalam sebuah organisasi dilakukan oleh segenap sumber daya manusia yang ada baik pemimpinan atau pekerja.Ada beberapa faktor yang dapat mempengaruhi sumber daya manusia dalam menjalankan kinerja nya. Baik dari faktor dalam diri sendiri, gaya kepemimpinan yang di terapkan oleh salah satu kepala ruangan adalah gaya kepemimpinan demokratis, dari gaya kepemimpinan yang diterapkan mempengaruhi kinerja perawat yang bekerja dalam memberikan pelayanan keperawatan yang konstan dan terus menerus selama 24 jam kepada pasien setiap hari, sehingga memberikan gambaran baik buruknya kinerja seoarang perawat dari gaya kepemimpinan yang diterapkan oleh kepala ruangan (Nitasari & Lataruva, 2012; Zainaro dkk, 2017).
Kemenkes 2009 menyebutkan bahwa kinerja dalam pelayanan kesehatan belum memadai.Hal ini disebabkan oleh jumlah tenaga kesehatan dengan jumlah penduduk masih rendah.
METODE Penelitian ini merupakan penelitian deskriptif analitik, dengan pendekatan cross sectional dimana penelitian ini dilakukan dalam satu waktu. Populasi pada penelitian ini adalah perawat pelaksana di ruang rawat inap rumah sakit umum daerah kota bekasi sebanyak 67
Untuk itu pada tahun 2010 sampai 2020, perawat dituntut untuk mampu memberikan pelayanan professional berdasarkan standar global, artinya perawat harus bersaing dengan munculnya rumah sakit swasta dengan segala kompetisinya, dimana perawat dapat meningkatkan mutu pelayanan, meningkatkan profesionalisme kerja, memperbaiki dan menyempurnakan sistem pelayanan yang lebih efektif (Wulansari, 2015; Raodhah dkk, 2017).
Dari studi pendahuluan yang dilakukan peneliti di Rumah Sakit Umum daerah Kota Bekasi Rabu 2 mei 2018, bahwa jumlah perawat di 4 ruangan di Gedung E terdapat 80 perawat pelaksana, dan jumlah ruangan dari 4 ruangan terdapat 36 ruangan, hasil wawancara dengan salah satu kepala ruangan bahwa gedung E baru beroperasi pada bulan desember 2017, sehingga sistem manajemen nya masih dalam tahap penyesuaian untuk di terapkan di masing-masing ruangan, salah satu kepala ruangan menerapkan gaya kepemimpinan demoktratis untuk mempengaruhi kinerja perawat yang berada diruangannya.

maka sampel sebanyak 60 repsonden.Waktu penelitian ini dilakukan pada tanggal 12-25 juni 2018.
Alat pengumpulan data dalam penelitian ini menggunakan kusioner modifikasi yang terdiri dari data demografi ( usia, jenis kelamin, status kepegawaian, pendidikan terakhir, lama kerja, ), 15 item pernyataan variabel independen yaitu gaya kepemimpinan kepala ruangan, dan 9 pernyataan variabel dependen
yaitu kinerja perawat dalam bentuk skala likert.
Analisa yang digunakan dalam penelitian ini adalah analisa univariat untuk mengetahui distribusi frekuensi variabel yang diteliti yaitu gaya kepemimpinan kepala ruangan dan kinerja perawat pelaksana. analisa bivariat menjelaskan hubungan gaya kepemimpinan kepala ruangan terhadap kinerja perawat pelaksana. uji statistik yang digunakan yaitu uji chi square.

Berdasarkan tabel 4 menunjukan bahwa 60 dengan gaya kepemimpinan otoriter dengan kinerja kurang baik sebanyak 13 responden (21,67%) dan berkinerja baik 1 responden (1,66%), dan gaya kepemimpinan demokratis dengan kinerja kurang baik sebanyak 17 responden (28,34%) dan yang berkinerja baik sebanyak 43 responden (71,66%).
PEMBAHASAN
Hasil uji statistik dengan menggunakan uji chi square diperoleh diperoleh p value (0.000) < alpha (0.05) maka dapat disimpulkan maka H0 ditolak, dimana hasil menunjukan bahwa ada hubungan gaya kepemimpinan terhadap kinerja perawat di ruang rawat inap Rumah Sakit umum Daerah Kota Bekasi Tahun 2018. Kekuatan hubungan dengan korelasi kuat dengan nilai r=0,620 karena berada pada interval r=0,5-0,75. Dari data yang didapatkan bahwa gaya kepemimpinan yang diterapkan adalah gaya kepemimpinan demokrasi dimana sebanyak 78,3% responden memilih gaya kepemimpinan demokrasi dengan kinerja yang baik sebanyak 28.3%.
Dari hasil analisa peneliti bahwa gaya kepemimpinan demokratis dengan kinerja kurang baik sebanyak 4 responden (6,67%) dan yang memilih gaya kepemimpinan otoriter dengan kinerja baik sebanyak 1 responden (1,66%), menurut peneliti hal ini di sebabkan karena faktor dari tingkat pendidikan perawat dan juga lama kerja perawat tersebut dimana perawat yang kinerja kurang baik dan baik dengan gaya kepemimpinan demokratis dan otoriter tersebut dengan lama kerja hanya < 1 tahun dan dengan tingkat pendidikan yaitu SPK dan D3. Hal ini sesuai dengan teori bahwa terdapat dua faktor yang dapat mempengaruhi kinerja perawat

KESIMPULAN
Berdasarkan hasil penelitian tentang “ Hubungan Gaya Kepemimpinan Kepala Ruangan Terhadap Kinerja Perawat Pelaksana Di Ruang Rawat Inap Rumah Sakit Umum Daerah Kota Bekasi tahun 2018” dan pembahasan yang telah dijelaskan sebelumnya, maka dapat disimpulkan sebagai berikut : 1. Hal ini sesuai dengan penelitian yang dilakukan oleh Hutahaen (2009), yang mengidentifikasi gaya kepemimpinan kepala ruangan dan pengaruh semangat kerja perawat pelaksana di Rumah Sakit Umum Pusat Adam Malik Medan. Dari hasil yang diperoleh, didapatkan gaya kepemimpinan yang sering dipakaikepala ruangan adalah demokrasi. Dimana semangat kerja perawat pelaksana tinggi dengan gaya kepemimpinan demokrasi .
2. Distribusi frekuensi kinerja perawat di ruang rawat inap Rumah Sakit Umum daerah Kota Bekasi tahun 2018 dari 60 responden (100.0%) terbanyak memiliki kinerja dengan kategori “Baik” yaitu sebanyak 43 responden (71,66%).
3. Hasil p value = 0.000 lebih kecil dari nilai α =0.05 maka dapat disimpulkan H0 ditolak, artinya ada hubungan gaya kepemimpinan kepala ruangan terhadap kinerja perawat pelaksana di ruang rawat inap Rumah Sakit Umum Daerah Kota Bekasi Tahun 2018.

The Influence of Nurse Manager Leadership Style on Staff Nurse Work Engagement

BACKGROUND: Nursing literature supports the
importance of an engaged nursing workforce as a
means to positively influence performance. Nurse man-
ager leadership style plays a critical role in engaging
staff nurses. These relationships have been minimally
studied in nurse managers and staff nurses.
OBJECTIVE: The aim of this study is to evaluate the
influence of nurse manager leadership style factors
on staff nurse work engagement.
METHODS: Using a descriptive correlational re-
search design, 441 staff nurses working in 3 acute care
hospitals were surveyed. Survey instruments included
the Utrecht Work Engagement Scale and the Multi-
factorial Leadership Questionnaire 5X short form.
RESULTS: Transactional and transformational lead-
ership styles in nurse managers positively influenced
staff nurse work engagement. Passive-avoidant lead-
ership style in nurse managers negatively influenced
staff nurse work engagement.
CONCLUSIONS: Nurse managers who provide sup-
port and communication through transformational
and transactional leadership styles can have a positive
impact on staff nurse work engagement and ulti-
mately improve organizational outcomes.
With the uncertainty of the long-term impact of health-
care reform, regulatory mandates, advancing tech-
nology, potential workforce shortages, and financial

pressures, decisions regarding where to allocate re-
sources are a challenge for nurse managers (NMs).1-3
This problem has not gone unnoticed and national
organizations have weighed in on the issue. The In-
stitute of Medicine (IOM) has recognized the impor-
tance of developing nurse leaders as a means to impact
staff nurse practice environments and improve organi-
zational outcomes.4 In healthcare organizations, NMs
play an important role in influencing supportive staff
nurse practice environments. Nurse managers not only
manage nursing units but serve as leaders to staff
nurses. The influence of the NM is actualized through
their leadership style.5,6 It is through the leadership
style of the NM that change can be intentional and
effective in achieving organizational goals. Nurse man-
ager leadership style can positively impact the health-
care work environment and ultimately patientoutcomes.
Previous studies have suggested high levels of work
engagement were associated with high levels of patient
satisfaction, quality of care, and work effectiveness.4,7
Work Engagement
Early studies on work engagement were conducted in
non-healthcare settings. Only recently has research
on work engagement emerged in nursing literature.
In a study by Bogaert et al,1 work engagement was
determined to be influenced by practice environment
factors such as NM support at the unit level. Laschinger
et al3 concluded that staff nurse work engagement
resulted in increased job satisfaction and organiza-
tional commitment. Staff nurse work engagement re-
sulted in increased organizational commitment and
improved quality of work life.8 Work engagement
literature suggests that there is a positive impact on
employee productivity and it is influenced by lead-
ership style. The aim of this study is to investigate

the influence of NM leadership style on staff nurse
work engagement.
Conceptual Framework
This study was grounded in 2 previously developed
conceptual frameworks: The Full Range Leadership
Model (FRLM)9 and the Work Engagement Con-
ceptual Framework.10 The FRLM was selected to
guide this study because it highlights 3 main lead-
ership styles. These styles will be investigated in NMs
and the impact they have on staff nurse work en-
gagement. The FRLM developed by Bass9 describes
3 styles of leadership in a continuum from transfor-
mational to transactional to passive-avoidant (PA).
Nine factors are divided among 3 leadership styles
(Table 1). There are 5 factors associated with trans-
formational leadership style: idealized influence (be-
haviors), idealized influence (attributes), inspirational
motivation, intellectual stimulation, and individual
consideration. Idealized influence (behaviors) is de-
scribed as the trust and confidence a leader builds
with followers through personal association. Ideal-
ized influence (attributes) is the way a leader develops
a collective sense of mission and values with followers.9
Inspirational motivation is the promotion of a clear
vision by the leader to followers.9 Intellectual stimu-
lation is the promotion of innovation and creativity
among followers by the leader. Individualized con-
sideration is the empowerment passed onto the fol-
lowers by the leader. Empowerment is facilitated
through the leader by encouraging autonomy and
voice of the staff.9
There are 3 factors of transactional leadership
style: contingent reward, management by exception
(active), and management by exception (passive).
Contingent reward is defined as the exchange between
leaders and followers in which there is an exchange
for a specific reward. Management by exception is
defined as negative feedback, negative reinforcement,
and corrective criticism and is subdivided into 2 factors:
active and passive. Management by exception (active)
provides more negative reinforcement and corrective
criticism than management by exception (passive). This
type of leader watches for mistakes and takes corrective
action when mistakes are identified. Passive-avoidant
leadership is defined as a Black of leadership.[ The
leader takes a Bhands-off approach,[ delays decisions,
provides no feedback, and makes very little effort to
satisfy the needs of followers.9
The work engagement conceptual framework is
the 2nd framework chosen to guide this study. De-
scribed by Laschinger, the framework is character-
ized by 3 subscales: vigor, dedication, and absorption.3
Vigor is characterized by the mental resilience and
high levels of energy experienced during work. It is
the willingness to exert effort in work and to persist
during difficult times. Dedication is characterized by
the strong involvement in ones work. It is the expe-
rience of enthusiasm, inspiration, pride, sense of sig-
nificance, and challenge. Absorption is characterized
by the Bhappy[ engrossment in ones work and a full
level of concentration where time passes quickly and
a person has difficulty detaching from work.10 The
9 leadership style factors will be measured in NMs
based on the FRLM and the 3 work engagement sub-
scales will be measured in staff nurses based on the
work engagement conceptual framework.
Methods
A descriptive, correlational design was used to analyze
and evaluate the research question. An anonymous
online survey was developed using SurveyMonkey
and e-mailed to staff nurses at 3 hospitals in the
southeastern region of United States. The 3 hospitals
were chosen because of their similar size, location, and

healthcare services provided. One of the 3 hospitals
received MagnetA recognition. A gatekeeper was iden-
tified for each of the hospitals. The gatekeeper sent the
e-mails containing the survey as well as the reminder
e-mails to all staff nurses at each hospital. The 1st
12 survey items included demographic questions and
were followed by the Utrecht Work Engagement Scale
(UWES)10 and Multifactorial Leadership Question-
naire (MLQ) 5X short form.9 Inclusion criteria in-
cluded consent to participate, completion of the entire
survey, currently working as a staff nurse, and not
currently in hospital orientation. The survey was open
for 6 weeks, with reminder e-mails going out at weeks
2 and 4. No identifying information was collected
from the participants. Before the study was conducted,
institutional review board approval was obtained from
the supporting university and hospital sites. Com-
pleting the online survey by the staff nurse partici-
pants implied consent. The data collected using the
online survey were maintained in a Microsoft Excel
spreadsheet on a computer with a secure password-
protected file. Research Question
Based on this study aim, the following research ques-
tion was proposed: What is the influence of NM
leadership style factors on staff nurse work engagement?
Survey Instruments
Multifactor Leadership Questionnaire 5X Short Form
The current study used 2 previously validated survey
instruments to investigate the study variables: the
MLQ 5X short form9 and the UWES.10 Staff nurse
perception of NM leadership style was measured using
the MLQ. This 45-item instrument measures a con-
tinuum of 3 leadership styles: transformational, tran-
sactional, and PA.9 This questionnaire has been used
internationally to measure leadership style across
a wide variety of disciplines, including nursing. The
questionnaire is rated using a 5-point Likert scale
ranging from 0 (not at all) to 4 (frequently, if not
always). Five factors are used to measure transfor-
mational leadership style, 3 factors measure transac-
tional leadership style, and 1 factor is used to measure
PA leadership style. A high degree of validity and
reliability has been established with the MLQ, with a
reported Cronbach"s ! of .76 to .89.9 Staff nurse
perception of NM leadership style was measured.
Permission to use this instrument was granted by the
instrument developers. Work Engagement
Work engagement was measured using the UWES.10
This 17-item instrument has been widely used in

various countries and across many disciplines, includ-
ing nursing. The instrument is rated using a 7-point
Likert scale ranging from 0 (never) to 6 (always) and
measures 3 subscales of work engagement: vigor,
dedication, and absorption. A high degree of validity
and reliability has been established with this instru-
ment, with a reported overall Cronbach ! of .71 to
.84. Permission to use this instrument was granted
by the instrument developers.10
Results
Sample
The survey was emailed to 2,008 registered nurses
(RNs) working in 3 acute care hospitals located in
southeastern United States. A total of 630 RNs ac-
cessed the survey; 441 met inclusion criteria and were
subsequently analyzed. A priori power analysis estab-
lished that adequate power existed to detect signifi-
cant relationships among the study variables based on
a sample size of 395. Table 2 depicts a breakdown of
the staff nurse participant demographics. Most of the
respondents were in the 30 to 39 years age range (n =
116, 26.3%) and held a baccalaureate degree in
nursing (BSN) (n = 225, 51.7) as their highest degree
of educational preparation. They reported employ-
ment in their current job for less than 1 year (n = 55,
12.5%), 1 to 5 years (n = 120, 27.2%), 6 to 10 years
(n = 133, 30.2%), and more than 10 years (n = 133,
30.2%). Most of the staff nurses worked full-time (n =
300, 68%) and on the day shift (n = 252, 57.1%).
Most of the staff nurses worked in both medical-
surgical nursing units (n = 109, 24.7%) and critical
care units (n = 107, 24.2%). The sample demo-
graphics were compared with the Health Resources
and Services Administration11 national sample of
RNs and were concluded to be similar to this study
sample of staff nurses.
Data Analysis
Scale means and standard deviations did not show
that the staff nurses viewed their NMs as strongly
transformational, transactional, or PA in their lead-
ership style. Table 3 depicts the MLQ leadership
style factors, UWES subscales, number of items for
each survey item, and mean (SD) scores. The highest
means belonged to transformational leadership style
factor of inspirational motivation (mean [SD], 2.69
[1.01]; with a 0 to 4 point scale range) and the work
engagement subscale of dedication (mean [SD], 4.49
[0.95]; with a 0 to 6 point scale range). The lowest
means belonged to the PA leadership style (mean
[SD], 1.43 [0.98]) and the work engagement
subscale of absorption (mean [SD], 3.65 [0.82]). JONA Vol. 46, No. 9 September 201

Multivariate analysis was used to conduct the
multiple regression of the study variables and deter-
mine the influence of the 9 NM leadership style factors
on the 3 staff nurse work engagement subscales. The
chosen method for the multiple regression data analysis
was a backward elimination technique. This data
analysis procedure tested the predicted value of the

dependent variables (work engagement subscales)
against a set of independent variables (leadership style
factors). In a series of steps, the independent variable
with the smallest decrease in R2 was removed (the fit
within the model along the regression line). Removal
of the independent variables continued in a stepwise
fashion until the only study variable remaining sig-
nificantly contributed to the model. Significance was
based on a predetermined ! value of P G .05. Tables 4
to 6 depict the multiple regression data analysis find-
ings for each of the work engagement subscales of
vigor, dedication, and absorptions, respectively. None
of the perceived NM leadership styles resulted in a
significant impact on the staff nurse work engage-
ment subscale of vigor (Table 4).
The data analysis revealed that transformational,
transactional, and PA leadership style factors signifi-
cantly influenced the staff nurse work engagement
subscale of dedication (Table 5). Each transforma-
tional leadership style factor (idealized influence behav-
ior and attributes, inspirational motivation, individual
consideration, and intellectual stimulation) (P G .001)
resulted in a positive and significant influence on staff
nurse work engagement. The transactional leadership
style factors of contingent reward and management by
exception (active) had a positive and significant influ-
ence on staff nurse work engagement (P G .001).
The transactional leadership style factor of man-
agement by exception (passive) and PA had a neg-
ative and significant influence on staff nurse work
engagement (P G .001) (Table 5). For the work en-
gagement subscale of absorption, the data analysis

revealed a negative and significant influence of PA
leadership style (P G .05) and the transactional leader-
ship style factor of management by exception (active)
(P G .05) on staff nurse work engagement (Table 6).
Discussion
The study findings for NM leadership style and staff
nurse work engagement were consistent with previ-
ous research findings and support the current liter-
ature by examining the influence of 3 leadership styles
in NMs on staff nurse work engagement. Bamford
et al12 concluded that transformational leadership
had a positive impact on staff nurse work engage-
ment. This positive impact on staff nurse work en-
gagement may be explained through the supportive
and relational leadership behaviors associated with
transformational leadership styles.7 Examples of sup-
portive leadership behaviors include role modeling,

promoting a clear vision, innovation, creativity, and
encouraging autonomy.13
The study findings for transactional leadership
style factors revealed a mixture of both positive and
negative predictive relationship with the study vari-
ables. This is consistent with previous research that
concluded that nurses may be engaged by the rewards
provided by transactional leaders.13,14 When the
transactional leader exemplified passive leadership
behaviors such as delayed feedback and limited com-
munication, a negative impact on work engage-
ment was identified.15 The transactional leadership
style factor of contingent reward and management
by exception (active) were both significant and positive
predictors for work engagement. The transactional
leadership style factor of management by exception
(passive) was a significant and negative predictor of
staff nurse work engagement. When a NM exempli-
fied a transactional leadership style that was passive,
the impact in work engagement was negatively im-
pacted. Passive behaviors include delaying feedback
and limited communication by the NM.15,16
The study findings for PA leadership style in NMs
revealed a significant and negative impact on staff
nurse work engagement. This is consistent with the
study conclusions by Negussie and Demissie,13 who
described the negative impact of PA leaders such as
delaying decision making and providing little to no
feedback to employees.
Findings from this study support the importance
of the leader communicating with followers. When
NMs provide frequent feedback, the result is increased
staff nurse work engagement. A lack of communica-
tion and feedback from the NM resulted in decreased
staff nurse work engagement. Communication is the
hallmark of transformational leadership style, and this

leadership style in NMs has the potential to positively
influence organizational success through staff nurse
work engagement. Transformational leadership style
should serve as a guide for leadership development
because it empowers, motivates, and fosters account-
ability among followers and ultimately improves sat-
isfaction, which increases organizational success.13,16
Limitations and Future Research
There were several limitations associated with this
study. Limitations include the use of a convenience
sample and use of a survey. The use of a convenience
sample prevents generalization of the findings to
the staff nurse population. The survey measured
staff nurse perception of NM leadership style and
staff nurse engagement. A 1-time survey is a limita-
tion because participants may not honestly answer
the survey questions and may score based on selec-
tive memory of their NM. Another limitation to con-
sider is that the perceptions of engagement may vary
over time and this study used a 1-time measure of the
study variables.
Future research should include the use of differ-
ent research strategies that can lead to new insights
on this topic. One example of an alternative research
strategy could include intervention research that uses
leadership development programs while measuring
organizational outcomes such as nurse retention and
patient satisfaction scores. Alternative research strat-
egies can be useful in obtaining more information
about how leadership development programs can
best impact organizational outcomes such as staff
nurse work engagement. Implications for Practice
The results from this study have direct implications
for nursing practice and support the call by the IOM
to promote the development of nurse leaders.4 Rec-
ommendations include development of leadership
skills in NM that emphasize transformational and
transactional management by exception (active) be-
haviors while minimizing management by exception
(passive) and PA leadership behaviors. Key differences
between these leadership styles are supportive lead-
ership behaviors and increased communication with
staff. Communication can include providing positive
or negative feedback. The recommendation for prac-
tice is for NMs to refine their skills as leaders. This
can be accomplished through leadership develop-
ment training, self-assessment, reflection, and men-
torship, all of which can have a positive impact on
organizational outcomes and can ultimately impact
organizational outcomes such as job satisfaction and
intent to stay.14-16

Nurse Practitioners’ perceptions of their ability to enact
leadership in hospital care

Abstract
Aims and objective: To gain insight into nurse practitioners‟ (NP) leadership roles in Dutch
hospital care, by exploring the perceptions regarding their current leadership role and the
differences with their previous role as a registered specialized nurse.
Background: To meet today‟s challenges of the increasing healthcare demands, the
employment of NPs is proliferating. NPs have the ideal position to play a pivotal role within
healthcare reforms, yet full expansion of their scope of practice and expertise is having
limited success. Long-term sustainability of NPs depends on the ability to perform and
develop a leading role.
Design and methods: This qualitative descriptive study was conducted in fifteen Dutch
hospitals. Data were collected from April to July 2018 and purposive sampling was used for
eighteen semi-structured interviews. This study is conducted and reported according to the
COREQ checklist (see Supplementary File 1).
Results: Three main themes concerning NPs‟ current leadership role emerged and they were
all linked to a successful positioning of NPs. All themes seemed to be of influence on NPs‟
scope of daily practice. Direct patient care was emphasized and leadership on other levels
appeared to be underused. Most NPs desired to reshape their profession. However,
unprofitable use of their leadership skills especially on professional- and organizational level
and lack of supportive factors seemed to hinder them.

Conclusions: An adequate use of leadership is crucial for role development and positioning
of NPs. Further development of the NP profession can help to better differentiate between the
tasks of registered specialized nurses and NPs.
Relevance to clinical practice: Leadership in nursing contributes to the improvement of the
quality and efficiency of healthcare. Further positioning of the NP profession depends on a
profitable use of leadership competencies. Besides, NPs should collaborate with healthcare
organizations, educational institutions, and professional associations to value the NP
profession in the current healthcare system.

le 1. Introduction
Rising healthcare costs, technological innovations, integrative care, the ageing population,
and the need to pursue equity increase the pressure on healthcare systems to maintain their
quality and high safety outcomes (Australian College of Nursing, 2015; Delamaire &
Lafortune, 2010). Nurses play a central role in this, as they work across all sectors, deliver
care in a broad range of health services, and represent the largest group of health providers
(Australian College of Nursing, 2015). The Institute of Medicine claimed that this is a
remarkable opportunity for nurses to employ leadership competences and meet the challenges
of an uncertain healthcare climate (Denker, Sherman, Hutton-Woodland, Brunell, & Medina,
2015).
Besides the increased focus on leadership among nurses, innovative advanced roles are
proliferating, and the employment of nurse practitioners (NP) is emerging (Vulto & Vianen,
2009; Zwijnenberg & Bours, 2012). Compared to registered specialized nurses, NPs have
developed more knowledge and skills, and possess other properties, such as the achievement
of new practice, responsibility for health promotion and significant role autonomy (Hamric,
Hanson, Tracy, & O‟Grady, 2014). According to Elliott, Begley, Sheaf, and Higgins (2016),
long-term sustainability of NPs depends on the ability to perform and develop the leadership
component of their role.
Denker et al. (2015) concluded that nurses‟ leadership development is a complicated process
since today‟s healthcare is fast paced and quickly changing. Whether NPs take the lead in
these areas relies upon their personal qualities and characteristics, demands of the patient,
characteristics of the organization and health policy issues (Elliott et al., 2016; Fealy et al.,
2018; Hamric et al., 2014). At this moment it is unclear how NPs in the Netherlands have
developed their leadership role during the educational program and how their current
competencies differ from the ones they possessed as a registered specialized nurse. Likewise,
scant information on NPs‟ experiences regarding their leadership role and position is
available.
2. Background
NP is the overall designation for nurses with an advanced degree in a nursing program
including a specific focus area of practice. NPs are expected to have a significant impact in

le the development of practice, professionalization of their teams, and to improve the quality
and safety of healthcare services (Elliott et al., 2016). NPs must understand the bigger
networks around the patient that may influence the provision of care, recognize the need for
innovation and implement strategies to improve safety and quality of care. NPs are more
autonomous in clinical decision making from a medical perspective and they are more often
consulted by the multidisciplinary teams than are registered specialized nurses (Begley et al.,
2010; Zwijnenberg & Bours, 2012). Although registered specialized nurses are also expected
to enact leadership in daily practice, many policy-makers believe that NPs have additional
responsibility for professional development and practice innovation. Besides, it is assumed
that NPs can connect their leadership performance with their job performance evaluation
(Begley et al., 2010).
Hamric et al. (2014) distinguished four different leadership roles: clinical-, professional-,
system-, and political leadership. Leadership tasks of NPs at system- and political level are
still underdeveloped because NPs are mainly occupied with clinical care and case-load
responsibility (Elliott et al., 2016). Even though NPs value the non-clinical aspect of their
profession, they frequently reported barriers to participate in education and research activities
(Dicenso & Bryant-Lukosius, 2010). The roles of NPs have been widely studied, although
with a primary focus on generic features of the profession and the direct influence of NPs on
the health care system (Carryer, Gardner, Dunn, & Gardner, 2007; Gardner & Gardner,
2005). Additionally, a broad interpretation of the term „Advanced Practice‟ exists worldwide
between countries, which makes it difficult to formulate a clear and consistent scope of
practice (Gardner & Gardner, 2005; Schober, 2017), and thus the extent of the NP leadership
role.
2.1 Aim
This study aims to gain insight into NPs‟ leadership roles in Dutch hospital care, which could
contribute to a better understanding of their scope of practice and its influential factors. The
study has the following research objectives:
- To explore NPs‟ perception regarding their current leadership role and to discover
which barriers and facilitators they experience.
- To explore the experienced differences in leadership role between the NP position and
the previous position as a registered specialized nurse

le 3. Methods
This study is conducted and reported according to the Consolidated criteria for Reporting
Qualitative Research (COREQ) checklist (supplementary file 1) (Tong, Sainsbury, & Craig,
2007).
3.1 Design
A qualitative descriptive design was used to obtain insights into the perceived leadership role
of NPs. The aim of qualitative description is to provide a rich and unbending description of
an event (Neergaard, Olesen, Andersen, & Sondergaard, 2009) and it contributes to exploring
people‟s experiences and opinions (Price, Jhangiani, & Chiang, 2015).
3.2 Registered specialized nurses and NPs in the Netherlands
3.2.1 Registered specialized nurses
A registered specialized nurse is a registered nurse with an associate degree basic registration
in the Dutch registry for healthcare professionals including supplementary education in their
area of expertise (European Qualifications Framework (EQF) level 4 or 6). Registered
specialized nurses‟ competencies are defined in accordance with the CanMEDS-system
(Lambregts & Grotendorst, 2012; V&VN, 2018).
3.2.2 NPs
In the Netherlands, after having completed a two-year master‟s program for Advanced
Nursing Practice (MANP), one can register as NP („Verpleegkundig specialist‟ in Dutch),
which is a legally protected title in the Dutch registry for nurse practitioners (Vulto &
Vianen, 2009; V&VN, 2018). The total study load consists of 3360 hours, or 120 EC. The
master program is offered by nine Universities of Applied Sciences and focuses on the
acquisition of function-specific skills and expertise. The program is based on the
apprenticeship model of learning, which refers to a combination work-based learning in their
healthcare settings and universities. The program can be followed after an initial nursing
degree program is completed at bachelor level and is classified as EQF level 7. NPs work in
somatic or mental healthcare and their competencies are also based on the CanMEDS-system
(Lambregts & Grotendorst, 2012). Vulto and Vianen described five competencies that
distinguish NPs from registered specialized nurses in the Netherlands: (1) clinical reasoning
and the ability to independently make a diagnosis; (2) possessing medical knowledge and

le skills to bridge the gap between physicians and nurses; (3) being informed about recent
scientific research; (4) being involved in division of management plan; and (5) showing
leadership in innovation processes.
3.3 Participants
A homogeneous purposive sampling of nurse practitioners across various specialties and
hospitals in the Netherlands was chosen. This sampling method can be used for describing a
specific subgroup with its aim to decrease variation (Palinkas et al., 2015). In this study, five
eligibility criteria for NPs were selected: (1) sufficient proficiency in the Dutch language; (2)
at least three years of work experience as a specialized registered nurse before starting the
MANP; (3) currently two to six years working experience as NP; (4) NP specialization is
equal to the specialization as registered nurse; and (5) the MANP degree must have been
obtained at one of the nine Universities of Applied Sciences in the Netherlands that offer the
MANP program. The above-mentioned work experience is required to ensure sufficient
leadership development. Characteristics of the study population can be found in table 1.
Invitations by e-mail were sent to program directors of six Master Advanced Nursing
Programs in the Netherlands. A convenience sample was selected to include various sizes,
geographic locations and diversity in education. Subsequently, five program directors
selected students who graduated in the years 2012 to 2017 and invited these by e-mail to take
part in this study. An information flyer about the research including the possibility to sign up
was published at a Facebook page for NPs. At one University, this study was also promoted
at a NP alumni event. NPs who agreed to participate in the study were contacted by the
principal investigator (JK) to plan a face-to-face interview and they received additional
information about the research. The interviews took place at a time and location convenient
for the participants. Allowing the participant to decide on where the interview will be held is
not only a technical matter since power relations will not be encouraged when doing so
(Herzog, 2005). At one University, none of the Alumni responded to the invitation. In total,
fifteen participants were included.
While conducting the interviews, it appeared that three participants did not meet the required
work experience as NP. Two participants had graduated for a year and one participant for
more than six years. Despite the specification of the strict inclusion criteria, the interviews

le were continued. After data saturation had been reached with the other fifteen interviews,
these three interviews were analyzed to see whether differences appeared when not meeting
the exact inclusion criteria on work experience. The duration of work experience did not
prove to be an obstacle to the overall picture of their leadership development. Following an
iterative decision-making process with all authors, they were included in the final sample.
3.4 Data collection
3.4.1 Interviews
One researcher (JK) conducted eighteen in-depth semi-structured interviews from April up to
and including June 2018. Conducting this type of interview makes it possible to seek deeper
information and knowledge than one can obtain from surveys or structured interviews
(Johnson, 2011). All participants were interviewed once. The interviews took between 45 and
60 minutes, were digitally recorded after participants‟ permission, and field notes were taken
immediately after each interview. An interview guide used during the interviews (Table 2)
functioned as a tool to embellish consistency in data collection. The guideline was based on
Hamric et al.‟s (2014) description of NPs‟ leadership domains and their corresponding
competencies. In addition, the guideline was discussed with a multi-jurisdictional Advisory
Board including seven researchers (JK, CO, HV, MH, AH, MA, JP) and one pilot interview
was conducted. For improvement of interviewing skills, it can be helpful to have a more
experienced researcher listen to the interview tapes (Streubert & Carpenter, 2001). Therefore,
the quality of the interviews was evaluated with the senior investigator (CO) who is an
interviewing expert. Participants were asked if they wanted to receive the transcript to give
comments. One participant requested a few minor changes.
3.4.2 Role of the researchers
JK is a female registered nurse and MSc graduate. CO is a female registered nurse with a
post-academic degree and working as a senior advisor and researcher. Personal relationships
between the participants and interviewer did not exist. Participants knew about the role and
education of the interviewer before the interview started. The Advisory Board members (HV,
MH, AH, MA, JP) are all senior researchers and were involved in creating the topic list,
analysis and interpretation of the findings and finalizing the article.

le 3.5 Data analysis
The interviews were conducted, transcribed verbatim, analyzed independently and coded.
Two researchers (JK, CO) were directly involved in the process of the inductive and constant
comparative analysis. All field notes and transcripts were coded into themes and categories in
order to form a conclusion. To strengthen the rigor, the trustworthiness of the data and
interpretations were considered (Milne & Oberle, 2005). Several main themes emerged from
the data and they were refined various times. The codes, categories and themes were also
discussed with members of the Advisory Board. Whenever there was a disagreement,
discussions and clarifications were continued until consensus was reached. This led to a
distillation of data into thematic description. Data analysis was conducted in Dutch, using
Atlas-ti version 8.2.0 software.
3.6 Ethical considerations
Participation was voluntary, participants were assured of confidentiality and anonymity, and
they had the right to withdraw at any point without prejudice. Prior to conducting the
interviews, participants were asked to sign an informed consent form. All data were to remain
confidential, and the anonymity of the participants was guaranteed by dissociation of names.
The medical ethics review board of the Radboud Institute of Health Sciences approved the
study but waived the need for ethical approval as the study had no effect on the participants‟
wellbeing. According to the board‟s rules and legislations, data will be archived for 15 years
and saved under identification numbers.

le 4. Results
All participants were convinced that being a nurse leader had a positive effect on the quality
of daily care as well as on the positioning of NPs. Main differences with their previous
function as a specialized registered nurse were found in the degree of accountability and
responsibility in patient care. Issues mentioned included patient consultation, demands from
the patient and influence over money issues. The analysis of the data has led to the
emergence of three major themes concerning NP‟s current leadership role: (1) focus on
clinical leadership; (2) shaping the NP profession; and (3) essential components for NP‟s
practice.
4.1 Focus on clinical leadership
Taking the lead in the development of high-quality care and direct patient care was pointed
out in all interviews. Participants indicated a higher level of medical knowledge and a
different sense of responsibility towards patient and colleagues. Integration of knowledge,
skills and attributes is required for competency building. These three indicators were
identified from the interviews.
4.1.1 Clinical knowledge
Nursing expertise and advanced knowledge of diseases of the patient population were present
and identified in the interviews. Participants mentioned knowledge as an important factor to
further elaborate their role as NP. A majority stated that they developed more advanced
medical knowledge during, and after their MANP education. Some of them indicated that this
expansion in clinical knowledge supported them to carry more responsibilities. According to
the participants, having sufficient knowledge provides more confidence – which was directly
related with being and feeling a nurse leader.
You are able to ask more questions in consultation. You notice connections and you can
better understand the cause of the problem. Medical knowledge has grown, which has been
nice. I certainly made a big step forward. (Participant 15)
Our leadership (NPs’; ed.) is mainly focused on knowledge and expertise on activities you
often execute. (Participant 3)

le 4.1.2 Skills for professional development
Participants mentioned several skills that helped expand tasks and responsibilities, such as
adaptive- and reflective capacity, setting and keeping personal boundaries, taking matters into
own hands, and effective communication skills.
A substantial number described themselves as the „spider in the web‟, based on their
coordinating and connecting role in daily practice. It appeared that the majority of the
participants fulfilled this role, even without mentioning this expression. Participants
frequently mentioned that they acted as the binding factor between various disciplines in
hospitals, like physicians, nurses and management.
In my team, I would describe myself as the spider in the web. Ensuring that the patient care
runs well and to get things done. If problems arise, or people are sick and at home, I have to
come up with a solution. (Participant 4)
All participants were convinced that conducting research assisted in enhancing the quality of
care and the development of the profession. Some of them were actively involved in
conducting research, while others stated that they did not have the ambition because they
preferred to be active in direct patient care.
[…] using research activities to explore the exact problem. Thanks to this development, I
think my love for practice-oriented research has arisen. (Participant 12)
Some of the participants‟ descriptions of their current role also included policy-related
activities. For example, few participants were active in supporting substitution of hospital
care with primary care and in financial affairs like managing budgets or cost-effectiveness
studies. Nonetheless, several participants indicated that their activities were mainly focused
on clinical care due to high workload on patient care and focus on medical practice.
In my opinion, as a nurse leader I also need to manage budgets […] I keep track of the
number of patients and how much money we spend on it. (Participant 9)
Most participants fulfilled a role in professional associations varying from secretary to board
member. For some participants this involved a monodisciplinary collaboration with other

le NPs, while others joined associations focusing on their own specialty population. These
activities were seen as a way to encourage knowledge sharing and promotion, networking and
to create unambiguity about the profession. Knowledge is also shared by giving presentations
and training programs. Few participants indicated that they teach at universities and
participate at national congresses.
I think knowledge sharing is the most important factor. That people learn about certain
diseases, how to recognize them and what you can do about it. I think that is important. And I
like doing that. (Participant 15)
Some participants mentioned that they were involved in educating NPs in training or in
coaching registered nurses within the hospital.
That is our strength as NP I believe. Especially people who just… who are anxious about it,
who do not want to be at the forefront and whose network is less clear […]; yes, we need to
help those people. (Participant 12)
4.1.3 Attributes for the improvement of patient care
Fundamental to the ideas of all participants was the intrinsic drive and high feeling of
responsibility to deliver high quality patient care. This appeared to be the driving force in
their leadership experience since they were all convinced of having a significant impact on
clinical practice. Repeated references to „being responsible for‟, „serving their interests‟ and
„patient‟s satisfaction‟ illustrate their strong connection with this theme.
Yes, my heart really lies in patient care and I feel like a nurse leader when I have clearly
defined the actual problem and when I have devised a plan to resolve this. (Participant 15)
To be primarily accountable for a patient and being an independent health professional as an
NP was a challenging shift from the previous position as a registered specialized nurse.
Carrying this responsibility made the participants feel capable of providing care at an
advanced level.

le I think a nurse leader must be able to function independently. Call a physician in the case of
an event, but in general you have to know exactly how care works. Being able to act and to
respond to problems […] I think that is important. Especially the responsibility and
independence. (Participant 2)
Showing initiative was seen as important for patient care, personal development and
positioning of NPs. Several participants stated that, as pioneers, they had to take the initiative
to create a role for themselves in the organization.
[…] eventually we decided to sit down with a lawyer. So, we invited one to check the exact
scope of our competencies and abilities. Sometimes we are seen as nurses, and they
(physicians; ed.) are like ‘we will arrange it’, and sometimes when it is not a good time, you
are portrayed as a physician and receive unwanted responsibilities while you do not even
have the authority to carry them. (Participant 10)
Almost all participants claimed that they felt more confident in their task execution as a NP
after having some experience in their profession and being familiar with the department.
[…] by doing it a lot. And when you notice that it runs smoothly, you know that you are
delivering good work. This gives you confidence. I have been prescribing medicine for four
years and in the first year it was different from how I do it nowadays. (Participant 7)
4.2 Shaping the NP role
Different barriers and facilitators to task execution and the leadership role were indicated
with regard to three subjects: (1) collaboration for a successful integration of NPs; (2)
productivity growth as priority; and (3) task- and goal fulfillment depending on personal
characteristics and drivers.
4.2.1 Collaboration for a successful integration of NPs
The value of a supportive work environment was mentioned many times. For example, the
physician‟s confidence to cede responsibility and let a NP be primarily accountable for a
patient‟s wellbeing was seen as supportive. Besides, participants indicated that getting

le support and approval from their management was important for further advancement of
knowledge and task execution at policy level.
I think it is something you have to organize with your own team and department within the
big hospital. There are short lines of communication and I think management in your own
department is the place where to find task commitment. (Participant 4)
If you do not have any commitment from the management in the hospital, they would not
involve us in a number of projects. This may result in having less insight and you could do
much less. Thus, the management must be committed to involve the NPs. (Participant 14)
Most of the participants indicated that being a NP and standing up for the profession was
easier when working together with two or more NPs at a department. Working alone could be
a barrier to an advanced leadership role.
One of my colleagues is leaving and there will be a gap. This makes me sad, first of all to
miss my nice colleague, but also because I will have to fight alone now. (Participant 10)
The participants came up with several wishes with regard to shaping their profession. For
example: a better and more intense collaboration with nurses and physicians; a better balance
between working in the outpatient unit and ward; and more involvement in research and
cross-disciplinary tasks such as policy-related or managerial and financial tasks.
It must be a combination. For me that would be a way to better position my function and
distinguish it from that of the nurses in my team. To tell them ‘this is my function. I execute
patient care, but I also pursue other activities which I can independently perform and are
given the freedom and time to actually realize them’. In my opinion, that is a must for a nurse
practitioner. (Participant 4)
4.2.2 Productivity growth as priority
High clinical workload and the demand for and high focus on efficiency and productivity
were seen as barriers for further development. Many participants indicated that they worked

le on own time to perform research, and to further elaborate activities on policy level and in
professional associations.
We are a company; the hospital is just a company and we are being judged on productivity
rates […] If you only focus on ‘patient first’, you are undermining lots of functions. In my
opinion, you will definitely not get there then (practice full scope of their education and
profession; ed.). (Participant 9)
4.2.3 Task- and goal fulfillment depending on personal characteristics and drivers
It appeared that personal skills and attributes played a substantial role in the participants‟ task
execution. Some participants indicated the importance of standing up for yourself and of not
being afraid to take responsibility if you wanted to be active in other domains. Many others,
however, mainly experienced external barriers which impeded further development and they
did not mention their own personal capacity, which may be of great influence.
I like to engage in the discussion because I really believe there is lots of added value; so
many underused opportunities associated with this function. I actually want to tell them that
we can be of great use. (Participant 13)
For most participants, the main drive to be a NP was knowledge and skills building regarding
their specialty as registered specialized nurse. Besides, authorization to prescribing
medication and the diagnostic decision-making were many times indicated as reasons to take
the MANP program. Several participants claimed that they encountered limitations in their
daily practice as registered specialized nurse.
To give an example how I ended up in this position. I was confronted with my limitations as
registered specialized nurse. I always resented the fact that I knew very well what treatment
these patients needed while I still needed a physician to receive a signature, prescription or
to put an order in the computer. All this despite the fact that I knew very well what to do with
it and how to interpret results. (Participant 13)

le 4.3 Essential parties for NP‟s practice
The analysis showed that all participants had ideas about the future positioning of their
function and they were all, in some way, actively involved in seeking opportunities for role
expansion and positioning. Important parties in this development were: (1) healthcare
organizations; (2) the MANP program; and (3) professional associations for NPs.
4.3.1 Healthcare organizations
An established common vision between NPs and healthcare providers often turned out to be
absent or underdeveloped. Most participants mentioned a lack of general awareness and
clarity about the NP function among staff members. Some participants mentioned that the
physician assistant role was often considered the same as the NP role, which may create
friction between different disciplines. Besides, overlap in domains between nurses and
physicians also appeared to have a significant impact.
Yes, sometimes they have that idea, in particular the junior physicians. They assume: ‘he is
going to take my job’. That is, of course, not my intention at all. (Participant 3)
And up to the present day I am struggling with that. Even this morning, three times I was
called a diabetes nurse while I thought by myself, no I am not. (Participant 9)
Having a board for NPs hospital-wide was often mentioned as solution to create a well-
known, nationally accepted profession. Though all participants were convinced of a possible
positive contribution to their position, the vast majority of participants stated that they did not
have a common board (yet) in their hospital or were not actively involved.
We are now in the process of setting up a NP board in the hospital, because that has not yet
been well arranged. In case of any changes, they do not know who to approach because we
simply do not have a board. (Participant 2)
[…] ‘now hold on’, I said. ‘If we cannot even agree on who we are, what we are and what we
stand for, how on earth do you expect the organization to know it?’ (Participant 9)

le It appeared that those involved in research were confronted with unpleasant or difficult
situations in their healthcare organization. They claimed that the hospital did not support
them and for this reason they went looking for other opportunities to execute their research.
I wanted to do more research, but the hospital did not support me. They said: ‘it does not fit
you nor the hospital. It is not a university hospital and we want you to run production, we
want to use you for other purposes.’ (Participant 12)
Almost all participants mentioned that they fulfill a relatively new function and that this did
not come without concerns. It is feared that NPs are the first to be cut out and fulfil a
profession in a grey area, which means that especially right now it is important to show the
exact added value.
Hereby, a smooth positioning is fundamental. That you never get contracted out the function.
Because if you get cut out, I think we are the first to struck out at the moment things are
getting difficult. We are the youngest and you cannot function without an emergency nurse,
you need them in daily practice. You also need an emergency physician, you cannot function
without it. I think we are of additional value until the moment it becomes problematic. You
have to make sure to position yourself in a manner that everyone knows what you do, without
thinking too highly of yourself. (Participant 3)
4.3.2 MANP program
All participants recognized the added value of the two-year master program regarding the
execution of their function. They especially highlighted an increase in medical knowledge
and skills which gave them the opportunity to take the responsibility that this profession
requires. In addition, various participants mentioned that it enabled them to further develop
on personal level, i.e. being aware of one‟s limitations and responsibilities to face the
demands of this new profession.
The MANP education was strongly focused on not falling back into your old job, your old
way of thinking and old status. I think that situation is dangerous, because it is also safe. It
feels safe to do what you used to do before, but then you will get what you previously got. You

le have made the switch to another job, so you should not want that. Your position has other
responsibilities. (Participant 17)
However, some participants felt that the program did not prepare them well enough for
difficulties they might encounter in practice. They wished that more attention would have
been given to positioning of the function.
I think it is very important that the educational institutes address positioning of the function.
Doing this during the program has the advantage of being with like-minded people. Everyone
is facing the same situation, although in different situations. Often, the question is how to
position yourself or how to receive a study budget. And in my opinion the institutes are the
perfect places to invite the right people to talk about it. (Participant 3)
4.3.3 Professional associations for NPs
According to most participants, professional associations for NPs could stimulate positioning
of the profession. It could help create awareness among other disciplines and patients.
Besides, this could also do away with the ambiguity about the job description and
responsibilities.
In my opinion, V&VN VS (the abbreviation for the professional association representing NPs
in the Netherlands), should negotiate with for example health insurances or the ministry.
They have to represent the relatively small group of NPs and they have to point out, ‘this is
still a group of young health professionals, this is their quality’. So, a bit of appreciation,
conferment or more clear rules. (Participant 18)

le 5. Discussion
Three main themes were identified regarding NP role development and utilization that
highlight the barriers and enablers participants perceived to enact their leadership role. It
appeared that the results emerged from these topics showed similarities and all seemed to be
of influence in the degree of accountability, responsibility and autonomy NPs experienced
and possessed in their daily practice
5.1 Main findings
5.1.1 Focus on clinical leadership
The results of this study confirmed that participants fulfill their nursing leadership role in
different domains of clinical-, professional-, system-, and political leadership (Hamric et al.,
2014). Notwithstanding, their actual performance mainly reflects the clinical leadership
domain. This is commensurate with Elliott et al. (2013), who concluded that leadership
domains on system- and political level continue to be underestimated among NPs. Different
skills and attributes for the participants‟ current leadership role were highlighted. For
example, advanced medical knowledge was considered of great importance for leadership
development. There is overlap in knowledge and skills needed for effective leadership
development, i.e. developing skills and knowledge in the clinical leadership domain makes it
easier for NPs to be active at policy level (Hamric et al., 2014). Knowledge about health
policy and nursing practice was an important component for the range of skills of effective
nurse leaders (Antrobus & Kitson, 1999). However, the present study showed that NPs‟
contribution to high quality care is mainly based on an increase in the degree of responsibility
and accountability in clinical practice. Development of clinical skills during education and
the different legislation in workplace appeared to be main differences between NPs and
registered specialized nurses. Where registered specialized nurses enact their clinical
leadership role to exert direct influence on patients, family, and fellow healthcare providers
(Patrick, Laschinger, Wong, & Finegan, 2011), NPs are skilled in providing integrative care
in complex and dynamic environments (Lamb, Martin-Misener, Bryant-Lukosius, & Latimer,
2018). Since being competent depends on the capability to use a set of related knowledge,
skills and behavior (Epstein & Hundert, 2002), the question arises if participants possess the
required competencies to further develop their leadership role in different domains. Even
though all participants possessed advanced knowledge, well developed skills and several
useful attributes, most of them said they do not yet hold the desired role, and were anxious to

le their further position, including more intense collaboration with nurses and physicians and
being more involved in research.
5.1.2 Shaping the NP profession
Being a NP nowadays seems to automatically demand a proactive attitude where taking
initiative is important for role development (Mantzoukas & Watkinson, 2006). Nevertheless,
only few participants actually performed their desired work activities. These NPs were active
in research and executed many tasks on policy level. The concept of positive deviance may
be at play here, as it assumes that it is likely to have some individuals within a given
population who have developed solutions to a complex problem (LeMahieu, Nordstrum, &
Gale, 2017). How did these participants stray from the straight path and went looking for
workarounds? To overcome the obstacles that were in the way of shaping their own desired
profession, e.g. the need for high production rates in hospitals, they found new opportunities
at other places. The degree of self-efficacy seemed to play an important role: they were all
confident about their added value and could clearly describe their vision as NP. Scott and
Miles (2013) concluded that the level of confidence in knowledge, skills, and abilities is
related to leading others. They described this as leadership self-efficacy and thus, individuals
with high self-confidence probably have more belief in the ability to be successful and having
an internal locus of control.
„Claiming autonomy in clinical practice‟ was regularly referred to in the interviews. This
included being directly responsible for a patient‟s wellbeing. Attaining autonomy relies upon
the capability to make independent decisions, on possessing sufficient knowledge and on
having reflective capacity (Skår, 2008). In this regard, the participants mainly mentioned
autonomously prescribing medicine and autonomous decision-making, but it also appeared
that there were still many differences in the extent to which authorizations had been
conferred. Petersen, Keller, Way, and Borges (2015) concluded that institutional and legal
restrictions can negatively affect the level of autonomy in NP practice. Even in the
Netherlands, NPs have to clear hurdles in their institutions, despite development of legislation
and regulation at national level. One of the aims of the professional association for NPs is to
nationally represent all NPs for further development of the profession and its position in the
Dutch healthcare system.

le 5.1.3 Essential components for NP‟s practice
The overlap in scope of practice between NPs and physicians had at times created tensions
between the parties. The combination of nursing care and medical care may also provoke
difficulties in creating autonomous practice for NPs (Weiland, 2015). A possible explanation
is the still-present hierarchy in hospitals, where physicians are in a leading position and
nurses cannot succeed in creating a position of one‟s own (van Oostveen, Goedhart, Francke,
& Vermeulen, 2017).
According to the participants, workplace context and organization-level factors were of great
influence on NPs‟ task allocation. They indicated factors such as high productivity rates, time
shortage, lack of support at the organizational level or healthcare budget cuts. Some studies
recognized organizational-level barriers as the largest category of barriers to enacting
leadership and suggested that removing these barriers will enable NP‟s leadership potential
(Elliott, 2017; Higgins et al., 2014). Furthermore, lack of a shared vision between NPs and
the managerial level was a potential barrier, too. It is likely that NPs‟ expertise will be more
effective in organizations with a clear communication about their roles and responsibilities
(Brooten, Youngblut, Deosires, Singhala, & Guido-Sanz, 2012). In accordance with the
participants, more support and effort from management, other healthcare professionals and
other NPs could facilitate the reallocation of tasks (Carnwell & Daly, 2003; Zwijnenberg &
Bours, 2011).
Some of the participants had difficulty to position themselves without having colleagues to
support them, in line with findings by Cronenwett (2012) and Brooten et al. (2012). Besides,
Nursing Advisory Councils within the hospital did not seem eligible for representation of
NPs hospital-wide, according to some participants. Furthermore, participants indicated that a
general job description could do away with any ambiguity about the NP function. Leadership
ability of NPs is dependent on having a national job description in which the leadership
component is specified. Development of a standard with the involvement of all clinicians will
enhance role clarity and reduce resistance of the multidisciplinary team (Higgins et al., 2014).
All participants had experienced that others were confused about the NP profession in view
of the name of the function and the overlap in scope of practice with the physician assistant.
Consistency and understandability are necessary for developing the full potential of NPs
(Bryant-Lukosius, Dicenso, Browne & Pinelli, 2004; Hamric et al., 2014)

le 5.2 Study limitations
Some limitations of the study need to be addressed. First, NPs are a young professional group
with a great deal of working areas. This makes it difficult to generalize this sample to all NPs
in the country. However, the completion of the sample of fifteen NPs with three NPs that did
not meet the exact inclusion criteria with regard to work experience demonstrated that the
results found might be representative for a larger group of NP professionals. Moreover, the
aim of the study was to explore the experiences of a sample of NPs. Second, several
participants saw similarities between their current role as NP and former function as
specialized nurse, although they do feel more accountable and responsible as a NP. Perhaps
we should have focused on the participants‟ feelings or personal effectiveness rather than the
role description, since both NPs and registered specialized nurses in the Netherlands are
trained to seamlessly integrate the competencies of all seven CanMEDS roles (Lambregts &
Grotendorst, 2012; Ter Maten-Speksnijder, Grypdonck, Pool, Meurs, & van Staa, 2015).
Third, the purposive sampling method used in this study could have led to selection bias as
participants were selectively approached by the educational coordinators. Furthermore, all
participants had knowledge of and interest in the topic and this may indicate they were more
actively involved in the positioning of NPs than others. Nevertheless, judgements and
inclusion criteria were based on clear criteria. Last, the findings from this study are limited to
the perspectives of the NPs themselves. Inclusion of physicians and management could have
provided different views on the leadership execution.

le 6. Conclusion
This study on the perception of one‟s ability to enact leadership shows that an adequate use of
leadership is crucial for role development and positioning of NPs. Although differences in
tasks between NPs and registered nurses appeared to be clear, NPs mainly enact leadership
on clinical level. It is a major challenge for individual NPs, healthcare organizations and
educational institutions to create a full and unambiguous scope of expertise and practice. This
development may enable NPs to clearly show the added value of this profession in the
healthcare system.

le 7. Relevance to clinical practice
A profitable use of NPs‟ leadership competencies could lead to further role development and
a better positioning of NPs. Leadership in nursing is of great importance since it is directly
linked to quality of care, positive patient outcomes, cost-effectiveness and productivity in
healthcare settings (Australian College of Nursing, 2015; Wong, Cummings, & Ducharme,
2013). Universities could focus on increasing NP‟s perceived self-efficacy to be prepared for
the challenging role. For NPs themselves it is important to be involved in daily work
interventions on different organizational levels, like more involvement in policy-related
decisions or research activities (Heinen, van Oostveen, Peters, Vermeulen, & Huis, 2019). A
primary focus on medical practice will not improve further positioning of the profession
(Hamric et al., 2014). Moreover, physicians‟ recognitions about the NP profession is of great
importance and their demands should be in line with the NP‟s qualifications. Besides, more
support and effort from management is needed for further development and positioning of
NPs (Elliott, 2017; Higgins et al., 2014). For example, being encouraged to conduct research
or being involved in policy-related activities will help NPs to adequately establish their
position. Managers need to consider the importance of an established and common vision
with all staff members. Nurse associations can play a more active role in the clarification of
NP‟s scope of practice. Establishing NPs as an independent professional group and clearly
define work profiles will contribute to further positioning of the NP‟s profession. Further
research on the perceptions of all disciplines may help achieving this.
