Our trained technicians will help to establish healthy target ranges and develop healthy lifestyles that will work for you.

I have Type 2 diabetes why is cardiovascular disease risk reduction important to me?

High blood pressure, family history of heart disease, lack of exercise, fatty diet . . . are all signs that you are at risk for heart disease and stroke.

✔ Monthly clinic visits until risks are reduced by reaching target ranges for blood pres- sure, cholesterol and blood sugar. After reaching target, visits will be quarterly.

✔ A registered nurse will help manage your care, provide education and be available to answer your questions in person or by phone

✔ Closely monitor your blood pressure, blood sugar, weight and cholesterol

✔ Group and individual educa- tion classes on diabetes and cardiovascular disease (CVD) risk reduction

✔ Regular laboratory testing

✔ Family participation encouraged through community events

✔ Consultation with Exercise Program Manager and personal exercise plan development

✔ Consultation with a Registered Dietician and assistance with personal meal planning

• Patients who receive health care within the Choctaw Nation Health Care System

• Have a diagnosis of Type 2 Diabetes

• Must be at least 18 years old

• Pregnant

• On dialysis or in End Stage Renal Disease

• Actively using alcohol or drugs as deter- mined by a physician
