BGRR Event On 11-29-11, a subcontractor fell approximately 16 feet from a
scissor lift with a hinged guardrail system. The worker was participating in the process of torch cutting. The worker was leaning against the extended rear guardrail of the scissor lift platform when it folded outward into the down (retracted) position, allowing the worker to fall. The worker was hospitalized with multiple fractures

Fall from Height Issues Investigation of recent injury at BGRR shows:
· Complacency using a scissor-lift; other hazards at the job site drew safety awareness away from the hazards of scissor-lifts
· No pre-use inspection and no pre-use inspection checklist; work planning did not ensure a pre-use inspection
· Teamwork and communication were less than adequate; each thought the other did the inspection; no questioning attitude
What we can do:· Show concern for others – co-workers should question each other
(e.g., did you do the inspection, do you have the PPE…)· Ensure yourself that all hazard controls are implemented for the
job before beginning the work· Reduce stressors in the workplace (i.e., report suggested cost
cutting may have been a stressor)
