The Burlington School District offers all eligible employees the opportunity to participate in a
tax sheltereã retirement plan for the benefit of its employees, It is intended that the plan comply
with the requirements of and qualify under Section 403(bX7) of the Internal Revenue Code' The
plan includås Annuity Contraóts and Custodial Agreements offered by vendors on the district's
provider list (Appendi* e of a03@) Plan Adoption Agreement: Funding Vehicles). The fact that
à particular conlract or custodial account may be available from a provider, does not constitute
an endorsement, recommendation, or approval of any kind'
General Information:
Each employee of the district is given an opportunity to participate in the district's 403(b) Plan
by entering Into a 403(b) Plan Salary Reduction Agreement except as limited below:
(a) Employees whose salary reduction would not exceed $200 per year'

Enrollment Procedures :
To participate in the 403(b) Plan, or to change salary reduction amounts from one provider on

At the beginning of each new Plan Yearwe need to have current documentation from your Doctor
or physicar rherapist stating your condition and treatment for the condition erigible for
reimbursement, e.g., swimming 3 times a week due to a specified condition' Expenses for your
generalwell being, such as "swimming is a good exercise, you would feel better if you swam three
times a week," would not be an eligible expense'
over-the-counter {oTc} prescriptions need to be updated each year as well'
Therapeutic massage must be a treatment for a medical condition, as with the swimming
example,ifthemassageisforyourgeneralwellbeing,itwouldnotbeaneligibleexpense'Proofof massage expenses must include a receipt or invoice that indicates the masseuse/masseur is
