Course at a Glance

Course Description

The focus of this course is on the application of advanced organizational principles in complex healthcare environments.

Organizational issues, administrative processes and applications are explored. The managerial perspectives of a mid-to

senior healthcare administrator are emphasized.

Course Design

Effective health care organizational management in a very complex environment is the “heart and soul” of this course.

The course is designed to provide opportunities and tools to equip participants to assume health administrator roles within

any healthcare organization. Participants will analyze current healthcare dilemmas and develop insights into the

organizational challenges facing administrators today –regardless of healthcare setting.

This course progressively builds up to provide students the ability to complete a project, covering basic health care

organizational principles, strategic planning, financial planning, human resource management, effective leadership, and

effective communication. Much of the learning will come from a series of discussion questions, short papers, and realistic

case studies.

Week One: Examines and evaluates principles of healthcare administration in complex situations. Students will be asked

to evaluate modern management theories that can be applied to their own work environment by means of two Discussion

Questions.

Page 5: Mha601.online.sg.2011.v1.1025.mn

Week Two: Examines and evaluates the concept of power, politics, and conflict within a health care organization. One

discussion question asks the students to evaluate power and political games in their own organization. For the second

discussion question, the students are asked to recommend a course of action from the point of view of conflict

management and interpersonal relationships. The written assignment for the week deals with conflict and teamwork

within a hospital department.

Week Three: Introduces, examines, and evaluates the principles of leadership and decision making within the health care

environment. In one discussion question, students will be asked to identify the most important factors concerning

effective leadership and then evaluate and recommend leadership qualities within their own organization. A second

discussion question deals with effective decision-making. A third discussion questions requires the students to review a

Case Study focusing on decision making and communication. The written assignment asks the students for a reflective

paper on their own personal leadership philosophy.

Week Four: Introduces and examines the concepts of values, ethics, and organizational dysfunction. For the first

discussion question, students are asked to view a slide presentation and to read and respond to a case study dealing with

racism rules. The second discussion question deals with organizational dysfunction. For the written exercise, the students

will reflect on and write about various stakeholders’ views of the uninsured.

Week Five: Focuses on change, human resources, and the development of the most effective management team. The

students, for the first discussion question, create their own thoughts on "whitewater changes" occurring in healthcare

today. The second discussion question deals with human resources and the effects of poor communication. For the third

discussion question, the students are asked to review a video focusing on the theme, “Why do some succeed where others

fail? What makes high-achievers different?”

Week Six: Concludes the course by focusing on the medical staff and the board of directors as well as the final paper.

The first discussion questions asks students to evaluate challenges faced by medical staff leadership, while the second

discussion question requires the students to review a case study dealing with the failure of the board of directors to act

effectively. Finally, there is a summative assignment allowing the students to utilize their reading and learning from the

six weeks of class to create a solution to a problem of an exodus of nursing personal within a health care organization.

Components of Course Evaluation

Multiple measures of evaluation are used in the course, allowing students opportunities to demonstrate their learning in

more than one way, and giving consideration to individual learning styles. Course components that will be evaluated

include:

Discussions

Each week students will participate in online discussions with classmates, which are related to the week’s readings. These

discussions replace the interactive dialogue that occurs in the traditional classroom setting. Each week, students’ initial

discussion posts are due by 11:59 p.m. (of the time zone in which each student resides) on Day 3 (Thursday). Students

will have until 11:59 p.m. on Day 7 (the following Monday) to make the required minimum number of response posts to

classmates. Discussions represent 46% of the overall course grade.

Assignments

Written Assignments

There is a written assignment due in weeks one through five of this course. These assignments must reflect college-level

writing and critical thinking. Assignments represent 29% of the overall course grade.

Final Paper

The final assignment for this course is a Final Paper. The purpose of the Final Paper is for you to culminate the learning

achieved in the course by creating a proposal for the Renfrey Memorial Hospital Board of Directors. The Final Project

represents 25% of the overall course grade.

Focus of the Final Paper

Renfrey Memorial Hospital (RMH) is a 200 bed regional hospital located in the Midwest. It has been in existence for 32

years and was named after a prominent philanthropist in the area, Mr. Gilbert H. Renfrey, who made many financial

contributions to projects in and around the community where Renfrey Memorial Hospital is located. The hospital

maintains the only 24 hour emergency department in the area, as well as an "extended hours" urgent care clinic. The

hospital is encountering a problem with nursing staff retention. Sixty-eight percent of the nursing staff is over the age of

Page 11: Mha601.online.sg.2011.v1.1025.mn

MHA601: Principles of Health Care Administration

Online

11

45, and facing retirement. The hospital commissioned a survey that revealed many of the nursing staff finds the work too

physically demanding and many are feeling physically exhausted and report emotional burn out as well. Several of the

survey respondents report they are stressed to the breaking point caring for aging parents in addition to working at

Renfrey. In the past two years, the hospital has seen an exodus of nurses. The hospital has been forced to use temporary

pool nurses from an agency. This has driven up personnel costs and also resulted in decreased rankings on patient

satisfaction surveys. In addition, the nursing shortage is making it difficult to recruit replacement nurses for the staff who

plans on retiring within the next couple of years. Renfrey Memorial Hospital faces significant staffing challenges ahead

as it grapples with these issues and the hospital board is very concerned. They know there must be some changes made in

order to prevent a major financial and human resources crisis in the future.

For your final course project, you will create a 10 -15 page proposal for the Renfrey Memorial Hospital Board of

Directors. In your proposal, you will:

Include an executive summary

Research and identify solutions implemented at five other hospitals in the US that were dealing with these same

issues (benchmarking).

Based on your research, identify two solutions that are the most viable for Renfrey Memorial Hospital to

implement within the next two years.

Present an analysis of the two solutions that you have selected in terms of at least 5 stakeholder groups involved

(e.g. patients, hospital administration, accounting, H.R., marketing, third-party payers, etc.) Who is impacted?

What change processes may be required? What fiscal impact would occur?

Make a recommendation with detailed justification and research support, on the single solution that you feel is

best for RMH. Explain why you feel it is the best and most viable solution given the financial impact, HR issues,

interpersonal dynamics of hospital personnel, cultural shift, and change management.

Utilize 10 -12 reference sources written within the last 5 years to prepare your board proposal.

The Final Paper:

1. Must be 10 to 15 double-spaced pages in length, and formatted according to APA style as outlined in the Ashford

Writing Center.

Discussions

Participate in the following discussions:

1. 1st Posting Due by Day 3. Cultural Values and Ethics. Review the Ethical Culture slide presentation and read

the case starting on page 251 of the course text. Post responses to following questions:

How do culture, values, and ethics enter into this case study?

Reflect on the term used in the slide presentation, “Ethical Wisdom.” How might the use of ethical

wisdom assist in this case study?

If you were in a leadership role of this health care organization, how would you have handled this

situation compared to the way the hospital’s president?

Your combined responses to all three questions should be at least three paragraphs. Respond to at least two of

your classmates' postings.

company around. He was eventually fired by the board having cut thousands of jobs and reducing the company to

practically a skeleton. Describe some of the factors that might lead a health care organization toward, as Graber

suggests in your text, “a neurotic style” (pg 286). Also, describe how health care organizations should avoid such

actions. Clarify/explain why you selected these. Respond to at least two of your classmates’ postings.

3. 1st Posting Due by Day 3. Abuses and Unethical Behavior in Health Care. [Watch Bad Medicine: The Glaxo

Case. Drugs provide society with enormous benefits: they reduce mortality and morbidity, relieve pain and

suffering, are less expensive forms of treatment than surgery and hospitalization, and are more readily accessible

to a larger portion of the population than are more expensive technologies and have enabled physicians to see

more patients with improved treatment outcomes. The U.S. spends over $300 billion drugs annually, and we

expect safe and effective drugs. Yet society seems to have a special place for the U.S. Drug Industry, because

they do produce these life savings/sustaining products. Yet within the past few years, issues have been raised that

question some of these company’s ethics and the safety of some of these drugs. Question: Should we treat

abuses/unethical behavior in the U.S. drug industry and our entire health care industry differently than we do in

the banking industry or auto industry? Why or why not? Please respond to at least two of your classmates’

postings.
