One out of every four Canadians is speculated to be in the early stages of developing metabolic syndrome, pre-diabetes, or type 2 diabetes.
The conventional approach is diagnosis-driven and until a precise diagnostic code is given to the patient, there is no treatment plan.
In the functional medicine or naturopathic medicine approach, the “underlying dysfunction” or ‘tolle causum” (causes) are identifi ed and a multi-pronged treatment approach is initiated.
Metabolic syndrome
was fi rst termed “Syndrome X” by Dr. Gerald Reaven, MD, a Stanford University endocrinologist more than 30 years ago.
Simply put, it is a disorder of human physiology that if untreated, eventually leads to type 2 diabetes, cardiovascular disease and atherosclerosis.
In 2002 (JAMA vol; 287:356-359), the defi nition was further clarifi ed that for a person to be diagnosed with metabolic syndrome, they had to meet three of the following criteria: triglycerides greater than 1.7, HDL cholesterol less than
1.03 in men and less than 1.3 in women, waist to hip ratio greater than 1, fasting blood sugar greater than 6.1 and blood pressure greater than 130/85.
Then in 2004 a follow up article in JAMA (vol; 292:2823-2824) added two more criteria: increased clotting tendency and low magnesium.
Normal insulin metabolism regulates the production and storage of sugar (glycogen) in the liver, it is anti-infl ammatory and stimulates the production of nitric oxide which relaxes blood vessels and lowers blood pressure.
The mechanism of metabolic syndrome starts with insulin resistance when normal insulin pathways are disrupted and more fat gets stored inside the cell instead of being stored as glycogen.
As fat builds up inside the cell, the mitochondria (the energy producer of all cells) becomes less effi cient. This results in the activation of infl ammatory
pathways and free radical production and increased insulin resistance.
This now becomes a vicious cycle.
The long term consequences of metabolic syndrome and type 2 diabetes are signifi cant and include among others: chronic infection, poor wound healing, neuropathy, non-alcoholic fatty liver disease, kidney disease, cataracts, retinopathy, sleep apnea and autoimmune disorders.
For this reason it is important to prevent and treat the earliest signs and symptoms of this metabolic syndrome.
Most are aware that dietary polyunsaturated fats are prone to becoming rancid as a result of oxidation. It is also important to understand that glucose itself oxidizes during some cooking processes creating clumps with proteins called AGE’s (advanced glycosylation end-
byproducts). For example
cooking foods with carbohydrate and/or protein will develop AGE’s when cooked at high temperatures.
Barbecuing meat with sugary sauces at high temperatures,
fl aming a crème brulée and sticky buns are all sources of AGE’s.
When these are consumed, even by a person with normal blood
sugar, infl ammatory pathways are up-regulated causing insulin to become more resistant and you have the beginnings of metabolic syndrome.
Similarly, a diet high in refi ned carbohydrates activates the same pathways.
Clearly, the conventional approach of prescription drugs is simply not effective enough, or we wouldn’t have such an escalating problem.
The good news is that lifestyle, dietary, and nutraceutical support strategies offer many advantages for the prevention and treatment of metabolic syndrome and type 2 diabetes.
A study in JAMA 2004; 292:2823–2824, it was shown than
lifestyle intervention reduced the incidence of type 2 diabetes by 58 per cent whereas Metformin reduced the incidence by 31 per cent.
This means that if a patient, average age 51, has a HgA1c (average blood sugar over 3 months) of 6.8 (not diabetic yet) and is prescribed Metformin, that same patient has the choice to exercise 150 minutes per week,(or 21 minutes per day) in order to increase muscle mass and lose seven per cent of their weight over 2.8 years and have a 27 per cent better outcome than if they had just taken the Metformin alone.
There is certainly a place for the use of Metformin as a stop gap measure or in the lowest dosage possible to control blood sugars.
The goal is to avoid prescription medications if at all possible.
In a future article I will review a naturopathic treatment program used with a patient to lower the risk factors for metabolic syndrome.

Curator Jane Hutton says a good year fi nancially is
making it possible to improve some permanent displays at the museum.
First a small tool shed structure will be
added to the existing settler’s cabin front to display tools.
“We have lots of old tools and someone just
gave us some trapper’s equipment,” said Hutton.
“We are also working on a little
theatre,” said Hutton. The Capitol Theatre in Port Alberni is replacing their old seats.
“I have my name on a little set of four,” said Hutton. “We show videos here all the time, particularly in the summer time, so I want to get a fl at screen TV and put in the seats and pretty it up a bit.”
The plans are possible because of revenue from the gift shop.
“I am very grateful
to the community for their support of the gift shop,” said Hutton. The gift shop features local jewelry, giftware and books.
Also in the works is a temporary display of fossils from the area. A local expert is assisting Hutton in identifying the fossils already at the museum and gathering and identifying those on loan from the community.
The display is scheduled to open in April.

BLACK CREEK — A long and winding driveway off Catherwood Road leads to the home of Father Charles Brandt, a priest of the Roman Catholic Church who lives a solitary life on a 30-acre spread next to the Oyster River.

followed him from the river. Brandt recalls it had been “absolutely quiet” that day while he sat on a bench above the river.
“I couldn’t see him,” he said of the cat. “I heard these robins excitedly calling. I looked over my shoulder, and couldn’t see anything. I walk into the house and closed the door. Then I looked out. At fi rst I thought it was a deer — that’s the path they come up — but there was a cougar. And it stayed around for a while.”
Using a digital Canon Mark 3 with 200-mm lens, he snapped a picture of the approximately 15-foot cougar, which promptly fell asleep in the shade underneath a tree at the bottom of the backstairs.
“There’s no vibration, so I can get pretty good shots,” said
Brandt, whose face and physique bely his age. “I always have a camera in the van, so if I see something I photograph it.”
Brandt started out by using a “simple camera” when he took a photography course at Cornell University in New York, from where he obtained a bachelor of science in ornithology. But he is mostly a self-taught photographer, as was his father.
“It’s an art form, I think. It’s interesting.”
Brandt became an islander in 1965. A year later, he was ordained to the Catholic priesthood at the Canadian Martyrs Church in Courtenay, which is now a theatre. He was mandated to live the life of a hermit priest.
“Which is pretty unusual,” he said. “I came from the Trappist Monks, that’s a Benedictine order. Everybody knew
about the hermits on Vancouver Island from all over the world. It was a group that wanted more solitude that you couldn’t fi nd in the big order...I came from New Melleray Abbey (Iowa) to join the hermitage on the Tsolum River — when the mine was going in on Mount Washington.”
As a member of the Hermits of St. John the Baptist, Father Charles fi rst lived in a cabin he constructed near Headquarters Creek in the Tsolum River watershed before moving his hermitage to the banks of the Oyster River. Prior to moving to Canada, Brandt had served as a navigator with the U.S. air force during the 1940s. He later graduated with the aforementioned BSc. and a bachelor of divinity from Nashotah House, a theological seminary in Wisconsin.
“I’m not just a hippie copping out of society,” he said with a chuckle.
Every couple of years or so he will pack his van and journey south of the border to visit family and friends.
Besides his skill behind the lens, Brandt is also gifted with the pen. His published books include Meditations From the Wilderness and Self and the Environment.
He is also the subject of a chapter entitled A Hermit of the Rivers, which appears in the Stephen Hume book A Walk with the Rainy Sisters.
Hume, a writer for the Vancouver Sun, was among a crowd
that squeezed into a Campbell River church in 2007 to celebrate Brandt’s 40th anniversary as a hermit priest. Hume writes: “Brandt represents an ancient tradition of wise men and women withdrawing from the world, the better to refl ect upon how best to serve God.”
