    T h e H o u r s (2002) Nicole Kidman, Julianne Moore, Ed Harris (dir.Stephen Daldry). Story of three women who are profoundly affected byVirginia Wolfs novel, Mrs.Dalloway. This excellent film has, as itsundercurrent, the reasons for a suicide, an attempted suicide, and a

    rational suicide. Kidman won an Oscar for her portrayal of Virginia Wolfwho drowns herself to escape advancing madness. Moore is the cityhousewife, bored and confused, who nearly commits suicide. Harris playsthe over-the-hill New York poet with advanced AIDS who can no longerbear to live and allows himself to fall to his death out of a window.Screenplay by David Hare from the novel by Michael Cunningham. It helpsto have read the Pulitzer Prize novel first.

    T h e Ev e n t (2003) Parker Posey, Olympia Dukakis, Sarah Polley (Dir.Thom Fitzgerald). An intense relationship drama that takes the form of amystery, The Event centers around a series of unexplained deaths that

    occur among the gay community in New Yorks fashionable Chelseadistrict.     New Film: "A Good Death"

    The film A Good Death is for patients with COPD or other serious lungconditions, their families, and the doctors and nurses who provide care forthem. It is not an easy film to watch, because it describes the struggles ofa real patient, Mr. Martin Cavanagh, and his family during the last monthsof his life. If you are living alone, it may be best to invite a family memberor friend to watch it with you.

    The film is designed to introduce the concept of ADVANCE CAREPLANNING. It is meant to prompt meaningful discussion and helpfulplanning for future care for patients whose respiratory illness is leadingthem to the end of life. It is about making thoughtful choices. If you are apatient, you may find this challenging or even upsetting at first. Butexperience tells us that these understandable reactions give way togenuine appreciation and satisfaction because sensitive issues have beenbrought out into the open and addressed.

    Professor Robin Taylor wants to change the way that we care for peoplewith long-term, but inevitably fatal medical conditions as they reach theend of their lives. Instead of writing another academic paper, he decidedto take a completely different tack and commission a film about theissues.

    Im a respiratory specialist and I realized that the care we were providing

    to patients with chronic respiratory disease at the end of their lives wasoften missing the mark.

    For more and more of us the end of life is shaped by chronic organ failure;the heart, the kidneys, or the lungs may gradually pack up. The journeyoften involves declining health over many weeks or months, and duringthat time the individual concerned and their families often struggle tocome to terms with what lies ahead death.

    The diagnosis of dying is often avoided by health care professionals asmuch as by the patient, and this may mean that treatment, especially ifthe patient arrives in hospital severely ill, may be inappropriate. Medicalinterventions motivated by a we can fix it mentality, may now be futileand simply prolong the discomfort rather than relieve it.

    Two families recently expressed their appreciation, but also their

    frustration at the way their loved ones had been looked after in DunedinHospital. The problem was that our efforts to help had prolonged theirdeaths rather than prolonged their lives. The patients families felt thatwhat was done was caring, but nevertheless inappropriate. We had beeninsensitive to what was really needed: an understanding of their situationand help in coming to terms with it. In response to all of this, we havebegun to change the way we do things and are now much more attentiveto the issue of dying well.

    Dying well is not euthanasia. It involves pro-active, supportive care andpalliative treatment. It involves being honest and having open discussions

    about dying sooner rather than later. The aim is to address uncertaintiesand relieve deep anxieties and make the end of life experience easier foreveryone involved.

    Ive spent a substantial part of my career writing papers for journalsread by specific sectors of the medical establishment, but this time Iwanted to reach beyond my own specialty. I wanted to influence thethinking of my colleagues in other disciplines and to change the perceptionof the general public as well. To do that I realized would need a differentapproach so I asked Dr. Paul Trotman, a doctor and filmmaker, to make afilm with me, one looking at how people are cared for as they are dyingfrom chronic respiratory disease.

    This film is the result. It follows the experience of Martin Cavanagh, apatient with severe emphysema, and his family during the last year of hislife. It provides a close-up look at the struggles that they went throughand explores the issues that needed to be reconciled in someone with aterminal illness which is not cancer.

    Its a poignant film, but not a depressing one. It deals with the concept ofadvanced care planning and challenges the health care model with whichwe currently approach end-of-life care. It encourages meaningfulresponses from patients, family members, doctors and nurses in the way

    they handle end-of-life suffering. Although Martin died from emphysema,his story is equally relevant to other common medical conditions.
