Inhalatiesteroïden zijn mogelijks een faciliterende factor voor het ontstaan van ABPA gezien
er significant meer gebruik is van inhalatiesteroïden zowel in de volledige groep patiënten als
in de groep chronisch gekoloniseerd met P. Aeruginosa, subgroep B, in het jaar voor de
diagnose van ABPA (indexjaar).

CFRD is potentieel een faciliterende factor van ABPA. We zien meer patiënten op het
indexjaar met CFRD voor de patiënten chronisch gekoloniseerd met P. Aeruginosa.

Als laatste parameter wordt gekeken naar de relatie tussen de BMI en het ontwikkelen van
ABPA.
Er is een lagere BMI-waarde in de ABPA-groep (groep A) op het indexjaar en dit vooral bij
volwassenen (P-waarde 0.05); dit is bij de P. Aeruginosa gekoloniseerde groep (subgroep B)
niet aanwezig (P-waarde 0.32).

4. Discussie
ABPA is een frequent voorkomende complicatie bij mucoviscidosepatiënten. Het is vaak
moeilijk om de diagnose te stellen, gezien veel symptomen samenvallen met de symptomen
van een infectieuze exacerbatie bij mucoviscidose. De ontwikkeling van ABPA is ook
afhankelijk van diverse factoren zoals P. Aeruginosa kolonisatie en luchtwegbeschadiging. Er
is eveneens nog geen duidelijk verband tussen A.fumigatus sensitisatie en het ontwikkelen van
ABPA.(17)
We hebben geprobeerd om het verband aan te tonen tussen ABPA en de achteruitgang van
longfunctie, intraveneus antibioticagebruik, CFRD, het gebruik van inhalatiesteroïden en de
nutritionele status (BMI). Deze parameters zijn retrospectief over een lange periode voor en
na het ontstaan van ABPA bekeken.
Gezien het om een retrospectieve studie gaat zijn, is er gevaar voor bias. De recall bias werd
geminimaliseerd doordat de gegevens gehaald werden uit het BMR, dat jaarlijks de gegevens
anoniem bijwerkt. Gezien de grote studiegroep was het ook mogelijk om verbanden af te
leiden.
We zien dat patiënten met de DF508 mutatie meer vertegenwoordigd zijn in de ABPA-groep;
deze zijn niet meer vertegenwoordigd in de groep chronisch gekoloniseerd met P. Aeruginosa.
Dit wijst, zoals eerder aangetoond, dat bepaalde mutaties een grotere voorbeschiktheid hebben
voor ABPA en P. Aeruginosa kolonisatie (15, 16). Verder onderzoek hieromtrent lag niet
binnen de doelstellingen van deze studie.
Als er gekeken wordt naar de longfunctiewaarden 1 jaar vóór het indexjaar, zien we in de
ABPA-groep (groep A) een lagere FEV1- en FVC-waarde dan bij de controlegroep (P-waarde
<0.001). In de groep gekoloniseerd met P. Aeruginosa, subgroep B, valt dit verschil volledig
weg. Dit kan verklaard worden doordat de patiënten gekoloniseerd met P. Aeruginosa meer
longschade hebben. Als de cumulatieve waarden voor het indexjaar bekeken worden zien we
dat de ABPA-groep, groep A, meer longschade heeft met significant lagere FEV1- en FVC-
waarden (P-waarde <0.0001). Hieruit kan besloten worden dat bronchiale schade met lagere
FEV1- en FVC-waarden vermoedelijk een faciliterende factor is voor het ontwikkelen van
ABPA (in patiënten niet gekoloniseerd met P. Aeruginosa). Als de cumulatieve waarden na
het indexjaar bekeken worden zien we een significant verschil in FEV1-waarden voor de
ABPA-groep, groep A, (P-waarde <0.01) en de groep chronisch gekoloniseerd met P.
Aeruginosa, subgroep B (P-waarde <0.03). Een eventuele verklaring is dat patiënten met P.

Aeruginosa al meer longbeschadiging hebben, waardoor er voor de diagnose van ABPA geen
significant verschil is in longfunctie met de controlegroep. Dit verschil treedt pas op na het
doormaken van ABPA(15). Een argument dat aantoont dat ABPA meestal optreedt in longen
die meer beschadigd zijn. Dit past bij de pathofysiologie van ABPA, gezien A. fumigatus
kolonisatie ook sneller plaatsvindt in beschadigde luchtwegen.(4) Als er gekeken wordt naar
het FEV1-verlies over de volledige studieperiode en de volledige ABPA-groep zien we vooral
een statistische significantie voor de diagnose van ABPA (P-waarde <0.0005) en over de
volledige studieperiode (P-waarde <0.02). Dat er geen significantie is na de diagnose van
ABPA kan verklaard worden doordat de ABPA-patiënten meer behandelingen krijgen en
hiermee hun longfunctie op een bepaald niveau houden of doordat de niet-ABPA-patiënten
ook ouder worden en daardoor meer longschade oplopen die gepaard gaat met verlies van
FEV1.
Als er naar de nood aan IV antibiotica en het aantal dagen hospitalisatie gekeken wordt,
zien we dat er na een ABPA-exacerbatie significant meer nood is aan IV antibiotica en
opnames in het ziekenhuis, dit zowel voor de volledige ABPA-groep (groep A) (P-waarde
<0.0001) als de groep gekoloniseerd met P. Aeruginosa (subgroep B) (P-waarde <0.05). Dit
kan verklaard worden door: de behandeling van ABPA bestaat uit systemische steroïden, en
toegenomen longbeschadiging, een faciliterende factor voor ABPA, leidt tot meer opnames
met nood aan IV antibiotica.(19). Dit verschil in nood aan IV antibiotica en
ziekenhuisopnames valt weg voor de subgroep B, chronisch gekoloniseerd met P. Aeruginosa,
vóór de diagnose van ABPA. Deze groep heeft door hun chronische kolonisatie meer nood
aan IV antibiotica. Men moet er op bedacht zijn dat continue blootstelling aan antibiotica
kolonisatie met gisten en schimmels kan bevorderen (12). Als we de 2 groepen (ABPA en
controlegroep) apart vergelijken, zien we dat er in de controlegroep significant meer
antibiotica gebruikt wordt na het indexjaar, wat niet significant is in de ABPA-groep. Dit kan
verklaard worden doordat, naarmate de patiënt ouder wordt, er meer longbeschadiging is, wat
leidt tot meer nood aan IV antibiotica. Een tweede verklaring is ook dat de ABPA groep reeds
voor het indexjaar meer nood had aan IV antibiotica.
Inhalatiesteroïden zijn mogelijks ook een faciliterende factor voor het ontwikkelen van
ABPA, gezien er significant meer inhalatiesteroïden gebruikt worden bij de volledige ABPA-
groep, groep A, en de groep chronisch gekoloniseerd met P. Aeruginosa (subgroep B) in het
jaar voor het ontwikkelen van ABPA (P-waarde <0.0001). Dit kan verklaard worden doordat

inhalatiesteroïden een kolonisatie met A. fumigatus kunnen bevorderen, wat verder op een nog
onbekende manier leidt tot de ontwikkeling van ABPA (13).
Er wordt gezien dat er een interactie is tussen CFRD en ABPA, die zou leiden tot meer
respiratoire exacerbaties (19). In onze populatie is er enkel meer CFRD in de groep
gekoloniseerd met P. Aeruginosa, (P-waarde 0.07). Een mogelijkse verklaring hiervoor is dat
kolonisatie met P. Aeruginosa, een bevorderende factor is voor het ontwikkelen van CFRD.
We tonen ook aan dat patiënten met ABPA een lagere BMI-waarde hebben op moment van
de diagnose van ABPA, zowel voor kinderen als volwassenen. Dit doet vermoeden dat een
slechte nutritionele status, met de daaraan gekoppelde lagere longfunctiewaarden, eveneens
een bijdrage heeft in het ontwikkelen van ABPA. Het feit dat er geen statistische significantie
bekomen wordt in de groep die chronisch gekoloniseerd is met P. Aeruginosa, kan eventueel
verklaard worden doordat deze patiënten een slechtere voedingstoestand hebben door hun
chronische kolonisatie.
5. Besluit
ABPA is één van de meest voorkomende complicaties bij mucoviscidosepatiënten boven de
6-jaar. ABPA heeft een hogere prevalentie in mucoviscidose patiënten met een DF508
mutatie.
Lagere longfunctiewaarden, meer nood aan IV antibiotica en gebruik van inhalatiesteroïden
zijn mogelijks faciliterende factoren voor het ontwikkelen van ABPA. Het longfunctie verlies
stabiliseert wel na het doormaken van een ABPA infectie. Een slechte nutritionele status heeft
ook een bijdrage tot het ontwikkelen van ABPA.
We concluderen dat ABPA een identiteit blijft met een complexe pathogenese, die
waarschijnlijk beïnvloedt wordt door veel factoren: klinische, genetische en nutritionele.

Allergic bronchopulmonary fibrosis (ABPA) is a TH2-mediated hypersensitivity reaction of the airways in response to Aspergillus fumigatus. Sensitization to A. fumigatus is common in cystic fibrosis (CF) patients. ABPA, a respiratory complication seen in +10% of the CF patients, is characterized by acute or subacute clinical symptoms (wheezing, pulmonary infiltrates, bronchiectasis). There is no specific laboratory test to establish the diagnosis of ABPA, the complication is suspected on clinical and immunological criteria: increased IgE, eosinophilia, positive specific IgE or IgG antibodies against A. fumigatus or a positive skintest. The mainstay treatment for ABPA consists of glucocorticoids and antifungal agents (itraconazole). The treatment aims to control the acute inflammation and limit progressive lung injury. Newer therapies are now being studied such as voriconazole and omalizumab. We present a case control study relying on data from the Belgian CF register. One hundred and fifty five ABPA patients, matched for age, gender and P. aeruginosa colonisation were compared to 356 controls. Patients were studied over a 10 year period before and after the index year (year of ABPA diagnosis). The following data were compared: FEV1, FVC, infectious exacerbations needing in-hospital treatment with IV antibiotics and BMI (body mass index). The data were statistically analyzed by the wilcoxon (rank sum) test. Mean FVC and FEV1 values are significantly lower in the ABPA group before and after the index year (p-value <0.05). Over a period of ten years before the index year there is no significant difference in FEV1 decline between the ABPA group and control group. After an ABPA infection the decline in FEV1 over a period of ten years is significantly steeper compared to the control group. ABPA patients are more prone to receive in-hospital IV antibiotics both before and after the index year (p-value <0.001). The BMI of patients with ABPA is significantly lower than the control group before and after the index year (p-value <0.001). These results suggest that ABPA is a major contributing factor to a steeper decline in lung function values over a period of 10 years after the index year. Lower lung function values, higher need of IV antibiotic treatment and a low BMI seem to be facilitating factors to develop ABPA. After ABPA the higher need of in-hospital days for IV antibiotics and a lower BMI remained.

Results
The objective of the study was to discover facilitators and consequences of ABPA. Lung function (FEV1, FVC), need of IV antibiotics before and after the diagnosis of ABPA were compared.
Methods
We present a retrospective case control study relying on data from the Belgian CF register (registry year 2000-2010). Each CF patient with ABPA (133 cases) was matched for age, gender and pseudomonas aeruginosacolonization with 2 controls. Patients were studied over a 10 year period before and after the index year (yearof ABPA diagnosis). The following data were compared: FEV1, FVC, infectious exacerbations needing in-hospital treatment with IV antibiotics. The data were statistically analyzed using the signed rank test or thewilcoxon rank sum test. The slopes of decline of FEV1 were analyzed by the ANACOVA model.
The prevalence of ABPA is significantly higher in homozygous deltaF508 patients (p<0.007). Mean FEV1values are significantly lower in the ABPA group on (table 1), before (table 2) and after (table 2) the indexyear. ABPA patients are more prone to receive in-hospital IV antibiotics both before and after the index year.Over a period of ten years before the index year there is a significant steeper FEV1 decline in the ABPAgroup compared to control patients. After the ABPA index year the decline in FEV1 is comparable to thecontrol group. The ABPA group has a steeper decline of FEV1 over de whole study period (table 4).

ABPA in Pseudomonas aeruginosa colonized CF patients.

Allergic Bronchopulmonary Aspergillosis syndrome (ABPAs) is a complication occurring in 10 to 15% of the CF patients. In case of late diagnosis bronchial damage can be considerably. A high degree of suspicion is mandatory and indicators of ABPAs must be followed carefully. We intend to study facilitators and consequences of ABPAs in Pseudomonas aeruginosa colonized CF
patients.
Methods: We performed a case control study relying on data from the Belgian CF register. Sixty four ABPAs patients matched for age, gender and P. aeruginosa colonisation were compared to 128 controls. FEV1, FVC, infectious exacerbations and BMI were studied over a period of 10 years before and 10 years after the index year. Results On the index year no differences were found for BMI, FVC and FEV1. Patients with ABPAs have significantly a higher prevalence of CFRD and significantly more were on treatment with inhaled corticosteroids, RhDnase, Azithromycine and inhaled Tobramycine. Cumulative values (FEV1, FVC, days of IV antibiotics) over the 10 years period before the index year were similar, over the 10 years period after the index year significantly lower values for FEV1 (p<0.03) were found in the ABPAs group. Before the index year no difference in IV antibiotic treatment days was recorded, after the index year patients with ABPAs showed significantly more treatment days (0.01). In ABPAs patients significantly more IV antibiotic days were found after the index year compared to before (p<0.001) which was not the case in the control group (0.56). Significantly more patients with ABPAs had at least one positive culture for A.fumigatus (p<0.009). Conclusion In chronic Pseudomonas colonized CF patients lower lung function values (c.q. more severe lung damage) are probably not a facilitating factor to develop ABPAs. The use of inhaled corticosteroids, RhDnase, Azithromycine and inhaled Tobramycine on the index day was significantly higher in the ABPA group suggesting more severe bronchial inflammation. Probably chronic use of inhaled corticosteroids, as an immunosuppressant drug, is a facilitating factor ABPAs patients needed significantly more antibiotic treatment days compared to controls and in contrast with controls more after the index year than before suggesting a higher number of infectious exacerbations.
