• Chief complaint: เจบ็สะโพกขวาทันที 5 ชัว่โมง• Present illness: เวลา 16:00 น. ขณะผู้ป่วยกำาลัง

ปีนขึ้นไปเปล่ียนหลอดไฟในหอ้ง ถกูไฟดดู แล้ว ตกลงมาสะโพกขวากระแทกพื้น ปวดมากทันที ลกุ

เดินไมไ่หว ไมม่ขีาขวาผิดรูป ไมม่ปีวดขอ้อ่ืน

Primary survey

• A: can speak, not tender along c-spine• B: normal breath sound, CCT;neg• C: BP stable, no external bleeding• D: E4V5M6 , pupil 3 mm RTLBE• E: no deformities, tender at lateral side of Rt.

tight, limit ROM active and passive due to pain, neurovascular; intact, no wound

Secondary Survey

• A: ยาแก้อักเสบไมท่ราบชนิด• M: none• P: U/D Hypertension (poor compliance)• L: last meal 15.00 pm• E: ถกูไฟดดู แล้วตกลงมาบนพื้นในหอ้ง สะโพกขวา

กระแทกพื้น ปวดทันที ลกุเดินไมไ่ด้ ไมม่บีาดแผล ภายนอก ไมม่ขีาผิดรูป

Classification• Stable : intact posteromedial cortex : will resist medial compressive loads once reduced• Unstable : comminution of the posteromedial

cortex: fracture will collapse into varus and

retroversion when loaded

Treatment

Non operative early out of bed!!• nonambulatory patients• patients at high risk for

perioperative mortality

!! high rates of pneumonia, UTI,DVT
