1 Intended use
Enzyme immunoassay for the detection of IgG antibodies to Toxoplasma gondii in human serum or plasma. The kit is intended for professional use. For “in vitro” diagnostic use only.
2 Introduction
Toxoplasmosis is a widespread parasitic disease caused by protozoan Toxoplasma gondii – a parasite with a complicated life cycle consisting of several morphologically different stadia. Primary hosts are members of the feline family. Humans and most warm-blooded animals can be infected by either primarily infected food (insufficiently heat-treated meat) or by ingestion of oocysts (secondary contaminated food or contaminated fingers, objects, etc.).
Acquired toxoplasmosis in immunocompetent individuals is usually asymptomatic or can manifest itself with flu-like symptoms (subfebrility, fatigue, lymphadenopathy, muscle aches), but without lasting ill effects. Severe life-threatening infections (encephalitis, hepatitis, chorioretinitis, myocarditis, generalized form of the disease) may develop in immunocompromised patients usually because of a reactivation of a latent infection.
Congenital toxoplasmosis is caused by transmission of infection from mother to foetus. The mother has been primary infected with toxoplasmosis shortly before becoming pregnant or during pregnancy. Congenital toxoplasmosis might result in severe damages of the foetus (brain calcification, hydrocephalus, vision disorders, mental affections), still birth or abortion.
Diagnosis of the disease is based on epidemiological anamnesis, clinical manifestation and laboratory tests. Direct detection of the parasite is not available for routine diagnostics. Serology is the most important tool for laboratory diagnostics of toxoplasmosis. Screening consists in determination of total antibodies by complement fixation test (CFT). Determination of specific IgA, IgE, IgM, IgG antibodies and IgG avidity is performed by ELISA and confirmation of results by immunoblot method.
IgA, IgE and IgM antibodies are significant markers of acute toxoplasmosis. IgM antibodies are a highly sensitive marker of acute infection and they can persist more than 1 year. IgA antibodies can persist for 6-9 months from the beginning of infection. IgE antibodies as a highly specific marker of acute infection can persist up to 6 months from the beginning of infection.

IgG antibodies reach maximum level in serum after 6 months from the beginning of infection and they can be detected for many years after past infection.
3 Test Principle
The kit is intended for detection of specific Toxoplasma IgG antibodies in a sample by means of a sandwich type of the ELISA method (i.e. a solid phase coated with specific antigen – antibody from the analysed sample – labelled antibody). The labelled antibody (conjugate) is an animal immunoglobulin fraction to human IgG conjugated with horseradish peroxidase. Peroxidase activity is determined in the test by a substrate containing TMB. Positivity is indicated when blue colour appears; after stopping solution has been added, blue changes to yellow. The yellow colour intensity is measured by a photometer at 450 nm, and it is proportional to the concentration of specific Toxoplasma IgG antibodies in the sample.

6 Storage and Stability
Store the kit at +2°C to +8°C. Do not freeze. If the kit is stored as described, the labelled expiration date is valid. The expiration date is indicated on the package. The opened kit should be used within three months.
Sample Preparation and Storage
The following human body liquids can be used for testing: serum and citrate plasma. Anticoagulants in the plasma (except for citrate) as well as bacterially contaminated, haemolytic or chylous samples can affect the test results.
Samples can be stored at +2°C to +8°C for one week. For a longer period, store samples at -20°C. Diluted samples should be used as soon as possible.
7 Preparation of Reagents
Dilute the Wash Solution 1:20 (1 part of solution and 19 parts of distilled water); e.g. 75 ml of the concentrated Wash Solution + 1425 ml of distilled water.
Salt crystals might develop in the bottle with the concentrated Wash Solution. Prior to use, it is necessary to dissolve the crystals by warming the bottle in a water bath. The diluted Wash Solution is stable at +2°C to +8°C for one week.
The Controls and the Calibrators are supplied ready to use, do not dilute further!
The Conjugate is supplied ready to use, do not dilute further!
TMB-Solution is a one-component chromogenic substrate solution ready to use, do not dilute further!
Interchangeability of reagents
The Sample Diluent, TMB-Solution and the Avidity Solution are interchangeable between some ELISA of DIAsource ImmunoAssays S.A, provided they have the identical numeric marking (e.g. Sample Diluent 2, Sample Diluent 3, etc.). Further information relative to this interchangeability can be requested to DIAsource ImmunoAssays S.A.

8 Preparation of Samples
Mix gently the Sample Diluent prior to use.
Dilution of sera and plasma samples
Dilute well mixed samples 1:101 with the Sample Diluent:
E.g.: 10 µl of sample + 1 ml of the Sample Diluent
Mix well.
Dilute foetal and neonatal sera 1:101 with the Sample Diluent:
E.g.: 10 µl of serum + 1 ml of the Sample Diluent
Mix well.
9 Assay Procedure
Allow all reagents to come to room temperature and mix well. If you do not use a whole microplate, return unnecessary strips into the bag with desiccant. Seal the bag tightly and store at +2°C to +8°C. Keep dry!
1. Dispense the controls (calibrators) and the diluted samples according to the working schedule.
Semiquantitative evaluation in Index of Positivity (IP)
• Leave A1 well empty (blank).
• Pipette 100 µl of the Negative Control (Calibrator 1) into 1 well.
• Pipette 100 µl of CUT-OFF (Calibrator 2) into 2 wells.
• Pipette 100 µl of the Positive Control (Calibrator 3) into 1 well.
• Pipette 100 µl of the diluted samples (see Chapter Preparation of Samples) into the other wells.
Quantitative evaluation in Units IU/ml
• Leave A1 well empty (blank).
• Pipette 100 µl of the Negative Control (Calibrator 1) into 1 well.
• Pipette 100 µl of CUT-OFF (Calibrator 2) into 2 wells.
• Pipette 100 µl of the Positive Control (Calibrator 3) into 2 wells.
• Pipette 100 μl of the Calibrator 4 into 2 wells.

2. Cover the microplate with the lid and incubate at 37°C for 60 minutes.
3. Aspirate the content of the wells and wash 5× with the working strength Wash Solution. Fill the wells up to the edge. Finally, tap the inverted microplate thoroughly on an absorbent paper to remove solution remnants.

4. Pipette 100 µl of the Conjugate into all wells except A1 well.
5. Cover the microplate with the lid and incubate at 37°C for 60 minutes.
6. Aspirate the content of the wells and wash 5× with the working strength Wash Solution. Fill the wells up to the edge. Finally, tap the inverted microplate thoroughly on an absorbent paper to remove solution remnants.
7. Pipette 100 µl of TMB-Solution into all wells. Avoid contamination – see Chapter Procedural Notes.
8. Cover the microplate with the lid and incubate at 37°C for 20 minutes. Keep out of light.
9. Stop the reaction by adding 100 µl of the Stop Solution in the same order and intervals as the substrate was added.
10. Read the colour intensity in wells against blank (A1 well) using photometer set to 450 nm. The absorbance should be read within 30 minutes after stopping the reaction.

11 Quality Control

12 Results Interpretation
Calculation of Index of Positivity (IP)
Divide the absorbance of a tested sample by the mean absorbance of CUT-OFF measured in the same test run:

Examination of borderline samples, i.e. samples with Index of Positivity from 0.9 to 1.1, should be repeated from a new sample collected after 2 to 6 weeks regarding to the disease specifics.
Quantitative evaluation in International Units (IU/ml)
Construct a calibration curve by plotting the concentration (X) of the calibrators in IU/ml against the corresponding absorbance (Y). Construct the calibration curve by single point cross connection. Read the values of antibody level (IU/ml) in samples from the calibration curve.
If the level of antibodies in a sample is higher than 240 IU/ml, dilute the sample in two steps and repeat the test (final dilution 1:1010).
Step 1. Dilute the sample 1:101 with the Sample Diluent: 10 μl of the sample + 1 ml of the Sample Diluent.
Step 2. Dilute 100 μl of the sample diluted in Step 1 with 900 μl of the Sample Diluent.
Multiply the concentrations of antibodies read from the calibration curve by 10.

Examination of borderline samples should be repeated from a new sample collected after 2 to 6 weeks regarding to the disease specifics.
Serological finding can be interpreted only in the context of results of other laboratory tests and patient clinical picture.
13 Performances characteristics
13.1 Specificity and Sensitivity
The diagnostic specificity was determined in the panel of negative sera, the number of tested sera was 50. The diagnostic sensitivity was determined in the panel of positive sera, the number of tested sera was 89. The specificity and sensitivity are based on the results found.
Specificity: IgG 100.0%
Sensitivity: IgG 98.9%
13.2 Precision and reproducibility
Intra-assay reproducibility was determined by testing samples of different levels of antibody reactivity for at least 16 times in one test. The coefficient of variation (CV) of the reactive IgG samples was 5.6%
Inter-assay reproducibility was determined by testing samples of different levels of antibody reactivity in 10 different test runs. The CV of the reactive IgG samples was 7.3%.
14 Safety Precautions
The kit is intended for in vitro diagnostic use only.
The sera used for controls were tested and found to be negative for HIV 1 and HIV 2, HBsAg, HCV, TPHA. In spite of this fact, they still need to be handled as potentially infectious materials.
Some reagents contain sodium azide, which is a toxic compound. Avoid contact with skin.
The Stop Solution contains diluted acid solution. Avoid contact with eyes and skin.

It is necessary to observe the local safety rules and regulations.

First aid
In case of contact with eyes, flush with copious amount of water and seek medical assistance. In case of contact with skin and clothing, remove all the contaminated clothes. Wash the skin with soap and plenty of running water. In case of contact with solutions containing plasma or clinical samples, disinfect the skin. In case of accidental ingestion, flush the mouth with drinking water and seek medical assistance.
Remnants disposal
All the materials used for performing the test must be treated as potentially infectious due to the contact with biological materials. Therefore they need to be disposed together with biological waste.
Expired kit disposal
Disassemble the kit and dispose the components as biological material. Discard the packaging material as required by local regulations.
15 Procedural Notes
In order to obtain reliable results, it is necessary to strictly follow the Instructions for Use. Always use clean preferably disposable tips and glassware.
Microtiter Plate – in order to prevent water condensation on the surface of the microplate, always allow the bag with the microplate to warm up to room temperature before opening.
Wash Solution – use high quality distilled water for preparing the working strength Wash Solution.
Washing procedure – keep to the prescribed number of wash cycles and fill the wells to the upper edge. The soak time (i.e. interval between two different wash cycles during which the wells stay filled up with the Wash Solution) should be approx. 30-60 seconds.
TMB-Solution – the vessel used for multichannel pipetting should not be used for other reagents. Do not return the surplus TMB-Solution from the pipetting vessel into the vial.
Non-reproducible results might be caused by improper methodology as following:
• insufficient mixing of reagents and samples before use
• improper replacement of vial caps
• using the same tip for pipetting different reagents
• reagent exposure to excessive temperature; bacterial or chemical contamination

• insufficient washing or filling of the wells (the wells should be filled to the upper edge), improper aspiration of Wash Solution remnants
• contamination of the well edges with Conjugate or samples
• using reagents from different kit lots

The kit might be used for sequential examinations. When preparing working strength solutions, use only the amount of reagents needed for the analysis.
The kit might be used in all types of automatic ELISA analysers.
If necessary, DIAsource ImmunoAssays S.A. can offer a certified modification of the Instructions for Use for the specific type of analyser.
The producer cannot guarantee that the kit will function properly if the assay procedure instructions are not strictly adhered to.
16 Index of Avidity
16.1 Introduction
Antibody avidity expresses the strength of bond between antigen and antibody. Low avidity antibodies are produced in the early stages of a primary infection. As the infection progresses, immune response of organism matures and avidity of antibodies increases. Antibodies show high avidity in the latent phase of the disease. High avidity IgG antibodies are produced by memory B-cells from the beginning of a secondary infection or reactivation.
Determination of IgG antibodies avidity enables differentiation of various stages of infection and is a useful addition to serological diagnostics.
16.2 Test Principle
Avidity determination is based on dissociation of antigen-antibody bond by means of Avidity Solution (urea solution). After Avidity Solution treatment, low avidity antibodies are released and washed out while high avidity antibodies remain bound to the antigen. The binding strength is expressed by the Index of Avidity (IAv). IAv determines the portion of IgG antibodies that remains bound to antigen after incubation with Avidity Solution. IAv assay procedure is a modification of the standard ELISA procedure using Avidity Solution. The IgG avidity is determined only in IgG positive samples.
Result interpretation of 90% of serum samples will not be influenced by different serum dilution, i.e. whether a sample has been diluted 1:101 or more. However, IAv analysis of 10% of sera diluted 1:101 leads to high-avidity results while further dilution of such sera may provide a low-avidity outcome. Optimal results are obtained for

antibody concentration of approx. 60 IU/ml. Therefore, one- or three-point analysis can be used for the test.
One-point analysis
Determine quantitatively antibody level in serum (in IU/ml) using the usual procedure (see Chapter Assay Procedure). Then dilute the analysed sample with Sample diluent to 60 IU/ml and determine the IAv in a subsequent ELISA test.
Three-point analysis
The IAv is directly determined without any previous analysis. In this procedure, three different serum concentrations are simultaneously analysed - the serum sample is diluted 1:101 (basic dilution) and then further 5× and 25× diluted. This procedure is especially useful in time-critical situations when rapid results are required (STAT examinations).
16.3 Preparation of Reagents
Allow all reagents including the Avidity Solution to come to room temperature and mix well. Crystals might develop in the vial with the Avidity Solution. Prior to use, it is necessary to dissolve the crystals by short-time warming up. The functionality of the Avidity Solution is indicated by yellow colour. The solution is thermolabile. The solution is deteriorated, if it changes its colour from yellow to red. A red-coloured Avidity Solution cannot be used further.
16.4 One-Point Analysis
16.4.1 Dilution of samples
Determination of antibody concentration in IU/ml
Determine antibody concentration (IU/ml) by VIR-ELISA ANTI-TOXO-IgG test as described (see Chapter Assay Procedure).
Predilution to 60 IU/ml
1. If antibody concentration is lower than 60 IU/ml, do not dilute the sample.
2. If antibody concentration ranges from 60 to 240 IU/ml, dilute the sample with the Sample Diluent to 60 IU/ml (e.g. dilute the sample three times if the content of antibodies is 180 IU/ml).
3. If antibody concentration is higher than 240 IU/ml, dilute the sample 1:1010, repeat the analysis and multiply the result by 10. Then, dilute the sample with the Sample Diluent to 60 IU/ml according to the obtained antibody concentration.

Dilution of Samples for IAv analysis
Dilute a prediluted serum sample (60 IU/ml) 1:101 with the Sample Diluent (10 µl of sample + 1 ml of the Sample Diluent). Mix well.

16.5 Three-point analysis
16.5.1 Dilution of samples
Dilute each sample in 3 subsequent steps and determine the IAv of all three dilution.
Step 1. Dilute serum 1:101 with the Sample Diluent:
10 µl + 1 ml of the Sample Diluent (final dilution 1:101)
Step 2. Dilute serum prepared in Step 1with Sample diluent:
100 µl + 400 µl of the Sample Diluent (final dilution 1:505)

7. Pipette 100 µl of Conjugate into all wells except A1 well.
8. Cover the microplate with the lid and incubate it at 37°C for 60 minutes.
9. Aspirate the content of the wells and wash 5× with the working strength Wash Solution. Fill the wells up to the edge. Finally, tap the inverted microplate thoroughly on an absorbent paper to remove solution remnants.
10. Pipette 100 µl of TMB – Solution into all wells. Cover the microplate with the lid and incubate it at 37°C for 20 minutes. Keep out of light.
11. Stop the reaction by adding 100 µl of Stop Solution in the same order and intervals as the substrate was added.
12. Read the colour intensity in wells against blank (A1 well) using photometer set to 450 nm. The absorbance should be read within 30 minutes after stopping the reaction.
16.7 Quality Control
Quality control of the test is performed in N strips (see Chapter Quality Control).

16.8 Results Interpretation
Calculation of Index of Avidity (IAv)
Divide the absorbance of a tested sample in Av strip by the absorbance of the tested sample in N strip measured in the same test run:
Interpretation of the test results is described in Table 3.
If the three-point analysis is used, calculate the IAv for 1:101, 1:505 and 1:2525 sample dilutions. Determine the sample dilution with the antibody level closest to 60 IU/ml. The IAv of this sample dilution represents the relevant IAv result.

Notes to interpretation
• Less than 5% of patients, who overcame toxoplasmosis and their disease, are not acute, show low avidity even after 4 month after the infection. The Index of Avidity remains lower than or equal to 30. It typically occurs in sera with low Anti-toxoplasma IgG concentration. Samples with specific IgG concentration lower than 25 IU/ml, especially when diluted (for three-point analysis), can give inadequately low IAv values.
• Three-point analysis cannot be evaluated if IgG concentration in a sample is higher than 5000 IU/ml.
• Examination of borderline samples should be repeated with a new sample collected after 3-4 weeks.
• Low as well as high levels of specific IgM and IgA antibodies can persist in some patients who have high IAv values.
• Results of IgG avidity examination should be interpreted only in the context of results of other tests, especially CFT, NIFR titres and IgG, IgM and IgA levels.

Serum oder Plasma
1 Verwendungszweck
Enzym-Immunoassay zur Ermittlung von IgG-Antikörpern gegen den Toxoplasma gondii in humanem Serum oder Plasma. Das Set ist für professionelle Zwecke bestimmt. Nur für „in vitro“-Diagnostika geeignet.
2 Einleitung
Toxoplasmose ist eine parasitäre Erkrankung, deren Erreger das Protozoon Toxoplasma gondii ist. Es handelt sich um einen Parasiten mit kompliziertem Entwicklungszyklus mit mehreren morphologisch unterschiedlichen Stadien. Primärwirt sind Katzen und katzenartige Raubtiere. Der Mensch und sonstige warmblütige Wirbeltiere können sich entweder primär durch infizierte Nahrung (unzureichend gegartes Fleisch) oder durch den Verzehr von Oozysten (schmutzige Finger, Gegenstände usw.) anstecken.
Die erworbene Toxoplasmose verläuft bei immunkompetenten Individuen gewöhnlich asymptomatisch oder nur mit leichten Anzeichen (Lymphadenopathie, Subfebrilie, Müdigkeit) und ohne Folgen. Die schwerwiegendsten Erscheinungen der Toxoplasmose (Enzephalitis, Hepatitis, Myokarditis, generalisierte Form durch Erkrankung) können bei Patienten mit Immunschwäche eintreten, die darüber hinaus auch durch die Reaktivierung der latenten Infektion gefährdet sind.
Die angeborene Toxoplasmose wird durch die Übertragung der Infektion von der Mutter auf den Fötus verursacht. Sie ist die Folge der Erstinfektion der Mutter kurz vor oder während der Schwangerschaft. Sie kann zum Abort, zur Geburt eines toten Kindes oder eines Kindes mit unterschiedlichem Schädigungsgrad (Gehirnverkalkung, Hydrocephalus, Sehstörungen, Beeinflussung der geistigen Entwicklung) führen.
Die Diagnostik der Erkrankung basiert auf dem klinischen Bild, der epidemiologischen Anamnese und Labortests. Der direkte Nachweis des Parasiten ist in der Routinediagnostik nicht möglich. Die Serologie ist die wichtigste diagnostische Möglichkeit zum Nachweis der Toxoplasmose. Als Screening-Methode wird die Bestimmung der Gesamtantikörper mit der CFT-Methode (complement fixation test) erachtet. Die Bestimmung der spezifischen Antikörper der Klassen IgA, IgE, IgM und IgG erfolgt durch die ELISA-Methode, die ggf. durch einen Immuniblot bestätigt wird.
Die Antikörper der Klassen IgA, IgE und IgM haben für die Diagnostik der akuten Toxoplasmose Bedeutung. Die Antikörper der Klasse IgM stellt einen hochsensitiven Marker einer akuten Infektion dar und können länger als ein Jahr persistieren. Antikörper der Klasse IgA können 6 bis 9 Monate nach der akuten Infektion

nachgewiesen werden. Antikörper der Klasse IgE, als hochspezifischer Marker einer akuten Infektion, persistieren im Serum weniger als 6 Monate.
Antikörper der Klasse IgG erreichen den Maximalspiegel im Serum ein halbes Jahr nach dem Beginn der Erkrankung und können viele Jahre nach der Infektion nachgewiesen werden.
3 Testprinzip
Das Kit ermöglicht die Bestimmung spezifischer Toxoplasma IgG-Antikörper in der Probe mittels der Sandwich-E-Methode (d.h. antigenbeschichtete feste Phase - Antikörper aus der untersuchten Probe - markierter Antikörper). Der markierte Antikörper (Konjugat) ist eine tierische Immunoglobulinfraktion gegen menschliches IgG, die mit Meerrettichperoxidase konjugiert ist. Die Peroxidaseaktivität wird durch das TMB-Substrat bestimmt, das sich bei einer positiven Reaktion blau färbt. Die Enzymreaktion wird mit einer Stopplösung beendet. Die blaue Färbung schlägt in eine gelbe um. Die Intensität der Gelbfärbung wird mit dem Photometer (bei einer Wellenlänge von 450 nm) gemessen und ist proportional zur Konzentration der spezifischen Toxoplasma IgG-Antikörper in der Probe.

6 Lagerung und Haltbarkeit des Kits
Das Kit muss bei einer Temperatur von +2°C bis +8°C gelagert werden. Bei Einhaltung der Lagerungsbedingungen gilt das auf der Verpackung des Kits angegebene Haltbarkeitsdatum. Der angebrochene Kit sollte innerhalb von drei Monaten verbraucht werden. Das Kit darf nicht einfrieren!
Proben und ihre Lagerung
Als Probe kann menschliches Serum und Zitratplasma verwendet werden. Mit Bakterien verseuchte Proben, hämolytische Proben oder Proben mit Chylus und im Plasma enthaltene Antikoagglutinationsmittel (Zitrat ausgenommen) können das Testergebnis beeinflussen.
Die Proben können bei einer Temperatur von +2°C bis +8°C höchstens 1 Woche lang aufbewahrt werden. Frieren Sie die Proben bei längerer Lagerung bei -20°C ein. Verdünnte Proben sind möglichst bald zu untersuchen.
7 Vorbereitung der Reagenzien
Verdünnen Sie die Waschlösung im Verhältnis 1:20 (1 Teil Lösung und 19 Teile destilliertes Wasser). Z.B. 75 ml konzentrierte Waschlösung + 1425 ml destilliertes Wasser.
In der Flasche mit der Waschlösung können sich Salzkristalle bilden. Diese Kristalle müssen vor Gebrauch durch Erwärmung im Wasserbad aufgelöst werden. Die verdünnte Lösung ist eine Woche lang bei +2°C bis +8°C haltbar.
Die Kontrollen und die Kalibratoren sind gebrauchsfertig, nicht weiter verdünnen!
Das Konjugat ist gebrauchsfertig, nicht weiter verdünnen!
Das TMB-Lösung ist eine chromogene Einkomponenten-Substratlösung und gebrauchsfertig, nicht weiter verdünnen!

8 Probenverdünnung
Mischen Sie die Verdünnungslösung für die Proben vor der Verwendung schonend.
Verdünnung der Serum- und Plasmaproben
Verdünnen Sie die gründlich gemischten Proben 1:101 mit der Verdünnungslösung für die Proben:
z.B: 10 µl Probe + 1 ml Verdünnungslösung für die Proben.
Gut mischen.
Verdünnen Sie fetale- und neonatale Seren 1:101 mit der Verdünnungslösung für die Proben:
např.: 10 µl Serum + 1 ml Verdünnungslösung für die Proben.

9 Testdurchführung
Es ist sehr wichtig, alle Reagenzien vor ihrer Verwendung auf Raumtemperatur zu bringen und gründlich zu mischen. Sofern Sie nicht die gesamte Platte verwenden, so legen Sie die nicht benötigten Strips in die Verpackung mit dem Trocknungsmittel zurück, verschließen diese luftdicht und lagern sie bei +2°C bis +8°C. Sorgfältig vor Feuchtigkeit schützen!
1. Pipettieren Sie die Kontrollen (Kalibratoren) sowie die verdünnten Proben gemäß dem Arbeitsschema.
Semiquantitative Auswertung im Index der Positivität (IP)
• Lassen Sie die Vertiefung A1 leer (Blank).
• Pipettieren Sie 100 µl der Negativen Kontrolle (Kalibrator 1) in 1 Vertiefung.
• Pipettieren Sie 100 µl CUT-OFF (Kalibrator 2) in 2 Vertiefungen.
• Pipettieren Sie 100 µl der Positiven Kontrolle (Kalibrator 3) in 1 Vertiefung.
• Pipettieren Sie 100 µl der verdünnten Proben (siehe 7. Probenverdünnung) in die restlichen Vertiefungen.

• Pipettieren Sie 100 µl der verdünnten Proben (siehe 7 Probenverdünnung) in die restlichen Vertiefungen.
2. Decken Sie die Platte mit dem Deckel ab und lassen Sie sie 60 Minuten bei 37°C inkubieren.
3. Saugen Sie den Inhalt der Vertiefungen ab und waschen Sie 5× mit Arbeitswaschlösung. Füllen Sie die Vertiefungen bis zum oberen Rand. Verbleibende Flüssigkeit zum Schluss durch Ausklopfen auf saugfähigem Papier entfernen.
4. Pipettieren Sie in alle Vertiefungen mit Ausnahme von A1 100 µl Konjugat.
5. Decken Sie die Platte mit dem Deckel ab und lassen Sie sie 60 Minuten bei 37°C inkubieren.
6. Saugen Sie den Inhalt der Vertiefungen ab und waschen Sie 5× mit Arbeitswaschlösung. Füllen Sie die Vertiefungen bis zum oberen Rand. Verbleibende Flüssigkeit zum Schluss durch Ausklopfen auf saugfähigem Papier entfernen.
7. Pipettieren Sie in alle Vertiefungen 100 µl TMB-Lösung. Achten Sie auf Verschmutzungen – siehe 13 Technische Anmerkungen.
8. Decken Sie die Platte mit dem Deckel ab und lassen Sie sie 20 Minuten bei 37°C im Dunkeln inkubieren.
9. Stoppen Sie die Reaktion durch Zugabe von 100 µl Stopplösung in der gleichen Reihenfolge und den gleichen Intervallen, wie das Substrat pipettiert wurde.
10. Messen Sie am Photometer bei einer Wellenlänge von 450 nm die Intensität der Verfärbung der Lösungen in den Vertiefungen im Vergleich zum Blank (Vertiefung A1), und zwar innerhalb von 30 Minuten nach dem Stoppen der Reaktion.

Die Untersuchung grenzwertiger Proben, d. h. Proben mit einem Positivitätsindex von 0,9 bis 1,1, muss nach 2 bis 6 Wochen unter Berücksichtigung der jeweiligen spezifischen Erkrankung nach erneuter Entnahme wiederholt werden.
Quantitative Auswertung in internationalen Einheiten (IU/ml)
Erstellen Sie eine Kalibrierungskurve, indem Sie die Konzentration der Kalibratoren in IU/ml (X-Achse) gegen die entsprechenden Extinktionen (Y-Achse) auftragen. Durch Verbindung der einzelnen Punkte erstellen Sie die Kalibrierungskurve. Bestimmen Sie den Antikörperspiegel in den Proben (IU/ml) durch Ablesen dieser Werte von der Kalibrierungskurve.
Sofern der Antikörperspiegel in der Probe größer als 240 IU/ml ist, verdünnen Sie die Probe in zwei Schritten und wiederholen Sie den Test (Endverdünnung 1:1010).
Schritt 1. Verdünnen Sie die Probe 1:101 mit der Verdünnungslösung für die Proben: 10 µl Probe + 1 ml Verdünnungslösung für die Proben.
Schritt 2. 100 μl der im Schritt 1 verdünnten Probe verdünnen Sie mit 900 μl Verdünnungslösung für die Proben.
Die von der Kalibrierungskurve abgelesenen Konzentration der Antikörper sind dann mit 10 zu multiplizieren (x 10).

Die Untersuchung grenzwertiger Proben muss nach 2 bis 6 Wochen unter Berücksichtigung der jeweiligen spezifischen Erkrankung nach erneuter Blutentnahme wiederholt werden.
Der serologische Befund kann nur im Kontext mit den Ergebnissen anderer Labortests und mit dem klinischen Bild des Patienten interpretiert werden.
13 Performances characteristics
13.1 Spezifität und Sensibilität
Die diagnostische Spezifität wurde in einer Reihe von negativen Seren ermittelt, die Anzahl der getesteten Seren betrug 50. Die diagnostische Sensitivität wurde in einer Reihe von positiven Seren ermittelt, die Anzahl der getesteten Seren betrug 89. Die Spezifität und Sensibilität basieren auf den ermittelten Ergebnissen.

13.2 Präzision und Reproduzierbarkeit
Die Intra-Assay-Reproduzierbarkeit wurde ermittelt, indem Proben mit unterschiedlicher Antikörperreaktivität mindestens 16 Mal innerhalb eines Testdurchlaufs geprüft wurden. Der Variationskoeffizient (VarK) der reaktiven IgG-Proben betrug 5,6 %.
Die Inter-Assay-Reproduzierbarkeit wurde nach 10 verschiedenen Testdurchläufen der Proben mit unterschiedlicher Antikörperreaktivität ermittelt. Der VarK der reaktiven IgG-Proben betrug 7,3 %.
14 Arbeitssicherheit
Der Test ist nur für Diagnosezwecke in vitro vorgesehen.
Die bei der Fertigung der Kontrollen verwendeten Seren wurden mit negativem Ergebnis auf HIV 1 und HIV 2, HBsAg, HCV, TPHA untersucht. Dennoch ist es

erforderlich, mit den Seren so umzugehen, als seien sie potentielles Infektionsmaterial.
Einige Reagenzien enthalten das giftige Natriumazid. Vermeiden Sie Hautkontakt.
Die Stopplösung enthält eine verdünnte Säurelösung.
Schützen Sie bei der Arbeit mit dieser Lösung Ihre Augen und Ihre Haut!

Erste-Hilfe-Maßnahmen
Spülen Sie bei Augenkontakt Ihre Augen gründlich mit lauwarmem Wasser und suchen Sie einen Arzt auf. Entledigen Sie sich bei Kleidungs- und Hautkontakt sämtlicher kontaminierter Kleidung. Spülen Sie die Haut gründlich mit Wasser und Seife ab. Desinfizieren Sie die Haut, sofern Spritzer einer Lösung, die Plasma oder eine klinische Probe enthält, auf diese gelangen. Spülen Sie bei zufälligem Verzehr Ihren Mund mit Trinkwasser aus und nehmen Sie ärztliche Hilfe in Anspruch.
Resteentsorgung
Sämtliche zur Testdurchführung verwendeten Hilfsmittel sind hinsichtlich des Kontakts mit biologischem Material als potentiell infektiös zu betrachten. Deshalb sind sie als biologischer Abfall zu entsorgen.
Entsorgung des Kits nach Ablauf der Haltbarkeitsdauer
Zerlegen Sie das Kit in seine einzelnen Komponenten und entsorgen Sie diese als biologisches Material. Entsorgen Sie die Verpackungen und Verpackungsreste gemäß den örtlichen Vorschriften als sortierten Abfall.
15 Technische Anmerkungen
Um zuverlässige Ergebnisse zu erzielen, ist die genaue Einhaltung der Anleitung notwendig. Verwenden Sie bei der Arbeit stets Hilfsmittel höchsten Sauberkeitsgrades. Bevorzugen Sie Hilfsmittel für den einmaligen Gebrauch.
Mikrotiterplatte – Bringen Sie vor dem Öffnen den Beutel mit der Mikrotiterplatte auf Labortemperatur, damit auf der Plattenoberfläche kein Wasserdampf kondensiert.
Waschlösung – verwenden Sie zur Vorbereitung der Waschlösungs-Arbeitsverdünnung hochwertiges destilliertes Wasser.
Waschen – halten Sie die vorgeschriebene Anzahl der Waschzyklen ein und füllen Sie die Vertiefungen bis zum oberen Rand. Die Zeitdauer, während der die Vertiefungen zwischen den Waschzyklen mit der Waschlösung gefüllt verbleiben (soak time), sollte etwa 30 – 60 Sekunden betragen.

TMB-Lösung – verwenden Sie die Pipettierwanne für TMB-Lösung für keine anderen Lösungen. Füllen Sie den Lösungsrest aus der Pipettierwanne nicht in die Flasche zurück.
Nicht nachvollziehbaren Ergebnissen können methodische Fehler zugrunde liegen, insbesondere:
• unzureichendes Mischen der Lösungen und der Proben vor der Verwendung
• Vertauschen der Flaschenverschlüsse
• Verwendung der gleichen Spitze zum Pipettieren verschiedener Lösungen
• die Reagenzien wurden zu hohen Temperaturen, bakterieller oder chemischer Kontamination ausgesetzt
• unzureichendes Auswaschen der Vertiefungen, die Vertiefungen wurden nicht bis zum Rand gefüllt, schlechtes Absaugen der Lösungsreste
• Verschmutzung der Vertiefungsränder mit Konjugat oder mit Proben
• Vertauschen der Reagenzien aus verschiedenen Kit-Chargen
• Kontakt der Reagenzien mit Oxidationsmitteln, Schwermetallen und deren Salzen.
• Das Kit kann in mehrere Tests aufgeteilt werden. Entnehmen Sie zur Vorbereitung der Arbeitslösungen nur so große Mengen Reagenzien, wie für die Analyse notwendig sind.
• Der Test kann auf allen Typen automatischer ELISA-Analysatoren durchgeführt werden. Bei Bedarf bietet DIAsource ImmunoAssays S.A. eine zertifizierte Modifizierung der Arbeitsanweisung für den jeweiligen Automatentyp an.
Bei Nichteinhaltung des Arbeitsverfahrens haftet der Hersteller nicht für die korrekte Funktion des Tests.
16 Index der Avidität
16.1 Einleitung
Die Avidität der Antikörper bringt die Festigkeit der Bindung zwischen dem Antigen und den Antikörpern zum Ausdruck. Bei der Primärinfektion werden zunächst Antikörper mit geringer Avidität gebildet. Im Verlaufe der Infektion reift die Immunreaktion des Organismus aus und die Avidität der Antikörper nimmt zu. In der latenten Phase der Erkrankung sind die Antikörper hochavid und bei einer Sekundärinfektion oder einer Reaktivierung produzieren die Gedächtnis-B-Zellen sofort IgG-Antikörper mit hoher Avidität. Die Feststellung der Avidität der IgG-Antikörper ermöglicht die genauere Bestimmung der Ansteckungsphase und ist eine sinnvolle Ergänzung zur serologischen Diagnostik.

16.2 Testprinzip
Die Bestimmung der Avidität der Antikörper basiert auf dem Prinzip der Beeinträchtigung der Bindung zwischen dem Antigen und den Antikörpern durch eine Aviditäslösung (Harnstofflösung). Antikörper mit geringer Avidität werden durch das Wirken der Aviditätslösung aus der Bindung an das Antigen freigesetzt und ausgeschwemmt. Im Gegensatz hierzu bleiben die hochaviden Antikörper auch nach dem Wirken der Aviditätslösung zum Großteil an das Antigen gebunden. Die Festigkeit der Bindung wird durch den Index der Avidität (IAv) ausgedrückt, der festlegt, welcher Anteil der Antikörper (in %) nach der Inkubation mit der Aviditätslösung an das Antigen gebunden blieb. Die praktische Bestimmung besteht in der Durchführung des modifizierten ELISA-Verfahrens unter Verwendung der Aviditätslösung, im Vergleich zum normalen Verfahren ELISA. Die Bestimmung der IgG-Avidität erfolgt nur bei IgG-positiven Proben.
Bei mehr als 90 % der untersuchten Seren wird die Interpretation des Ergebnisses nicht durch unterschiedliche Probenverdünnungen beeinflusst, z.B. ob die Probe 1:101 oder stärker verdünnt wurde. Bei weniger als 10 % der Seren kann der Aviditätsindex einer 1:101 verdünnten Probe ein hochaffines Ergebnis ergeben, während dieselbe Probe bei höherer Verdünnung zu einem niederaffinen Ergebnis führt. Optimale Ergebnisse wurden mit einer Antikörperkonzentration von ca. 60 IU/ml erzielt. Der Test kann daher als Einpunkt- oder Dreipunkttest durchgeführt werden.
Einpunkt-Bestimmung
In der Serumprobe wird die Antikörperkonzentration quantitativ (in IU/ml) in der üblichen Weise bestimmt (siehe Kapitol Testdurchführung). Außerdem wird die Probe mittels der Verdünnungslösung auf 60 IU/ml verdünnt und der Ides der Avidität anschließenden mit dem ELISA-Test bestimmt.
Dreipunkt-Bestimmung
In der Serumprobe erfolgt ohne vorheriges Testen die Bestimmung des Aviditätsindex. Das Serum wird für diese Bestimmung in drei Konzentrationen untersucht: In der Ausgangsverdünnung des Tests (1:101), außerdem fünffach verdünnt und fünfundzwanzigfach verdünnt. Diese Titrationsanalyse ist insbesondere für eilige Bestimmungen geeignet (STAT-Bestimmungen).
16.3 Vorbereitung der Reagenzien
Bringen Sie alle Reagenzien, einschließlich der Aviditätslösung, auf Labortemperatur und mischen sie gründlich. Im Fläschchen mit der Aviditätslösung können sich Kristalle bilden. Diese Kristalle sind vor Gebrauch durch kurzes Erwärmen aufzulösen. Die Funktionsfähigkeit der Aviditätslösung wird durch gelbe Färbung angezeigt. Die

Lösung ist temperaturempfindlich. Ein Farbumschlag von gelb auf rot bedeutet eine Verschlechterung, rotgefärbte Aviditätslösung kann nicht mehr verwendet werden!
16.4 Einpunkt-Bestimmung
16.4.1 Probenverdünnung
Bestimmung des Antikörpergehalts in IU/ml
Führen Sie die Bestimmung des Antikörpergehalts in IU/ml mithilfe des Toxoplasma IgG ELISA-Tests durch (siehe Punkt 8, Kapitol Arbeitsanleitung).
Vorverdünnung auf 60 IU/ml
1. Falls der Antikörpergehalt in der Probe weniger als 60 IU/ml beträgt, wird nicht vorverdünnt.
2. Falls der Antikörpergehalt in der Probe 60–240 IU/ml beträgt, ist die Probe mit der Verdünnungslösung für Proben auf 60 IU/ml zu verdünnen (z. B. bei einem Gehalt von 180 IU/ml wird dreifach verdünnt = 1+2).
3. Falls der Antikörpergehalt in der Probe mehr als 240 IU/ml beträgt, ist die Bestimmung des quantitativen Antikörpergehalts mit einer 1:1010 verdünnten Probe vorzunehmen und das Ergebnis mit 10 zu multiplizieren. Verdünnen Sie die Probe anschließend mit der Proben-Verdünnungslösung entsprechend dem Antikörpergehalt auf 60 IU/ml.
Verdünnung der Probe für die Bestimmung des Aviditätsindex
Verdünnen Sie das vorverdünnte Serum (60 IU/ml) mit der Proben-Verdünnungslösung 1:101 (10 µl Serum + 1 ml Proben-Verdünnungslösung). Gründlich mischen.
16.4.2 Testdurchführung
1. Pipettieren Sie die Kontrollen sowie die verdünnten Proben gemäß dem Arbeitsschema.
• Lassen Sie die Vertiefung A1 leer (Blank).
• Pipettieren Sie 100 µl der Negativen Kontrolle (Kalibrator 1) 0,1 IU/ml in 1 Vertiefung des Strips N.
• Pipettieren Sie 100 µl CUT-OFF (Kalibrator 2) 6 IU/ml in 1 Vertiefung des Strips N.
• Pipettieren Sie 100 µl der Positiven Kontrolle (Kalibrator 3) 60 IU/ml in zwei benachbarte Vertiefungen des Strips N und Av.
• Pipettieren Sie 100 µl von Kalibrator 4 (240 IU/ml) in zwei benachbarte Vertiefungen des Strips N und Av.
• Pipettieren Sie 100 µl der verdünnten Proben in zwei benachbarte Vertiefungen des Strips N und Av.

16.5 Dreipunkt-Bestimmung
16.5.1 Probenverdünnung
Führen Sie die Probenverdünnung in drei Schritten durch und bestimmen Sie von jedem Schritt den Aviditätsindex.
1. Schritt – verdünnen Sie die Serumprobe 1:101 mit der Verdünnungslösung für die Proben:
10 μl + 1 ml (Gesamtverdünnung 1:101)
2. Schritt – verdünnen Sie das verdünnte Serum aus Schritt 1 weiter im Verhältnis 1:5:
100 μl + 400 μl (Gesamtverdünnung 1:505)
3. Schritt – verdünnen Sie das verdünnte Serum aus Schritt 2 weiter im Verhältnis 1:5:
100 μl + 400 μl (Gesamtverdünnung 1:2525)
16.5.2 Testdurchführung
• Lassen Sie die Vertiefung A1 leer (Blank).
• Pipettieren Sie 100 µl der Negativen Kontrolle (Kalibrator 1) 0,1 IU/ml in 1 Vertiefung des Strips N.

Die weitere Arbeitsschritte sind für die Einpunkt- und die Dreipunktbestimmung identisch
1. Bedecken Sie die Platte mit den Proben und Kontrollproben mit einem Deckel und lassen Sie sie 60 Minuten bei 37°C inkubieren.
2. Saugen Sie den Inhalt der Vertiefungen ab und waschen Sie 5x mit Arbeitswaschlösung. Füllen Sie die Vertiefungen bis zum oberen Rand. Verbleibende Flüssigkeit zum Schluss durch Ausklopfen auf saugfähigem Papier entfernen.
3. Pipettieren Sie in alle Vertiefungen der Strips Av 100 µl Aviditätslösung.
4. Pipettieren Sie in alle Vertiefungen der Strips N 100 µl der Arbeitswaschlösung.
5. Decken Sie die Platte mit dem Deckel ab und lassen Sie sie 10 Minuten bei 37°C inkubieren.
6. Saugen Sie den Inhalt der Vertiefungen ab und waschen Sie 5× mit Arbeitswaschlösung. Füllen Sie die Vertiefungen bis zum oberen Rand. Verbleibende Flüssigkeit zum Schluss durch Ausklopfen auf saugfähigem Papier entfernen.
7. Pipettieren Sie in alle Vertiefungen mit Ausnahme von Blank (A1–Blank) 100 μl Konjugat.
8. Decken Sie die Platte mit dem Deckel ab und lassen Sie sie 60 Minuten bei 37°C inkubieren.
9. Saugen Sie den Inhalt der Vertiefungen ab und waschen Sie 5× mit Arbeitswaschlösung. Füllen Sie die Vertiefungen bis zum oberen Rand. Verbleibende Flüssigkeit zum Schluss durch Ausklopfen auf saugfähigem Papier entfernen.
10. Pipettieren Sie in alle Vertiefungen 100 µl TMB-Lösung.
11. Decken Sie die Platte mit dem Deckel ab und lassen Sie sie 20 Minuten bei 37°C im Dunkeln inkubieren.
12. Stoppen Sie die Reaktion durch Zugabe von 100 µl Stopplösung in der gleichen Reihenfolge und den gleichen Intervallen, wie das Substrat pipettiert wurde.
13. Messen Sie am Photometer bei einer Wellenlänge von 450 nm die Intensität der Verfärbung der Lösungen in den Vertiefungen im Vergleich zum Blank (Vertiefung A1), und zwar innerhalb von 30 Minuten nach dem Stoppen der Reaktion.

16.6 Testvalidität
Die Auswertung der Validität des Tests erfolgt anhand der N-Strips (siehe Kapitol Testvalidität)

16.7 Auswertung der Ergebnisse
Berechnung des Aviditätsindex (IAv)
Dividieren Sie die Extinktion der getesteten Probe auf dem Strip Av durch die in der gleichen Serie der Untersuchung gemessene Extinktion der auf dem Strip N getesteten Probe:
Die Interpretation der Untersuchungsergebnisse siehe Tabelle (Tabelle 3).
Bei der Dreipunkt-Analyse wird der Aviditätsindex IAv für die 1:101, 1:505 und 1:2525 Probenverdünnung berechnet. Bestimmen Sie die Probenverdünnung bei der der Antikörperspiegel am nächsten bei 60 U/ml liegt. Der IAv dieser Probenverdünnung ist der relevante Avititätsindex.

Hinweise zur Interpretation:
• Bei weniger als 5 % der Patienten, die eine Toxoplasmose durchmachten, erhöht sich auch nach 4 Monaten ab der Infektion die Avidität nicht und der Aviditätsindex bleibt kleiner oder gleich 30, obwohl es sich nicht um eine akute

Toxoplasmose handelt. Dies ist insbesondere für Seren mit einem geringen Gehalt an Anti-Toxoplasma-IgG charakteristisch. Proben mit einer spezifischen IgG-Konzentration von weniger als 25IU/ml, insbesondere bei verdünnten Proben (Dreipunkt-Analyse) können zu geringe Aviditätswerte aufweisen.
• Die Dreipunkt-Aanalyse kann nicht ausgewertet werden, wenn der IgG-Gehalt in einer Probe > 5000 IU/ml ist.
• Ergebnisse mit einem grenzwertiger Aviditätsindex sollten nach 3 – 4 Wochen mit einer neuen Probe wiederholt werden.
• Manche Patienten mit hohem IAv können persistierende, niedrige als auch hohe Spiegel von spezifischen IgM- und IgA-Antikörpern haben.
• Die Ergebnisse der IgG-Aviditätsbestimmung sollten im Kontext der Ergebnisse anderer Untersuchungen, insbesondere CFT, NIFR-Titer und IgG-, IgM- und IgA-Spiegen interpretiert werden.

1 Uso previsto
Inmunoensayo enzimático para la detección de anticuerpos de clase IgG contra el Toxoplasma gondii en suero humano o plasma. El kit está diseñado para uso profesional. Exclusivamente para uso diagnóstico “in vitro”.
2 Introducción
La toxoplasmosis es una enfermedad parasitaria generalizada causada por el protozoo Toxoplasma gondii, un parásito con un ciclo de vida complicado que incluye varios estadios morfológicamente distintos. Los huéspedes primarios son miembros de la familia de los felinos. Los seres humanos y la mayoría de los animales de sangre caliente pueden infectarse por alimentos infectados en primera instancia (carne no tratada lo suficiente con calor) o por la ingesta de ooquistes (alimentos secundarios contaminados o bien dedos, objetos, etc. contaminados).
La toxoplasmosis adquirida en individuos inmunocompetentes normalmente es asintomática o se puede manifestar por medio de síntomas similares a los de la gripe (estado subfebril, fatiga, linfadenopatía, dolor muscular), pero sin efectos negativos duraderos. Los pacientes inmunocomprometidos pueden desarrollar infecciones graves potencialmente mortales (encefalitis, hepatitis, coriorretinitis, miocarditis, forma generalizada de la enfermedad), normalmente debido a una reactivación de una infección latente.
La toxoplasmosis congénita está causada por la transmisión de la infección de la madre al feto. La madre ha sido infectada en primera instancia con toxoplasmosis poco antes de quedarse embarazada o durante el embarazo. La toxoplasmosis congénita podría provocar daños graves al feto (calcificación cerebral, hidrocéfalo, trastornos de la visión, afecciones mentales), mortinato o aborto.
El diagnóstico de la enfermedad se basa en la anamnesis epidemiológica, el cuadro clínico y las pruebas de laboratorio. La detección directa del parásito no está disponible para el diagnóstico de rutina. La serología es la herramienta más importante para las pruebas diagnósticas de laboratorio para la toxoplasmosis. La detección consiste en la determinación de los anticuerpos totales mediante la prueba de fijación de complemento (PFC). La determinación de anticuerpos IgA, IgE, IgM, IgG específicos y de la avidez de IgG se realiza mediante ELISA y la confirmación de los resultados mediante electrotransferencia.
Los anticuerpos IgA, IgE y IgM son marcadores significativos de toxoplasmosis aguda. Los anticuerpos IgM son un marcador altamente sensible de infección aguda y

pueden persistir durante más de 1 año. Los anticuerpos IgA pueden persistir durante 6-9 meses desde el inicio de la infección. Los anticuerpos IgE son un marcador altamente específico de infección aguda y pueden persistir hasta 6 meses desde el inicio de la infección.
Los anticuerpos IgG alcanzan su máximo nivel en suero tras 6 meses desde el inicio de la infección y se pueden detectar muchos años después de la infección.
3 Principio del test
El kit permite la detección de anticuerpos específicos de la clase Toxoplasma IgG en una muestra mediante un ELISA tipo sándwich (es decir, una fase fija recubierta con un antígeno específico – anticuerpo de la muestra de ensayo – anticuerpo marcado). El anticuerpo marcado (conjugado) es una fracción de inmunoglobulina animal contra IgG humana, conjugada con peroxidasa de rábano picante. En la prueba, la actividad de la peroxidasa viene determinada por un sustrato con TMB, que adquiere un color azul en el caso de positividad. Después de añadir una solución de parada, el color azul cambia a amarillo. La intensidad del amarillo se mide utilizando un fotómetro a 450 nm, y es proporcional a la concentración de anticuerpos Toxoplasma IgG específicos presentes en la muestra.

6 Almacenaje y caducidad de los juegos
Almacene los kits a temperatura entre +2 ºC y +8 ºC. No se deben congelar. Si se observan las condiciones de almacenamiento, la fecha de caducidad es válida. Tras la apertura del kit se recomienda utilizarlo dentro de los tres meses siguientes.
Dilución y almacenamiento de muestras
Como muestra para el examen se pueden usar los siguientes fluidos del cuerpo humano: suero y plasma citratado. Las muestras con contaminación bacteriana, hemolíticas o de quilosano y los anticoagulantes contenidos en el plasma (excepto el citrato) pueden afectar el resultado de la prueba.
Las muestras se pueden almacenar a entre + 2 ºC y + 8 ºC durante una semana. Para un almacenamiento más prolongado, almacene las muestras a -20 ºC. Las muestras diluidas deben utilizarse lo antes posible.
7 Preparación de soluciones de trabajo
Diluya la solución de lavado 1:20 (1 parte de solución y 19 partes de agua destilada). Por ejemplo, 75 ml de solución de lavado concentrada + 1425 ml de agua destilada.
En el frasco con solución de lavado concentrada se pueden formar cristales de sal. Es necesario disolver estos cristales antes del uso calentando el frasco en un baño de agua caliente. La solución de lavado diluida es estable durante una semana a entre +2 ºC y +8 ºC.
Los controles y los calibradores se suministran en la solución de trabajo, ¡no diluir más!
El conjugado se suministra en la solución de trabajo, ¡no diluir más!
Solución de TMB es una solución de sustrato cromogénico de un componente en dilución de trabajo, ¡no diluir más!
Intercambiabilidad de las soluciones

8 Dilución de muestras
Mezcle bien antes del uso la solución de dilución de muestras.
Dilución de muestras de suero y plasma
Diluya las muestras bien mezcladas 1:101 en solución de dilución de muestras:
p. ej.: 10 µl de muestra + 1 ml solución de dilución de muestra
Mezcle bien.
Diluya suero fetal y neonatal a 1:101 con solución de dilución de muestras:
p. ej.: 10 µl de suero + 1 ml de solución de dilución de muestras
Mezcle bien.
9 Procedimiento de trabajo
Deje que todos los reactivos se alcancen la temperatura del laboratorio y mezcle bien. Si no utiliza toda la placa, devuelva las tiras sobrantes al embalaje con secante, ciérrelo herméticamente y almacénelo a entre +2 ºC y +8 ºC. ¡Protéjalo de la humedad!
1. Dosifique los controles (calibradores) y las muestras diluidas según el esquema de trabajo.
Valoración semicuantitativa en el Índice de positividad (IP)
• Deje el pocillo A1 vacío (blank).
• Pipetee 100 µl de control negativo (calibrador 1) en 1 pocillo.
• Pipetee 100 µl de CUT-OFF (calibrador 2) en 2 pocillos.
• Pipetee 100 µl de control positivo (calibrador 3) en 1 pocillo.
• En los pocillos restantes pipetee 100 µl de muestras en la dilución correspondiente (véase el capítulo Dilución de muestras).
Evaluación cuantitativa en unidades UI/ml
• Deje el pocillo A1 vacío (blank).
• Pipetee 100 µl de control negativo (calibrador 1) en 1 pocillo.
• Pipetee 100 µl de CUT-OFF (calibrador 2) en 2 pocillos.
• Pipetee 100 µl de control positivo (calibrador 3) en 2 pocillos.
• Pipetee 100 μl del calibrador 4 en 2 pocillos.

2. Cubra la placa con una cubierta e incúbela a 37 ºC durante 60 minutos.
3. Absorba el contenido de los pocillos y lave 5 veces con solución de lavado de trabajo. Llene los pocillos hasta el borde superior. Finalmente, sacuda bien los restos de solución en material absorbente.
4. Dosifique 100 µl conjugado en todos los pocillos excepto A1.
5. Cubra la placa con una cubierta e incúbela a 37 ºC durante 60 minutos.
6. Absorba el contenido de los pocillos y lave 5 veces con solución de lavado de trabajo. Llene los pocillos hasta el borde superior. Finalmente, sacuda bien los restos de solución en material absorbente.
7. Pipetee 100 µl de solución de TMB en todos los pocillos. Cuidado con la suciedad, véase el capítulo Condiciones técnicas.
8. Cubra la placa con una cubierta e incúbela a 37 ºC durante 20 minutos. Manténgala protegida de la luz.
9. Detenga la reacción añadiendo 100 µl de solución de parada en el mismo orden e intervalos como se dosificó el sustrato.
10. Mida en el fotómetro a una longitud de onda de 450 nm la intensidad del color de la solución en los pocillos contra el blank (pocillo A1), hasta 30 minutos tras pararse la reacción.

12 Valoración de los resultados
Cálculo del índice de positividad (IP)
Divida la absorbancia de la muestra de prueba por la absorbancia CUT-OFF media, medida en la misma serie de exámenes:

El examen de muestras de límite, es decir, con un índice positivo de 0,9 a 1,1, se debe repetir a partir de una nueva muestra recogida entre 2 y 6 semanas después, teniendo en cuenta las especificidades de la enfermedad.
Evaluación cuantitativa en unidades internacionales (UI/ml)
Cree una curva de calibración trazando la concentración (X) de los calibradores en UI/ml contra la absorbancia correspondiente (Y). Cree la curva de calibración mediante conexión transversal de un solo punto. Lea los valores de nivel de anticuerpos (UI/ml) en las muestras de la curva de calibración.
Si el nivel de anticuerpos en la muestra es superior a 240 UI/ml, diluya la muestra en dos pasos y repita la prueba (dilución final a 1:1010).
Paso 1. Diluya la muestra a 1:101 con la solución de dilución de muestras: 10 μl de la muestra + 1 ml de la solución de dilución de muestras.
Paso 2. Diluya 100 μl de la muestra diluida en el Paso 1 con 900 μl de la solución de dilución de muestras.
Multiplique las lecturas de concentraciones de anticuerpos de la curva de calibración por 10.

El examen de muestras límite se debe repetir a partir de una nueva muestra tras entre 2 y 6 semanas, teniendo en cuenta las especificidades de la enfermedad.
El hallazgo serológico se puede interpretar solo en el contexto de los resultados de los otros tests de laboratorio y con el cuadro clínico del paciente.
13 Características de rendimiento
13.1 Especificidad y sensibilidad
La especificidad diagnóstica se determinó en el panel de sueros negativos, el número de sueros analizados fue 50. La sensibilidad diagnóstica se determinó en el panel de sueros positivo, el número de sueros analizados fue 89. La especificidad y la sensibilidad se basan en los resultados observados.
Especificidad: IgG 100,0 %
Sensibilidad: IgG 98,9 %
13.2 Precisión y reproducibilidad
La reproducibilidad intraensayo se determinó mediante el análisis de muestras en distintos niveles de reactividad de anticuerpos durante al menos 16 veces en un análisis. El coeficiente de variación (CV) de las muestras de IgG reactivas fue del 5,6 %.
La reproducibilidad interensayo se determinó mediante el análisis de muestras de distintos niveles de reactividad de los anticuerpos en 10 procedimientos de análisis diferentes. El CV de las muestras de IgG reactivas fue del 7,3 %.
14 Seguridad en el trabajo
El kit está destinado solo para fines de diagnóstico in vitro.
Los sueros utilizados en la producción de controles se examinaron para detectar VIH 1 y VIH 2, HBsAg, VHC, TPHA, con resultado negativo. A pesar de ello, es necesario tratarlos como material potencialmente infeccioso.
Algunos reactivos contienen un componente tóxico, la azida de sodio. Evite el contacto con la piel.

La solución de parada contiene solución diluida de ácido. Evite el contacto con los ojos y la piel.
Es necesario seguir la normativa local en relación con la seguridad en el trabajo.

Primeros auxilios
En caso de contacto con los ojos, enjuague los ojos con abundante agua tibia y busque ayuda médica. En caso de contacto con la ropa y la piel, retire toda la vestimenta contaminada. Lave la piel con abundante agua y jabón. Desinfecte la piel en caso de contacto con soluciones que contengan plasma o muestras clínicas. En caso de ingestión accidental enjuáguese la boca con agua potable y busque ayuda médica.
Eliminación de los restos
Todos los accesorios utilizados para el test es necesario considerarlos como potencialmente infecciosos debido al contacto con material biológico. Por ello, debe deshacerse de ellos junto con los residuos biológicos.
Eliminación de kits caducados
Desmonte el kit y deseche los componentes como material biológico. Deseche el embalaje y los residuos del embalaje de acuerdo con las normativas locales.
15 Condiciones técnicas
Para lograr resultados fiables, es necesario cumplir estrictamente con el manual. Utilice siempre puntas y materiales de vidrio limpios y preferiblemente desechables.
Placa de microtitulación – antes de abrir, deje siempre que la bolsa con la placa de microtitulación alcance la temperatura ambiente para evitar la condensación del vapor de agua en la superficie de la placa.
Solución de lavado – utilice agua destilada de alta calidad para preparar la solución de lavado en dilución de trabajo.
Lavado – siga el número prescrito de ciclos de lavado y llene siempre el pocillo hasta el borde superior. El tiempo en que los pocillos permanecen llenos con la solución entre ciclos de lavado (tiempo de remojo) debe ser de unos 30-60 segundos.
Solución de TMB – no utilice para otras soluciones de trabajo el recipiente utilizado para el pipeteado multicanal. No devuelva el resto de la Solución de TMB del recipiente de pipeteado al vial.
Los resultados no reproductibles pueden venir de errores metodológicos, en especial:
• mezcla insuficiente de la solución y las muestras antes del uso
• reemplazo inadecuado de las tapas de los viales
• uso de la misma punta para pipetear diferentes soluciones

• exposición del reactivo a una temperatura excesiva; contaminación bacteriana o química
• lavado o llenado insuficiente los pocillos (los pocillos se deben llenar hasta el borde superior), mala aspiración de los restos de solución de lavado
• contaminación de los bordes de los pocillos con conjugado o muestras
• uso de reactivos de distintos lotes de kits
• contacto de los reactivos con antioxidantes, metales pesados y sus sales
El kit se puede utilizar para análisis secuenciales. Cuando prepare las soluciones de trabajo, utilice solo la cantidad de reactivos que se necesite para el análisis.
El kit se podría utilizar en todos los tipos de analizadores de ELISA automáticos.
Si es necesario, DIAsource ImmunoAssays S.A. puede ofrecer una modificación certificada de las instrucciones de uso para tipos concretos de analizador.
En caso de no observarse estrictamente las instrucciones del procedimiento de trabajo, el fabricante no está en disposición de garantizar el correcto funcionamiento del kit.
16 Índice de avidez
16.1 Introducción
La avidez de los anticuerpos expresa la fuerza de la unión entre el antígeno y el anticuerpo. En las fases iniciales de la infección primaria se producen anticuerpos de baja avidez. A medida que progresa la infección, la respuesta inmunitaria del organismo madura y la avidez de los anticuerpos aumenta. Los anticuerpos muestran una gran avidez en la fase latente de la enfermedad. Desde el comienzo de una infección secundaria o una reactivación, las células B con memoria producen anticuerpos de clase IgG de gran avidez.
La determinación de la avidez de los anticuerpos de clase IgG permite diferenciar distintas etapas de la infección y es una adición útil para el diagnóstico serológico.
16.2 Principio del test
La determinación de la avidez se basa en la disociación de la unión antígeno-anticuerpo por medio de la Solución de avidez (solución de urea). Después del tratamiento con la Solución de avidez, se liberan y eliminan mediante lavado los anticuerpos de baja avidez, mientras que los anticuerpos de gran avidez permanecen unidos al antígeno. La fuerza de la unión se expresa por el Índice de avidez (IAv). El IAv determina la porción de anticuerpos de clase IgG que permanece unida al antígeno después de la incubación con la Solución de avidez. El procedimiento de análisis del IAv es una modificación del procedimiento ELISA estándar utilizando

Solución de avidez. La avidez del anticuerpo IgG únicamente se determina en muestras positivas para IgG.
La valoración de los resultados del 90 % de las muestras de suero no se verá afectada por la diferencia en la dilución del suero, es decir, si la muestra se ha diluido a 1:101 o más. Sin embargo, el análisis del IAv del 10 % del suero diluido a 1:101 provoca resultados con una avidez elevada, mientras que una mayor dilución de dicho suero puede producir resultados de baja avidez. Los resultados óptimos se obtienen para una concentración de anticuerpos aproximada de 60 UI/ml. Por lo tanto, para la prueba se puede utilizar el análisis de un punto o el de tres puntos.
Análisis de un punto
Determine cuantitativamente el nivel de anticuerpos en suero (en UI/ml) utilizando el procedimiento habitual (véase el capítulo Procedimiento de análisis). A continuación, diluya la muestra analizada con solución de dilución para muestras a 60 UI/ml y determine el IAv en análisis ELISA posterior.
Análisis de tres puntos
El IAv viene directamente determinado sin ningún análisis previo. En este procedimiento, se analiza simultáneamente tres concentraciones séricas distintas: la muestra de suero se diluye a 1:101 (dilución básica) y luego se diluye 5× y 25×. Este procedimiento resulta especialmente útil en situaciones en las que el tiempo es un factor crítico cuando se necesitan resultados rápidos (exámenes STAT).
16.3 Preparación de soluciones de trabajo
Deje que los reactivos, incluida la Solución de avidez, alcancen la temperatura ambiente y mézclelos bien. Podrían formarse cristales en el vial con la Solución de avidez. Antes del uso, es necesario disolver los cristales mediante un calentamiento breve. La funcionalidad de la Solución de avidez se indica por la aparición de color amarillo. La solución es termolábil. Si el color de la solución cambia de amarillo a rojo, indica que se ha deteriorado. Una Solución de avidez de color rojo ya no se puede utilizar.
16.4 Análisis de un punto
16.4.1 Dilución de muestras
Determinación de la concentración de anticuerpos en UI/ml
Determine la concentración de anticuerpos (UI/ml) mediante la prueba ELISA de IgG de toxoplasma tal como se describe (véase el capítulo Procedimiento de análisis).
Dilución previa a 60 UI/ml
1. Si la concentración de anticuerpos es inferior a 60 UI/ml, no diluya la muestra.

2. Si la concentración de anticuerpos se encuentra en el rango de 60 a 240 UI/ml, diluya la muestra con solución de dilución de muestras a 60 UI/ml (p. ej., diluya la muestra tres veces si el contenido de anticuerpos es 180 UI/ml).
3. Si la concentración de anticuerpos es superior a 240 UI/ml, diluya la muestra a 1:1010, repita el análisis y multiplique el resultado por 10. A continuación, diluya la muestra con solución de dilución de muestras a 60 UI/ml en función de la concentración de anticuerpos obtenida.
Dilución de muestras para análisis del IAv
Diluya la muestra de suero diluida previamente (60 UI/ml) a 1:101 con la solución de dilución de muestras (10 µl de muestra + 1 ml de solución de dilución de muestras). Mezcle bien.
16.4.2 Procedimiento de trabajo
1. Dosifique los controles y las muestras diluidas según el esquema de trabajo.
• Deje el pocillo A1 vacío (blank).
• Pipetee 100 µl de control negativo (calibrador 1) 0,1 UI/ml en 1 pocillo de tira N.
• Pipetee 100 µl de CUT-OFF (calibrador 2) 6 UI/ml en 1 pocillo de tira N.
• Pipetee 100 µl del control positivo (calibrador 3) 60 UI/ml en dos pocillos adyacentes de tiras N y Av.
• Pipetee 100 µl del calibrador 4 (240 UI/ml) en dos pocillos adyacentes de tiras N y Av.
• Pipetee 100 µl de las muestras diluidas en dos pocillos adyacentes de tiras N y Av.

16.5 Análisis de tres puntos
16.5.1 Dilución de muestras
Diluya cada muestra en 3 pasos consecutivos y determine el IAv de las tres diluciones.
Paso 1. Diluya suero a 1:101 con la solución de dilución de muestras:
10 µl + 1 ml de la solución de dilución de muestras (dilución final 1:101)
Paso 2. Diluya el suero preparado en el Paso 1 con la solución de dilución de muestras:
100 µl + 400 µl de la solución de dilución de muestras (dilución final 1:505)
Paso 3. Diluya el suero preparado en el Paso 2 con la solución de dilución de muestras:
100 µl + 400 µl de la solución de dilución de muestras (dilución final a 1:2525)
16.5.2 Procedimiento de trabajo
1. Dosifique los controles y las muestras diluidas según el esquema de trabajo.
• Deje el pocillo A1 vacío (blank).
• Pipetee 100 µl de control negativo (calibrador 1) 0,1 UI/ml en 1 pocillo de tira N.
• Pipetee 100 µl de CUT-OFF (calibrador 2) 6 UI/ml en 1 pocillo de tira N.
• Pipetee 100 µl del control positivo (calibrador 3) 60 UI/ml en dos pocillos adyacentes de tiras N y Av.
• Pipetee 100 µl del calibrador 4 (240 UI/ml) en dos pocillos adyacentes de tiras N y Av.
• Pipetee 100 µl de muestras diluidas a 1:101 en el Paso 1 en dos pocillos adyacentes de tiras N y Av.
• Pipetee 100 µl de muestras diluidas a 1:505 en el Paso 2 en dos pocillos adyacentes de tiras N y Av.
• Pipetee 100 µl de muestras diluidas a 1:2525 en el Paso 3 en dos pocillos adyacentes de tiras N y Av.

El procedimiento de seguimiento es idéntico para el análisis de un punto y para el de tres puntos.
1. Cubra la placa con una cubierta e incúbela a 37 ºC durante 60 minutos.
2. Absorba el contenido de los pocillos y lave 5 veces con solución de lavado de trabajo. Llene los pocillos hasta el borde superior. Finalmente, sacuda bien los restos de solución en material absorbente.
3. Pipetee 100 µl de solución de avidez en todos los pocillos de tiras Av.

4. Pipetee 100 µl de la solución de lavado en dilución de trabajo en todos los pocillos de tira N.
5. Cubra la placa con una cubierta e incúbela a 37 ºC durante 10 minutos.
6. Absorba el contenido de los pocillos y lave 5 veces con solución de lavado de trabajo. Llene los pocillos hasta el borde superior. Finalmente, sacuda bien los restos de solución en material absorbente.
7. Pipetee 100 µl de conjugado en todos los pocillos excepto A1.
8. Cubra la placa con una cubierta e incúbela a 37 ºC durante 60 minutos.
9. Absorba el contenido de los pocillos y lave 5 veces con solución de lavado de trabajo. Llene los pocillos hasta el borde superior. Finalmente, sacuda bien los restos de solución en material absorbente.
10. Pipetee 100 µl de solución de TMB en todos los pocillos. Cubra la placa con una cubierta e incúbela a 37 ºC durante 20 minutos. Manténgala protegida de la luz.
11. Detenga la reacción añadiendo 100 µl de solución de parada en el mismo orden e intervalos como se añadió el sustrato.
12. Mida en el fotómetro a una longitud de onda de 450 nm la intensidad del color de la solución en los pocillos contra el blank (pocillo A1), hasta 30 minutos tras pararse la reacción.
16.7 Validez del test
La validez del test se realiza en tiras N (véase el capítulo Validez del test).
El test es válido si:
El Índice de avidez del control positivo (Cal 3) es inferior al 30 %.
IAv < 30 %
El Índice de avidez del calibrador 4 es superior al 60 %.
IAv > 60 %

16.8 Valoración de los resultados
Cálculo del Índice de avidez (IAv)
Divida la absorbancia de una muestra analizada en una tira Av entre la absorbancia de la muestra analizada en la tira N medida en la misma serie de exámenes:
La valoración de los resultados del test se describe en la Tabla 3.
Si se utiliza el análisis de tres puntos, calcule el IAv para diluciones de la muestra a 1:101, 1:505 y 1:2525. Determine la solución de dilución de muestras con el nivel de anticuerpos más próximo a 60 UI/ml. El IAv de esta solución de dilución de muestras representa el resultado de IAv relevante.

Notas a la valoración
• Menos del 5 % de los pacientes que superaron la toxoplasmosis y su enfermedad no son agudos y muestran una avidez baja incluso transcurridos 4 meses desde la infección. El Índice de avidez sigue siendo inferior o igual a 30. Normalmente ocurre en suero con una baja concentración de IgG antitoxoplasma. Las muestras con una concentración de IgG específico inferiores a 25 UI/ml, especialmente cuando están diluidas (para análisis de tres puntos), pueden arrojar valores de IAv indebidamente bajos.
• El análisis de tres puntos no se puede evaluar si la concentración de IgG en una muestra es superior a 5000 UI/ml.
• El examen de muestras con valores límite se debe repetir con una nueva muestra recogida al cabo de 3-4 semanas.
• Tanto los niveles altos como bajos de anticuerpos IgM e IgA específicos pueden persistir en algunos pacientes con valores de IAv elevados.

• Los resultados del examen de avidez de IgG deben valorarse únicamente en el contexto de los resultados de otras pruebas, especialmente CFT, títulos de NIFR y niveles de IgG, IgM e IgA.
