- Launched 2012 as part of Europe 2020, aims to add 2 Healthy Life Years by 2020 through QoL, sustainability and growth/jobs.
- Stakeholder-led, cross sectoral, collaboration instrument.
- Showcases good practices, builds critical mass, aligns funding.

EIP on AHA Action Groups, formed of stakeholders working on commitments (projects), engage in collaborative work which both
creates pan-European cooperation structures and experience
creates useful knowledge products and tools for better ageing.
Outputs of the EIP on AHA Action Group on Integrated Care
• Maturity Model and results from 12 regions self-assessment
• Concrete demonstrations of integrated care models / services in these regions
• Demonstrations of practices covering range of themes / topics
• Tools and resources from Toolkit – covering organisational models, change management, workforce development, risk stratification, care pathways, citizen empowerment, ICT / Teleservices and Finance / funding models
Main Successes of the Action Group on Integrated Care
• Growing integrated care stakeholder network as a result of dissemination at European and regional forums.
• Scaling up integrated care pathways – e.g. AIRWAYS ICP – through successful engagement with regional health ministries
• Evidence of inter-regional collaboration – e.g. Memorandums of Understanding between Scotland with Catalonia, Greece and Basque Country
• Development of regional EIP networks – e.g. Greece, Puglia, Languedoc Roussillon, etc
• 2015: Good Practice “pioneer” regions will coach “follower” regions
JRC Monitoring Tool (MAFEIP project) Develop a model to link interventions to QoL, Sustainability and Jobs/Growth
2015: Development of tool (Excel-based) to estimate outcomes of the EIP on AHA on health as well as the sustainability of health and care systems
Tool aimed to be useful individually or on aggregated level.
Distribution of the pilot first to EIP on AHA partners to see how it can be used.

Objective: provide an overview of available:
Assessment tools of citizens' well-being in countries, regions or cities, and performance of health systems.
Decision tools to decide on new policies regarding well-being of citizens, and the introduction of new health technologies including:
