INTERNATIONAL INSIGHTS: MEDICAL EDUCATION INNOVATIONS IN COLOMBIA AND THAILAND On June 26th, UCSF Fresno’s Global Health Curriculum presented Dr. Bjorn Nilson and Dr. Pegah Mashayekhi, who reflected upon their journeys and experiences in Thailand and South America. In Thailand, Dr. Nilson directs a program to educate medics serving the hill regions of the Thai-Myanmar border, a region beset by both geographical and medical challenges. Dr Nilson’s curriculum has allowed local citizens to learn and practice basic medical tactics guided by “Where there is no Doctor” and continuously enhanced with digital photography and internet-based spot education. Meanwhile, in South America, Dr. Mashayekhi rotated with an internal medicine team and sleep clinic service in Colombia.
The program where Dr. Nilson teaches consists of 23 students who attend for 6 weeks over the course of 3 years, after which they apply their knowledge and skills within their geographically isolated and culturally diverse communities. These students came from various parts of Myanmar to the northern Thai city of Chiang Mai, many travelling long distances over several days. The use of technology, such as audience response systems and the internet, has allowed the clinical materials to be taught efficiently and encourage communication between the teachers and students. Dr Nilson, in his always engaging and informal style, shared 8 memorable cases with our group: trauma, djenkolic acid poisoning,
pediculosis capitis, community acquired pneumonia, palliative care, meliodosis, dental cariers, and leprosy or Hansen’s disease. It was evident right away that a world class education was being delivered to the learners with whom Dr Nilson interacted, because the images and photos were excellent in quality and the cases were spot-on in their clinical details. This lecture was more than just show and tell: what Dr Nilson described was a brand new way of practicing and teaching medicine, one which will end up empowering the local providers long after the program ends. The images that Dr Nilson’s students are collecting are indeed a sort of atlas of life in the remote Burmese mountains— the students are given a camera and journal to record various cases in their villages. In the second and third year the students present their cases to the instructors, and together they discuss management goals and followup parameters for the next time they see the patient or a similar case. This use of real world cases, despite being separated by time and distance, promises to result in a much
more nuanced and resilient educational model than the conventional 1-size-fits-all packaging that much global health education aspires to. Additionally, the use of audience response clickers helps identify trouble spots in the didactic curriculum, so learner misconceptions are dispelled immediately during lectures and meetings. This makes sure that the group’s time is spent as well as possible when everyone is gathered together once a year. In Colombia, Dr. Mashayekhi completed a month rotation— two weeks on a medical wards team and two weeks doing sleep medicine clinic at an academic medical center amidst the beautiful culture and food of South America. She saw patients with tropical diseases, along with cases of cardiovascular disease, HIV, AIDS, TB, and pneumonia. A typical day at the hospital started with a lecture at 7 am, followed by rounds starting at 9 am, lunch around 1-2 pm, ending with another lecture at 4 pm; education was highly emphasized throughout the program. And, to top it all off, all lectures and patient interactions were conducted entirely in Spanish, allowing her to polish her language skills every minute of the day. Dr. Mashayekhi will now continue her education in sleep medicine at Baylor University, Texas.
