BACKGROUND AND OBJECTIVES: In recent years, circannual variations in incidence and mortality for venous thromboembolic disease have been demonstrated, with a peak in winter. However, several investigators have observed no seasonal variation in these diseases. The aim of our study was to investigate whether a seasonal variation, in terms of atmospheric pressure, humidity, and temperature, exists for pulmonary thromboembolism. METHOD: We retrospectively included 206 patients with a diagnosis of pulmonary embolism (PE) between 1 June 2001 and 31 May 2006. RESULTS: The highest number of cases in the 5 years concerned occurred in May (29 cases). Although PE occurred most commonly in the spring (72 cases) and autumn (51 cases), the difference was statistically significant (P = .003). There were no case correlations with months and pressure, temperature, or humidity. However, there was a statistically significant positive correlation between case incidence and atmospheric pressure (r = 0.53, P < .0005) and humidity (r = 0.57, P < .0005). In terms of risk factors, seasonal distribution was not statistically significant as regards cases of embolism occurring for surgical or nonsurgical reasons (r = 0.588).
CONCLUSION: In terms of the relationship between seasons and embolism cases, despite the determination of an insignificant positive correlation, a statistically significant positive correlation was determined between air pressure and humidity and case incidence. There is now a need for further wide-ranging prospective studies including various hematological parameters to clarify the correlation between PE and air pressure.

In a one month prospective study of 62 rheumatic patients-16 with
rheumatoidarthritis (RA), 24 with osteoarthritis (OA), 11 with
inflammatory arthritis, 11 with fibromyalgia joint pain-swelling and
everyday activity was compared with changes in daily weather
conditions. In most patients weather changes increased arthritic
symptoms. Women were more sensitive to weather than men (62% v
37%). Pain was affected positively by
barometric pressure and temperature in RA,
by temperature, rain, and barometric pressure in OA,
and by barometric pressure in fibromyalgia. These results support
the belief of most rheumatic patients that weather conditions
significantly influence their day to day symptoms.

In daily clinical practice, many patients attribute joint pain to weather conditions. It was a prospective, double-blind study including 92 patients with rheumatic disorders (80 with OA, 12 with RA) compared to a control group of 42 subjects. The evaluation of pain (VAS) and functional capacity HAQ) were determined daily during one month. The climatic variables studied were temperature, humidity and barometric pressure. Our data demonstrate that osteoarthritic patients experience increased joint pain in response to a decrease in pressure, indicating that low atmospheric pressure conditions exacerbate joint pain in these patients. Our work also suggests that some meteorological variables affect the occurrence of pain in rheumatoid arthritis, since we have found that low temperature increases the risk of joint pain. Therefore, these data suggest that in the future it may be possible to modulate pharmacological and non-pharmacological treatments for some osteoarthritic patients depending on the predictable weather conditions in order to avoid, as much as possible, the disease-associated joint pain and functional incapacity, thus improving patients' quality of life.

Objectives: To assess the effectiveness of climatotherapy at the Dead Sea on the production of vitamin D in Norwegian patients suffering from various rheumatic diseases and to investigate possible associations between increased vitamin D serum levels, musculoskeletal symptoms and disease severity.
Methods: Sixty Norwegian patients who came to the Dead Sea area for 21 days of medical rehabilitation were divided into three groups according to their diagnosis: chronic pain syndromes, i.e., low back pain or fibromyalgia (Group 1, n=33); rheumatoid arthritis (Group 2, n=16); and osteoarthritis (Group 3, n=11). Serum 25-hydroxyvitamin D (25-OHD) levels were determined at arrival and prior to departure. The treatment protocol included daily sun exposure (climatotherapy), bathing in the Dead Sea and mineral spring water (balneotherapy), mud applications and fitness classes.

The aim of this study was to evaluate the effectiveness of aerobic exercise in water pool compared with aerobic exercise performed in sea by women with fibromyalgia (FM). A total of 46 patients were randomly allocated into two groups: pool group (23 patients) and sea group (23 patients) that performed the same aerobic exercise program. Patients were evaluated baseline and after 12 weeks using: VAS, number of tender points, FIQ, SF-36, PSQI, and BDI. Both groups improved significantly in post-treatment for all the evaluated variables. There were no significant differences between two groups, except for BDI (F = 2.418, P < 0.0001). Aerobic exercise program performed in water (pool or sea) was effective for patients with FM. However, sea water exercises have been shown to bring more advantages related to emotional aspects. Then, exercise performed sea water (thalassotherapy) is an option for effective treatment with low cost for patients with FM.

Objectives. To study the effect of a combination of thalassotherapy, exercise and patient education in people with fibromyalgia.
Methods. Patients with fibromyalgia, selected from a rheumatology out-patient department and from members of the Dutch fibromyalgia patient association, were pre-randomized to receive either 2½ weeks of treatment in a Tunisian spa resort, including thalassotherapy, supervised exercise and group education (active treatment) or treatment as usual (control treatment). Primary outcome measure was health-related quality of life, measured with the RAND-36 questionnaire. Secondary measures included the Fibromyalgia Impact Questionnaire, the McGill Pain Questionnaire, the Beck Depression Inventory, tender point score and a 6-min treadmill walk test.
Sonuç: Talassoterapi, egzersiz ve hasta eğitimi fibromiyalji semptomları ve sağlıkla ilişkili yaşam kalitesinde geçici iyileşmeye yol açabilir

Objectives: To investigate the long-term effect (week 16) of a 4-week rehabilitation program for patients with rheumatoid arthritis (RA) and to compare the effect of this intervention given in a
Mediterranean or a Norwegian climate. Methods: A randomized, controlled, parallel group design, where 124 RA patients applying for rehabilitation were
randomized to a rehabilitation program either in Norway or in a Mediterranean climate. The participants were examined clinically immediately before (week 0) and after (week 4) the rehabilitation period as well as in week 16 and answered a mailed questionnaire in week 28. The 28-Joint Disease Activity Score (DAS28), American College of Rheumatology (ACR) response and physical tests were used to measure clinical response.
Conclusions: RA patients showed immediate positive effects with regard to disease activity, physical function, and symptoms during a 4-week rehabilitation program . The effects on disease activity and symptoms were larger and better maintained at least 3 months after rehabilitation in a warm rather than in a cold climate.
Sonuç: Sıcak iklimde yapılan rehabilitasyon uygulamalarının hastalık aktivitesi ve semptomlar üzerine etkileri soğuk bölgede yapılanlardan daha fazlaydı ve takip edilen 3 ay boyunca böyle devam etti.

Strumse YAS, Stanghelle JK , Utne L et al. Treatment of patients with postpolio syndrome in a warm climate. Disability and rehabilitation, 2003; vol. 25, no. 2, 77 ± 84
Purpose: Treatment in warm climate of various patient groups
including patients with postpolio syndrome is controversial. Method: Eighty-
eight patients with postpolio syndrome (61 women) were recruited, stratified according to sex, age (above/ below 60 years old) and use/not use of electrical wheelchair, and randomized to three groups. Group 1 (n=30) underwent treatment in a rehabilitation centre in Tenerife for four weeks in November/December 1999. Group 2 (n=29) were treated in two similar centres in Norway for the same period of time, while Group 3 (n=29), the control group, followed their ordinary health care programme. All patients were tested at the start of study, and 3 and 6 months later, including physical tests and several questionnaire and qualitative interviews. Patients in Group 1 and 2 were also tested after the rehabilitation period. Results: Group 1 and 2 improved significantly both in physical tests and subjective ratings. The positive effects in Group 1 tended to exceed the positive effects in Group 2, and the effects lasted longer. Six minutes walking distance in the two groups was 347 m and 316 m, respectively, before the treatment period, 429 m and 362 m immediately after, and 431 m and 356 m 3 months later. Subjective rating of pain (VAS-scale) was 42 and 43, respectively, before treatment, 17 and 31 immediately after, and 28 and 44 3-months later. In the control group, only minor changes were found.
Conclusions: The study seems to document a positive effect of treatment of patients with postpolio syndrome in warm climate.
Sonuç: Bu çalışma sıcak iklimde yapılan postpolio sendromu tedavisinin olumlu etkilerini dokümente etmektedir.

Objective: Several studies have shown positive effects of treatment of chronic diseases in a warm climate.
The aim of this study was to evaluate the long-term effect of a 4-week rehabilitation programme in a warm climate for patients with neuromuscular diseases. Design: A randomized controlled trial with a cross-over
design. One period of intervention and one period of “life as usual”. Patients: A total of 60 persons with a neuromuscular diagnosis. Methods: Long-term effects were defined as changes in physical and psychological functions persisting after 3 months. Several scales were used according to the World Health Organization’s classification of functioning. Results: A comparison of the changes in the 2 periods showed significantly better results for all primary outcome scales in favour of the intervention. Mean difference in changes in pain (VAS scale), 6-min walking test and “timed up and go” were 9.0 (SD 28.8) units, 52 (75) m and 1.0 (2.3) sec, p = 0, 03, < 0.01 and 0.01, respectively. Median difference in changes in “Fatigue Severity Scale” and “Life Satisfaction Scale” were 0.4 (–
0.5, 1.7) and 0.0 (0.0, 1.0), p = < 0.01 and 0.01, respectively. Conclusion: This study shows positive long-term effects on different dimensions of health after a 4-week rehabilitation programme in a warm climate for patients with neuromuscular diseases. This effect might be due to the programme, the warm climate, or a combination of both.
Sonuç: Bu çalışma nöromuskuler hastalığı olan kişilere sıcak iklimde uygulanan 4 haftalık rehabilitasyon programının sağlık üzerine farklı boyutlarda uzun süreli etkilerini göstermektedir. Bu etki programdan, sıcak iklimden veya her ikisinin kombinasyonundan kaynaklanıyor olabilir.
