INFORMATION1. All children who will be three (3) years old by October 1, 2010 are eligible for the Three (3) Year

Old Preschool Program.

2. Allchildren who will be four (4) years old by October 1, 2010 are eligible for pre-kindergarten.

3. Allchildren who will be five (5)years old by October 1, 2010 are eligible for kindergarten.

4. Children MUSTbe present at registration.

5. Parents/Guardians must accompany ALLchildren.

6. Guardians must have guardianship papers.• - ?

7. Proof of residence must be in the name of the child's parent or guardian. If it is in someone else'sname, that person must be present at the time of registration with proof of residence and a personalPhoto ID.

8. Any student with a history of positive Mantoux (Tuberculin Test) must present the nurse withdocumentation of a chest X-ray report and/or INHtherapy at the time of registration.

9. Bilingual services are available at the Enrollment Center.10. Parents/guardians of special needs students must call for an appointment prior to registration,
