YAYASAN RAMA SESANA Creating a healthy generation through women
We are a non-profit, non-government organization. We were founded in 1999 and have provided free education
programs and donation-based health services focusing on sexual and reproductive health since 2004. We work in Bali’s
traditional markets because they are the centre of Balinese community life.
Our Vision:
To create a healthy generation through women’s empowerment. We want the disadvantaged women of Bali to become
empowered through knowledge and take control of their sexual and reproductive health. Then we want them to share
their knowledge with others, especially their daughters, their family and their friends to extend that empowerment. We
also want to improve the immediate health outcomes of these women so we will provide clinical services focussing on
sexual and reproductive health.

Dear Friends and Supporters,
I am very glad to share with you Yayasan Rama Sesana (YRS ) Annual Report 2017-2018, which
provides the summary of our achievements during the year through our challenges in the
temporary office at temporary traditional market. This year will be remembered as a year of
challenges, struggle and development.
YRS supported women in increase the awareness, knowledge, empowerment and access for
health services particularly low income women work in traditional market and women live in
the villages
The achievement of our program in this year bigger that the estimation and the projection
number we had. This happen because massive of promotion and increasingly of frequency of
services through the mobile clinic. The interesting part was also some of our repeat client felt
loss their clinic after the fire of Pasar Badung, they was find our temporary clinic at temporary
traditional market, and come again to our clinic with introduce the services to their friend and
the neighbor and bring them for the services.
We conducted the services in the two based clinic around traditional market and twenty times
of mobile clinic to the villages around Bali.
All of our achievement was not out from support of people, communities, organization, donors
and government as a part of our stakeholders.
We are very grateful to all our partners, donors and associates, who have constantly supported
us in all our endeavours, in particularly for Inspirasia Foundation to support us from the
beginning of YRS project more than money to develop YRS become the sustainable organization
in the future.

PROGRAM ACHIEVEMENT

To echo and instill reproductive health knowledge in people who work in traditional markets, we disseminate
information about reproductive health devices, STIs, HIV / AIDS, cervical cancer and breast cancer. The most important
thing besides disseminating information is educating them to be able to understand and prevent or treat existing
complaints and disseminate information obtained to their immediate family. Thus it can increase awareness of
reproductive health and slowly reduce the incidence of disease.

We educated 4933 people about cervical cancer prevention test especially Pap
test and See & Treat, 3487 people received breast cancer and breast check
examination and 3804 people received sexual reproductive health include HIV.
We delivered 1591 cervical cancer brochures, 955 breast cancer brochures and 363
HIV test brochures

Outreach is informal, face-to-face interaction where our
staffs deliver sexual and reproductive health information.
Outreach is the most convenient way for the women of
the traditional market communities’ method to get
information. Our staff approaches the beneficiaries with
light and friendly conversation before they start talking
about a variety of health topics. This ensures the women
are engaged and receptive.

OUTREACH
Small group discussions (SGD) usually include just 3 to 5
people so the participants feel comfortable enough to
share experiences and discuss sensitive issues. A staff
member guides the discussion and delivers information
according to the experiences and stories shared. The
discussions provide the opportunity for more complex
sexual and reproductive health issues to be explored
and explained. The discussions are conducted there are
enough people who are interested in a topic, most
commonly in the market.

MEDICAL PROGRAM Sexual & Reproductive Health Screening and Treatment in Traditional Market
Creating a healthy generation starts with a healthy
mother. Based on WHO data, the highest cause of
female mortality in the world is women cancer
which increases during years. In line with the
organization's mission, we provide reproductive
health services of cervical cancer screening, breast
cancer and STI to help decrease female mortality
rate in cancer.

The program activities are carried out in base clinic
in traditional markets in Denpasar, Tiara Grosir and
Sanur Market. From both clinic we serve cervical
cancer screening; breast cancer screening through
breast check; HIV test through VCT; STI test, family
planning and general health care. The graphic show

Cervical Cancer Screening We screen cervical cancer by Pap test and See & Treat.
This year is the second year we provide see and treat
optimal services method at both base clinic. Client with
suspicious cancer result in Pap test and See &Treat cure
with cryotheraphy and refer to higher health facilities.
Breast Cancer Screening Screening by ultrasound only conduct at Tiara Grosir
market clinic and mobile clinic. With this machine, mass
suspected from palpation can be allowed to more
specific diagnostic. Screened with ultrasound can help
women in the market and also low income women to
cut the burden on examination fees compared to if they
went to the hospital. Suspicious breast cancer result in
our USG test refer to the Prima Medika Hospital.
STI Test Sexual Transmitted Infection (STI) do not occur in high
risk community like sex worker but can be in low risk
community like housewife and traders. Our target in low
risk community aim to increase their sexual reprudcitive
health status by screened, cured and counseling of STI
problem.

WOMEN EMPOWERMENT
Community participation in YRS
program was very high, as evidenced
by the involvement of beneficiaries
become volunteer as Peer educators
(PEs). Those who are PEs are workers,
traders, housewives who are around
traditional markets and have been
trained by YRS staff so that they have
the capacity to help their friends in
raising awareness about sexual and
reproductive health.
PEs activities include training and
education about health from YRS
staff, PEs monthly meeting and
outreach in Tiara Grosir market,
brochure and condom distribution
and also as active participated in
various themes health campaign
every month. We had 12 PEs
actively join our peer activities and
reach 835 people educated by PEs

MOBILE CLINIC Sexual & Reproductive Health Education and Services in Remote Area
Thousands of Balinese women living in rural areas of the island have
extremely limited access to reproductive and sexual health services.
In order to increase their access and to improve their lives and
communities we travel to these areas to conduct a mobile clinic.
The mobile clinic services are for the entire community, not only for
women (though women of reproductive age are the demographic
accessing the services the most). The day before a mobile clinic our
staff prepares the equipment, supplies, medicines, forms, brochures
and information materials.

Medical activities An ad hoc‟ clinic is set up in the available space, and medical
services are provided from 07:30 am to noon. Services include
general health checks, breast self-examination, VCT consults and
Pap tests.
Educational activities At every mobile clinic a Big Group Discussion is conducted, as well
as outreach activities with clients who are waiting to be assessed
by staff.

SUCCESS STORY
Komang Suartono and his wife received counseling from our staff
Impact of Education Program
YRS educates traders and visitors about reproductive health every day in Tiara Grosir
Market both male and female. One of them is a fried rice seller named Mr Komang
Suartono. Mr. Komang suartono sells fried rice every day from morning to noon and he also often
checks his health to YRS. After educated by YRS specifically about women's reproductive
health, Mr. Nyoman Suartono recalled that his wife had a problem with her reproductive
organs.
"Arriving at home I told my wife to immediately examine herself related to her complaints,
because it is very dangerous if left silent and afraid become cancer, that's the explanation
that I got from YRS"
After routine visits to YRS, now her wife has not experienced complaints about the
reproductive organs and able to daily activity without any fear of complaints that were felt
before.
“I am very grateful to know YRS. YRS has provided information, does not burden the costs
and helps with the treatment that I complain about in the reproductive organs so that now I
feel healthy and know the importance of maintaining reproductive health. If my husband
does not get information about reproductive health from YRS and my husband does not
continuously persuade me to go to YRS, maybe my illness is now very severe"
Mr. Nyoman Hartono’s wife was one of the patients who had complaints about the
reproductive organs that were successfully treated by YRS. There are still many more
women who have the same complaints that YRS has managed to treat.

CHALLENGES
The bigger concern of YRS after the accident the fire in PasarBadung is, we have no permanent space for the
based clinic, and uncertain information related the new Pasar Badung after the renovation. To address this
issues and while waiting the certain information about the Pasar Badung movement to the new building, we
also consider the suggestion of Inspirasia Board member to provide the services in ALC, and approach the
Market Management and Government to provide the space for YRS when the the re-build Pasar Badung has
been complete and the market back again to Pasar Badung.
We conduct the feasibility studies to support the analysis that the potential space will be feasible for YRS
project. One of feasibility studies we conduct in ALC for the community around of ALC location. The result of
this study will use for the baseline data and recommendation to YRS base clinic and office.

Capital Management Policy
Our objectives when managing capital are to safeguard our ability to continue operations for
the benefit of the women of Bali. We manage our capital through a budgeting and strategic
planning process to ensure adequate funds and maintained to meet our current and future
objectives.
Capital Reserve
Central to our strategy is the awareness that we are dependent on restricted and unrestricted
donations. There is a constant risk that anticipated donations may not materialize and to
manage this risk we have in place a capital reserve. The purpose of this reserve is to make sure

we have sufficient cash to pay all liabilities including staff entitlements. The reserve is also
intended to fund operations for as long as possible in any period unsupported by donors.

Our major donor is Inspirasia Foundation and the grants that provided are tied to agreed
activities and projects. To ensure we are able to acquit the grants transparently we maintain
separate reserve accounts for major restricted donations.
