STUDENT INFORMATION PLAN (SIP)

COURSE DESCRIPTION

PRE-REQUISITES: Acceptance into the LVN program CO-REQUISITES: VNSG 1160 Clinical I, VNSG 1227 Essentials of Medication Administration VNSG 1423. Basic Nursing Skills (4 credits). Description: Mastery of entry level nursing skills and competencies for a variety of health care settings. Utilization of the nursing process as the foundation for all nursing interventions. Lab required. Learning Outcomes: The student will demonstrate competency in basic nursing skills; identify the steps in the nursing process and how each relates to nursing care; and discuss the delivery of basic nursing skills in a variety of health care settings. (3 lecture and 6 laboratory hours per week). The following required texts can be purchased at the ACC bookstore, which is open Monday through Thursday 7:30 a.m. to 7:00 p.m. and Friday 7:30 a.m. to 2:00 p.m. Bookstore hours can vary during holidays so please call the bookstore at 281-756-3681 for more information. COURSE OBJECTIVES

(EXPECTED OUTCOMES) The student will be able to: 1. Learn how to provide patient centered care by gaining understanding of how the nursing process

is utilized to meet the total needs of the patient during adaptation along the health - illness continuum.

2. Understand how to meet the patient’s physiological needs for safety against bacteriological,

environmental, and chemical injury in preparation for the nurse’s role of patient safety advocate. . 3. Learn how to meet the patient's physiological needs for environmental management, oxygen,

hygiene, comfort, rest, sleep, activity, nutrition, elimination, and fluid balance in order to provide safe, patient centered care.

4. Understand and begin to develop effective communication skills in order to provide patient

centered care, and in order to become an effective member of the health care team and member of the nursing profession.

5. Learn how to become a provider of patient centered care by understanding the patient's needs

for self-esteem, self-worth, dignity, and respect. 6. Learn how to become a provider of patient centered care by understanding the patient’s needs to

be useful, to belong, and to be productive. (Rehabilitation) 7. Learn how to provide patient centered care and prepare to become a member of the health care

team by assisting to meet the patient’s needs to know and understand. (Teaching) 8. Prepare to become a provider of patient centered care by understanding how to meet the

patient's needs for privacy and spirituality. 9. Understand how to become a member of the health care team, a member of the nursing

profession, and a provider of safe patient care by developing behaviors reflective of professional, ethical, legal, and responsible standards of practice consistent with the role of the vocational nurse.

VNSG 1423 BASIC NURSING SKILLS

COURSE COMPETENCIES

At the completion of this course, the student is expected to have achieved the following competencies. Each of the following competencies is correlated with the corresponding SCANS competencies and foundation skills. For further information on SCANS see the Appendix at the end of this SIP. 1. With at least 74.5% accuracy on written exams , identifies components of the nursing process in

providing basic nursing care that meets client needs for safety (environmental, bacteriological, chemical), oxygen and circulation, hygiene, comfort, communication, activity, nutrition, elimination, fluids, medication, oxygen supplement, aging and death. (SCANS C08, F1-F17)

2. Demonstrates competency in specified nursing knowledge and nursing skills via attaining a

minimum of 74.5% average on all exams combined and mastery of all skills taught in skills lab. 3. Assists to write a Nursing Care Plan for the adult / geriatric client in the acute care setting.

(SCANS F1-F17) 4. Learns how to execute nursing roles and fundamental nursing skills specific for the adult or

geriatric client. (SCANS C1-C20; F1-F17)

Role I. PROVIDER OF CARE

A. ASSESSMENT (SCANS C5, C8; F1-F17)

1. Completes a focused head-to-toe assessment of the client's health status using interviewing, inspection, palpation, auscultation, and olfaction.

2. Uses observation skills. 3. Measures objective data; e.g. vs, ht. wt, body measurements, specimen collection. 4. Utilizes various forms of communication to include verbal and written records, graphs,

forms, and computer entries (e.g. the Chart, Kardex, NCP, MAR).

B. PLANNING (SCANS C5-C7; F1-F17) 1. Implements the nursing process of assessment, planning, implementation, and

evaluation in contributing to the development of the NCP using nursing diagnoses (NANDA).

2. Prioritizes a plan of care so that it is time efficient and cost effective.

C. IMPLEMENTATION - Executes interventions (According to the Human's Hierarchy of Needs along the Wellness-Illness continuum) (SCANS C11, C14, C15, C19, C20; F1-F17) 1. Safety: HWT, universal precautions, sterile gloving, dressings, topical medication

applications. Environmental hazards, restraints, fall precautions. Performs procedures safely. Management of lines, tubes, and equipment.

2. Physiological needs for oxygen & circulation: PA, VS, NVS, CPR, O2 supplement, airway management

3. Hygiene and Environmental management: Bathing, AM care, oral hygiene (dependent and assisted). Bed making, esthetic environment.

4. Communication: Establishes nurse/patient relationship. Functions as a team member. 5. Comfort: Decubitus prevention and care. Rest, sleep, skin and back care, pain

management. Hot and cold applications. Administers topicals and toiletries. 6. Activity: Maintains body alignment. Turning, positioning, ROM, progressive

mobilization, transfer to chair, stretcher, amb. 7. Nutrition: NPO, serves tray, feeds client, NGT/GT feed/flush, bolus, pump regulation. 8. Elimination: Bedpan, urinal, incontinent care, catheter, enema, bladder train,

diversions. 9. Fluid & electrolytes balance: I&O, monitors IVs, measures weights and

circumferences. 10. Emotional / Learning needs: Supports and encourages the patient. Prep for Dx test,

surgery, specimen collection, procedures, and exams. Fosters developmental growth.

D. TEACHING (SCANS C10, C14; F1-F17) Reinforces patient teaching (of steps and rationale of nursing actions and treatments and in preparation for discharge care and rehabilitation).

E. EVALUATION & DECISION MAKING (SCANS C5, C7, C11; F1-F17)

1. Recognizes, records, and reports overt outcomes of individualized nursing care and client responses.

2. Considers alternative nursing actions based on evaluation of client responses that are consistent with established nursing protocols. Identifies client compliance.

EVALUATIVE PROCEDURES

(METHODOLOGY)

Exams: A minimum of 4 unit exams will be given and their average will be worth 50% of the total grade for the didactic portion of the course. A comprehensive final exam worth 50% of the total grade for the didactic portion of the course will be given. The average of these two values will comprise 90% of the Fundamental Skills course grade. A student who fails any exam, except the final exam, will be required to complete instructor-provided remediation material prior to the next exam for the student to take that exam. Quizzes: Students will be provided with a “Medical Terminology & Medical Abbreviation Independent Learning Module” during the first week of class. It contains a total of nine weekly quizzes. There is no final exam attached to this learning module. The scores of these nine quizzes will be averaged together. The average of these nine quizzes will comprise 10% of the final Fundamental Skills course grade.

90% for total Fundamental Skills didactic grade

The Fundamental Skills final course grade must be 74.5 or greater in order to pass the course. At the end of the semester there will be a proctored comprehensive Fundamentals ATI exam. Skills lab: The lab portion of the Fundamental Skills course is pass/fail. Students will be checked off on skills after being given sufficient time to practice the skills. If a student fails to successfully pass a skill, they must complete remediation via guided practice before attempting the skill again. The student will be given three opportunities to successfully complete a skill. If a student is unable to pass a required skill on the third attempt, he / she automatically fails the Fundamental Skills course, and may not move forward and must withdraw from the course.

LATE COURSE WORK POLICY

Students are expected to complete exams on date scheduled. Exam may be taken early if requested by student. Exams taken early or late will cover the same topics as the scheduled exam but may consist of alternate test items. Missed exams must be taken within 2 weeks of scheduled exam date. The final grade achieved by a student on a late exam will have a 15 point late penalty deducted from it (see Testing Policy in Student Handbook).

Absences for Exams: Exams missed due to absence will be made up in the Learning Lab or in the Vocational Nursing Department. MAKEUP EXAM POLICY

Any exams you miss will be made up in the nursing department or will be sent to the Alvin Community College Learning Lab for you to take as a makeup exam. Makeup exams are available in the Learning Lab 2 business days after the day you missed them.

Any makeup exams you take will be returned to your instructor and graded 2 to 4 business days after you took the makeup exam. Do not ask the VNSG secretary for exam grades; she does not have access to grade records.

The Learning Lab is located in the college library on the second floor of building A, to the left as you enter the front doors. Each semester the Learning Lab publishes an updated policy describing how to take exams in the Learning Lab. Refer to the hardcopy of this policy distributed by your instructor at the start of each semester, or online at http://www.alvincollege.edu/Current/Learning_Lab.cfm

If you have any problems using the Learning Lab to take makeup exams, notify your instructor or the VNSG secretary.

1. A 15 point penalty is subtracted from the score achieved by the student on the makeup exam. The highest grade that may be attained on makeup exams is 85%.

2. You must complete each makeup exam no later than 2 weeks after the exam was originally given to the class, or by the end-of-semester exam deadline published in the current semester’s Learning Lab policy, whichever occurs first.

3. Any makeup exams you do not complete within the above timeframe will be averaged in the applicable course grade as a zero ("0").
