Innovations in Teaching Technology: Virtual Microscopy / Andrea Barrett Slide 1 WHO is Virtual Microscopy?

Slide 2 Drs. Ralph Zottola, Roger Craig, and Tom Smith led the team that…
Slide 3 … investigated Virtual Microscopy a potential educational tool for both the first-year Histology and second-year Biology of Disease (Pathology component) courses. They gained approval for the project,

Slide 4 …worked with the product’s vendor (Bacus Labs) and our Systems Group to determine the requirements needed to deliver Virtual Microscopy to the students seamlessly,

Slide 5 …and followed through on the multiple detailed decisions and work that put it all in place here at UMass.
Slide 6 The core of the Virtual Microscopy working group also included Drs. Sue Gagliardi, John Cooke, Anca Stefan, and our most recent member, Dr. Krista Johansen. Andrea Barrett has led the current IS effort to integrate Virtual Microscopy technology into our online curriculum.

Slide 7 The team was nominated and received the Educational Achievement Award (the STAR award) for having taken Virtual Microscopy from an “idea” to “rollout for first- and second-year students” in less than two years.
Slide 8 WHAT is Virtual Microscopy?
Slide 9 An optical microscope, often referred to as a "light microscope", is an instrument that uses visible light and a system of lenses to magnify images of tissue sections mounted on glass slides. Optical microscopes are the oldest and simplest of the microscopes. The medical students have been using them for decades to study in the Pathology and Histology Labs.

Slide 10 Creating a virtual slide starts with a patient's prepared specimen slide. A virtual slide is a digitally captured, high-resolution, diagnostic quality image-set of hundreds, if not thousands of graphics taken of a physical glass slide with a special scanner.
Slide 11 The digital files are then stored on a server here at UMass. Every slide scanned becomes a folder on the server.
Slide 12 One slide folder consists of hundreds if not thousands of individual jpgs– note the scrollbar at the bottom of the screen, indicating that there are many, many more files in this one folder. Most slide folders are between 750MB – 1GB in size. We currently have 475 UMMS slides on our server, and we also provide links to slides from other Universities via the Virtual Microscopy Public Collections that are available from Bacus Labs.
Slide 13 So now what? The traditional way to view a glass slide is to place the slide on the microscope’s stage, then focus on the specimen using the lowest-power objective. In order to zoom in you need to manually switch the objectives and use oil between the lens and the specimen to increase the magnification. With Virtual Microscopy, things have changed. The only way to knit these individual jpgs together and view them as if you were looking through the eyepiece of a microscope is to use the viewers that are part of the product, which are free downloads.

Slide 14 So first we open Internet Explorer (or any other web browser) and type in http://islide.umassmed.edu. This is the page that is generated by the application, offering several ways to access the slides.
Slide 15 There are three viewers included in this suite: the JAVA applet viewer is the first one, which is cross-platform (for MAC and PC users). The folders on the left are basically the containers for a group of slides, so in this case the H_GI main folder which stands for (Histology Gastrointestinal) has a group of the individual slides that belong in that category. Each small thumbnail represents a scanned slide, and when you click on one …

Slide 16 … the JAVA viewer pops up. The small box on the top left with red crosshairs indicates the exact location on the slide. Right-clicking on the image brings up the menu bar, which is used to change focal lengths. The second graphic is the result of choosing the 40.0x Objective view; again the red crosshair tells the student exactly where he or she is looking on the slide.
Slide 17 Let’s see the ActiveX view. Again, log into http://islide.umassmed.edu. If you’re a PC user you can choose the ActiveX link.
Slide 18 The exact same list that the JAVA view offered appears, but the slides now will have ActiveX functionality.

Slide 19 ActiveX provides some additional functionality, such as saving a view of a slide to a student’s computer, but is essentially the same as the JAVA view.

Slide 20 The third choice is WebSlide Browser, which is also a free download.

Slide 21 It functions primarily as an administrative tool for people in the system who are authors, such as Drs. Smith, Craig, Johansen, and Cooke. It’s a stand-alone application that is installed on your computer, and it functions as a local application that provides many uses, such as creating overlay files as well as viewing the slides on our network.

Slide 22 Faculty can use the WebSlide Browser to create overlay files, which when added to the server provide more detailed and specific information for the students.

Slide 23 With Overlays you can essentially “draw” a slide, pointing to a specific spot on a specific slide. Overlay annotations do not affect the contents of the slide and can be turned off or on by the viewer. Students can also use this tool to create their own overlays that can be shared with other students or e-mailed to faculty with specific questions

Slide 24 We can’t change this first page (islide) when users access the server, so we’re tied to the Bacus design. The JAVA and ActiveX views can only offer the folders alphabetically in a list. However, as the administrator of this system, Andrea can use the WebSlide Browser tool to generate the html code that allows me to create individual web pages of each slide. In BLS Vista, she created separate Media Library entries for each slide and offered two links – one JAVA, one ActiveX. Using a naming convention that we all agreed on, links to the entries appear automatically throughout a course.

Slide 25 So by having the choice of three viewers plus the Media Library entries, students are provided an experience that is analogous to viewing a glass slide under a microscope in that you can move the specimen, change the objectives, focus, place a pointer, and easily make linear and area measurements – all from an Internet browser.
Slide 26 WHY are we investing in Virtual Microscopy?

Slide 27 One of the drivers for pursuing this technology was the fact that our glass slide collection is aging. Many of the slides are irreplaceable, so digitizing the slides is a way to preserve the collection. The slide images remain pristine, and are safe from breakage and fading.

Slide 28 No two sides are identical, so offering the slides digitally provides standardization of the collection.

Slide 29 The other obvious benefit is the ability to provide students with anytime / anywhere access;
Slide 30 … it encourages group learning,

Slide 31 …so several students and faculty can view and discuss an image at the same time.

Slide 32 The use of new and emerging technologies like VM in medical school exposes students to powerful tools not previously available, and also increases their comfort level with the use of technology in general.
Slide 33 When did all this happen?

Slide 34 Virtual Microscopy at UMass Medical School went live in August 2007 in Dr. Tom Smith’s Biology of Disease course. It was supposed to be a “pilot” launch but it went straight to production! There was no turning back.
Slide 35 Not long after, Dr. Roger Craig began incorporating slides into his Block One Histology Lab Manual,

Slide 36 … and this year Dr. Krista Johansen has completely re-worked the Histo Blocks Two through Four Lab Manuals. Roger and Krista have built up a large collection in Histology’s Media Library.
Slide 37 Wow! The presentation will now be turned over to Tom, Roger, and Krista to share how they use Virtual Microscopy in their teaching.
Any attribute declarations that have use = "default" changed to use="optional" - attr.structure.req. Any attribute declarations that have value="somevalue" changed to default="somevalue", attr.structure.req (hierarchical). Removed references to IMS MD Version 1.1. Modified attribute group "attr.resourcetype.req" to change use from optional to required to match the information model. As a result the default value also needed to be removed Name change for XSD. Changed to match version of CP Spec Inclusions and Imports Attribute Declarations element groups Any namespaced element from any namespace may be included within an "any" element. The namespace for the imported element must be defined in the instance, and the schema must be imported.
2001-04-26 T.D.Wason. IMS meta-data 1.2 XML-Schema. 2001-06-07 S.E.Thropp. Changed the multiplicity on all elements to match the Final 1.2 Binding Specification. Changed all elements that use the langstringType to a multiplicy of 1 or more Changed centity in the contribute element to have a multiplicity of 0 or more. Changed the requirement element to have a multiplicity of 0 or more. 2001-07-25 Schawn Thropp. Updates to bring the XSD up to speed with the W3C XML Schema Recommendation. The following changes were made: Change the namespace to reference the 5/2/2001 W3C XML Schema Recommendation, the base type for the durtimeType, simpleType, was changed from timeDuration to duration. Any attribute declarations that have use="default" had to change to use="optional" - attr.type. Any attribute declarations that have value ="somevalue" had to change to default = "somevalue" - attr.type (URI) 2001-09-04 Schawn Thropp Changed the targetNamespace and namespace of schema to reflect version change Any namespaced element from any namespace may be used for an "any" element. The namespace for the imported element must be defined in the instance, and the schema must be imported.

Any attribute declarations that have use = "default" changed to use="optional" - attr.structure.req. Any attribute declarations that have value="somevalue" changed to default="somevalue" - attr.structure.req (hierarchical). Removed references to IMS MD Version 1.1. Modified attribute group "attr.resourcetype.req" to change use from optional to required to match the information model. As a result the default value also needed to be removed Name change for XSD. Changed to match version of CP Spec 2001-11-04 Chris Moffatt: 1. Refer to the xml namespace using the "x" abbreviation instead of "xml". This changes enables the schema to work with commercial XML Tools 2. Revert to original IMS CP version 1.1 namespace. i.e. "http://www.imsglobal.org/xsd/imscp_v1p1" This change done to support the decision to only change the XML namespace with major revisions of the specification i.e. where the information model or binding changes (as opposed to addressing bugs or omissions). A stable namespace is necessary to the increasing number of implementors. 3. Changed name of schema file to "imscp_v1p1p3.xsd" and version attribute to "IMS CP 1.1.3" to reflect minor version change Inclusions and Imports Attribute Declarations element groups 2003-03-21 Schawn Thropp The following updates were made to the Version 1.1.3 "Public Draft" version: 1. Updated name of schema file (imscp_v1p1.xsd) to match to IMS naming guideance 2. Updated the import statement to reference the xml.xsd found at "http://www.w3.org/2001/03/xml.xsd". This is the current W3C schema recommended by the W3C to reference. 3. Removed all maxLength's facets. The maxLength facets was an incorrect binding implementation. These lengths were supposed, according to the information model, to be treated as smallest permitted maximums. 4. Added the variations content model to support the addition in the information model. Any namespaced element from any namespace may be included within an "any" element. The namespace for the imported element must be defined in the instance, and the schema must be imported.

The specification states: "Each activity must have one and only one objective that contributes to rollup". The following type describes an unbounded set of elements all named "objective" that do not contribute to rollup, and one element called "primaryObjective" that contributes to rollup. Contributes to rollup of the activity. Does not contribute to the rollup of the activity. The type that describes an individual objective mapping. See http://www.w3.org/XML/1998/namespace.html and http://www.w3.org/TR/REC-xml for information about this namespace. This schema defines attributes and an attribute group suitable for use by schemas wishing to allow xml:base, xml:lang or xml:space attributes on elements they define. To enable this, such a schema must import this schema for the XML namespace, e.g. as follows: <schema . . .> . . . <import namespace="http://www.w3.org/XML/1998/namespace" schemaLocation="http://www.w3.org/2001/03/xml.xsd"/> Subsequently, qualified reference to any of the attributes or the group defined below will have the desired effect, e.g. <type . . .> . . . <attributeGroup ref="xml:specialAttrs"/> will define a type which will schema-validate an instance element with any of those attributes In keeping with the XML Schema WG's standard versioning policy, this schema document will persist at http://www.w3.org/2001/03/xml.xsd. At the date of issue it can also be found at http://www.w3.org/2001/xml.xsd. The schema document at that URI may however change in the future, in order to remain compatible with the latest version of XML Schema itself. In other words, if the XML Schema namespace changes, the version of this document at http://www.w3.org/2001/xml.xsd will change accordingly; the version at http://www.w3.org/2001/03/xml.xsd will not change. In due course, we should install the relevant ISO 2- and 3-letter codes as the enumerated possible values . . . See http://www.w3.org/TR/xmlbase/ for information about this attribute.
