Welcome to the Christian Medical College (CMC), aregistered voluntary, non-profit making organization. We hope yourstay here will be pleasant and your recovery as speedy aspossible.
From 1900 when the founder, Dr. Ida Sophia Scudder startedCMC with a single bed in her father’s home, to the present day,CMC has been growing both in numbers and services rendered tothe people. Today, CMC serves over 2000 inpatients and 5000outpatients daily, with 67 wards, 92 clinics each day and121 departments / units.
As a teaching institution CMC trains doctors, nurses andallied health workers with a high degree of technical competence toserve with love and compassion in the spirit of Christ. CMC is uniquein being not only one of the pioneering institutions in the country inthe area of medical research and high-tech health care, but also asan institution which reaches out to the practical needs of the poorthrough low cost inpatient wards and outpatient clinics in the slumareas of Vellore town with concessional or free care for the mostneedy. In addition, through large rural health and developmentprojects in the K.V. Kuppam, Kaniyambadi and Anaicut blocks, thewholistic concept of healing is advocated wherein rural folk are giventraining in various vocational fields, by which they can augment theirearning power, thus enhancing their standard of living and thegeneral physical and economic well being.
With this constant growth and expansion, patients/visitorscoming for the first time to CMC may have difficulty getting aroundthe campus and may fail, through ignorance, to take full advantageof the available facilities. Therefore, this booklet was developed toprovide basic information necessary to make use of our facilitiesspeedily, to the fullest extent and with the least inconvenience andloss of time. A map of CMC is also provided for your convenience.
Dr. Ida Scudder gave the institution the Biblical motto "Notto be ministered unto but to minister" challenging CMC’s studentsand staff to heal and serve the needs of the whole person whichtoday remains the most important objective of CMC.

CMC has beencategorized as a super speciality teaching hospital of thehighest quality and assigned the A1 grading by theInvestment Information and Credit Rating Agency (ICRA)Ltd.

A SHORT HISTORY OF CMC
Christian Medical College, Vellore, is the outcomeof a single woman’s obedience to God's call. In 1890,Ida Sophia Scudder, founder of this institution, then aschoolgirl, was visiting her missionary parents in SouthIndia. One eventful night she was asked to help threeyoung women from different families struggling in difficultchildbirth. Being without training at that time, Ida herselfcould do nothing. She offered the help of hermissionary father, who was also a medical doctor. How-ever, to her amazement, the men responded saying,“What! Take a man into my house to care for my wife?It is better that she should die than another man shouldlook upon her face.” Custom prevented them fromaccepting the help of a male doctor. The next morningIda found out that all the three women had died. Shebelieved that it was a calling and a challenge set beforeher by God to begin a ministry to the health needs ofthe people of India, particularly women and children.
Consequently, she went back to America, enteredmedical training and in 1899 graduated from CornellUniversity Medical College and was among its first groupof women students.
Ida returned to India in 1900 and opened aone-bed clinic in Vellore. In 1902, she built the 40-bedMary Taber Schell Memorial Centre for women andchildren, named after the donor’s wife. The original siteis now CMC ’s Ophthalmology Department. The maincampus moved to the present site in the Thottapalayamarea of Vellore in 1924 and treatment was extended tothe whole family.
Early in her career, Dr. Ida Scudder realised thatthe women of India themselves should be trained to helpin medical work. She started a school for women nursesin 1909 which was upgraded to a College of Nursing in1946. A training programme for women doctors began in1918 which was raised to MBBS level in 1942 and men Dr. Ida Sophia Scudder

THE MAIN CAMPUS:In accordance with the vision of the founder, CMC
has, through the last many decades, through sheer hardwork, prayer and dedication evolved into one of thepioneering institutions in specialized medical carerendering invaluable service to large number of patientsfrom every state in India and neighbouring countries inthe spirit of Jesus Christ.
CMC located on Ida Scudder Road, in the heartof Vellore town, is a large 2512 bed hospital with121 specialized departments/units. The variousconsultants and the days they conduct outpatient clinicsare listed from page 63.
Being a referral hospital CMC is in a position toprovide specialized medical care to patients requiringtreatment in more than one area / ailment / disease. Thisis due to the unique availability of all the specializeddepartments/units under one roof and due to the exist-ence of close understanding and harmony amongst thesedepartments.
Moreover CMC has a system by which it givesexposure to all consultants by periodically sending themfor various national/international conferences, sympo-siums and specialized training overseas to acquire thelatest knowledge and skills in their respective areasso that they can provide high quality medical service toall patients coming to the institution.

In addition to the main campus on Ida ScudderRoad, there are a number of peripheral facilities whichare also departments of CMC.
OPHTHALMOLOGY DEPARTMENT
CMC’s Ophthalmic Department, is Schell EyeHospital 2 km away from the main campus, on Arni Road.Patients with eye problems should go directly tothis department for registration, investigations andtreatment. This department, equipped with all moderndevices does computerised field testing, cataract surgerywith intraocular lens implantation, phaco-emulsification,corneal transplantation and vitreo-retinal surgery.Inpatient services are also available.

In order to reach the Ophthalmology Department,take town bus route no.2 from the CMC bus stop androute no.1 from Bagayam, near CHAD hospital.
MENTAL HEALTH CENTRE (MHC)
CMC’s Department of Psychiatry, is situated onthe Christian Medical College Campus at Bagayam, 7km.south of the main campus. Patients with psychiatricproblems may go directly to this department forregistration, evaluation and treatment. Bothinpatient and outpatient services are available.

NAMBIKKAI NILAYAM
A centre for the evaluation and training of childrenwith mental retardation is housed at the same locationas the Mental Health Centre. Here it is essential thatatleast one parent of the child should be presentduring the first consultation and subsequent careand training. The names of the consultants and theregistration time for outpatient clinics are given in theOutpatient service schedule. Inpatient services are alsoavailable.

REHABILITATION INSTITUTE
The Physical Medicine and Rehabil i tat ionDepartment started by Dr.Mary Verghese,(herself aparaplegic) in 1966, is located in the Christian MedicalCollege campus, Bagayam. This Department provideslong term therapy and training for physically handicappedpatients to cope with the activities of daily living withinthe limits of their disability.

COMMUNITY HEALTH AND DEVELOPMENT (CHAD)
The 135 bed CHAD Hospital is also situated inthe Christian Medical College, Bagayam Campus. It is aprogramme under the Community Health Department of

CMC, Vellore. This comprehensive health programme,which serves a population of 2,50,000 in the rural, urbanand tribal community areas of Kaniyambadi, is effectivethrough the joint efforts of public health teams, socio-economic development workers and part-time communityhealth workers. In addition, the department also providesconsultancy services to a population of 1,20,000 inAnaicut block which is south-west of Vellore. Primaryhealth care in both the blocks is carried out in closecollaboration with the existing government services.

RURAL UNIT FOR HEALTH AND SOCIAL AFFAIRS(RUHSA)
Twenty five kilometres north west of Vellore in theK.V.Kuppam block, is RUHSA. This is an integrated ruralhealth and development project which runs a variety offield programmes actively involving the villagers, govern-ment and banks. RUHSA provides inpatient care, mobilehealth service, vocational and many other trainingprogrammes to empower the rural youth, programmesfor the poorest of the poor, with over 110 local familycare volunteers who live in the villages. It serves apopulation of 1,35,000 in 39 panchayats.

HOW TO REACH VELLORE AND CMC (Main Campus)
CMC is located at Vellore, the district head-quarters of Vellore District, Tamil Nadu. The main campusis situated on the Ida Scudder Road about 2 km fromthe main bus-stand. Those coming to Vellore by broadgauge train should get off at Katpadi Junction. The maincampus is about 6 km from Katpadi Junction. Town buses(No.1 & 2 )operate regular day and night service fromKatpadi railway station via CMC to Bagayam. Taxis andauto-rickshaws are always available at Katpadi railwaystation and will charge approximately Rs.250/- for taxisand Rs.60/-for auto-rickshaws to bring you to the maincampus. For any help contact the "CMC Help Desk" onplatform No.1 at the station.
Patients travelling to Vellore by meter gauge train(Villupuram - Tirupathi line) should get off at Vellorecantonment station which is 2 k.m. from the main campus.
Auto-rickshaws and cycle-rickshaws are availableat approximately Rs.30/- and Rs.25/- respectively to takeyou to the main campus.
Patients who arrive at Chennai and wish to comeby bus to Vellore should take bus no.102 from the newbus stand in Koyambedu. This service is available every15 to 20 minutes and is about 3 hours journey, adistance of 140 k.m.
For those coming by air, the options availableare either to come into Chennai city from the airport (bybus, taxi or suburban train) and find a suitable connectingtrain from Chennai Central to Katpadi or a bus fromKoyembedu or take a taxi right from the airport to CMC,main campus at an approximate cost of Rs.1500/-

c) After triage proceed to the cash counters (MCTT) andsubmit the filled in registration forms with registration feesof Rs.105/- for General patients and Rs.550/- for privatepatients. There are separate cash counters 4-8 for newprivate, 9-12 for new general and repeat registration16-19 counters in the ground floor of the ISSCC Building.Every new private patient will receive a CMC CHRIS cardwhich will also double up as your hospital number card.d) Proceed to the appropriate clinic with the paymentreceipt. This receipt will have your name, hospitalnumber, date, reporting time and location of the clinic.e) Submit the payment receipt to the Medical Record Officerat the clinic you are visiting. It is important that the HospitalNumber Card be kept safely.Please visit the OP at the specified time on your registrationslip.No extra charges is levied for appointments. Do not paymoney to middlemen to get appointments. Pleasecontact the floor Managers in the OPD G 52/ MedicalSuperintendent's Office for help.

The Outpatient block is afour storeyed building situatedon your right as you enter themain gate.
Most of the outpatient clinicsare conducted in this building.
The outpatient department is a very busy areacatering to more than 5,000 patients per day. Receptionstaff at the OPD and Centenary receptions will assistpatients with information and accompany them, if needbe, and provide any sort of procedural assistancepatients may require. Contact G-12 (OPD Reception) forassistance.
You can approach the Social Worker (G-14) in theOutpatient Block if you need any help or if you have anysuggestions or encounter problems during your treatmentat the hospital. You may also contact the OP managersoffice (G-52), OPD, Ground Floor.
REGISTRATION PROCEDURESAll Patients coming for the first time to the hospital must:
a) Fill in the registratrion forms kept at the OPD andthe ISSCC receptions. Please fill all details accuratelyas they are permanent records which you may requirelater for reimbursement or further correspondence.If you know the area Pin Code, please enter it correctlyon the registration form.b) After filling the form, proceed to 600A in the mainbuilding adjacent to OPD building for triaging. REVISITSGeneral patients must pay Rs.55/- for re-visit
towards repeat registration charges and private patientsmust pay Rs.155/- for revisit. Private Patients who havepaid the consultation fees of Rs.550/- need to payRs.465/- only as consultation renewal - cum registration feesfor re-visit after three months.Revisit amount of Rs.155/- (for private) + Rs.55/- for Generalis not refundable.
PROCEDURE FOR PAYMENT OF BILLSCash can be paid at cash counters only. Demand
drafts are accepted at all cash counters.
Payments can be made through credit cards in thePCF Counter (No.1), in ISSCC (2 to 19) in OP block(5 to 8) Casualty counter, A Block Counter and 'B' Ward.
Very poor patients who cannot afford to pay theregistration fees of Rs.105/- or revisit fees of Rs.55/- arerequested to approach the Social Worker (G14) who willassess and issue a free registration slip.
To avoid over-crowding in the clinics only one relativewill be permitted to accompany a patient. Identity wristbands are provided at the registration counter of the OPbuilding. You are requested to wear this till you leave thepremises. In case you don't receive the wrist band beforethe day of your visit to the doctor, produce your appointmentslip and get it from the band counter at the OPD entrance.Please remember that access to most service areas ispermissible with wrist bands only. We need your co-operation in this regard.
GENERAL PATIENT / PRIVATE PATIENTIf you choose to be a General Patient, you will be
seen by one of the doctors in the appropriate clinic. Butif there are concerns a second opinion is always asked forfrom a consultant at no extra cost.
If you are a Private Patient, you will meet theconsultant in the appropriate outpatient clinic or by priorappointment at other times. All fees and charges collectedgo to the institutional funds and no doctor working in CMChas any private practice.
When you (as a general patient or private patient)are referred to another department for opinion or treatment,the option is available to you to be seen as a general patientor as a private patient.
CMC Health CheckThis facility is being offered to those who wish to
have a general medical check up. It includes laboratory testsfor screening of common diseases and physical evaluationby a consultant physician to ensure good health. This checkup will be completed in 10 hours. Cost of this package isRs.2490/-. General health checkup is offered from Mondayto Friday. Please contact 600A for assessment. Pleasecome fasting in the morning of the checkup. Pleaseapproach PCF reception help desk, room No.101, in theOPD Building first floor for further processing. Patients withmultiple problems should not opt for this. They should see

REFUNDSPrivate Patients :1. In case of cancellation, for a new patient, (private) Rs.150/
- only will be refunded, after deducting a service chargeof Rs.400/-.
2. If you are a repeat patient, and have paid Rs.465/-, theconsultation fee of Rs.310/- will be refunded.

ESTIMATION OF COSTIf you would like to know the approximate cost of
your treatment write directly to the Head of the concerneddepartment or unit. It is advisable to estimate yourtotal cost before coming here. The expenses willpossibly include charges for such items as registration,revisi ts, tests, invest igat ions, medicine, diet,theatre,anaesthesia, blood, oxygen, bed and nursing,radiation therapy, physiotherapy and occupationaltherapy, whenever these are advised by the doctor.
Your total cost will depend on the choices youmake, whether you wish to be seen as a general patientor a private patient; whether you choose a private, semi-private or general bed as an in-patient.
Patients who require open-heart surgery,kidney transplant, dialysis and other highly specializedprocedures, should obtain an estimated cost ofsuch procedures from their doctors before arriv-ing at Vellore. Such patients should generate fundsfrom their own contacts. However, some economicallybackward patients have been able to collect money fromsources like Prime Minister’s relief fund, Chief Minister’srelief fund, various charity organizations and appealsin news-papers for expensive treatment.
Another important factor that will affect the totalcost is the length of admission which is determined byyour illness and the subsequent treatment. Thoughdoctors will give you information as to the approximateduration of hospitalization, it is sometimes difficult togive an accurate estimation as it will, depend primarilyon your condition, the number of investigations and

Apart from the above, there will be other costsincurred such as boarding and lodging. It is wise tobudget for all the above and to provide for sufficientreserve. It is important to keep in mind that specializedtreatment is expensive and takes time.
ADVANCE DEPOSIT FOR TREATMENT :
You can deposit an amount of Rs.40,000/- and above withCARE section under patients name for in-patients andRs.10,000/- for out-patients.
REFERRALSIf you are referred to another department for
opinion/treatment, the option is available to you to be seeneither as a General or Private patient. If you chooseto be a General patient and the clinic you are referred tois on the same day, contact the MRO of that clinic for anappointment.
However, if the clinic you are referred to is onanother day, you have to pay Rs.55/- for a General patientat payment counters showing the referral slip. If you donot show the referral slip your chart will be sent to yourprevious clinic.
To see a consultant get an appointment by makinga payment of Rs.465/- at the cash counter in the ISSCC,ground floor.
SAFETY OF YOUR REGISTRATION CARDThe registration card issued to you is an important
document. Please retain it carefully. To obtain a duplicateregistration number card contact G-31/G-32 (Supervisorcounter) in the ground floor of OPD building. Do notregister for a new card as this is expensive and timeconsuming for you and us.
OUT-PATIENT CLINICSOn reaching the appropriate out-patient clinic, submit
the appointment slip to the MRO, please be seated in thewaiting hall. When your medical chart reaches the clinic,the MRO in the hall will call your name. On producing thepayment slip which you had obtained during registration, theMRO will send your chart to one of the doctors. If youhave registered as a Private patient, your chart will go toa consultant or his/her approved assistant. Your name willbe called out when your turn comes and you will be seenby the doctor. Report to MRO according to your reportingtime.
When you meet with your doctor feel free to discussall your health problems with him/her, giving all relevantinformation about your sickness and follow the advice given.
If the doctor decides to have some clinical testsdone, such as examination of blood, urine, stool etc., youwill be issued the required forms. The steps to befollowed are described below under the heading “Testsand Investigations”.
If the doctor decides to put you on treatment, youwill be given prescription slips for purchase of medicine/injections. and / or treatment procedures. Steps to befollowed are given below.
If the doctor decides to admit you as an inpatient,he/she will give you an admission order. The steps to befollowed then are given below.

MEDICINES AND INJECTIONSIf the doctor thinks that you need certain medicines,
he will give you signed prescription forms. Take them tothe Pharmacy payment counters for pricing and payment.
These counters function from 8.00 am to 8.00 pm. TheOPD Pharmacy dispensing counter will issue medicines onlyfor the out patients. Prescriptions of inpatients will not beprocessed at this counter.
If you need an injection take the procedure formgiven to you by your doctor and proceed to the 24 hourinjection room, which is next to the post office, afterpayment.
If you are claiming reimbursement, remember tohave your receipt endorsed by the pharmacist.
It is safe to buy medicines from the hospitalpharmacy to ensure quality and authenticity, correctdosage and strength of the drug. The Pharmacist willexplain how to take the medication.
IMMUNIZATION FOR CHILDREN (Location : ISSCC No.450)Children may be taken to the Immunization and Well
Baby Clinic in the ISSCC straight away for immunization.
After weight and temperature recording, the doctorwill examine the child and give a prescription for immunizingyour child. There is no need for OP registration for this.
MINOR SURGERY DRESSINGS (location : OP buildingG-41)
If you require minor surgical procedure or dressings,you will be given a prescription for the same, specifyingthe charges for the treatment. Pay the amount at thepayment counter after which proceed to the O.P.Surgerydressing room (G-41) in the ground floor of the OPbuilding.
OTHER TREATMENT..................................................For all other treatment you will be given referral slips
which should be taken to the concerned doctors / units.You will be registered and given instructions to follow.
DAY CARE FACILITYFor the convenience of out-patients while they
undergo specific time-consuming investigation/procedures, aday care facility has been established at A Block / O4 East,where patients can avail a bed and hospital diet, onpayment. This facility will be available only during the periodof the actual investigation/procedure done during the dayi.e. between 7.00 a.m. to 7.00 a.m the following day. Patientscan make a request to the doctor who will make necessaryarrangements.
The charge for the day-care facility is Rs.1000/- forA Block and Rs.750/- for O4 East. These charges and thefee for the investigation/procedure should be paid andreservation made before 3.00 p.m. the previous day. Themedical chart of the patient will be sent to the wardautomatically.
BIRTH CERTIFICATE
(Location : M.S.Office 3 main building)To obtain birth certificate, death certificate, recent
corrections, Insurance contact the Medical Superintendent’soffice(No.3) in the main building ground floor. A form hasto be filled in and a fee of Rs.50/- has to be paid at thepayment counter in the OP building. The birth certificate canbe collected in person or by post.
MEDICAL REPORTSTo get a medical report, please request the doctor
at the time of your final visit. He will give you a voucherfor payment and make necessary arrangements for themedical report or guide you to the person who may beresponsible to arrange this. After making the necessarypayment, the receipt may be submitted to the secretary inthe concerned department along with a self-addressed

IN-PATIENT SERVICESADMISSIONSAll admissions are made only on the advice of the doctors.If the doctor feels you require admission he/she will giveyou an admission slip which indicates the amount you willhave to pay in advance. The admission procedure variesslightly depending upon whether the patient seeksadmission in general ward or private ward.
All clinical departments have wards attached tothem. All these wards have few semi-private rooms. At thetime of admission patient must indicate their preferencefor general, private or semi-private room.
GENERAL WARD ADMISSIONSi) After making the necessary payment you will have totake the admission slip to the ward and show it to the wardsister who will confirm your admission.
PRIVATE WARD ADMISSIONSi) The private wards are A Block, O-3 East,O-3 West,O-4 East, O-4 West, O-5 East, O-5 West, O-6 East,O-6 West, Q-5 North and G7.ii) Take your admission slip to the PCF help desk counterNo.101 on the first floor of the R.T. building in the mainOPD where your admission will be confirmed after payment.iii) At this counter you will be allotted your bed as andwhen your turn comes. This is because there is a demandfor private rooms and a waiting period of a few days oreven weeks may become necessary. You are expected tokeep checking with this counter periodically.
iv) When your turn comes, your admission order slip willbe endorsed. You should take this slip to the concernedward where the necessary files will be prepared and youwill be admitted.
Advance reservation can be made for private wards:Get the admission slip from your doctor and proceed
to PCF help desk No.101 where a bed can be reservedfor you. On the day of admission proceed directly to theward mentioned on the admission slip.
MATERNITY PATIENT ADMISSIONSi) Maternity patients in labour can go straight to the
labour room in the ISSCC.ii) They need not wait to complete admission formalities.
AT THE TIME OF ADMISSION PATIENTS MUST KEEP INMIND THAT :1. All admissions are made only on the advice of the
doctors and not at the request of patients.
2. Routine admissions are made between 8.00 a.m. and4.00 p.m.
3. Emergency admissions are made at all times.
4. General, semi-private and private beds/rooms areavailable at varying costs. For details of room/bedcharges refer to page 33. It is important to note thatthe charge is only for the room/bed and does not includeother costs which will depend on the type of bed facilityavailable i.e. private, semi-private or general.

NURSING SERVICESWhen you are at C.M.C. as an in-patient, your
requirements will be looked after by the nursing services.They will carry out the treatment planned by your doctors,makearrangements for your tests/investigations and give constant

SPIRITUAL & PASTORAL CARE :
Spiritual and pastoral care are offered to all patients andtheir families by the Chaplaincy Department. It is locatedin the ground floor of the main building near the Chapel.The telephone number is 2016. For further information onChaplaincy, see page 53.
SUPPORT SERVICES FOR INPATIENTSDIET
CMC has a Dietary Department which servesvegetarian, non-vegetarian,western and special diets at thebedside of patients at meal times. Food for the attendantof the patient also can be had from the dietary. Nutritiousfood is prepared and served under strict standards ofhygiene using modern equipment and fully competent staff.
It is not advisable to give the patient food purchasedfrom outside since the method of preparation and theenvironment in which it is prepared, may not be clean andhygienic.

ATTENDANTSIt is compulsory to bring one lady attendant to stay
in the ward with the patient. In private rooms a camp cotis available for the attendant.
If a private patient does not bring a lady attendantbut is in need of one he should approach the ward sisterthrough whom an ayah can be arranged at a nominal cost.Male attendants are permitted only during visiting hours andmeal times. Patients should obtain passes(2) from the wardsister for attendants and relatives bringing food. It shouldbe returned to the sister-incharge on discharge. Onlya lady attendant is allowed to stay with the patientduring the night.
CHAPLAINCY LIBRARYThe Chaplaincy Department runs a library which is in
the Ida S.Scudder Centenary Center. A wide range of readingmaterials, religious books and magazines areavailable for reading. This library is open from 8:00 am to4.30 pm, with lunch break between 12.30 and 1 pm.
MOBILE LIBRARYThe Chaplaincy Department also provides books to
patients in all the wards. Patients can make use of this facility.
DHOBIES AND BARBERSAuthorised dhobies and barbers can be engaged for
their services on payment. The ward sister will provide youinformation on this.PROCURING MEDICINE FOR PATIENTSGeneral Ward Patients :1) The medicines your doctor prescribes will be written on
a prescription which will be given to you.2) You should take the prescription to the appropriate
pharmacy counter (location and timings of the pharmacycounters are listed on pages 58-59 for payment.)
3) After payment collect your medicines from thepharmacy.
Private Ward Patients :All medicines will be automatically procured by
the nurses and the cost added to the hospital bill.

CHARGES AND SETTLEMENT OF BILLSAt the time of admission patients usually pay an
advance. After admission weekly bills are presented to allin-patients on every Monday evening. If you do not receivea weekly bill contact the ward sister or ward clerk. Thesebills should be settled on a weekly basis within 48 hours.A final bill will be presented on the day of discharge whichshould be paid before the patient vacates the room/bed.The final bill includes special procedures, majorinterventions etc. You can settle bills through cash, demanddrafts (D.D) or credit / debit cards.

Then the CARE staff will decide on the crediteligibility and credit extension, based on which the pleasantMCTT staff would print out the needed slips to meet thedoctor. 6. Patients who require major neurological procedures,cardiothoracic surgery, bone-marrow or kidneytransplantation may need to pay the estimatedamount as advance.
7. Payment of bills by companies will be carefullymonitored. Credit facility will be discontinued for thosecompanies that are irregular in payment.
8. CARE Section functions 24 hours. So you can getassistance for credit patients from CARE, during nonoffice hours and from ACCESS during office hours.
9. When a credit billed patient is referred as anoutpatient to Ophthalmology Department/Mental HealthCentre/Rehabilitation Institute /Nambikkai Nilayam he/she should come to ACCESS for an authorisationletter. Proceed to the referred centre with authorizationletter from ACCESS,

CHRIS CARD for self-deposit patients: Self-payingpatients can also make deposit of Rs.10,000/- andabove as drafts / cheques for outpatient treatment,and they can avail credit facility against their deposit.This deposit will be your own money. 11. Patients who pay advance to us, are requested toadvise the ACCESS / CARE section regarding the bills/ further treatment or refund, if any, after discharge /treatment is over.
12. Credit billed patients have separate counters for billtransactions, blood tests and pharmacy.
II. REIMBURSEMENT CERTIFICATE Phone 0416 22825931. Registration for Reimbursement Certificate (hereafter
referred to as RC) can be done only after completingthe treatment for out-patients and discharge from theward for inpatients.CMC has its own RCs for out-patients (Form A) and inpatients (Form-B).
2. All payments have to be completed before registeringfor R.C. Please note that if you have received anyconcession in your bills, you can register for R.C,after paying the reduction amount.
3. The registration fee for RC for out-patient andin-patient treatment will be Rs.215/- and Rs.270/-respectively. Combined RC will be issued on paymentof Rs.400/-. For each bill belonging to one periodof admission or treatment the fees need to be paid.
4. In-patients are requested to submit all original cashreceipts including inpatient cash advance receipt, ifany. (These are receipts issued from C M C, Vellorefor any payments made by you).
5. In-patients are requested to submit all weekly anddischarge bills in original.
6. In-patients are requested to submit a xerox copy ofMedical Report / Discharge Summary in order tohasten the despatch of RC.
7. Once a registration is made we will not be able toaccept anymore cash receipts for inclusion in thesame registration. However, you may make a newregistration with extra fees, or you may submit italong with future receipts.
8. If you need RC in your own institution’s forms,please submit the blank forms to us, at the time ofregistration for RC and not later.
9. Patients who need RC in their employer's officialforms, if any, after issuing it in our forms, would berequired to pay an additional sum of Rs. 95/- for thesame.
10. Ensure that you receive the receipt for registrationwith your registration number. If you would like tocontact us regarding RC later on, please quote thisnumber, along with your Hospital Number.
11. We will provide a summary of your bills, with detailedbreak-up of items, drug details and consolidated cashreceipt. Detailed break up of items are available inweekly bills for inpatients.
12. Company patients with credit facility can register forreimbursement certificate, after the settlement of billsby the company.
13. Medicines which are bought from outsideCMC pharmacy cannot be included in RC. Howeverthese receipts can be attested by the treating doctorif found genuine, and claimed directly from yourinstitution. These can be submitted to us along withCMC receipts and bills, for signature. But in case,you need a separate reimbursement certificate, foroutside purchases alone, you need to pay extra fees.
14. We strive to dispatch Reimbursement Certificate toyou within 5 working days under ordinary circum-stances.
15. In case you were not able to register for RC whenyou were at Vellore, you may send all documents(items 2 to 6) and send the fees as Demand Draftfavouring Treasurer, C M C, Vellore, payable atSBI Vellore town, Code : 1618 and a covering letter,to the Accounts Officer, CARE Section, AccountsDepartment, CMC (Town campus), Vellore 632 004,Tamil Nadu.

However, in this connection, you need toremember the following :
i) TPAs or Health Insurance companies do not pay forinadmissable items. So you need to carry cash inorder to pay for these. This usually amounts toapproximately Rs. 2,000/- or so. Apart from this,you need to pay for diet too.
ii) They allow only hospitalization and do not pay forinvestigations done and medicines bought as anout-patient. So you should be willing to pay forthese, by yourself. This amount may vary basedon the number and type of investigations.
iii) In order to get hospitalized one needs to get thepre-authorisation form (request) to be filled up byour doctor whom you consult. This will be faxedor emailed by ACCESS / CARE to the TPA whichshall in turn give or deny authorisation, forhopitalisation for you. If we receive the authorisation,you can have the treatment on credit. But if it isdenied, you need to pay for the treatment.

If you returnto C.M.C. for a follow-up examination or treatment, it willbe helpful to bring this document with you. This dischargesummary is given to you free of charge. You can alsoarrange for it to be sent by post to you.

GENERAL INFORMATIONWHAT TO BRING / NOT TO BRING
Your length of stay at CMC would be considerablyshortened if you remember to bring with you thefollowing :
1. All relevant reports, X-rays, Biopsy Slides, blocks, andprescriptions for drugs, which you might have ob-tained from other doctors.
2. Patients requiring surgery and/or blood transfusionsare advised to bring relatives or friends as blooddonors.
3. Patients requiring in-patient care should bring alongone lady attendant to stay in the ward during thenight. Male attendants or relatives are not allowed tostay with the patient in the ward after visiting hours.
4. For a longer stay you may find it convenient to bringitems such as flask, a tiffin carrier, plates, tumblers,knife, spoons, etc.
5. Patients are advised to bring a lock and key to keeptheir personal belongings safe in the cupboard,cabinet or drawer provided.
6. During the months of (November to February), youmay require light warm clothes.
7. Sheets, blankets, pillow-covers and towels areprovided for inpatients.
8. Bring only essential personal items of clothing andtoilet articles. It is not advisable to bring jewelleryor other expensive articles. For inpatients requiringsurgical procedures, it is the responsibility of therelatives/ attendants to take possession of allvaluable personal belongings. CMC will not take anyresponsibility for the loss of personalbelongings of patients.
9. Patients should not bring money (large amounts) inthe form of a draft as they will find if difficult to findan introducer.
10. Children below the age of 12 are not allowed to visitpatients in the wards.

THE CHAPLAINCY
Spiritual and pastoral care are offered to all patientsand their families by Chaplaincy. It is located on the groundfloor of the main hospital building near the Chapel. CMCbelieves that at the heart of all its health care spiritual healthforms a unity with physical and mental health. Therefore,ministering to the spiritual needs of patients and theirrelatives is seen as an integral part of the healing process.The CMC staff as a whole strive to demonstrate in all theirdaily work the belief that faith is a necessary component ofhealing. The chaplains bear a special responsibility for thisaspect of health care. The chaplains try to visit every patientand offer help, prayer, counselling, etc., according to the need,irrespective of the language and religion. A chaplain can becalled to service at any time either by notifying the ward sister
or contacting the Chaplaincy by phone on 2016. Upon therequest of the patient the Holy Communion will beadministered at the patient’s bedside. After Life ServicesCMC's mission constitutes patient care, education andresearch. We aim at compassionate care right from the time apatient arrives at the doorstep of our hospital till the patientand the family leave us. If a patient dies it is our responsibilityto reach out to the bereaved family and help them. The staffof the Public Relations Department will help the bereavedfamily members in all possible ways to cope with their loss andco-ordinate the after life services. They will assist inobtaining all required documents, in cremation, arranging forburial if needed or make arrangements for transporting thebody home. This service will be made available roundthe clock and we request the wards to inform PRO orthe main reception in the event of death.

The deceased family who wishes to have the burial oftheir loved ones, at the government cemetry may contact theChaplaincy Department for necessary arrangements. All donations to CMC are eligible forincome tax exemption, under 80G of the Income Tax Act. Thisallows you to deduct 50% of the donation from your taxableincome, up to 10% of total income for the financial year. Wewill send you a receipt with IT exemption certificate to enableyou to make the claim.Every day thousands of suffering people come to CMC forhealing. Many of them cannot pay for the treatment that theyneed. CMC tries its best to provide concessional or free careto the poor. But our resources are limited and the need ismuch more. Kind friends enable us to do far more than ispossible from our own funds. Some contribute to buyequipment, others give for the special needs in the unit underwhich they were treated. Many people are keen to supportthe care of very poor patients coming for treatment.The Person To Person Fund enables us to give small grantsto several patients, requiring inpatient care. CMC depends entirely on patient's relatives andvolunteers for their blood requirements. When you meetyour doctor discuss the blood requirements for yourtreatment or surgery. Your relatives can donate bloodinto the blood bank at any time, even if it is much beforeyour scheduled date of surgery, Blood Bank will creditit in your patient. However, if unexpected emergenciesarise there might be rare instances where Blood Bankmight not be able to provide blood despite yourdonation. However, maximum effort will be made to seethat this does not happen. Most of the time, unless bloodbank is facing an acute shortage of a particular bloodgroup, donations do not have to be group specific. Bloodbank welcomes volunteer donors at any time.
