- Over 1 million patients have been “prescribed” an Emmi program

- Programs span chronic condition management, acute care, and patient safety

WHO ARE WE?

Emmi Solutions is developing a Multimedia program to help practices communicate the medical home concept to patients in a consistent, understandable way

- To be made available for free to physician practices and others

- Help meet CMS pilot requirements for educating the patient on the medical home

- Could also be used to educate providers and other stakeholders
