Prague **Institute of Information Theory and Automation Czech Academy of Sciences ***1.st Medical Faculty Charles University Prague Breast cancer is the most common malignancy in women and the second most common cause in cancer –related mortality. The usefulness of mammography, hand held ultrasound and clinical evaluation for the diagnosis was analyzed at cohort of 145 women with breast cancer followed at our oncology department. The median age was 61 years, minimum 30 years and maximum 90 years, 55% of patients was between 50 and 70 years. The mammography alone reveal 96% true positivity, 1.82% false positive rate and 3.64 % false negative rate, 96 % sensitivity and 67 % specificity. The hand held ultrasound alone had 87.5 true positive rate, 2.5% false positivity but unacceptable 12.5 % false negative rate, 87% sensitivity and 50% specificity. Combining mammography and hand held ultrasound lead to 97% sensitivity and 50% specificity. The clinical evaluation had 28% false negative rate, 70% sensitivity and 40% specificity. We are developing the 3D breast ultrasound technique to ensure the whole breast exam and better depicting of lesions. We propose an affordable solution to improve the image quality of ultrasound devices using electromagnetic tracking technology and Bayesian inference algorithm for reconstructing 3D volumes. We can thus monitor the quality of the exam and regions that were examined and minimize the risk of not detecting harmful lesions. The proposed method thus ultimately reconstructs the 3D volume from 2D images while removing noise, blur, increasing resolution and surpassing the quality of the original 2D images. Mammography provides almost complete breast pictures at two projections with high sensitivity for microcalcifications. Ultrasound helps to distinguish normal findings like cysts or fat lobules from suspicious breast changes which need to be biopsied. The test does not expose patient to radiation. The use of ultrasound for breast cancer screening is limited by skill of the examiner, resolution of device and uncertainty, whether the whole breast was scanned. Breast ultrasound is frequently used to evaluate breast problems found during mammography, especially in women with dense breast tissue or with palpable lesions on physical exam. The ultrasound cannot be used as sole examination because of the inappropriate high false negative rate. The intervals between mammography could lead to interval carcinomas. The women with dense breast also could be misinterpreted on mammography. The false negative results still harm the patients. This study confirms the necessity of using the combined investigative techniques for detection of breast cancer and is intended as base for the development of more sensitive 3D ultrasound with certainty of whole breast examination and better lesions depicting. Videokymography: Theory and practice Jitka Vydrova Medical Healthcom ,Voice and Hearing Centre Prague, Czech Republic This presentation will be devoted to the fundamentals of videokymographic imaging and to the application of videokymography in clinical practice. The presentation will provide case examples of characteristic pathologic conditions, which were diagnosed thanks to videokymographic findings. When is videokymographic imaging applied in clinical practice, videokymography allows detailed diagnosis of vibration disorders of the vocal folds in small developing lesions, which are difficult to detect using other methods. It enables an early detection of cancerous tissue on the vocal folds. It serves as a great help in detecting organic basis of damaged vocal fold tissue, which has otherwise been considered a functional disorder. It allows detecting tiny scarred areas and increased stiffness of the mucosa and of the submucosal tissues. Videokymography is a high-speed videolaryngoscopic examination method, which allows obtaining detailed information of the vibratory behavior of the vocal folds and surrounding tissues. In contrast to the standard strobovideolaryngoscopic methods, which work properly only on regular voices and do not allow detecting irregularities, the high-speed imaging does not suffer from this limitation and allows detecting real vibration of the vocal folds. Videokymography differs from the other high-speed videolaryngoscopic methods in that it uses a special videokymographic camera which provides two images simultaneously – in the left half it shows the standard laryngoscopic image (50 or 60 images per second), in the right half it provides the kymographic image showing the vibratory behavior of a selected part of the vocal folds in high speed (7200 line images per second). The high-speed kymographic images are available immediately, which makes the method friendly for use in busy clinical practice. Research on videokymography in voice disorders identified over 30 vibratory features which reveal on various causes of vibration disorders of the vocal folds.

Nutritional Habits in Adolescences and Examination of Body Mass Index Çiftçi EK*, Aydın D** *Department of Pediatric Nursing, Faculty of Health Sciences, Zirve University, Gaziantep, Turkey **Department of Pediatric Nursing, Faculty of Health Sciences, Bandirma Onyedi Eylul University, Balikesir, Turkey Aim: This study was conducted to determine nutritional habits of the adolescences and body mas indexes. Materials and Method: Population of the study of descriptive and cross-sectional study type consisted of adolescences having education in high schools in center of Şanlıurfa. Sample of the study consisted of a total of 413 adolescences having education in high school and selected with simple random sampling method and accepting to participate in the study. In collection of the study data, personal information form developed by the researchers and comprising questions about sociodemographic features and nutritional behaviors of the adolescences, and nutritional habits scale were used. Questionnaire form was filled by the adolescences in face-to-face interviews. The obtained data was assessed in SPSS 13.0 package program, and analyzed with number, percentage and chi-square tests. Results: It was determined that 56.7% of the participants were females and 70.9% lived with their families. The rate of the adolescences stating that they paid attentions to adequate and balanced nutrition was determined as 51.3%. It was detected tat 64.9% of the adolescences skipped meal, 61.5% skipped the breakfast meal at most and only 31.7% has education on adequate and balanced nutrition. Among the reasons of skipping meal of the adolescences, there is inappetency of 31.7%, and insufficient time of 24.2%. It was detected that 38.3% of adolescences dined out 1-2 times a week. In the study, body mass indexes of the adolescences were examined and it was determined that 71.2% were normal, 19.6% were thin and 9.2% were slightly fat. Considering Nutritional Habits Scale sub-dimensions of the adolescences, it was detected that they got 30.33±7.84 points from the dimension of prejudices against fatness, 52.39±10.21 from the dimension of positive and negative thoughts resulting from fatness, 20.47±5.48 from the dimension of self-perception against fatness, and 26.79±5.29 from the dimension of eating behaviors. Conclusion: At the end of the study, it was determined that the adolescences were not qualified about the information relating to adequate and balanced nutrition.

Contribution of environmental factors in untreated paranoid schizophrenia for long term remission Vikram TANWANI Medical Officer , General Psychiatry , Institute of Mental Health , Singapore [Objectives] Explaining the role of environmental factors in prevention of relapse of schizophrenia [Methods] A Case Report [Results] We present the case/example of a middle age female patient who in her thirties suffered the onset of schizophrenia followed which in this presentation for relapse of the same. It was indeed the matter of interest for us to note no relapse of schizophrenia from the age of 28 tp 55 years of age without any Mental interventions. This may led us to explore the possible other factors which may prolong the relapse and could be considered stronger than any pharma intervention usually employed. Her environmental conditions are regarded as protective with firm support from family and pre occupation with business activities which heralded full blown relapse. [Conclusion] This case reports possibly explains the efficacy of protective environmental factors for achieving long term remission in untreated schizophrenics. ETIOLOGICAL AND EPIDEMIOLOGICAL ASSESSMENT OF PEDIATRIC PATIENTS WITH SENSORINEURAL HEARING LOSS IN THE PROVINCE OF ŞANLIURFA Çiftçi M.A, Iynen I, Karaca Ciftci E, Zirek A, Yükkaldıran A, Beklen H. Gaziantep, Turkey Childhood hearing loss in period affects adversely the cognitive, social and emotional development and particularly the language and speaking skills of the child. Difficulty in establishing a diagnosis, and limited treatment and rehabilitation methods distress the doctors and families of the children. Therefore, early diagnosis of sensorineural hearing loss in children is very important. Our study was conducted on 71 pediatric patients diagnosed with sensorineural hearing loss, who applied to Harran University Medical Faculty Otorhinolaryngology policlinic, and 30 pediatric patients who did not have anamnesis of hearing loss and applied to our policlinic. When hearing loss reasons were classified, it was determined that while 42 patients (59.2%) had familial reasons, 23 patients (32.3%) had acquired reasons. No reason was detected for 6 patients (8.5%). Syndromic reasons were determined in 8 of the familial reasons (11.3%). 34 patients (47.9%) were evaluated as non-syndromic reasons. Connexin 26 (GJB2) and 32 (GJB1) sequence analyses were examined on 59 of our patients. No mutation was determined in GJB1 gene in none of these 59 patients. 3 of 59 patients (5.1%), who were analyzed in terms of GJB2, were homozygote for 35 del G mutation and 3 patients (5.1%) had heterozygote mutation in the GJB2 gene. Anti Toxoplasma Ig G, Anti Rubella Ig G, Anti CMV Ig G, Anti HSV-1 Ig G, Anti HSV-2 Ig G antibodies of 59 of the 71 patients with sensorineural hearing loss and 30 children without sensorineural hearing loss constituting the control group were examined. When the patient and the control groups were compared statistically in terms of TORCH, there was a significant difference only in HSV-1. Temporal CT or MR values of 32 patients in the patient group were reassessed and diagnoses were reviewed. 4 different inner ear malformations were determined in 7 of the patients (21.9%). Mondini deformity was determined in 2 patients, cochlear hypoplasia in 1 patient, enlarged vestibular aqueduct in 3 patients, and enlarged internal acoustic canal in 1 patient. 66.2% of parents of the patients had consanguineous marriage. On the other hand, modern dentistry is also associated with rapid penetration of advanced technologies, new dental materials and drugs into the diagnostics and therapeutic procedures resulting in the new branches of contemporary dentistry, significantly changing undergraduate curricula, need of permanent postgraduate reeducation, re-education of dental practitioners, and the necessity of close collaboration of researchers, teachers, and clinicians. Prevalence of musculoskeletal disorders in Czech dental practitioners Zdenka Šustová*, Martin Kapitán*, Lenka Hodačová** *Department of Dentistry, **Department of Social Medicine, Faculty of Medicine in Hradec Králové, Charles University in Prague, Czech Republic Aim: The aim of this study was to present results of a questionnaire survey concerning musculoskeletal disorders (MSDs) conducted among dental practitioners in the Czech Republic. Methods: Dental practitioners filled in the questionnaire within educational events in. The questionnaire consisted of three parts. The first part included general information about the respondents, the second one concerned the characteristics of workplace, and the third part involved the prevalence of MSDs. Collected data were analysed statistically. Results: A total of 581 (72.6%) out of 800 questionnaires were returned . A total of 96.9% (n = 557) of respondents stated at least one kind of MSD regardless its intensity during last 12 months. 30.6% (n = 176) of respondents identified their subjective complaints as a mild pain. Middle or strong pains were declared by 66.3% (n= 381) of all respondents; by 69.3% (n = 287) of women and 58.4% (n = 94) of men. The difference between men and women groups was statistically significant (chi-square test, p = 0.012744). Of the respondents with middle or strong pain 22.6% (n = 86) decreased their work load for MSDs during the last 12 months. Of the dentists suffering from middle or strong pain 86.9% (n = 331) treated themselves by doing exercises, 48.6% (n = 185) by medicaments and 46.7% (n = 178) turned for help to a specialist. The most often MSDs were pain in the area of cervical spine, back and shoulders. Conclusions: The prevalence of MSDs is quite high among active dental practitioners in the Czech Republic, predominantly in women. Supported by PRVOUK P37/13

Self-Care Compliance by Elapsed Time after Kidney Transplantation Min-young Kim Ulsan University, South Korea The purpose of this study was to survey the self-care compliance by patients in each elapsed time after the kidney transplantation. The research subjects were patients who had operations of kidney transplant at university hospital, and the data was collected from October2014 to March 2015 included 235 patients. In order to measure self-care compliance, the Follow-up care compliance was used : that was revised and modified by structured scale. Collected data was analyzed by using SPSS Window IBM 20.0 programs. The conclusion of this study was as follows: The self-care compliance after the kidney transplantation showed a significant difference in each elapsed time and in particular, the decrease of self-care compliance was most evident in the group for which more than one year but less than five years elapsed after the operation. With respect to sectorial self-care compliance, which includes drug adminstration domain self-care compliance, diet domain self-care compliance, everyday life domain self-care compliance, had significant differences by elapsed time. Therefore, a nursing strategy that provides consistent education and developing self-care improvement programs is needed to strengthen self-care based on the point of time when the self-care compliance starts to decrease in each elapsed time, thereby increasing the self-care compliance is necessary. Fall Risk Factors and Characteristics of an Acute Hospital Setting in Korea InSil Jang Ulsan University, South Korea Purpose: This study was done to determine inpatient fall rates in an acute hospital setting and to explore risk factors and characteristics across clinical departments in Korea. Methods: The medical records and standardized fall reports of 416 patients admitted between January 1 and December 31, 2012 were reviewed. Descriptive statistics and statistical tests were used, including: t-test, χ2-test, ANOVA. Results: The total fall rate per 1,000 inpatient days was 0.49. Fall rate, fall risk factors and characteristics such as age, type of fallers and mean MFS (Morse Fall Scale) differed significantly among clinical departments. Conclusions: The analysis results show that the fall rates, fall risk factors and characteristics of acute hospital inpatient falls varied significantly across clinical departments. The findings of this study suggest that hospitals should consider differences in fall related characteristics across clinical departments when implementation fall prevention strategies and interventions.

Effects of Nurses Image, psychological wellbeing , career preparation behavior on career decision level in Nursing Students in Korea Kyoung Sook, Lee Ulsan University, South Korea Purpose: This study was done to explore the relationship among nurses image, psychological wellbeing , career preparation behavior on career decision level and related factors in Korea. Methods: Nurses image, psychological wellbeing , career preparation behavior on career decision level scales were used as the measurement tools with a sample of 482 students from 2 universities and 1 college in J-city and 1 college in U-city in Korea. T-test, ANOVA, Pearson correlation and stepwise multiple regression with SPSS/WIN 14.0 version were used to analyze the collected data. Results: Nurses Image, psychological wellbeing , career preparation behavior was positively correlated with career decision level . Stepwise multiple regression analysis showed that 26.2% he variables, nurses Image, psychological wellbeing, school score, economic states. Conclusion: These results suggest that earlier screening and intervention programs for low nurses image, and low psychological wellbeing in nursing student should be helpful in developing and assisting student career decision level. ETIOLOGY OF END-STAGE RENAL DISEASE, PREVENTABLE CAUSES AND THE ROLE OF NURSES Nilgun AKSOY Akdeniz Universıty Faculty of Nursing, TURKEY End-Stage Renal Disease (ESRD) is a prevalent disease in which the functions of kidneys are progressively and irreversibly lost, and a serious public health problem that affects all the organs and the systems, threatens life and considerably leads to loss of labor force and life quality as a consequence of its frequency, complicated treatments and high financial expenses. ESRD both affects the life quality and causes morbidity and mortality at high rates. The existing evidences indicate that the adverse results of Chronic Renal Disease can be prevented or delayed with early diagnosis and treatment. The distribution of causes leading to ESRD vary from country to country as well as race and gender. Climate, socioeconomic conditions, cultural differences and environmental factors play a significant role in the development of ESRD. By determining these differences and reducing the risk factors likely to occur at prenatal stage, renal failure developing until the stage of organ transplantation could be prevented, and additionally contributions will be made to both the public health and in the country’s economy. However, the development frequencies, the causes, the relationship related to sex and age, the accompanying clinic problems, the treatment methods and the protective methods for these diseases are not known well in Turkey. Determining the factors causing ESRD, and early diagnosis of the preventable and treatable disorders are significant in order not to encounter any recurrence after kidney transplantation stage. In countries like Turkey, where the health costs are limited, knowing about the etiological causes and the frequency of ESRD and treating these diseases from the moment of its diagnosis are considered to be an effective method in slowing down the ESRD process and decreasing the costs. The nurses can have individuals at risk group adopt and apply the healthy life style changes to provide protection from chronic renal disease at a high rate. To prepare strategies aimed at preventing ESRD, the frequency and prevalence of which is increasing gradually, it is important to determine the changeable risk factors related to the development of ESRD. The Effects of Divorce on Parents and Children Leyla Erdim* and Ayşe Ergün** *Istanbul University Faculty of Health Science, Istanbul, Turkey **Marmara University Health Sciences Faculty, Istanbul, Turkey Divorce is an important decision that ends the unity of family when a marriage cannot be maintained due to various reasons. Divorce is defined as an individual process where partners undergo crisis. This process, which starts with marital conflict of different length and intensity, continues with separation, legal divorce, and building a new life. Today, divorce is considered as an important risk factor that negatively affects healthy child development. Divorce leads to the structural, emotional, and psychological separation of the family and in turn, refers to the beginning of a new status for all family members and for children in particular. Therefore, children are mostly affected from this negative situation. Following divorce, children or adolescents may exhibit various psychosocial problems such as low self-esteem, academic failure, separation anxiety, intense fear and anxiety, feelings of unworthiness, accusing or attacking the parent they live with, asking less questions and playing less, delinquency, and having negative opinions about getting married and having children in the future. Children of divorces families are regarded as risky groups within the population. Nurses play an important role in developing protective and preventive health services for these children in health institutions and in schools. Nurses can support parents by handling the problems children encounter following divorce. They can help families express and share their feelings as well as preparing them for their new roles by sparing enough time for the family and the children. Nurses can provide that children and adolescents identify and use active coping methods appropriate for their age in the face of divorce. They can also develop school based intervention programs for children and adolescents. They can help children receive support from parents, friends or other adults. Effect of Leucine on MAPK/ERK Pathway of Liver Regeneration After Parcial Hepatectomy in Rats Emre CEYHAN Eskisehir Osmangazi University Eskisehir / TURKEY Studies have shown liver has more advantages with the ability to proliferation and begins to replicate the mass against damage various reasons towards completing from other organs in the body. Surgical removal of a portion of the liver lobe (partial hepatectomy - PH) and hepatocyte damage from viruses or chemicals in such cases is seen to increase hepatocyte replication. Liver regeneration is accepted to regulate as a result of the complex interaction stimuli between stimulators and inhibitors. The start of the growth response of hepatocytes and non-parenchymal cells, is a result of complex interactions between extracellular matrix, metabolites and nutrients. When the signaling molecule binds to the receptor, initiates intracellular responses involved in the regulation of a number of significant events such as cell proliferation, cell differentiation, migration, behavior, metabolism and apoptosis. MAPK (Mitogen-activated protein kinases) / ERK (Extracellular signal regulated kinase) is One of the most important signaling pathway stimulated liver after PH. The importance of nutritional and metabolic support in liver regeneration is demonstrated by studies. Especially glucose, fatty acids and certain amino acids have benificial effect in liver regeneration. Especially branched-chain amino acids (BCAA, such as valine, leucine or isoleucine) being a substrate for protein synthesis, as well as in many cell function is demonstrated by studies that it controls. The studies with BCAA after liver damage is about mostly the effects on serum concentration levels and HGF. In our study of the amino acid leucine in rats after partial hepatectomy, liver regeneration effects on MAPK / ERK pathway was to investigate out. The effects of Leucine on liver regeneration were assessed in Wistar albino male rats after 70% partial hepatectomy (PH) in vivo, and taken 6h PH. 5 groups with n = 6 were formed. Groups I were the sham control groups; a single dose of saline, Groups II were the PBS control groups; a single dose of PBS, 1g.kg-1dose Leucine, Glutamine and Valine was dissolved in 1ml 10 mM solution PBS. Leucine, Glutamine and Valine was injected intraperitoneally to the other 3 groups immediately after PH. Using RT-PCR and Western blot analysis, the Raf-1, Mek-1, Mek-2, Map-44, Map-42, Elk, ETS (E-26), and cmyc gene expression and protein levels were measured Western blot. According to the data obtained, Leucine had a significant role (P < .05) in increasing the process of liver regeneration when given intraperitoneally. In rats receiving 1g.kg-1dose leucine intraperitoneally, this agent induced hepatic regeneration 6 hours after PH (70%).

Polymorphism of genes as marker of effectiveness of statin therapy in patients with hypercholesterolemia Mal G.S. Kursk State Medical University, Russia The aim of this study was to evaluate whether the polymorphism of the gene determines responsiveness to rosuvastatin therapy. Methods. The study was designed as a prospective trial to assess effects of lipid-lowering therapy with a focus on the LDLc level from baseline up to 48 weeks in patients receiving rosuvastatin therapy. The study included 62 hypercholesterolemic men with ischemic heart disease aged 40-60 years (predominantly of Russian origin) who were upon rosuvastatin treatment (10 mg/day) and whose plasma lipid profile was determined before and at 4, 8, 24, 48 weeks. The percent change in the median LDLc level at each follow-up time point were calculated as concentration at week minus concentration at baseline (before treatment) divided by concentration at baseline, times 100.. The HindIII polymorphism of the LPL gene was genotyped through a real-time PCR TaqMan assay. Median and 25-75% inter-quartile range were used to describe lipid parameters in patient at 4, 8, 24, and 48 week of the treatment. Changes in the LDL level over time were analyzed by the Friedman two-way ANOVA. Nonparametric Wilcoxon’s matched pair test was used for inter-group comparisons from baseline to week 48. Effects of the LPL genotype on the LDL level at each time point was assessed through Kruskal–Wallis one-way ANOVA and Mann-Whitney U-test. Statistical analysis was performed using Statistica v.10 (StatSoft Inc., USA). Results. The frequencies of LPL genotypes +495GG, +495GT, +495TT were 52,5, 44 and 0,5% respectively. The genotype frequencies were in an agreement with the Hardy–Weinberg equilibrium. The median LDLc was decreased on 3.32, 1.92, 1.74 and 1.65 at weeks 4, 8, 16 and 48, respectively. Overall compared with the baseline value, the median LDLc decreased on 20-fold at week 4 and on 60-fold at week 48 (Friedman two-way ANOVA test, p<0.0001). Three genetic models were tested (additive, dominant and recessive models). Kruskal–Wallis ANOVA test did not reveal any significant effect of the LPL genotype the LDLc level at baseline or each follow-up time point, however, Mann-Whitney U-test did in recessive model. +495TT genotype of HindIII polymorphism found a statistically significant effect on the basal level of triglycerides (p=0.031) and +495TT genotype of HindIII lead to the low efficacy of the rosuvastatin treatment (p=0.012). Conclusion. The HindIII polymorphism of the LPL gene has significant effect on the LDLc levels in hypercholesterolemic men with ischemic heart disease who were upon rosuvastatin therapy.

RESORBABLE POLYMER HYDROGEL IN PREVENTION OF ABDOMINAL ADHESIONS V.A. Lipatov Kursk Medical University, Russia, Department of Operative Surgery and Topographic Anatomy The Aim of the research is to decrease the risk of the adhesion formations and adhesive complications after the operations on the abdominal organs. The biocompatible aquatic solution of an indifferent polymer compound of cellulose and it represents a gel which has optimum physical and chemical properties to use in a surgery. This can be used as a self-maintained preventive agent, and as well as basis for the multicomponent agents with the incorporation in a compounds of the compositions of Anesthetics, immunomodulating factors, anti-inflammatory preparations, reparants, cytostatics, antiseptics, antibiotics and cell-like substances etc. Treatments by the gel can be used for the prevention of the desiccation of the peritoneum during the long-time operations, by an injection in laparoscopic surgeries after an adhesiotomy (including, after adhesiolysis of the pelvic organs in a treatment of the tubo-peritoneal infertility. The preparation of multicomponent compositions and their application is done "ex tempore", and without losing the viscosity the gel can be admixed with the small volumes of solutions of medication. The preparations which entered in the compound of the gel dose not decrease its efficiency and the gel dose not influences the effects of the medications too. The combination of the gel has strengthened its’ anti-adhesive effects. Control of the preparation in many areas of the intervention in medicine is important and it has not solved the problem. In use of the antiadhesive gel there is a possibility to decrease the probability of the formation of an amount of the connective tissue and the various complications. Its’ used in common surgery, cardiosurgery, neurosurgery, vascular surgery, traumatology, operative gynecology, otolaryngology, cosmetic medicine, urinology, oncology and in other ranges. In experiments it was confirmed absence of the negative properties of the hydrogel. It can’t be ground for the breeding of germs, it doesn’t have toxicity, teratogenicity, oncogenicity, adjuvanticity. The main principal mechanism of the hydrogel is to effects the "hydroflotation" of the tissues and prevention of its consolidation and formation of adhesions. By using experimental animal model we showed the usage of the hydrogel significantly reduces the severity of adhesions. This was proved both the primary adhesion and the repeated formation of adhesions after their separation.

Effect of Rhythmic Auditory Stimulation On Gait In Patients With Stroke Mohamed Fouad Faculty of Physical Therapy, Cairo University, Egypt Background: Stroke is the most leading cause to functional disability and gait problems. Objectives: The purpose of this study was to determine the effect of rhythmic auditory stimulation combined with treadmill training on step length (on both affected and non affected sides) in stroke patients. Methods: Thirty male stroke patients participated in this study. The patients were assigned randomly into two equal groups, (study and control). Patients in the study group received treadmill training combined with rhythmic auditory stimulation in addition to selected physical therapy program for hemiparetic patients. Patients in the control group received treadmill training in addition to the same selected physical therapy program including: strengthening, stretching, weight bearing, balance exercises and gait training. Biodex gait trainer 2 TM was used to assess step length (on both affected and non affected sides) before and after six weeks training period (end of treatment) for both groups. Results: There was a statistical significant increase in step length (on affected and non affected sides) in both groups post treatment. The improvement in step length post treatment was significantly higher in the study group compared to the control. Conclusion: Rhythmic auditory stimulation combined with treadmill training is effective in improving step length in stroke patients when added to the selected physical therapy program. The Relationship between Sociodemographic Characteristics of Dyslexic Children and their Higher Perceptual Function after Cognitive Training Program Sanaa Shaheen Alexandria University, Egypt Dyslexia is a failure to read at expected level, it has been the focus of much discussion and research among physicians, educators, and psychologists. The sociodemographic characteristics of dyslexic children suggested to being affect on their perceptual function. Nurses are the key persons to identify the risk factors for reading disability; hence they have an important role in assessing children for any reading disability and help them to cope normally with any psychological distress. The aim of the study: identifying the relationship between sociodemographic characteristics of dyslexic children on improving their perceptual function Subjects and methods: The study subjects were composed of 50 children (30 boys and 20 girls). The study was conducted at child outpatient clinic of Psychiatric Mental Health Hospital and outpatient clinic of Assiut University Hospital during one year duration from 1st November 2012 until 30th November 2013. Childern were assessed through visual and auditory memory tests, in addition to Socio economic assessment scale. Results: there was significant difference as regard sex in which females outperform than males but there was no significant difference as regard the age, residence, and socioeconomic levels with visual and auditory memory tests. Conclusion and Recommendation: Sex only plays an important role in improving the cognitive function of dyslexic children in which girls outperform boys so intensive attention should be given to male dyslexic children to improve their cognitive function. The prevalence of depression, anxiety, and stress among Saudi dermatological patients Yazeed Faris Altebainawi Riyadh, Saudi Arabia Background: Little is known about Saudi dermatological patients regarding the prevalence of depression, anxiety, and stress and their associations with socio-demographic characteristics. Methods: A cross- sectional study was conducted at King Abdulaziz Medical City-Riyadh (KAMC-R) on 254 Saudi patients with a response rate of 85%. The Arabic version of the Depression, Anxiety, and Stress Scale (DASS) was used to screen for symptoms of depression, anxiety, and stress in the study population. Socio-demographic characteristics were obtained from each patient. Results: Of the 254 dermatological Saudi patients surveyed in this study, 24.4% had at least one of the three psychological symptoms. The prevalence of depression, anxiety, and stress in our samples was 12.6%, 22%, and 7.5%, respectively. According to our study, depression was significantly higher in Saudi dermatological patients with lack of family support than those with family support (26.5% vs. 10.7%, P=0.006). Anxiety was less common in patients who engaged in physical exercise compared to patients did not (14.5% vs 29.4%, P=0.005). Poor quality of life was found to be associated with stress (9.5% vs. 0.0%, P=0.016). Conclusion: It was estimated that approximately 2 out of 10 Saudi dermatological patients suffer from at least one psychological symptom. The primary reasons for the worsening psychological symptoms were lack of family support and poor quality of life. Interventional study is needed to examine the effects of social and family support on psychological symptoms among Saudi dermatological patients. EXPLICATING THE MEDICATION ADMINISTRATION AND FORMULATION PREFERENCES OF SCHOOL-AGED CHILDREN: A CONJOINT ANALYSIS Adrienne Dionisio, Helen Joy Dizon, Theuzelle Ann Dipasupil, and Les Paul Valdez University of Santo Tomas College of Nursing, Manila Philippines Background: By and large, the need to construct guidelines, increase availability (Kristensen, 2012; Gupta & Khan, 2013) and access to age specific pediatric formulations(Kristensen, 2012; Cram, Breikreutz, Desset-Brethes, Nunn & Tuleu, 2009; Kristensen, 2014) cannot be underestimated. However, school-aged children's administration and formulation preferences remain to be a blind spot in research. Objectives: This study aims to explicate the medication administration and formulation preferences of school-aged children as it relates to the flavor, consistency and frequency of administration of medication. Methods: A conjoint analysis survey using orthogonal cards was fielded to three hundred-seventeen (317) school-aged children. Importance and utility values relative to medication administration and formulation were generated using SPSS version 21. Results: Results of the conjoint analysis indicate that medication form (38.647%) and palatability (35.468%) received the highest importance values. Frequency (25.855%) for its part, was the least important attribute. In addition, school-aged children prefer orange-flavored (.226) chewable (.315) medications administered twice a day (.055). Implications: This study affords policy and practice implications on how interdisciplinary collaboration can put premium to drug development, medication formulation and administration. Today, more than ever, members of the health care team are challenged to enter a realm where flexibility is balanced with medication efficacy.

It is aimed in this study “the examining of relationship between body mass index (BMI) with the social appearance anxiety levels and self-esteem of adolescents””” The research was realized between dates of May 25-29, 2015, Samiha Ayverdi Anatolian High School and Vatan Vocational and Technical Anatolian High School with receive education students in 270 adolescents in the Fatih district of İstanbul province. The datas were collected using prepared in accordance with the literature Data Collection Form by researcher, Social Appearance Anxiety Scale and Rosenberg Self-Esteem Scale. NCSS 2007 program was used for statistical analysis. The level of significance p<0.01 and p<0.05 level were evaluated. Of the adolescents in the research group were determined %55.6 girl, %44.4% male, average age of 17.25±0.74, %, 20.7 slim, %63.0 normal weight, %9.3 overweight and %7.0 were obese. Self-Esteem is a high percentage of adolescents in social appearances anxiety levels were observed that statistically significant low (p=0.001; p<0.01). Adolescents, BMI with body image determined by measuring between the actual of body structures level of are found to be statistically significant difference (p=0.003; p<0.01). Adolescents, between the social anxiety levels and self-esteem with BMI (p> 0.05) was not observed statistically significant difference (p> 0.05) body image and self-esteem showed a statistically significant difference between (p=0.024; p<0.05). Consequently, while there was not determined the relationship between BMI with the social anxiety levels and self-esteem in adolescents, self-esteem high levels that adolescents the social appearances anxiety levels were found to be low. In this study, we address the problems and nursing care and education in patients with enterocutaneous fistulas. Introduction: Enterocutaneus fistulas most frequently occur postoperative period (about 75-85%). Enterocutaneous fistulas may appear the first 5 or 6 days after surgery. A fistula becomes apparent within 24 hours and enteric contents from the open wound surface. The aims of therapy for an enterocutaneous fistulas effective management plan must achieve the following goals: skin protection, patient comfort and mobility, containment of drainage and odor, correct metabolic and nutritional deficits, close the fistula, and reestablish continuity of the gastrointestinal tract. Discussion: The main objective is to provide healing of the wound. Skin protection and prevention is key components in wound care. Control of the fistula drainage is important for skin integrity. Skin discomfort can seriously affect the patient if the proper management technique is not used. Various materials such as: skin barriers, adhesives, dressing and pouches, can be used to protect the skin. The skin around a fistula must be clean and dry for effective pouch adherence. The success of surgical therapy may be improved if excoriation or superinfection of the skin surrounding the fistula tract can be prevented. Odor control is best controlled with the use of a pouch. Deodorants are available in tablet, liquid, or powder forms. Albumin level (More than 3 g/dl) that is required for the optimal recovery of intestinal function. Adequate protein and calories, vitamins and trace elements should be added to enteral formulas and parenteral formulas to prevent nutritional deficiencies. Total calorie requirements are calculated according to clinical condition of the patient. Nitrogen balance should be achieved in order to restore protein synthesis. Protein requirements are adequately replaced (1.5-2.5 g/kg/day). Adequate nutritional support is essential in the postoperative process. Zinc supplementation may be necessary with high output fistulas. The prevention fluid and electrolyte imbalance is required for wound care. Fluid requirements are calculated according body weight. Carefully monitored replacement of the losses fluids is essential and is often paired with central venous monitoring to accurately estimate fluid deficits. Conclusion: Successful management of patients with enterocutaneous fistulas, especially nurses, multidisciplinary team such as; dieticians, pharmacists, physicians, surgeons etc. play important roles. The Rate and Factors of Turnover of Bahraini Nurses and the Implications For the Health Services Mariam Tarada Kingdom of Bahrain Nursing turnover presents real challenges to healthcare managers all over the world. The problem has been linked to many demographic and organizational factors. Job satisfaction and intent to leave are closely related to both categories of factors and could play a role, on their own, in the turnover issue. The aim of the study is to measure the rate of turnover among Bahraini nurses in five government hospitals. The study was done on a random sample of 113 nurses in five government hospitals in Bahrain. The study was done on two phases. During the first phase, statistical information and data were collected from official sources about the actual numbers of resigned nurses, those who transferred to other non-hospital settings in the Ministry of Health, and those who have submitted official requests to change their location of work. The second phase utilized a survey questionnaire to collect information about causes of the turnover. Twenty percent of the questionnaire questions utilized Likert scales and the rest were closed-end questions. The response rate is 52.2%. The findings of this study identified certain organizational factors like poor nursing management, the lack of proportionality of pay to effort, and injustice in the promotion systems as the main reasons for turnover. Other factors of importance were the weak participation in the decision making process and the feeling of unhappiness in the place of work. The demographic factors that were reported to be highly linked to turnover were young age, longer years of experience, lower levels of academic qualifications and kinship responsibilities. However, job satisfaction rated high as it was linked to the pride and commitment to the profession. The nurses who expressed their intent to change their location of work rated the highest among the turnover group whereas the lowest rate was among the resignation group.

Determinants of Skilled Birth Attendants' Service Utilization in Asunafo North Municipal Area in Ghana George Achempim Kwame Nkrumah University & Technlogy, Ghana To address the discrepancy in antenatal care (ANC) attendance and skilled birth attendant (SBA) service utilization, a descriptive cross-sectional study was conducted to examine the socio-cultural and economic factors; health seeking behaviour; and service provider factors influencing access and utilization of SBA service in Asunafo North municipal area in Ghana. With current SBA utilisation of 48.9%, and anticipated 56% use of SBA services among the study population at 95% confidence interval, powered at 80%, a total of 392 women (who had delivered within 24 months prior to the study) participated in the study; taking into account 10% refusal rate. Sixteen (16) health facility managers in the study area were also involved in the study. The 392 women were selected using multistage sampling technique, while the 16 health facility managers were selected using purposive sampling technique. With the aid of two survey instruments (questionnaires); the women were interviewed, and the survey instrument targeting the facility managers was self administered. The results showed that poverty and distance were the most important barriers preventing pregnant women from seeking ANC and supervised delivery followed by low levels of education, and trust & confidence in Traditional Birth Attendants (TBAs). Marital status was found to be significantly associated with SBA service use. Women in cohabitation relationship were 8 folds more likely to use SBA services compared to those who were single. Women in marriage were 6.5 times more likely to use SBA services at odds ratio=6.54 (95% CI: 1.04, 11.37, P=0.04). Eighty five percent (85%) of all births among the respondents were attended by skilled birth professionals as opposed to 15 percent of births that occurred outside health faculties (8% attended TBA, and 7% unassisted birth). The study concludes that poverty (lack of money), distance and the client perception of quality of service are the most important determinants access and utilization of skilled birth attendant's service in the study area. It is recommended that the free maternal health in Ghana's National Health Insurance Scheme should be strengthened to ensure increased access and utilization of SBA services and health staff should improve quality of service delivery in municipality to motivate pregnant women in the municipality to utilize skilled birth attendant's services to improve birth outcomes of mother and child.

Talk Active As a Promoting Program for Mental Toughness through Peer Educator for Youth in Junior High School Jakarta 2015 Anita Putri Damayanti and Anida Hanifah University of Indonesia Health is a state of complete physical, mental, social, sexual, emotional and spiritual (WHO, 2014). Because health is a human right so each person is entitled to the same health services. Mental Health in Indonesia has not been a priority in the planning of the program. The prevalence of mental health disorders in Jakarta around 5.7% .Where WHO says that half of people with mental health disorders 14 Years old. Age is a productive adolescence. In the demographic bonus dependence rate of 0.4-0.5 wherein the non-productive age 40-50 will be covered by 100 people of working age. When the productive age declined because of mental health problems will decrease the quality of human resources in Indonesia. Therefore, it takes mental toughness high in adolescents to be productive. One of the efforts made through the promotion of mental toughness in junior high school students in Jakarta. Depression in adolescents leads to suicide. The main cause of this problem is the inability to resolve the problems encountered. The inability to resolve this issue is one of mental health disorders. In the curriculum taught in junior high are subjects that can be inserted mental counseling endurance material. In addition, one of the objectives and functions of this study is to assist students in making healthy decisions. The method of data collection is done by distributing questionnaires SDQ (Strengths Difficulties Questionnaire) as many as 50 pieces in junior high school students in East Jakarta X at random, as well as some interview questions on students and counseling teachers. Furthermore, the authors undertake the collection of information through the research literature, books related and some journals, moreover the authors collecting secondary data on health services in Jakarta. Some suicide cases that committed by Junior High Students. One of them is the junior high school students X in East Jakarta. In addition, based on interviews to the students the implementation of counseling subject less proactive and focused on discipline problematic students. Hence, Prevalence of junior high school X students who have an indication mental health disorders around 29.8%. And 36.2% students stated that sometimes bullied by friends or seniors. They have been feeling insecure while in school and in need of mental endurance. Hereby, the Talk Active program is an innovation in the promotion of mental toughness in junior high school students X. This program with peer educators approach which makes the peers as tutors who can share with each other related problems. Moreover, through this approach allows teachers in early detection of indication of mental health disorders so will be easier in prevention. Talk Active program in collaboration with stakeholders such as the Department of Health, Schools and others. Peer educators and teachers will be trained and given the information related to mental health. In the implementation also uses manual on mental health. Peer educators as much as 4-5 people in each class with the criteria helpfulness. Implementation of the program is at the moment Counseling class with methods of Focus Group Discussion (FGD). While evaluation program teachers and families will use research instrument SRQ (Self Reporting Questionnaire). The process of evaluation program will conducted like a talk show between teachers, Family and psychiatry. There will be discussions on the problem and its prevention measures. This process is carried out in order to create sustainable long-term goal of the program so that the decline in adolescent mental health disorders, especially in the junior X can be achieved.

Language Barrier in Geriatric Hospital Buthaina Al Asfoor Kingdom of Bahrain Health care providers in elderly residential institutions find many impediments to health care provision such as culture, finance, ethics, and language issues. Among those, the role of language deserves special attention due to its pervasive effect on the health care delivery. People are usually familiar with the problem of finding it hard to communicate their needs in a language that is not their first language. Elderly, in specific, has a worsen situation in which they may suffer from communication dysfunctions such as aphasia and dysphonia that may hinder them from conveying messages. The ability of elder people to effectively communicate is essential to maintain their independence and relationships. Literature review revealed that the language barrier in hospitals may result in patients feeling of loneliness, anxiety, frustration, helplessness, or depression. In addition, misunderstanding can lead to serious consequences, such as misdiagnosis, patients’ inability to comply with the treatment, and distrust caregiver. Therefore, the effectiveness of communication between clinicians and patients found to be correlate with patients’ satisfaction and compliance. Irrespectively that the native language in the kingdom of Bahrain is the Arabic, non-Arab health care providers constitute around 70% of the total number of nurses working in geriatric hospital. This paper aims to study the problem of language barrier in geriatric hospital in Bahrain. It is assumed to help nurses highlight the factors contributing to the problem of language barrier, analyze them and set practical recommendations. A qualitative approach is used. A sample composed of eight participants; four nurses and four elderly residents are interviewed. Analysis by using fishbone and SWOT tools is used. Results show that the psychosocial and physical needs of elderly are partially neglected. The main reason is the language barrier evoked by a large number of multicultural nurses in geriatric hospital. The conclusion involves implications in order to overcome the language barrier in elderly health care settings. Dominancy for nurses is training to break the language barrier, which might be hard, but the professional rewards are great.

Prevalence of Alleles Associated with HIV Resistance in Turkish Population Ayşe BALCI*, Zeynep YEĞİN*, İsmet BERBER *, Haydar KOÇ** *Sinop University, Turkey **Çankırı Karatekin University, Turkey Background: Genetic polymorphisms of CCR5, CCR2, and SDF1 genes exhibit protective effects against HIV-1 infection and disease progression. Individuals homozygous for CCR5-Δ32 mutation are resistant to infection by HIV-1 virus while those heterozygous for this allele have a limited resistance or have a delayed onset to AIDS when infected with HIV. Individuals carrying CCR2-64I variants have a decreased risk of AIDS progression after infection and also AIDS patients carrying these variants show favorable prognosis to HAART (highly active antiretroviral therapy). SDF1-3’A has also been associated with protection to AIDS progression however this effect may be modified by CCR2 genotype. The aim of the study was to determine these three allele frequencies in healthy individuals in Turkey. Methods: A total of 205 unrelated healthy Turkish participants were enrolled in the study. The healthy individuals were randomly selected mostly among the university staffs of both genders aged over 18 years. DNA extraction was performed with the standard salting-out method of Miller et al. Genotypes were detected by polymerase chain reaction (PCR) for CCR5-Δ32 or by PCR/RFLP (Restriction Fragment Length Polymorphism) assay with the digestion of restriction endonuclease BseGI for CCR2-64I and MspI for SDF1-3’A. Results: Allele frequencies of these genes in the population were: 3.17%, 17.80% and 27.07% respectively for CCR5-Δ32, CCR2-64I and SDF1-3’A. All the observed genotype frequencies were in accordance with Hardy-Weinberg equilibrium. No homozygous CCR5-Δ32 mutation was identified in our study, of a total of 205 individuals 94% was homozygous wild type (wt/wt) and 6% was heterozygous (wt/Δ32). The prevalence of 69%, 27% and 4% was found respectively for the CCR2-64I homozygous wild type (GG), heterozygous (GA) and homozygous mutant (AA). The prevalence of 52%, 42% and 6% was found respectively for the SDF1-3’A homozygous wild type (GG), heterozygous (GA) and homozygous mutant (AA). CCR5-Δ32 allele frequency is relatively lower compared to European countries but higher compared to Asian and African countries. CCR2-64I and SDF1-3’A allele frequencies seem similar with many countries and ethnicities in different continents. Allele frequencies of all three polymorphisms are quite similar with neighbour countries Greece, Crete, Greek Cypriots, and especially Bulgaria. Conclusion: The investigation of genetic polymorphisms conferring resistance to HIV-1 is important in terms of a better characterisation of genetic epidemiology of HIV-1 infection, determination of higher-risk people and individualized treatment approaches for HAART in future. Kurdish Women’s Labour and Birth Experience Kazhan Mahmood, Tony Blackett, Anne Peat, and Sharron Hinchliff The University of Sheffield - United Kingdom Introduction: The journey of having a baby may be similar for all women in terms of physiological details but its meaning to each woman is individual and unique because of the social, cultural, spiritual, emotional and psychological context of her life. Every woman experiences labour and childbirth differently. Women’s labour and childbirth experience is considered one of the essential aspects for health policy makers in developed countries, but Kurdish women’s labour and birth experience has received little attention. No previous studies have explored women’s experiences during labour and birth in Iraq generally or the Kurdistan region particularly. Therefore, this study is intended to examine this concern in Iraqi Kurdish settings, which is a context with unique environmental and cultural beliefs. Purpose: This study aims to assess Kurdish women’s experience of labour and birth and identify awareness of labour pain relief of those women who went through labour and delivery. The study also intends to identify women’s desire for pain relief. Methods: A survey using the ‘Childbirth Experience Questionnaire (CEQ)’ was conducted on women who underwent labour and birth in a Maternity Teaching Hospital in one of the Kurdish governorates in the Kurdistan region of Iraq. Formal ethical approval was obtained from the Ethics Committee of both the University of Sheffield and Hawler Medical University. Permission to carry out the study was obtained from the director of the hospital. Results: The data were analysed using a software package (SPSS version 22): both descriptive and inferential statistics were derived. The total number of participants was 256 and the mean age of the study sample was 26.39. Most (69.2%) of the study sample were multiparous with 30.8% of them primiparous. There were highly statistical significant relationships between participants’ educational level and greater number of antenatal visits (p<0.001). Regarding the whole labour and birth experience, 58.6% of the study sample were dissatisfied with their whole experience whereas 41.4% of them were satisfied. The majority (83.59%) of women graded their childbirth as painful yet 81.65% were unaware of labour and birth pain relief options. Surprisingly, antenatal visits had no effect on women’s awareness concerning labour and birth pain relief as none of them stated antenatal visits as a source of their information. Approximately 92% of women wished to have pain relief for any future birth. Conclusion and Recommendation: It was evident that those women who were educated to degree level were more knowledgeable regarding pain relief options than those who were not. More informative childbirth classes should be provided in the antenatal clinics in order to prepare pregnant women for labour and birth. Pain relief measures should be available to all women going through labour and birth.

Investigation of multidrug-resistant Acinetobacter baumannii Nosocomial Infection in Uthai Thani Hospital, Thailand Kanchana Usuwanthim, Chanihcha Anudit and Nateelak Kooltheat Department of Medical Technology, Faculty of Allied Health Sciences, Naresuan University, Phitsanulok 65000, Thailand Background and aim: Infections caused by Acinetobacter baumannii have become an important problem of patients in Asia and worldwide. The aim of the study is to investigate multidrug-resistant Acinetobacter baumannii (MDR A. baumannii) in Uthai Thani Hospital. Method: There were 2 study designs consisted of observational and experimental for bacteriology data. The observational study was performed during 2006- 2014 from antibiogram data. Bacterial isolation and drug susceptibility testing were characterized from 200 random hospital surface sample swabs and clinical samples from over 48 h hospitalized patients between September and December 2010. The relationship between A. baumannii strains was resolved by repetitive extragenic palindromic (REP)- and enterobacterial repetitive intergenic consensus (ERIC)-polymerase chain reaction. Result: The antibiogram data of Uthai Thani Hospital showed that there was increasing antibiotic resistance in A. baumannii from 2006 to 2010 and then trend was decreasing from 2010 to 2014. While the bacterial identification of clinical samples and hospital surface sampling swabs between September and December 2010 found 35 MDR A. baumannii clinical isolates and 5 MDR A. baumannii hospital environment samples. REP- and ERIC-PCR demonstrated that all isolates belonged to a unique clone. Conclusion: There were the spread of a clonally related strain of MDR A. baumannii of patient and environment swab cause of nosocomial infection in Uthai Thani Hospital.

Session 3: Technology and Innovation Sensitivity Analysis of An Unwinding-Idle-Winding System for Steel Plate Based on Vibrational Data of A Roller YONGHUI PARK Graduate school of Engineering Mastership, POSTECH, Korea We establish an unwinding-idle-winding system for steel plate that is based on a winding system for a web material. Previous linearized model for the web is developed by including non-linear properties about geometric variation of the roller and the steel plate. Additionally, friction problem between the steel plate near entrance of the unwinder and the winder, planar motion of the roller, unbalanced mass problem on the roller and wounded steel plate, and pre-wounded the steel plate in the unwinding roller are considered to check dynamic behaviors according design variables. We arrange an equation of motion about generalized coordinates by Lagrange’s equation. Also, automatic code that derives geometric information about position and orientation of each roller and the steel plate is established and is reflected into numerical iteration. We classify dynamic results into design variables and environment conditions, and give a physical mean. Tangential and adjoin points between the roller and the steel plate is important factor to determine the steel plate’s tension. The steel plate’s tension acts as a linear spring between each roller. Especially, bearing of the roller interacts each span; the steel plate, because of high elasticity of the steel plate. The unwinder and the winder has symmetric relation by winding and unwinding the steel plate, but physical quantity including bending stress of the coiled-steel plate and inertia of moment of the unwinder and the winder is not exactly same due to winding tension. Based on the fact that all design variables are co-related, we suggest several detecting methods to find a resource of physical phenomenon by comparing vibrations in the time-frequency domain. Especially, PLC(Programble Logic Control) technology based on IEC-61131 already use widely. The purpose of this paper is to forecast technology diffusion using patent information. First, we make an experiment with bass model and patent data. Second, we estimate suitability between forecasting and real data. Plant-Assisted Synthesis of Nanoparticles using South African Medicinal Plants Roshila Moodley University of KwaZulu-Natal, Durban, South Africa Certain metal and metal oxide nanoparticles such as Ag, Au, Se or ZnO exhibit biological activity, e.g. antioxidant, antibacterial or anti-cancer activity. Indigenous plants containing phytocompounds have been used medicinally by South African traditional healers to treat a variety of ailments for centuries. In our laboratory, we focus on extracting, isolating and identifying these compounds and systematically testing their biological activity. The combination of nanoparticles and secondary metabolites from medicinal plants where these act as reducers and end-capping agents and impart their biological activity on the nanoparticle is a promising “green” approach to nanoparticle synthesis, presumably resulting in more compatible nanoparticles for biomedical applications. The combination of nanoparticles and phytocompounds is expected to lead to combined and synergistic biological effects. In this study, gold and silver nanoparticles were grown from a solution of their salts using secondary metabolites of South African medicinal plants including Moringa oleifera and Harpephyllum Caffrum as reducing and end-capping agents. Grown nanoparticles were characterised using UV-vis Spectroscopy, Fourier-Transform Infrared Spectroscopy (FTIR), Nuclear Magnetic Resonance Spectroscopy (NMR), Gas-Chromatography Mass-Spectroscopy (GC-MS), X-Ray Diffraction (XRD), Scanning (SEM) and Transmission Electron Microscopy (TEM). The effects of varying concentrations of plant extract, pH and reaction temperature on size and shape of the nanoparticles were systematically studied. The antioxidant activity of crude plant extracts, pure isolated phytocompounds, free-standing nanoparticles and nanoparticles end-capped with the active phytocompound was tested using the DPPH• (2,2-diphenyl-1-picrylhydrazyl) assay. It could be shown that the DPPH radical scavenging activity was significantly higher for the end-capped nanoparticles. Recently, carbon dots (CD) another 0D carbon nanomaterial has emerged as new class of carbon nanomaterials. While either top-down or bottom-up methods have been utilized for the preparation of water soluble CDs with interesting optoelectrical properties, bottom-up methods based on the naturally abundant small molecules have many advantages including the scalability and non-toxicity, which is critical for the mass production of CDs. In this research, microwave-assisted preparation of CDs is attempted based on an amino acid, lysine. Simple microwave treatment of lysine by household microwave oven within 10 min accomplishes the formation of strongly blue-emitting CDs with high mass yield and good solubility in aqueous media. The fluorescence quantum yield of lysine-based CDs approaches 23% and typical excitation-wavelength-dependent fluorescence emission is demonstrated. Through the low cytotoxicity of lysine-based CDs, CDs can be utilized as bioimaging agents into living cells.

Microwave-assisted simple preparation of carbon dots from A2 and B3 type polyamide monomer sets Ari Chae, Yujin Choi, and Insik In Department of Polymer Science and Engineering and Department of IT Convergence (Brain Korea PLUS 21), Korea National University of Transportation, Chungju 380-702, South Korea Recently, intensive research interests have been focused on the formulation of water soluble and highly fluorescent carbon dots (CDs) as new functional carbon nanomaterials. While most of previous researches have attempted the utilization of A3 (A: -COOH) and B2 (B: -NH2) type polyamide monomer sets for the synthesis of CDs through either hydrothermal or microwave operation, the utilization of A2 and B3 type monomer sets can be advantageous because various types of naturally abundant dicarboxylic acid compounds can be used as A2 type monomers. In the detailed experiments, succinic acids (SA) and tris(2-aminoethyl)amine (TAEA) have been used as both A2 and B3 type polyamide monomer sets. After simple microwave operation within 10 min, highly fluorescent and dispersible CDs are easily prepared and isolated with high mass yield. The resulting CDs demonstrated the quantum yield (QY) up to 17.5% and intense blue fluorescence upon the irradiation of ultraviolet light. Through the good solubility of prepared CDs in polar solvents including water, methanol, and ethanol, various applications of CDs are able to be attempted. Formation of coagulants as hydroxometallic ion complexes from the soluble anode such as iron and/or aluminum oxidized due to the applied current are used to destabilize and coagulate the suspended particles and to adsorb dissolved contaminants in electrocoagulation (EC). For the iron electrode some of the resulting polymerized hydroxometallic ion complexes can be Fe2(OH)2

+4 and Fe2(OH)4+5 (1), (2). In a variation of the process called the peroxi-

electrocoagulation externally added hydrogen peroxide generates Fenton reactive system and synergistically works with electrocoagulation (3). In this study peroxi-electrocoagulation of mixed raw industrial wastewater from the inlet of organized industrial district wastewater treatment plant was investigated using soluble iron electrodes. Studies on the parameters such as current density, supporting electrolyte concentration and initial pH, which have significant effects on COD removal and hence the energy consumption, were performed. Hydrogen peroxide was used in different concentrations to observe it’s effects on COD removal efficiency and the energy consumption. Sludge productions as gram sludge per gram COD removed were calculated for all experiments. COD removal efficiency of >90% was obtained at the best experimental conditions. We report in this work how to elaborate the In203 at the reduced dimensional scale by adopting the stimulated oxidation by the electron beam on the indium phosphcm2. The characterization of the surface is done using the sensitive spectroscopy techniques AES ( Auger Electron Spectroscopy) and ELS ( Electron Energy Loss Spectroscopy) to display the reactive character between oxygen and indium. We note that indium develops on the InP by the argon ion bombardment. The electron beam of 3KeV energy and 10 –3A.cm-2 current density stimulates the oxidation of that indium by the small amount of oxygen present in the residual vacuum of 10 -9 Torr of the analysis chamber of the hemispherical spectrometer operating in direct mode N(E).

Optimum overhang dimensions for a sustainable housing in northern Algeria Khaled Imessad Renewable Energy Development Center, Algeria An analysis of the energy balance of Algeria shows that energy exports fell by 120 Million Tonne of oil equivalent (Mtoe) in 2010 to 101.5 Mtoe in 2013, while domestic final consumption is increased from 31.7 Mtoe in 2010 to 38.5 Mtoe in 2013, reflecting an annual growth of 7% since 2010. Rising conventional energy prices and environmental considerations increase interest in the integration of energy efficiency solutions and the use of free and inexhaustible energy sources. From the current distribution of energy consumption by sector, it appears that 41% of energy consumption in Algeria is allocated to the building sector. Most of this energy consumption is due to space heating and cooling. Due to rapid economic growth and growing desire for better indoor environment, energy demand for heating and cooling will continue to increase steadily. Thus, building sector has the greatest potential for energy savings. Beside the U-value of external walls, solar gains through windows are an important parameter for designing energy-efficient buildings. Direct solar radiation transmitted into the rooms is considered as an important contributor to building cooling load. At the same time, solar heat gain through fenestrations contributes to reducing energy demand for heating in winter period. Above south facing windows, fixed horizontal shading (eaves or overhangs) are provided as a way of obtaining a good compromise between these two effects provided that they are correctly sized to block direct solar radiations during summer but still allowing them to pass through during winter. This study has helped develop a correlation that optimizes overhangs length projection for a south-facing windows. This correlation is valid for buildings located at latitudes of about 36 N. This study concludes, considering the tradeoff between solar heat gains in summer and winter to optimize energy consumption as the best option in designing external horizontal shadings.

The magnitude of air pollution by vehicular traffic evident in genotoxic aspects of Tradescantia pallida (Rose) D.R. HUNT var.purpurea Amanda Izadora Roman, Juliana Spósito, Bruno Crispim, Liliam Candido, Alexeia Grisolia, Rosilda Mussury Federal University of Grande Dourados, Mato Grosso do Sul, Brazil. High pollutant concentrations, depending on their toxicity, may induce genotoxic effects not only in humans but also in animals, plants and bacteria, compromising the health of ecosystems. Among the cytogenetic tests used to assess genotoxic effects on living organisms, the Micronucleus Test (Trad-MCN) applied in Tradescantia pallida (Rose) Hunt. is considered a valuable method of investigation. In Mato Grosso do Sul, Brazil, the studies conducted in one of the most important agricultural hubs in the country, the city of Dourados, show a progressive effect of vehicular traffic on air quality. To accurately elucidate these effects, we evaluated the air quality in 12 municipalities that compose the Metropolitan area of Dourados, based on the genotoxic aspects of T. pallida. Material and methods: The cities involved were: Dourados, Itaporã, Deodápolis, Douradina, Rio Brilhante, Nova Alvorada do Sul, Caarapó, Jutí, Fatima do Sul, Glória de Dourados, Vicentina, Jateí and Mata do azulão (taken as a control because of an absent vehicular traffic). We selected two collection spots of T. pallida in each city according to their traffic flow (one with a heavy traffic and the other with a low traffic), where plants were already established. The samples were collected during four months of 2014 (May, July, September and November). The spots where the collections took place were marked with GPS Garminetrex LEGEND HCx. The vehicular traffic flow was obtained by counting the vehicles passing nearby the collection spot three times a day (from 8:00 am to 9:00 am, from 11:00 am to 12:00 pm and from 17:00 to 18:00 pm). Afterwards, the collected data were compared with the information provided by the IBGE\DENATRAN, the National traffic authority. The young inflorescences collected in each point were transported to the Botany Laboratory of the College of Biological and Environmental Sciences (FCBA) of the Federal University of Grande Dourados (UFGD) and fixed in Carnoy solution (3: 1), alcohol and acetic acid for 24 hours and subsequently stored in 70% ethanol. After fixing and conserving the flowers, samples were prepared in microscope slides. Five slides for each collection spot were prepared and analyzed with a light microscopy (400x). The micronucleus counting was performed on a group of 300 tetrads per slide. The frequency of micronucleus was calculated by the number of micronucleus observed in 300 tetrads, divided by the total of analyzed slides (100 slides per collection spot). The experiment was conducted in a completely randomized design (factorial 12 x 4) with 12 cities, four months and five repetitions. The means of data were subjected to analysis of variance and compared, whether significant, using Tukey test at 5% probability with the help of SAS statistical program. The relation between the frequency of micronucleus (MCN), the ambient temperature (Ta), relative humidity (Rh) and leaf temperature (Tf) was established using Pearson correlation at a probability of 5%. Results and discussion: In May, the cities of Vicentina (4.50), Dourados (3.66), Nova Alvorada do Sul (3.33) and Rio Brilhante (3.00) had a higher frequency of MCN when compared to control (0.00). In July, the cities of Fatima do Sul (12.66) and Dourados (10.00) had the highest frequencies. In September (18.33) and November (40.00), the City of Dourados showed the highest frequency of MCN when compared to other cities and control. There was a direct increase in the frequency of MCN over time and the city of Dourados showed the most significant results among the analyzed cities. The variables Ta and Tf showed, in September and November, a significant positive relation with the frequency of MCN, meanwhile Rh relation with frequency of MCN was inversely proportional and significant in every analyzed city. The increased frequency of MCN occurs when there is a drop in Rh. Conclusion: Vehicular traffic is a major factor and is directly related to the observed variables, suggesting that the efficiency of the current measures implemented to control the pollutant emissions by vehicle traffic may be reconsidered and rigorously analyzed. Vehicular traffic directly interferes in the quality of air in these cities and emerges as a health risk for the urban population.

Insecticide effect of medicinal plants from the Brazilian Cerrado Irys Fernanda Santana Couto, Bruno Sena Araujo, Fabricio Iglesias Valente, Rosilda Mussury Rosilda Mussury Federal University of Grande Dourados, Brazil. Introduction: Plutella xylostella occurs in all regions of the world and is one of the larvae that most damage the genus Brassica, occasionally resulting in a production loss of 100%. For being resistant to many synthetic pesticides, new studies aim at other alternatives of larvae control, including plant-based insecticides. This study involves plants from the Cerrado biome, which has a rich diversity of insecticides and medicinal plants, both threatened by the expansion of agriculture. Therefore, to preserve the Biome and emphasize its biological importance, we seek to assess an alternative method of insect control by evaluating the effect of aqueous extracts of marginata Serjania and Schinus terebinthifolius on the larvae and pupae of P. xylostela. Material and methods: The plants have been picked and gathered in a farm named Santa Madalena, in the city of Dourados, Brazil, from 7 am to 9 am. Afterwards the samples were washed and dried in a forced air oven for three days, with a maximum temperature of 40 ° C (± 1 ° C). The aqueous extracts were prepared through a maceration process, in which 10 grams of plant powder plus 100 ml of distilled water remained refrigerated and in maceration for 24 hours. Subsequently, the material was filtered and used immediately in the experiments. Cabbage leaves were treated with extracts of S. marginata, S. terebinthifolius and distilled water (control) and placed in Petri dishes containing a newly hatched caterpillar (<24 hours) of P. xylostella. The cabbage leaves were daily replaced by new treated leaves, until complete pupation. The pupae were weighed and separated until the emergence of adults. We evaluated the larval and pupal viability, larval and pupal period, and pupal biomass as biological parameters. The experimental design was completely randomized with three treatments (two species of plants and one control) and five repetitions, each involving ten caterpillars, with a total of fifty caterpillars per treatment. Means were compared using the Tukey test at a probability of 5%. Results and discussion: The aqueous extract of S. marginata was more effective in the larvae control when compared to S. terebinthifolius and Control. While comparing the number of days of larval development, S. marginata (9.84 days) was more successful in retarding the process, when compared to S. terebinthifolius (6.12 days) and Control (6.06). The larval viability was also more affected by S. marginata (62%) than by Control (94%) and the aqueous extract of S. terebinthifolius (86%). However, while comparing the influence over other biological parameters (such as pupal biomass, duration of the pupal stage and pupal viability), there was no significant difference. Phytochemical Studies on the genus Serjania reveal a great number of secondary metabolites involved in plant defense against herbivores (such as tannins, terpenes and flavonoids) known to reduce insect growth and survival. The delay in larval development may be due to nutritional inadequacy of the food offered to the insect or due to less efficient digestion caused by excessive energy expenditure in the degradation of toxic substances. The extension of the larval stage without a corresponding reduction of pupae weight is possibly due to low doses of toxic substances in an extract concentration of 10%. A low dose may lead to growth inhibition but does not inhibit alimentation. Conclusion: The aqueous extract of S. marginata showed a higher mortality and an extended larval stage of P. xylostella. Therefore, further studies analyzing other stages of the insect's life cycle may indicate a viable alternative control, replacing or reducing the use synthetic insecticides.

The aims of the study were to optimize the production of fish protein hydrolysate (FPH) by enzymatic hydrolysis of sardine solid waste using crude pepsin, and to scale up the process in a bioreactor. Results showed that the crude pepsin prepared by autolysis of the mucous membranes of a sheep stomach at optimal conditions (i. e. pH=1.5 - 2 and incubation time of 6 hours) could be satisfactory used for the enzymatic hydrolysis of fish solid waste. The optimal conditions for enzymatic reaction were: temperature 48 °C, and pH 1.5. The scale up of the enzymatic hydrolysis gave good results with the yield at pilot scale in the range of 63 % after 4 hours. However, the HPLC analysis showed the presence of all key amino acids in the concentrated product exception the cysteine. The major risk is related to the toxicokinetic interactions. Verapamil, an inhibitor of the calcium channels, is prone to this type of interactions when it is taken in combination with valproate or with diazepam. This study investigated the effect of valproate and diazepam on the toxicokinetics of verapamil in rats after oral (10 mg/Kg) and intravenous(1 mg/Kg) administration of verapamil in the presence or absence of valproate or diazepam. Valproate and diazepam altered verapamil pharmacokinetics compared to verapamil alone. The presence of valpraote and diazepam significantly (p<0.05) increased bioavailability of verapamil and decreased its total plasma clearance (Cl). Valproate and diazepam enhance the oral and intravenous bioavailability of verapamil by inhibiting the CYP2C9-mediated metabolism and UGT inhibition. Therefore, concurrent use of verapamil and valproate/diazepam should be monitored closely to potential drug interactions for safe therapy of cardiovascular diseases. Palynological data are used in wide range application, the pattern recognition of pollen grains is very important in the determination of the original floral honey, and the prediction of the allergy, which touch lot of people. Due to the high computing time needed for classification of the pollen grains and complex architecture of the neural networks, the genetic algorithm is used in order to find the optimal architecture of the Multilayer perceptron (number of hidden layers and the number of neurons within each hidden layer). In this paper, a methodology for pollen grains classification based on the using of the MLP optimized by genetic algorithm (GA) called MLP-GA is described. A database of pollen images has been used in this work. Firstly, for each image we have calculated some morphological and geometric features. Subsequently, the MLP-GA network has been used for classification of the image pollen. The best classification performance is achieved by using an experimental data base of pollen grains images. The classification rate is 90% which is very promising, and by comparing the hybrid MLP-GA with others ANN architectures, we can note that the MLP-GA is faster (the convergence time is 500 iterations). Ignition Characteristics of n-heptane/alcohol Blend Fuels in Rapid Compression Machine under Low Oxygen Concentration Conditions Kijoong Kang, Taeyoung Shim, and Gyungmin Choi Pusan National University, South Korea Auto-ignition characteristics of n-heptane/alcohol blended fuels diluted with argon and nitrogen were investigated both numerically and experimentally. The effects of fuel blending ratio and O2 concentration on the auto-ignition characteristics were evaluated at compressed ignition engine operating conditions. The results revealed that the ignition delay increased with increasing the alcohol fraction in the mixture due to a decrease of oxidation on n-heptane at low temperature conditions. The negative temperature coefficient behavior of blended fuels was weakened compared to pure n-heptane. Due to the effect of high EGR rate condition, the operating range was narrowed at lean condition and the 2nd stage ignition delay was sharply increased compared with normal condition. However, 1st stage ignition delay was affected negligibly by O2 concentration. Task assigning to employee is a difficult process. A reduced efficiency due to absenteeism, lack of job satisfaction, etc. can have consequences errors in such assignments. In this paper, we interest in a task assignment problem in home Health Care (HHC).HHC organizations provide coordinated healthcare services to patients at their homes. Due to the growing need for home-based care, the assignment of tasks to available healthcare staff is a common and complex problem in homecare organizations. Designing high quality task schedules is critical for improving worker morale, job satisfaction, service efficiency, service quality, and competitiveness over the long term. The desire is to maximizing schedule fairness in terms of workload assignments, avoiding task time window violation, and meeting management goals as much as possible. We describe a method for scheduling tasks and balancing healthcare staff’s workload. This paper develops a decision support model approach for task assignment in a home healthcare. The proposed approach uses an optimization method for task assignment. Results from computational experiments show that the approach is promising. Roles of mycorrhizas, soil carbons and soil aggregate stability under various land uses on C storage Suphathida Aumtong Soil Science Program, Agricultural Production Faculty, Maejo University ,Thailand Global atmospheric concentrations of CO2 have increased markedly as a result of human activities since 1750, and the contribution of land-use change has been significant. This study aimed to investigate the relationships between mycorrhiza activity as glomalin–related soil protein content, carbon fractions and aggregate stability of various land uses as climate change from mountainous areas of northern Thailand. The results showed that the average C storage ranged from 3.48 to 7.36 kg m2 according to land use, and was lowest in paddy soils (PAD).Mean while, primary forest (PF) was highest in C storage. The average E-glo and T-glo content ranged from 0.02 to 0.06 % and 0.02 to 0.04 % of total C, respectively between various land-use classes. In this study the SF areas which had clearly a highest coarse POM content (28.28 % of TOC) had a moderate to highest fine POM content (45.28 % of TOC).Mean while, the PAD areas had lowest coarse POM content (9.46 % of TOC). This could reflect a higher degradability of the coarse POM in that area. From the results, C in coarse POM and % C fine POM were closely related to glomalin and aggregate stability. Land use type with primary forest (PF) which occurred in mountainous area showed the highest aggregate stability, and amount of glomalin-related soil protein, due to lack of tillage and least disturbed managements. This study found positive relationship between soil aggregate stability and glomalin content and carbon fractions content.This study presented soil property e.g. the aggregate stability could be related C storage.

ENVIRONMENTAL IMPACT ASSESSMENT OF QUARRY ACTIVITIES IN OBA-ILE, ONDO STATE, NIGERIA Michael Ajide Oyinloye Federal University of Technology, Akure, Nigeria The study was carried out to assess the environmental impact of quarry activities in Oba-Ile, Ondo State, Nigeria. Structured questionnaire and field observations was administered to elucidate data on socio-economic characteristics of the residents; vibration effects; building damages; health effects and impact of quarry activities on the environment among others. Soil and water samples data were collected and analyzed in the laboratories. The study also employed ArcGIS software to determine the extent of land area that is affected by pollution as a result of the quarry activities in the study area. The research also made use of IKONOS imagery for Geographical Information Systems GIS) to buffer and extract buildings that are within 3km radius. Results obtained show that most buildings that fall within buffer zone to quarry site have high risk such as air pollution, noise pollution and health effects. The Study recommended total relocation for the residents of the study to reduce health risk problems. Sound propagation in a Salinity Stratified Environment. The case of NE Aegean. Petros Bitsikokos Hellenic Navy, Greece It is a common fact, during oceanographic surveys to calibrate the measurements taken with sound based instrumentation (e.g. Side scan sonar, ADCP, Multibeam Sonar etc) according to in situ XBT readings, as a cost and time effective measure. In addition, as the salinity measurements are commonly taken by CTD drops, they are time consuming and therefore are not executed prior to every deployment of the equipment mentioned above. This paper examines the differences that are arising to the predicted sound paths/rays in a salinity stratified environment, with or without the Salinity factor in mind, such as the one found at NE Aegean. The water masses coming from the Black Sea through the Bosporus straits are totally different in terms of T and S to the Levantine waters found in E Mediterranean and NE Aegean Sea. By using one of the many 2D Matlab ray tracing models available, based on Snell law, this paper tries to present a preliminary qualitative analysis of the shadow zones that are not taken into account when ignoring the salinity effect on sound propagation, in such an environment. The ultimate goal for this analysis is for the scientific community to rethink and reorient the basic in situ planning / positioning of measurements, in order to incorporate the differences in expected ray paths, during the use of sound based oceanographic instruments, in known or expected Salinity Stratified Environments (SSE). In this paper, ANN classifier was trained by two alternative methods, i.e. BP (Back Propagation) and PSO (Particle Swarm Optimization) on car evaluation dataset. In addition, PSO was implemented two different modes, i.e. linearly increasing and decreasing inertia weight modes. Finally, totally three classifiers’ experimental results in this study and SVM (Support Vector Machine) trained by PSO classifiers’ result were compared on car evaluation dataset. PSO with linearly decreasing inertia weight method provided an important improvement to ANN training with respect to BP method.

This work shows the improvement of a Hybrid Electrical Power Plant which is controlled by Fussy Logic. The aim is to control the opening angles of three different replaced valves in the Hybrid System. These valves are controlling the warm fluid coming from a geothermal power plant. The binary system where the heat exchangers are replaced are at the same time provided with Parabolic Collectors to increase the heat of the geothermal fluid. A developed fuzzy program will find the most suitable valve angles for maximum energy production. Where a straight line on a flat piece of paper minimizes the distance between two points, a geodesic minimizes the distance between two points on any surface; be it flat or not.Supposing that we have a surface in space given by the equation z = f (x, y).The search for a geodesic line on this surface, or more generally in the plan provided by an arbitrary metric, may be made by solving the coupled differential second order equations of Euler-Lagrange system. More precisely, the search of the shortest path connecting two given points may be made by solving that system for a specific initial velocity. In this paper we determine the geodesic lines corresponding the metric of type g=f(x,y)*(dx^2+dy^2) for f (x, y) defines positive. Starting from a metric of this type, we determine the Euler-Lagrange system correspondence; its solutions are geodesics. We designed geodesics and the shortest path for the given metric and a specific function f (x, y). We will need to determine the appropriate initial velocity for the system's numerical resolution of two differential equations of second order. ANOVA was performed to determine whether tool feeds and speed levels or their interactions have significant effects on mist concentration and particle diameter. The results show that all the independent variables levels studied and their combinations had a significant effect on the resulting concentrations. Moreover the study showed that all levels of concentrations obtained exceeded those currently permissible by OSHA and NIOSH. A significant and conventional calculation methodology to suitably estimate BIM's advantages is yet to be established; therefore an assorted outlook from professionals with regards to the benefits of BIM is creating a general misunderstanding of the expected outcomes. This study examines a domestic new build BIM home construction project at BRE Innovation Park: e4 Wienerberger. The return on investment (ROI) and maturity levels of BIM will be evaluated via collaboration with Valliant Group (International heating innovations corporation) as a key case study to un-earth the efficiency that BIM can have in terms of overall competences. The wide implementation of BIM among stakeholders of the e4 Wienerberger Project could produce vast investments from pre-construction to post-construction and enhance the proficiency of the process for the full scheme. A radicalisation to the roles of stakeholders within such new build projects could be improved to further this procedure. From background observation, it is evident that a framework can be devised in order to uncover significant: carbon/cost/time/lean savings, value increases from the use of BIM processes and innovative technologies.

For example, health system reconfiguration has shifted care from the hospital setting as the central focus of care, to more primary and preventive care throughout the community (Albejaidi, 2010). Rapidly expanding clinical knowledge and mounting complexities in health care mandate that professional nurses possess educational preparation commensurate with the diversified responsibilities required of them. The Saudi government is trying to meet international standards when it comes to nursing degrees, it still has a lot of issues that need addressing. There is a vast number of nurses within Saudi Arabia that are not currently employable due to the restricted requirement and limited seat for bridging programme to convert their nursing diplomas into Bachelor’s degree. Diploma nurses and funding for health care fall under the authority of the MoH, in order to bridge the gap above, case study used to critically assess the implications of Bachelor degree nurse education, as a baseline from which to develop a national nursing strategy for workforce planning. METHODS: Goffman (1974) frame analysis used as a framework to collect and analyse the data on three level of analysis, macro, meso, and micro levels. Goffman's frame analysis is the way to explore what happens and determine which is exceptional in a given case or experience. the “case” is ( the policy of entry requirement to nursing practice) that needs an in depth analysis by exploration of the boundaries that may have an external or internal effect. Semi structure interview used to collect data from three level (macro (stakeholder n=4), meso (regional nursing director, n=6)and micro level (practice nurses , n=14) complemented by documentary analysis. CONCLUSION: Combining and analysing the conclusions of each of the three groups (micro, meso, and macro level participant groups) by use content analysis, the data revealed commonality across groups, demonstrating significant findings that support cross-case themes in the data. These themes support the following conclusions drawn from across the three levels. Theme 1: General acceptance and agreement with the minimum requirement of BSN for nursing staff to support adequate knowledge and communication requirements for quality nursing practice Theme 2: Factors affecting degree attainment included personal commitment/passion for self-improvement, private versus government institution education quality, financial factors (incentives or promotional opportunities or obstacles). Theme 3: Benefits of BSN of knowledge supporting confidence and decreased absenteeism, broader knowledge base, greater communication and language skills, enhanced nursing skills and responsibilities, opportunities for advancement and increased pay, and increased quality of nursing practice, patient safety and outcomes, and quality of care. Theme 4: Perceived importance of both education and experience, with a focus on education supporting knowledge and patient care through advancement and in depth understanding. Theme 5: Recommendations for improvement in the differentiation of distinct nursing job descriptions based on education level attained and a focus on quality of patient care through education, validation and regulation of standardization of BSN requirement, and adequate supervising. Material and Methods: This randomized prospective trial includes the cases that admitted to Konya Training and Research hospital with chronic anal fissure between May 2011 and September 2012. The cases were randomized into four groups. Group 1: The patients would apply 15% of zinc oxide pomade (Zinc Oxide pomade, Bikar İlaç San. Ve Tic. Ltd. Şti, İstanbul,Turkey) to the inside of the anus twice a day after the 10 minutes of hot water sitting bath for 6 months. Group 2: The patients would apply 5% of lidocaine pomade (Anestol pomade, Sandoz Ürünleri İlaç Gıda Kimya ve Tohum San. A.Ş, İstanbul, Turkey) to the inside of the anus twice a day after the 10 minutes of hot water sitting bath for 6 months. Group 3: The patients would make only 10 minutes of warm sitz bath twice a day. No treatment, other than that was recommended. Group 4: Lateral internal sphincterotomy was performed to the patients under spinal anesthesia. The age, gender, symptoms (pain and bleeding), fissure localization, treatment modality, changes in symptoms after the treatment (pain and bleeding) and complications (perianal irritation, incontinence and postoperative bleeding) were recorded. Findings: There were 38 patients in group 1 (zinc oxide), 37 in group 2 (lidocaine pomade), 31 in group 3 (warm sitz bath) and 32 patients in group 4 (sphincterotomy). There were not any difference within the groups in terms of age and gender. While there is not any significant statistical difference within the groups in terms of pain scores (VAS) before and after the treatment (p>0.05); the decline in the pain scores in both the third and sixth weeks after the treatment is statistically highly significant compared with the pre-treatment scores (p<0.01). The healing in the Sphincterotomy group (group 4) significantly much more when compared to the other groups at both the third and the sixth weeks (p<0,001). The other groups were similar with the each other (p>0,05). Conclusion: The treatment of zinc oxide pomade after warm sitz bath can be used as the first choice of treatment. Yet, lateral internal sphincterotomy preserves its place as the most effective method.
