Universal Subscription Questionnaire The Universal Subscription Questionnaire (USQ) allows you to invite other entities to complete a questionnaire and share with any of their investments. This step by step guide outlines how to send a USQ and connect it to other entities.
The USQ can be accessed by:
▪ Logging into The ID Register ▪ Select your Connected Profile ▪ Click on the 'Subscription' tab
The questionnaire illustrates the existing connected entities.
If you wish to connect to another entity you can:
▪ Search existing entity profile and add the relationship connection ▪ Create an entity profile and add the relationship connection ▪ Invite new user to create a USQ by Invitation
Search Existing Entity The USQ allows a user to search for an existing entity name. If it exists, the entity name will appear in the results set and the user can add the connection by determining the relationship type.
Relationship Types:
▪ Beneficial Owner or Investor ▪ Borrower ▪ Investment ▪ Lender ▪ Parent Company ▪ Subsidiary
Create a New Profile
If the profile doesn’t exist, the user can create the profile manually by clicking ‘Create Profile’
The user will be automatically directed to the ‘New Person’ or ‘Legal Entity’ page where they
should include profile related detail.
When all details are created the user should click ‘Create Profile’.
Once the profile is created, the user that created it will automatically be assigned as an active user
on the given profile.
Additionally, the serviced profile that created new profile will automatically be connected to the
profile.
To complete a USQ the user should click ‘Subscription’.
See ‘Universal Subscription Questionnaire Overview’ for detailed steps on answering the
questionnaire.
Send a USQ Invitation:
▪ Click on the 'Subscription' tab ▪ Search for an existing profile in the Search bar using the profile name. If the profile does not
exist click 'Send Invitation'.
Within the 'Send Invitation' Window, enter:
▪ Name ▪ Email address you wish to send this invitation to ▪ Name of the Entity ▪ Your relationship to this entity (dropdown list) ▪ Tick 'CC' (if you wish to be CC’d in the invitation) ▪ Tick 'Request Universal Subscription Questionnaire’ ▪ Click 'Send Invitation'
The invitee will receive an email from The ID Register inviting them to complete USQ.
To register, the recipient much click the link in the email.
Once clicked, the user will automatically be directed to The ID Register log in page.
Existing User:
Enter your username and password and click 'Log in'
If you're connected to multiple profiles you can select which profile you want to use to complete a
USQ.
New User:
If the user is new to The ID Register, they should ‘Click here to register’ and enter the new user
profile details.
Enter Profile Address level data and click ‘Create Profile’
After the user creates their profile they will be brought to the ‘My Account’ page. Here they will
receive a message stating that they cannot view other pages on the site unless they verify their
email.
The users will then receive an email with the link to verify their profile.
Once clicked the user will automatically be redirected to the USQ homepage in the ‘Subscription’
tab.
Universal Subscription Questionnaire Overview
The USQ is divided into 19 separate sections.
Each section needs to be completed in order for the overall questionnaire to be considered
complete.
When working through the questionnaire the user will be prompted to save their work as they
progress:
Unsaved Banner:
The ‘Unsaved Sections’ banner highlights the questionnaire sections that need to be saved.
Unsaved changes:
As changes are made to each questionnaire section, an ‘Unsaved changes’ message will appear
highlighting to the user of any unsaved data.
The Questionnaire is not considered to be complete until all 19 sections have been answered and
you've signed the Questionnaire Declaration.
The USQ provides a % progress bar that indicates what percentage of the Questionnaire has been
successfully answered.
When each section has been successfully completed, the overall status will turn from Red to
Green.
Once all sections have been successfully completed and each section is highlighted in Green,
the Questionnaire Declaration should be signed.
When the Questionnaire Declaration has been signed you’ll receive a pop up confirming the
questionnaire has been completed.
Upon completing the questionnaire, the profile will automatically be connected to the profile that
invited you to complete the USQ.
Downloading Universal Subscription Questionnaires
The USQ allows the serviced entity to download completed questionnaires against any of its
related parties.
The questionnaires can be downloaded in numerous locations:
My Profiles
▪ Log into the ID Register
▪ Select the relevant profile
▪ Navigate to the ‘Profile connections’
▪ Click the ‘Download’ icon next to the requested profile
When the ‘Download’ icon is pressed, a ZIP file will download.
Click on the Zip file to Open
The USQ and the CDD pack will be downloaded and viewable as a PDF.
Report Download
The report can be downloaded directly from the ‘Subscription’ page:
▪ Log into the ID Register
▪ Select the relevant profile
▪ Navigate to the ‘Subscription’ tab
▪ Click the ‘Report’ button
Once clicked, a PDF version of the report will open in a new window and will allow you to download
