WELCOME FROM THE ASSOCIATE DEAN

Welcome to Stage 3 at Northern! Over the next two years you will gain experience in a variety of clinical settings. There are four 8 week “Core” blocks and four 8 week “Specialty” blocks as well as an Elective period and a Pre-internship term. This handbook contains information about the running of the Core blocks in Medicine, Surgery and Critical Care. The Specialty blocks are run by each relevant discipline and you will receive information on them at the appropriate times throughout the year. I am sure you will enjoy the clinical attachments arranged for you at the various locations throughout our Clinical School. In this part of the medical program, you will concentrate on perfecting your clinical skills of history-taking and physical examination, while broadening your knowledge-base and experience of patient management in the various disciplines. The staff at the Clinical School have put a lot of work into preparing materials to support your learning, as well as making the arrangements for you to join the clinical teams involved in the program. Remember that you will each experience a wide range of placements, but at the same time no-one will have a rotation in every specialty, as this is not possible in the 32 week period available to us over the next two years. Great care is taken to ensure that each student has a balanced series of attachments, including the required experiences in Critical Care (Emergency Medicine, Anaesthetics, and Intensive Care) and in Geriatric Medicine. In addition, most students will have the opportunity to experience a placement in a rural centre, either in the Northern Rivers area (Lismore/Grafton), in Dubbo/Orange, or in the Northern Territory. In all locations you will find there are well-briefed and enthusiastic supervisors to look after you. Your principal activity during Stage 3 is ward-based clerkships, and most of your learning will be in the context of active patient care. However, during the Core Blocks there is an important program of structured teaching scheduled to occur on each Friday. For students placed at hospitals in Sydney, but outside RNSH, this will involve travelling to St Leonards for these sessions. Your supervisors will be aware of this commitment and will support your attendance, but at all other times you should do as much as you can to take an active role in the clinical teams to which you have been attached. An important part of your assessment will be the report of your nominated Supervisor on your participation. Don’t forget also that the SMP web site contains a variety of useful materials to support your learning in this part of the program. Look out particularly for the learning objectives provided online and in the various handbooks, and for the assessment requirements. Sophie Cher and all the staff in the Clinical School Office are available to help you with any logistical problems which may arise, while Rory Clifton-Bligh (Medicine) and Leigh Delbridge (Surgery) are the academic coordinators for the year and can offer advice if problems arise in your clinical placements. You should also feel free to approach Kirsty Foster, Sub Dean (Education) & Senior Lecturer in Medical Education, especially if you need assistance in learning or studying skills, or for other personal guidance. My own door is open at all times, and I would be happy to see anyone who wants a word about their progress through this part of the course. General Information - Stage 3 Core Blocks Students are reminded that it is their responsibility to check the SMP web site for all course information and assessment information. The standard themes have specific information available as well as information on assessment, who to contact etc. Log in regularly to the SMP Bulletin board to keep informed of any changes or clarifications from the Office of Medical Education. Students each have a unique experience throughout Stage 3 Core Blocks, with a basic pattern of 4 days clinical experience and 1 day structured teaching. All scheduled activities, clinical, teaching and visits are compulsory, 100% attendance is required. Important:

Please go through this handbook, check all your personal dates for visits, presentations, clinics etc and place them in your diary immediately. In addition, please check the assessment section of the SMP site.

There are a number of group and individually scheduled tutorials and visits you must attend. It is essential that you attend as per the schedules outlined in the handbook as it’s not possible to reschedule missed activities.

Please ensure that you regularly check your email for the confirmed Friday teaching timetable and any other communication from the Sydney Medical School Northern staff.

The weekly structured teaching timetable may change, due to lecturer availability, examinations and other unforeseen circumstances.

For contact details of those above, please see the contact list on page 20. Core Block Attachments & Teaching

Students MUST contact the Supervisor of their new Clinical Attachment one week prior to commencing to organise the details (time/place), for the commencement day. See the Clinical School website: http://sydney.edu.au/medicine/northern/student/index.php for the supervisor’s contact details.

Please advise your Clinical Attachment Supervisor, in advance of any dates, where you will not be present at your Clinical Attachment due to a scheduled activity.

Forms Please ensure you keep your forms/log books in very good condition, use black pen only and don’t staple them as they need to be scanned for filing. It is your responsibility to submit the forms to Sophie Cher by hand, fax, email or mail no later than one week after completing each attachment. Please ensure you keep copies of all forms for your records as the clinical school will not photocopy them for you. Core Block Clinical Attachment Feedback Forms Towards the end of each Clinical Attachment you must complete the “Student Self-Assessment” sections on both pages of the Clinical Attachment Feedback Form, sign it and then submit it to your supervisor to complete and sign.

Required Forms

From Term A to C you must submit: o Medicine Bedside form o Surgery Bedside form o Rectal examination form

On completion of each Clinical Attachment you must submit: o Clinical Attachment form (details above)

On completion of the Critical Care block you must submit: o 4 x Clinical Attachment forms (ED, ICU, Anaesthetics & Acute Surgery)

On completion of year 4 you must submit: o Ophthalmology log book o CRS forms (details next page)

CRS The Clinical Reasoning tutorials are student led. At the beginning of each block, please allocate the topics amongst your group. It is expected that each student will lead at least two tutorials during the Stage 3 Core Blocks. For the cases you lead, ensure you have the CRS forms (pages 21 & 22) completed, signed by the facilitating tutor on the day and submit copies to Sophie. EBM/PEARLS Tutorials & Presentation

Each student is required to complete 3 compulsory EBM tutorials followed by an EBM Presentation; these usually occur in 3 consecutive weeks.

Prior to attending tutorial 1 students should read ‘A guide to Student PEARLS’ on the SMP web site on the EBM PEARLS page.

The EBM presentation needs to be 15 minutes (maximum) in duration followed by a couple of minutes for question and answers.

If presenting at RNSH, please ensure you don’t go over time due to the tight lecture schedule. Also ensure your presentation is loaded on the auditorium PC prior to the commencement of the presentations. If you encounter any IT issues, contact the AV Coordinator on x64528.

If you’re allocated to complete the EBM at a rural/peripheral site, please ensure you coordinate the dates/details with the Student Coordinator at that site.

Assessment of your EBM PEARLS includes tutorial attendance, as well as your presentation and handout. The handout must be no longer than one page and you must make photocopies at your own expense.

The ‘Pearls Assessment’ forms (filled out by your Assessors) will be returned to you via the Education Support Officer/Student Coordinator from the presentation site.

After your presentation, please take the ‘Pearls Audience Feedback’ forms with you for your reflective statement.

All students, regardless of where they present, must then submit a reflective statement about the process to the Student Coordinator at their site within one week of their presentation.

Once you satisfy all of these requirements you will be notified by Professor Lyn March, Academic EBM Coordinator.

All students allocated to clinical attachments in Sydney must attend all presentations as audience members. ‘Pearls Audience Feedback’ forms must be completed by all audience members on the day. Students who do not attend the presentations that occur whilst they’re allocated to Clinical Attachments in Sydney may fail the EBM component of Stage 3.

Acute Pain Round

RNSH is a world leader in pain management! For the students completing the Critical Care block at RNSH, you'll be allocated a date to attend the Acute

Pain Round. For the students completing the Critical Care block at a peripheral hospital who would like to attend the

RNSH Acute Pain Round, there are plenty of places available. A date can be arranged by contacting Sophie. On the Acute Pain Round, you'll be the only student accompanying the Consultant and occasionally a

Registrar. You'll have the opportunity to see how to deal with complicated patients, severe burns, spinal cord injuries, etc.

Once a date has been confirmed, please meet the team at the Department of Anaesthetics and Pain Management, Level 4, Main Block promptly at 8am. The Round will be completed by 12 noon.

If you arrive late, or need to speak to someone, please contact the Pain Service Nurse on pager 41219. Critical Care RNSH Timetable

For the students allocated to Critical Care at RNSH, you’ll receive the timetable via email prior to commencement of the term.

Attendance, Dress Code & General Information

Attendance Attendance is expected at all activities during the stage 3 Core and Specialty blocks. Planned leave for exceptional circumstances must be approved by the Associate Dean or the Sub Dean, Education and the faculty Stage 3 Coordinator. To seek permission for planned absence, submit a ‘Short leave’ form (SMP, Admin/Forms) to the Education Support Officer (ESO). For sick leave, contact the ESO and your placement supervisor on the first day of illness. A ‘Short leave’ form and medical certificate must be submitted as soon as possible and certainly by the end of the first week of absence. Please see the attendance policy on SMP for more information. Non attendance is a serious PPD issue and may result in failure to progress. Students who are repeatedly absent will be referred to the Sub Dean, Education and/or the Associate Dean.

Dress Code for Medical Students As a medical student you will be involved in day-to-day interactions with patients, members of the public and medical staff, including the discussion of sensitive issues and the performance of a range of clinical duties. Therefore, while attending the ward, clinics or general practice, your dress and general appearance should be appropriate. As an overall guideline:

A medical student should appear well groomed and professional. Your official hospital ID badge should be displayed prominently at all times so that your status as a student

is clear to all. For males, a collared shirt is strongly recommended. Ties have been discouraged in some areas because of

infectious control issues. Be guided by the senior male doctors on your team. Females should avoid wearing tops that are low-cut or bare the midriff. Shoes should be closed in. Sandals or open-toed shoes are inappropriate for ward areas as sharp objects such

as dropped needles may pierce the foot. Stilettos and high platform shoes are also inappropriate for ward areas. This is an OH&S issue.

Trainers, jeans, shorts and mini-skirts are not acceptable items of clothing while on ward attachments. Theatre gowns/clothing can only be worn within designated areas. Wearing of jewellery in Theatres is not

Lockers: Lockers are located in the Common Room Level 4 (Ground Floor) Kolling Building. They can only be used on a daily basis. Find a locker that is empty and follow the instructions for use. Pigeon holes are also provided in the common room.

Copying facilities: The BEAR system is working for the photocopier in the Library 24/7 room and this is the copier that the students will have access to. Copy cards can be purchased from the front desk of the Library during opening hours (Mon-Fri 8am-5pm) at a cost of $2.20. The students can then add money to the card as required (also during Library opening hours only at this stage). Receipts will be issued for card purchases and recharges. Copies cost 11c per page for black and white. Printers are available in the common room and the 24/7 room. You must provide your own paper.

Student Support: Every student should have their own GP. Please refer to SMP (Course Info, Student Support) for more information.

Student Access to Patient Medical Records

Health Information Services (HIS) primary functions are managing and guaranteeing the availability, security, privacy, and confidentiality of patient information

It is your responsibility to ensure medical records are safe, secure, available and that the information is kept private to maintain the patient’s confidentiality

It is important that you are aware of the policies in place to safeguard medical records, and its enclosed information, and of the impact on you.

Medical records must not leave HIS unless for patient care eg. admission, emergency, or outpatient clinic appointment.

Under no circumstances is a medical record to leave the hospital grounds including private consulting rooms, NSP, nursing homes etc. There are exceptions to this between RNSH and Ryde Hospitals – however always check with your supervisor

Medical records must not be photocopied – the more copies of personal health

information that exist the more likely it is that a breach of privacy may occur.

You are permitted to look at patients’ medical records and to enter appropriate content (e.g. history or examination). All entries you make must be signed and dated with your name and designation legibly printed in black or blue pen

It is also possible for you to visit HIS at RNSH where you may view records for patient care. You must have your identification badge with you.

If conducting research, you must have a Research Request Form signed by the Clinical School Executive Officer before you request records from HIS. Forms are available from the Student Common Room and through HIS at RNSH. Any notes you make must not include information which would allow the patient to be identified.

In preparing case presentations you must ensure that the patient is not identifiable. If you incorporate results or images you must ensure that the patient’s name is not legible and that there are no other identifying features (DOB, Address etc).

Ask for permission from any person whose photograph you wish to take or to use in a visual presentation. You are of course required to comply with any University or Health Service policies and directives at all times and this forms part of your PPD theme learning.

Applying for an Elective placement at

Sydney Medical School Northern

For a number of reasons, there are a very limited number of placements available at Sydney Medical School Northern* during the Elective Term. Applications will be considered on an individual basis by the Electives Coordinator. Due to problems in the past, we’ve had to include a rule that students don’t apply directly to potential supervisors. Even if a Supervisor says they can accommodate the student, this is often not the case as the Department may have other Elective or Stage 3 students booked in at that time, which the Supervisor is unaware of. This is just one of the reasons why initial enquiries MUST go via the Clinical School Electives Coordinator. NOTE:

the Electives Coordinator will only assist with arranging clinical placements at Northern. You should refer to SMP (stage 3, elective section) for information on faculty requirements with regard to the Elective term, including the Faculty Elective Term application form, deadlines, insurance, Elective Term report, etc.

• Students should view the ‘Available placements’ chart at: Process:

http://sydney.edu.au/medicine/northern/electives/placements/index.php If the Department a student would like to do an Elective in, is not listed on the website, this means it is not an available Elective. Students can only apply for Electives listed on the Available placements chart.

• Students who then wish to then apply should email Kelly Bird, Electives Coordinator, at

[email protected] for an application form to complete.

• The completed application form should be returned to Kelly Bird who will then liaise with the student regarding details of the elective placement(s).

• The online faculty elective application must only be completed after Kelly Bird has

approved the place. Sydney Medical School Northern accommodates visiting elective students all year round. Places fill up quickly with most students applying at least 6 months in advance. An early application is strongly advised. Northern students are not required to pay fees if their application is accepted. Due to the limited number of places available over the Christmas/New Year period, there are no places available for medical students from other Clinical Schools of the University of Sydney during this time. * Northern hospitals for this purpose include Royal North Shore Hospital, Hornsby, Manly, Mona Vale, Ryde and a few places at North Shore Private Hospital.

Dermatology Objectives

• Students will be expected to be familiar with morphological terms used in dermatology, and will be expected to be able to confidently describe diseases encountered in the clinic and formulate an appropriate differential diagnosis for cases examined

• Students will be expected to be familiar with the management of common skin diseases

DERMATOLOGYStudents will attend 4 Dermatology clinics during a Medical or Surgical attachment (attendance is compulsory)

Students are responsible to advise relevant Clinical Attachment Supervisors that you'll be absent for:

4 x Dermatology Clinics on Thursdays 9-1030am - Clinic 8, Outpatients Department

The Australasian College of Dermatologists online teaching support modules need to be completed prior to commencing clinics. The website details, login & password have been emailed to each student.

CLINICOPATHOLOGICAL CONFERENCES (CPCs)

CPCs involve presentation and discussion of the clinical, radiological and pathological findings associated with specific disease processes and will be run by a multidisciplinary team comprising physicians, surgeons, radiologists

and pathologists

CPCs are scheduled fortnightly on Friday afternoons as part of the lecture schedule

Attendance is compulsory for all students

For each CPC session 2-5 students are rostered to be involved in the presentation

Those rostered for the CPC should arrange to meet the CPC co-ordinator Dr Anthony Gill in the Department of Anatomical Pathology (x 67085) 2 weeks before the scheduled presentation to discuss their roles. If Dr Gill is

unavailable, either Dr Bryn Atmore or Dr Antony Kaufman (contactable on the same no) will provide assistance.

SYDNEY EYE HOSPITAL VISITS

Please ensure you take your Ophthalmology log book to Sydney Eye Hospital

Students have been allocated to a day at SEH in various Outpatients Clinics to enhance your Ophthalmology teaching.

Students are to arrive by 9am in the clinic and ask for the Nurse in charge of Outpatients (Lesley Thwaites, Office in ground floor, Unit D).

If there is a problem, students should seek assistance at the Save Sight Institute (Ph 93827300) University of Sydney Clinic located across the courtyard from the main Outpatients (If you encounter problems, you can contact Tony Succar

The Outpatients Clinic is located on the Ground Floor, Clinical Services Block, Sydney Eye Hospital, Macquarie Street, Sydney.

Students are required to complete an Ophthalmology log book during stage 3 (by the end of year 4)The log book is located on SMP, Links, On-Line Ophthalmology Teaching

Its recommended you take your log book onto the wards during all Core & Specialty blocks.Students are encouraged to test their clinical reasoning skills by using The Virtual Ophthalmology Clinic (VOC) to

diagnose and manage virtual patients with simluated eye disorders. Please contact Tony regarding the VOC program.
