Project Order Process

1. Inform participants who have completed the survey that we are now collecting money orders for their purchase total amount by Sunday May 20, 2012.

Money Orders should be completed like this

2. Verify their order information and check the total amount a) sign top form, tear off the bottom and use it to enter info on the Live Data Spreadsheet

3. Collect Money Orders and distribute receipts.a) Buy a three layer purchase order receipt book (1st layer for you, 2nd layer for Mosque or Study Group Secretary, 3rd layer for purchaser)b) Collect money orders and issue a receipt for orders (be certain to sign your name as receiver of funds)c) Verify that purchaser’s name and complete address is filled in properly.d) Be sure that money order reads as in the illustration above

5. Prepare documentation to be sent to The National CenterPackage should include the following: • Each individual money order • A copy of your Live Data Spreadsheet

DO NOT WRITE IN ANY ORDERS ON THE SPREADSHEET. TO RECEIVE GOODS, ALL ORDERS MUST BE ON THE LIVE DATA SPREADSHEET. WE USE THAT TO PLACE THE ORDERS.

6. Mail money orders and receipts to 3 Year Economic Program by May 21, 2011 Place the above documents in a large envelope and mail to :

7. Please act in a timely manner. We cannot make the order as a group until everyone has paid.
