WHY YOU SHOULD CARE INCREASING FREQUENCY OF VTE

Incidencia ETV
• El riesgo de ETV parece haberse incrementado en las
dos últimas décadas en los pacientes con cáncer:
(Khorana 2007)
– Mejoría métodos detección (TC helicoidal)
– Nuevos tratamientos: antiangiogénicos, EPO
– Incremento supervivencia (a mayor tiempo de supervivencia
mayor riesgo): CCR avanzado incremento supervivencia x4

Limitaciones del Modelo de Khorana
• 2,5 meses de seguimiento (escaso seguimiento)
• Era pre-angiogénica
• Pacientes con buen PS (infrarrepresentación de
pacientes con pobre PS)
• Mediana de edad desconocida
• Infrarrepresentación de neoplasias de alto riesgo
• Se desestima cirugía y el paciente inicia tratamiento
con QT Oxaliplatino - Capecitabina -Trastuzumab
(catéter port-á-cath)
Caso Clínico
• ¿Propondría tratamiento profiláctico con HBPM a este
paciente?
– Sí, al presentar un score de Khorana de alto riesgo y/o
factores de riesgo
– No
Caso Clínico
• Factores de alto riesgo de sangrado:
– Neoplasia con datos de sangrado (anemia ferropénica)
• Score de Khorana 2 – riesgo intermedio
Caso Clínico
• A las 6 semanas de tratamiento se realiza TAC de
reevaluación donde se demuestra respuesta parcial
según criterios RECIST 1.1
• Se continua tratamiento con mismo esquema de QT
• Dos semanas después el paciente acude al Servicio
de urgencias por aumento de disnea de esfuerzo
• Se diagnostica de TEP segmentario bilateral
Caso Clínico
• Inicia tratamiento con Tinzaparina 14.000 UI sc/día
• Inicialmente se realiza monitorización con anti-Xa por
riesgo de sangrado (insuficiencia renal + tumor no
operado con sangrado previo)
• Continua tratamiento con mismo esquema de QT y
tras 6 meses de tratamiento se suspende oxaliplatino
por neuropatía grado III y se mantiene capecitabina -
trastuzumab
• TAC tras 6 meses: RP>80% según criterios RECIST
Caso Clínico
• De acuerdo con el paciente se decide mantener
tratamiento con Tinzaparina a misma dosis más allá
de 6 meses
• Durante toda la terapia anticoagulante no se objetivan
sangrados menores o mayores
TAKE HOME MESSAGE
• ¿ Debería realizarse una profilaxis antitrombótica en todos los pacientes con una neoplasia? - No de forma indiscriminada, es importante estratificar
el riesgo
• ¿En qué neoplasias? – En neoplasias con alto riesgo de ETV y bajo riesgo de
sangrado. • ¿Por cuánto tiempo?
– La duración de la profilaxis no se ha establecido. El mayor riesgo de padecer un evento tromboembólico venoso es mayor en los 3-6 primeros meses.

• Se necesitan modelos para predecir el riesgo de sangrado en pacientes con anticoagulación profiláctica
