Internationally renowned Gehwol® foot care line

Gehwol®

has received international recognition as a highly effective,

quality line of foot care products, since its creation in Lübbecke,

Germany in 1868 by the pharmacist Eduard Gerlach.

As a village pharmacist catering to the needs of alpine hikers

and farmers, Eduard Gerlach understood how extremely painful

sore feet can be. He was able to successfully mix fundamental

substances such as potassium carbonate, unsaturated fatty acids,

mineral based lubricants and disinfecting essential oils to create

“E.Gerlach’s Preservative Cream”, the first industrially manufactured

cream for the prevention and alleviation of foot ailments. Edward

Gerlach received numerous international honors for his “Preservative

Cream” nowadays known as Foot Cream.

Since this first product introduction, GEHWOL®

has survived and expanded into a full scale line of foot

and nail care products made with the highest quality natural ingredients. The extensive line offers over

30 different foot care, nail care, and body care products as well as professional pedicure equipment.

Deodorizer, Antibacterial & Antifungal Ingredients

Climbazole is a topical antifungal chemical commonly used in the treatment of human fungal skin infections like dandruff

and eczema. Its chemical structure and properties are similar to other fungicides like ketoconazole and miconazole.

It is most commonly found as an active ingredient in OTC anti-dandruff and anti-fungal products, including shampoos,

lotions and conditioners. It may be accompanied by other active ingredients like zinc pyrithione or triclosan.

Clotrimazole a broad-spectrum antifungal agent used for the treatment of skin infections ranging from toenail fungus to

athlete’s foot and ringworm. It is easily available as an over-the-counter drug, usually in the form of a cream or lotion. It treats

fungi by stopping and inhibiting the growth of the membranes that surround the fungal cells. This results in the weakening of

the sensitive fungal cell, which makes the cell content leak out, ultimately leading to the death of the fungus.

Chloroxylenol (4-chloro-3,5-dimethylphenol) is a chemical compound that is commonly used in antibacterial soaps

Studies have shown a low anti microbial activity which is enhanced by additives. Its antibacterial action is due to disruption

of cell membrane potentials.[2]

Farnesol is a natural organic compound which is an acyclic sesquiterpene alcohol found as a colorless liquid. It is insoluble

in water, but miscible with oils.

Farnesol is present in many essential oils such as citronella, neroli, cyclamen, lemon grass, tuberose, rose, musk, balsam

and tolu. It is used in perfumery to emphasize the odors of sweet floral perfumes. Its method of action for enhancing per-

fume scent is as a co-solvent that regulates the volatility of the odorants.

Farnesol is a natural pesticide for mites and is a pheromone for several other insects.

Triclosan This organic compound is a white powdered solid with a slight aromatic/phenolic odor.

Triclosan is an ingredient added to many consumer products to reduce or prevent bacterial contamination. It may be found

in products such as clothing, kitchenware, furniture, and toys. It also may be added to antibacterial soaps and body washes,

toothpastes, and some cosmetics—products regulated by the U.S. Food and Drug Administration (FDA).

Undecylenic Acid for Nail Fungus

Medication - Toenail Fungus Medication

General Overview

Undecylenic acid has been FDA approved as an over-the-counter treatment for nail fungus. It is also the active ingredient in

medications to treat skin conditions like psoriasis and athlete’s foot, and can relieve irritation and itching that can come with

a nail fungus. The main ingredient naturally has anti-inflammatory properties that help soothe the area around the nail if it is

irritated or damaged slightly.

How it Works

Undecylenic acid is a natural fatty acid derived from castor oil. It is a natural antifungal and it is used in many different prod-

ucts besides a cure for nail fungus, such as perfume, cosmetics, acne medication, and anti-dandruff shampoos. The oil is

extracted from the bean and cold-pressed, which renders it odorless and tasteless. The molecule size of Undecylenic acid is

small enough to penetrate the skin instead of staying on the surface, which is one reason why it is effective as an antifungal

treatment.

How to Use

After using Undecylenic acid products consistently, you should see some improvement within four to six weeks, when your

nail begins to grow in healthy.

User Experiences

Users of Undecylenic acid report that consistent use and patience is key to making this treatment work – more than a few

months of daily treatment is necessary to see results, and the nail should start to grow back in healthy within a few weeks.

Users also report having to cut the infected nail very short so the liquid can penetrate the infected area. Some also report

that it is more effective on athlete’s foot than on a nail fungus, but it varies by individual.

Special effectivenessThe main purpose of the leg- and feet-peeling is to exfoliate and smooth excess skin

on shinbones and knees which are often dry and rough. Additionally, skin on the feet

is smoothed and rough spots can be reduced. Massaging the product into the feet

and legs promotes circulation. Aches and pains in the legs and feet can be reduced.

Gehwol FUSSKRAFT leg and feet peeling

prepares the skin for the following:

When using a self-tanner (many body lotions for the daily care also contain

self-tanners) the regular use of exfoliating products is recommended in order to get

a consistent tan. Ingrown hairs can be prevented. A careful removal of the cuticle

of the toes is also possible.

Gehwol FUSSKRAFT Leg and feet peeling

is not a substitute for a proper footcare treatment:

Calluses and cracks on the sole of the foot and on the heel cannot be removed by

the peeling. The main areas of use are the legs (especially the shins and knees) and

the bottoms and back of the feet.

Usage:The treatment can either be done by the end user at home while taking a shower

or a bath or by the podiatrist or cosmetician during a footcare treatment.

According to the instructions for use, Gehwol leg and FUSSKRAFT feet peeling is

massaged into moistened skin. After that, the skin will be washed, dried and a

caring footcare product can be applied.

The most important target groups for this product are women who go for regular

foot treatments such as pedicures or foot massage.

These clients pay attention to the care of their feet and spend time on treatments

for them. The podiatrist or the cosmetician can offer a peeling as an addition to

a pedicure or other footcare treatment such as reflexology or removal or corns

and calluses.

Products for follow-up care:GEHWOL FUSSKRAFT Leg-Vitality and GEHWOL Leg-Balm, caring products for

legs and feet GEHWOL FUSSKRAFT Hydrolipid-Lotion and GEHWOL med

Application knowledge CURRENT

Toenails are required to protect the toes. They prevent pathogens from entering thesensitive nail bed. They also serve as“counter-buffers” when sensing throughtouch. This is important, since the senseof touch in the feet is decisive for positioning within space. If the toe nail isdamaged or missing entirely for a longerperiod, the nail bed dies back. The consequence: The nail bed is too smallfor the growing nail, which then has togrow in. In order to prevent this, a nailprosthesis should be made immediatelyafter a nail is lost. For apart from the factthat remodelled nails are optically morepleasing, the prosthesis exercises thenecessary pressure on the nail plate sothat it does not die back.

CausesThe most frequent reason for which a nailmust be partly or wholly replaced aregrowth disturbances (onychodystrophy)which are usually caused by nail fungusdiseases (onychomycosis). Brittle nails(onychorrhexis) or detaching nails (onycholysis) also require a nail prosthesis. But nail removal is also oftenunavoidable in keratosis or subungualcorns (beneath the nails). In some cases, trauma (injuries) also lead to partial orcomplete nail loss. Herein, wrong shoes –too small, too tight or too high – are oftenthe cause. Medications or chemicals canalso cause onychodystrophy which requires nail reconstruction.

Varied helpSince the first nail prosthesis technologywas patented (Josef Greppmayr, 1953)many other methods have been developed. The most common ones are:

Acrylate pour-on technique for a partialprosthesis (air hardening two component resin, according to Greppmayr)

Full prosthesis made from cold formingplastic foil (classic method according toGreppmayr)

Deep drawing technique for full prostheses (according to Eckle)

fibreglass prosthetics acrylic polish partial prostheses from air hardening,

ready-made nail compound prostheses from light hardening

polymerisation resin (single phase gel)The methods sometimes show major differences in terms of the required effort, base equipment and the optical results. And it is not possible to use every technique for every problem. For all techniques, a small remnant of the nail

should be present for an optimal fit of theprosthesis.

Full prostheses according to EckleA completely missing nail can only bereplaced with the deep drawn techniqueaccording to Eckle. The method wasdeveloped from the classic method according to Greppmayr. It is particularlyprecise, but also requires a lot of effort. At the first visit of the patient, a precise silicone imprint of the previously treatednail is made with an “imprint spoonaccording to Eckle”. This negative form isused to produce a plaster model of thenail. When the plaster cast has been correspondingly prepared, the new nailcan be modelled on it with a special plaster mass. This completed model, which only requires some smoothing, ispressed into a heated special foil (deepdrawing). After the foil has cooled, theartificial nail is cut to a precise fit.

At the second patient visit, the artificialnail plate is fitted and roughened on theunderside. Prepared in this way, it is thenaffixed to the nail bed and – insofar aspresent – the remaining nail with a twocomponent resin or a single phase gel.Excessive adhesive is best removed with a spatula while it is still soft.

Nail replacementNail prosthetics: not only for optics, but particularly for protection

Nails are important. They protect the sensitive tips of the fingers and toes. A partly or completely missing nailshould therefore always be replaced. Only in this way can further complications be prevented from developing.Using the many different methods, the nail problem is optimally cared for and usually very good optical results areobtained as well. In this way, no sick toe has to be hidden.

Artificial or natural? A well done nail prosthesis is not recognisable as such at first glance.

An indication for a prosthesis: The reduced nail (left) can be filled up with a highlyviscous gel. The result is a nail with natural appearance (right).

Page 38: /PROFESSIONAL-GUIDE-2011-2012

Nail massAn especially simple method for thereconstruction of partially damaged nailsor to reinforce them is the use of finishednail mass which hardens in the air. Forthis purpose, GEHWOL Nail Compoundis applied to the nail in a thin layer directly from the tube. The compoundhardens within five minutes. During thistime, the nail can be formed. As a reinforcement, a fleece can be applied tothe still moist nail compound. The artificial nail provides optimal protectionfor the nail bed and also has many natural characteristics: It is difficult to differentiate from artificial nails, grows, can be cut and filed.

Single phase gelThe method of choice, which is also usedby most foot specialists, is to reconstructthe nail with a light hardening single phase polymerisation gel. This technologyis good value for the price, does notrequire many accessories, is easily learned and can be applied in manypatients.

With the single phase gels of the GEHWOL Nail Repair Sets for hands andfeet, you can provide not only medicallyflawless, but also optically optimal nailprosthetics for your customers. For thedifferently coloured gels can be mixedand thereby offer the option of adaptingthe colour of the artificial nail to that ofthe natural nail. Furthermore, the nail prostheses of light hardening single phase gels offer good wearingcomfort, since they remain more elasticthan the air hardening two componentresins. For if the replacement nail is toorigid, it may exercise too much pressureon the nail bed on the one hand and notyield sufficiently on the other hand ifpressure is exerted on it. In the worstcase, the regrowing nail is levered out ordamaged.

Advance workThe preparation of the nail or nail remnant is the same in all reconstructionmethods. It is required in order to avoidhaving the remnant nail detach from thenail bed, causing irritation through enclosed particles, and detachment of the artificial nail.

Step 1Before the artificial nail can be produced, thickened nail parts and nail areas whichare infected with fungi must be removed.Depending on the thickness and size ofthe nail, a coarse (such as HM 79 GM040 coarse or HM 251 GM 060 coarse)or slightly finer hard metal milling tool such as HM 75 GX 040) may be suitable

for this. Then the prepared nail is freed ofloose nail particles. Using a fine grindingbody (such as 732/050), you obtain aneven surface.

Step 2The nail edge must be carefully cleaned.Only in this way can the new nail growin straight and without pain. Using asmall diamond grinding body (such as D 407 H 010), ingrown nails in the nailedge can be reached gently and withoutdifficulty.

Step 3The remnant nail plate must be free offats so that the artificial nail can adhere tothis prepared plate. The nail remnant iscleaned and de-greased using GEHWOLNail repair Cleaner. The nail plate can beslightly roughened with a blunt instrument (such as the double instrument HH 106 R).

ModellingFor optimal results, toe nails should bereconstructed with two to three layers. Inthis way good flexibility of the artificialnail is guaranteed and the nail bed isoptimally protected. After the pre-treatment, the first layer of the GEHWOL Nail repair Gel is applied ontothe nail directly out of the tube.

The new GEHWOL Nail repair Gel highlyviscous is especially suited to detailedmodelling work. The three colours of clear, opal and pink can be mixed witheach other so that the nail colour can beoptimally adapted to the natural colour. The clear gel serves as a filling base.Depending on the natural colour, addopal (for rather white nails) or pink. Naturally, both colours can also be addedto design a colour. Mixing is most easilydone directly on the nail bed. Due to theincreased viscosity, the gel can be appliedwith point precision without running. Thismakes it much easier to work with. Andsince the gel only solidifies under UVlight, the new nail can be modelled with a brush without time pressure. Specialmodelling brushes with exchangeableheads are good choices. They can bechanged after every treatment, so thathygienic work is guaranteed. If your customer has a free, white nail edge onthe other toes, you can easily form thiswith the new gels without needing a template. Draw a fine line along the distal(upper) end of the artificial nail with thehighly viscous opal gel. Then the layer ishardened for two minutes under a speciallamp with pure UVA light (380–400 nm).A firm, but simultaneously elastic nail isbest produced with three layers of gel.Important: The polymerisation process

The damaged nail areas are completelyremoved with a hard metal milling tool.

Free of fungus and without loose nailresidue, the nail bed is optimally prepared for reconstruction.

To build up the artificial nail, the highlyviscous gels opal…

…and pink are applied directly to thenail bed…

…and mixed with a brush, then modelled.

develops a dispersion layer (“sweating layer”) on the nail. Since this acts toencourage adhesion between the individual layers, it should not be removed.

The chemical process of hardening canalso develop heat on the nail bed. If yourcustomer feels an unpleasant sensation, interrupt the UV irradiation. However thehardening process is only complete aftertwo minutes. Therefore continueirradiation after a brief pause if required.

To seal the finished nail, it is best to applya thin layer of GEHWOL Nail repair Gelmoderately viscous. Depending on thecoloration of the artificial nail, use thecolour “clear” for this, or if the nail appears too white, use “pink”, or for anunnaturally pink nail, “opal”. The moderately viscous gel equalises unevenareas extremely well. The nail also receives a beautiful shine. This layer isalso hardened under UV light. The sweating layer of the last gel layer can beremoved with a cellulose pad and theGEHWOL Nail repair Cleaner.

Follow-up treatmentThe nail prosthesis is then formed andbrought to a shine. Using cylindrical(such as HM 175 FX 060) or conical hardmetal milling tools (such as HM 186 FX050), more major work is easily performed on the solid gel surfaces. Finehard metal milling tools such as HM 251FX 060, HM 250 FX 040 and HM 79 FX040) are suitable for working on the nailedge and performing slight surface corrections.

Polishers provide a beautiful shine at thefinish. These are available in differing gritsizes. The polish usually starts with acoarse attachment and ends with a fineattachment. The cylindrical polishers areexcellent for lending a beautiful finish to

large surfaces. The conical polishers aresuitable for fine work on the edge of thenail.

ProtectionWhen it is treated this way, the nail canregrow under the prosthesis without problems. The special property of the Nailrepair Gels is that it detaches whereverthe skin forms fats. This means that thenew nail is not hindered in its growth. And the highly effective antimycotic additives in the nail gels mean that nonail fungus disturbs the regrowth. In addition you should protect the nail bedwith antifungicides such as GEHWOLmed Protective Nail and Skin Oil. It distributes well between the artificial nailplate and the nail bed, protecting againstmycosis. It is recommended to continuethis nail care on a regular basis. Thepatient can continue to do this himselfwithout problems until the next check-up.

Follow-up careIn the event of complete nail loss it cantake up to one year for a new nail toform. In this time the nail must be regularly checked and the artificial nailmust be shortened, or replaced from timeto time. It is generally recommended tocarefully grind down the new nail everysix to eight weeks and renew it.

Application knowledge CURRENT

A white free nail edge makes the prosthesis appear more natural. It canbe formed with the Nail repair Gelhighly viscous, opal.

Every gel layer is hardened for twominutes with pure UVA light. But yourcustomer does not have to worry abouta tanned toe. The short exposure time isnot enough for this.

Using a fine grinding body, the nail isnow shaped.

GEHWOL med Protective Nail and SkinOil then protects the nail against fungalinfections.

Hand & Foot

The finger nails are exposed to numerous strains. Using the GEHWOLNail repair Set, you can provide optimalcare and protection to the finger nails.

Also suitable for manicuresFor this the untreated nail is cleaned, slightly roughened and a layer of Nailrepair Gel, moderately viscous is applied and hardened. Use your choiceof the gel clear, opal or pink and optimally adapt the colour to the naturalcolour. The result is a well cared for,

naturally shiny nail which can no longereasily tear or break. The GEHWOL Nailrepair Gel is also highly suitable as apolish base. The gel fills unattractivegrooves and uneven areas and ensures asmooth nail surface.

Of course the GEHWOL Nail repair Setcan also be used to repair a damagedfinger nail or create a finger nail prosthesis.

Axel Pelster, podologist and nurse, Übach-Palenberg:

I have tested and worked with many artificial resins. I have already workedtogether with a variety of companies inthe field of polymerisation resins andpointed out the strengths and weaknessesof the various products. Since more thanten years, I have been working with nailprosthetics and since then, I have beenholding regular seminars and lecturesabout this subject within the context ofthe "Beauty International".

The development of the single phasepolymerisation gels was already an enormous advancement. The reconstruction of a full or partial nail wasgreatly simplified by this. But initially thepolymerisation resins were only offered inpots, which did not guarantee safe, hygienic work. Eduard Gerlach GmbHwas able to make a breakthrough in thisregard. Since the market introduction ofthe GEHWOL Nail repair Gel in the fallof 2005, Gerlach is offering the light hardening resins in tubes. This made thecompany the first provider of polymerisation resins in this practicalform. The resin can be applied to the nailremnant directly from the tube withoutconstantly having to dip the brush back

into the container. This advantage particularly meets the hygienic requirements of a medical foot care practitioner. Patients will also be happy. For the tubes massively reduce the risk ofinfections. For instance no fungal sporesare transported into the resin and fromthere, to the nail bed of the next prosthetic patient.

Eduard Gerlach GmbH and I have nowfurther developed these gels. We primarily worked on the consistency ofthe gels. Now the classic gel isadditionally available in a highly viscous formulation. The three colours pink, clear and opal ofthe highly viscous gel are mixable witheach other without difficulty. This makesit possible to develop a natural colourtone. For this, one simply puts a dropfrom each tube (or the required mixingquantities) directly onto the area being filled and mixes them, for instance, with abrush.

With the new gel, we foot care specialistsnow have a choice of two viscosities. Wecan use the moderately viscous gels torepair and seal. They flow easily, thereforeeasily distribute into small grooves anduneven areas and ensure a smooth finish. The highly viscous polymerisation resinallows longer modelling. The resin doesnot flow apart as quickly. This makes itpossible to be creative for a longer timewithout difficulties and model the prosthesis into a natural form. It is alsovery suitable for filling out larger areas onthe nail. In this way it is possible to builda stable and long lived nail prosthesis.

Personally I like to work on the highlyviscous resin with a brush. It is also possible to work directly from the tube. But with a brush, it is easier to distributethe material and mix the colours. In orderto obtain the greatest possible flexibilityand stability, the prosthesis should be

produced in two to three layers. Herein it is important to observe the hardening times of two minutes. Anincompletely hardened prosthesis remainssoft, has too little hold and does not provide sufficient protection to the sensitive nail bed.

The moderately viscous resin can be usedto finish. This gives the nail a smooth andeven surface and a pleasant shine. Whenthe last layer has hardened, the resin iseasily ground with all common rotatinginstruments. The nail which has been formed in this way can be covered withpolish if desired.

Since the highly viscous gel hardly flowsat all, it is also optimally suited for fixingorthonyxia braces. A small drop isenough to keep the brace in place. Theincreased viscosity also has the advantage that the gel is more easily dosed – savingyou material and money.

With the new highly viscous resin, theGEHWOL product range has been expanded with a truly useful productwhich makes work on feet much easierfor us.
