– Login: (Network= Computer/Email)• Username: ab012345• Password: Network Password

FORMAL OBSERVATION

Formal Observation

• After your administrator has scheduled your Formal Observation, you will receive an outlook email notification

• Login to your MyLearningPlan and Accept or Decline the schedule as necessary

ACKNOWLEDGING FORMAL OBSERVATION

Acknowledging Formal Observation

• After your administrator has completed your Formal Observation form, you will receive a notification for acknowledgement

• Under your Evaluation Tab in MLP: Action Required: Open your Formal Observation Summary

Acknowledging Formal Observation

• After reviewing the form you can click on the following options:

• Print: Print form to PDF • Acknowledge: Verify that you have reviewed the

Formal Observation Summary• Comment: Add a comment to the form (sends an email

notification to your assigned administrator)

Acknowledging Formal Observation

• Administrator receives email notification once you have acknowledged the form

• Your form will show as “In Progress” – awaiting administrator finalization

• You can view this form at anytime

SUMMATIVE EVALUATION

Summative Evaluation

• After your administrator has scheduled your Summative Evaluation, you will receive an outlook email notification

• Login to your MyLearningPlan and Accept or Decline the schedule as necessary

Summative Evaluation• Once Accepted, you will see the form in your

“Scheduled Components”

• After your administrator has completed your form, it will show in your “Complete” window for you to view at anytime
