BAB I
PENDAHULUAN
1.1. Sirosis hepatik
a. Definisi
Sirosis didefinisikan sebagai proses difusi yang ditandai dengan fibrosis
dan perubahan struktur hati normal menjadi nodul dengan struktur abnormal.
Pada akhirnya, akan terjadi kerusakan hepatosit yang nantinya akan digantikan
dengan jaringan fibrosis.

Sirosis menyebabkan peningkatan tekanan darah porta karena perubahan
fibrotik dalam sinusoid hati, perubahan kadar mediator vasodilatasi dan
vasokonstriktor, dan peningkatan aliran darah ke pembuluh splanknik. Kelainan
patofisiologis yang mendasarinya menyebabkan masalah yang sering ditemui,
seperti asites, hipertensi porta, dan varises esofagus, ensefalopati hepatik, dan
gangguan pembekuan darah.
Asites merupakan akumulasi patologis cairan limfe dalam rongga
peritoneum. Asites merupakan tanda sirosis yang paling awal dan paling sering
ditemukan. Perkembangan asites terkait dengan vasodilatasi arteri sistemik yang
menyebabkan aktivasi baroreseptor di ginjal dan aktivasi sistem renin-
angiotensin, dengan retensi air dan natrium serta produksi vasokonstriktor.
Kelanjutan hipertensi portal yang terpenting adalah terbentuknya varises dan rute
aliran darah alternatif. Pasien dengan sirosis memiliki risiko varises ketika
tekanan porta melebihi tekanan vena kava sebesar > 12 mmHg. Perdarahan

akibat varises terjadi pada 25 – 40% pasien sirosis dan setiap episode perdarahan
lopati memiliki risiko kematian sebesar 25 – 30%.
Ensefalopati hepatik merupakan gangguan sistem saraf pusat dengan
rentang gejala neuropsikiatri yang lebar terkait dengan insufisiensi dan gagal
hati. Gejala – gejala ensefalopati hepatik diduga terjadi karena akumulasi
senyawa-senyawa nitrogen di dalam sirkulasi sistemik sebagai akibat shunting
melalui kolateral portosistemik yang melewati hati. Senyawa-senyawa ini
kemudian memasuki sistem saraf pusat dan menghasilkan perubahan
neurotransmiter yang mempengaruhi kesadaran dan perilaku. Perubahan amonia,
glutamat, agonis reseptor benzodiazepin, dan mangan dikaitkan dengan
ensefalopati hepatik. Akan tetapi, kadar amonia serum tidak begitu berhubungan
dengan status mental pada ensefalopati hepatik. Ensefalopati hepatik tipe A
diinduksi oleh gagal hati akut, tipe B disebabkan oleh bypass porta-sistemik
tanpa penyakit hati intrinsik, dan ditipe C terjadi pada sirosis. Ensefalopati
hepatik dapat diklasifikasikan menjadi episodik, persisten, atau minimal.
Gangguan pembekuan darah dapat terjadi pada sirosis, mencakup
Pasien tidak mengalami mual dan muntah, perut tidak terasa sakit. Tekanan
darah 120/90 mmHg, denyut nadi 80 kali/menit, frekuensi pernafasan 22
kali/menit, suhu tubuh 36°C.
Terapi dilanjutkan, obat injeksi dihentikan.
e. Hari ke-5 (10 Mei 2013)
Pasien tidak mengalami mual dan muntah, perut tidak terasa sakit. Tekanan
darah 110/70 mmHg, denyut nadi 79 kali/menit, frekuensi pernafasan 20
kali/menit.
Terapi dilanjutkan.
f. Hari ke-6 (11Mei 2013)
Kondisi pasien sudah membaik diperbolehkan pulang dengan catatan harus
kontrol secara teratur. Obat yang dibawa pulang adalah sebagai berikut:
Tilidon 3 x 10 mg
Spasmomen 2 x 1 tablet
Curcuma 2 x 1 tablet

1. Demam tifoid
Pemilihan obat, dosis, dan lama pemberian dinilai sudah tepat.
Tidak ada reaksi obat yang tidak diinginkan (ROTD) yang ditemukan
pada pasien.
2. Gastritis
Pemilihan obat, dosis, dan lama pemberian dinilai sudah tepat.
Tidak ada ROTD yang ditemukan pada pasien
3. Tuberkulosis
Pemilihan obat, dosis, dan lama pemberian dinilai sudah tepat.
Tidak ada ROTD yang ditemukan pada pasien
Pasien gagal mendapatkan obat pada hari kedua pengobatan.
Terdapat masalah interaksi obat antara OAT dan antasida, tetapi tidak
signifikan secara klinis.
BAB VI
EDUKASI PASIEN
13
1. Sefiksim diminum 2 kali sehari (setiap 12 jam) setelah makan dan harus
diminum sampai habis.
2. Ottopan diminum 4 kali sehari satu sendok takar bila panas.
3. Acitral diminum 3 kali sehari 2 jam sesudah makan.
4. Rifampisin diminum 1 jam sebelum makan, INH dan Pirazinamid diminum
setelah makan pada pagi hari
5. Jika air seni, air liur, atau air mata berwarna oranye kemerahan, jangan panik
karena hal itu merupakan efek samping yang biasa terjadi setelah menggunakan
OAT (Rifampisin).
6. Vistrum diminum 3 kali sehari satu sendok takar setelah makan.
7. Kurangi aktivitas berat dan perbanyak istirahat
8. Selama 2 minggu, jangan makan makanan yang keras, pedas,
berempah,ataupun berserat.
9. Perbanyak minum air putih.
10. Makan secara teratur, jangan melewatkan waktu makan.
11. Pasien perlu kontrol secara teratur ke dokter untuk follow-up pengobatan
tuberkulosis.
12. Bila pasien mengalami muntah yang mengganggu/tidak bisa dikendalikan, kulit
atau bagian putih mata berwarna kuning, segera konsultasikan ke dokter.

Domperidon merupakan antagonis dopamin
Adverse EffectsPlasma-prolactin concentrations may be increased,which may lead to galactorrhoea or gynaecomastia.There have been reports of reduced libido, and rashesand other allergic reactions. Domperidone does notreadily cross the blood-brain barrier and the incidenceof central effects such as extrapyramidal reactions ordrowsiness may be lower than with metoclopramide(p.1748); however, there have been reports of dystonicreactions.Domperidone by injection has been associated withconvulsions, arrhythmias, and cardiac arrest. Fatalitieshave restricted use by this route.Effects on the cardiovascular system. Sudden death has occurredin cancer patients given domperidone intravenously inhigh doses.1-3 Four cancer patients experienced cardiac arrest afterhigh intravenous doses4 and 2 of 4 similar patients had ventriculararrhythmias.5 After such reports the injection has beenwithdrawn from general use in many countries, including theUK.Prolongation of the QT interval has been reported in an infantgiven oral domperidone, with normalisation after the drug wasstopped.61. PrecautionsDomperidone is not recommended for chronic use orfor the routine prophylaxis of postoperative nausea andvomiting. Domperidone should be used with great cautionif given intravenously, because of the risk of arrhythmias,especially in patients predisposed to cardiacarrhythmias or hypokalaemia.Breast feeding. No adverse effects have been seen in breastfedinfants whose mothers were given domperidone, and theAmerican Academy of Pediatrics considers1 that it is thereforeusually compatible with breast feeding. However, the FDA in theUSA has issued a warning against the use of domperidone to increasemilk production because of the possibility of serious adverseeffects.2 Others have commented that these warnings werebased on data from patients with malignant disease receivinghigh doses of intravenous domperidone, and that if the motherwere taking smaller oral doses, the total amount of drug ingestedby an infant would be extremely small. They recommend thatlow-dose domperidone should still be considered for lactatingwomen with decreased milk supply who are unresponsive tonon-pharmacological measures to enhance lactation. However,patients should be warned of the risk of arrhythmias at high doses,and women with known cardiac disease should not take domperidone.31. American Academy of Pediatrics. InteractionsAs with other dopamine antagonists (see Metoclopramide,p.1749), there is a theoretical potential that domperidonemay antagonise the hypoprolactinaemic effectof drugs such as bromocriptine. In addition, theprokinetic effects of domperidone may alter theabsorption of some drugs. Opioid analgesics andantimuscarinics may antagonise the prokinetic effectsof domperidone.Domperidone is metabolised via the cytochrome P450isoenzyme CYP3A4; use with ketoconazole has beenreported to produce a threefold increase in plasma concentrationsof domperidone, and an associated slightprolongation in QT interval. Similar increases in domperidoneconcentrations might theoretically be seenwith other potent inhibitors of CYP3A4 such as erythromycinor ritonavir, and such combinations may bebest avoided.PharmacokineticsAlthough absorption is rapid, the systemic bioavailabilityof domperidone is only about 15% in fasting subjectsgiven an oral dose; this is increased when domperidoneis given after food. The low bioavailability isthought to be due to first-pass hepatic and intestinalmetabolism. The bioavailability of rectal domperidoneis similar to that after oral doses, although peak plasmaconcentrations are only about one-third that of an oraldose and are achieved after about an hour, comparedwith 30 minutes after an oral dose.

Domperidone is more than 90% bound to plasmaproteins, and has a terminal elimination half-life ofabout 7.5 hours. It undergoes rapid and extensivehepatic metabolism. The main metabolic pathwaysare N-dealkylation by cytochrome P450 isoenzymeCYP3A4, and aromatic hydroxylation by CYP3A4,CYP1A2, and CYP2E1. About 30% of an oral dose isexcreted in urine within 24 hours, almost entirely asmetabolites; the remainder of a dose is excreted in faecesover several days, about 10% as unchanged drug. Itdoes not readily cross the blood-brain barrier.Small amounts of domperidone are distributed intobreast milk; concentrations are 10 to 50% of those inmaternal serum.Uses and AdministrationDomperidone is a dopamine antagonist with actionsand uses similar to those of metoclopramide (p.1749).It is used as an antiemetic for the short-term treatmentof nausea and vomiting of various aetiologies (p.1700).It is not considered suitable for chronic nausea andvomiting, nor for the routine prophylaxis of postoperativevomiting.Domperidone is also used for its prokinetic actions indyspepsia (p.1695) and has been tried in diabetic gastroparesis(see Diabetic Complications, p.433). It hasbeen given with paracetamol in the symptomatic treatmentof migraine (p.616).Domperidone is used as the maleate in tablet preparationsand as the base in suppositories and the oral suspension;doses are expressed in terms of the base.Domperidone maleate 12.73 mg is equivalent to about10 mg of domperidone. Domperidone has been givenparenterally, but this route has been associated with severeadverse effects (see above).For the treatment of nausea and vomiting domperidonemay be given in oral doses of 10 to 20 mg threeor four times daily up to a maximum daily dose of80 mg or it may be given rectally in a dose of 60 mgtwice daily. For doses in children see below.For the symptomatic management of non-ulcer dyspepsiasimilar oral doses of 10 mg taken up to fourtimes daily (the last dose to be taken at night) have beenrecommended; if necessary, an increase in the dose to20 mg may be prescribed. An initial course of treatmentshould not normally exceed 2 to 4 weeks. oesophageal reflux (p.1696), which identified 4 randomisedcontrolled studies of such use, considered that there was very littleevidence of its efficacy in reducing symptoms.1 Some suggestthat it has been overused because of the lack of a suitable alternativeafter withdrawal of cisapride in many countries.21. Menetralkan asam lambung sehingga berguna untuk menghilangkan nyeri
tukak peptik. Antasid tidak mengurangi volume HCl yang dikeluarkan
lambung, tetapi peninggian pH akan menurunkan aktivitas pepsin.
Bakteriostatis dan bakterisid terhadap Mycobacterium tuberculosa tergantung
dosis pemberian.
b. Indikasi:
Sebagai OAT yang digabung dengan obat anti TB yang lain.
c. Kontraindikasi:
Hipersensitivitas, gangguan fungsi hati, dan gangguan fungsi ginjal.
d. Efek Samping:
Kerusakan hati, mual, muntah, malaise, demam.
e. Perhatian:
Hati-hati pada penderita hiperurisemia dan encok akut, harus dilakukan uji
fungsi hati dan ginjal.
f. Interaksi Obat
Dapat berinteraksi dengan probenesid dan ziduvudine
g. Farmakokinetika
Pirazinamid mudah diserap di usus dan tersebar luas ke seluruh tubuh. Dosis
1 gram menghasilkan kadar plasma 45µg/mL pada 2 jam setelah pemberian
obat. Eksresinya terutama melalui filtrasi glomerulus.Asam pirazinoat yang
aktiv kemudian mengalami hidroksilasi menjadi asam hidropirazinoat yang
merupakan metabolit utama.Masa paruh eliminasi obat ini adalah 10-16 jam.

h. Dosis dan bentuk sediaan
Dosis anak :15-30 mg/kg BB per oral dosis tunggal.
Bentuk sediaan: Tablet 500 mg.
