WHAT IS "LET’S GET HEALTHY CALIFORNIA"?California leaders in health and health care came together to develop Let’s Get Healthy California as a vision to improve the health of all Californians. Let’s Get Healthy Califor-nia’s charge is ambitious:

The Let’s Get Healthy California framework identifies six goals with specific indicators to advance the Triple Aim and to promote greater health equity.

Health Across the Lifespan addresses the question of “What will it look like if California is the healthiest state in the nation?” We should aspire to be a state where Californians at all ages and stages of life can thrive. Health Across the Lifespan sets out key milestones of health and well-being in three critical life stages:

HEALTH ACROSS THE LIFESPAN

Getting a healthy start sets the stage for health and well-being for a person’s entire life. The nine priorities represent a spectrum of important dimensions of children’s health and well-being from infancy to the teenage years. There is increasing evidence that a number of adult health and medical conditions have their origins in early childhood, which is why tracking a range of issues in childhood is critical.

The World Health Organization (WHO) defines health as a “state of complete physical, mental and social well-being and not merely the absence of disease or infirmity.” The following six priorities represent key aspects of living well. A particular focus is placed on preventing and managing chronic disease, given the rising prevalence of chronic diseases and the impact they have on the state’s residents.

This is one of the most difficult stages of life, and yet, in great need of attention to improve the care and experience of individuals who are dying. These are three priorities to track whether patients facing advanced age and illness are obtaining the kinds of services that would enable them to maintain independence and dignity consistent with their wishes to the greatest degree possible.

End of Life - Maintaining Dignity and Independence

Pathways to Health addresses the question of “What will it take for California to be the healthiest state in the nation?” Three goals are identified which cover the practice and policy changes needed to improve the quality and efficiency of the health care system and to make community environments more conducive to being healthy.

Creating Healthy Communities - Enabling Healthy Living

Numerous studies have demonstrated that where we live plays a major role in our health. A variety of community conditions enhance or create barriers to health, from the level of air pollution to the availability of parks and “green space” and access to fresh produce. People need communities that are safe and provide opportunities for active living and healthy eating. These three priorities support people in developing and maintaining healthy behaviors, especially those related to two of the main contributors of chronic disease—diet and physical activity.

Being the healthiest state in the nation will require the health care system to be better aligned toward population health goals and outcomes. The system should be patient-centered and look beyond illness to health. To achieve these four priorities, health care systems and plans across the state are already innovating ways to redesign the health delivery system.

Lowering the overall cost of care is critical for all Californians to be able to have access to affordable coverage and care, as well as for the fiscal health of the state. Although Califor-nia’s per capita spending on health care is the 9th lowest in the country, it is still growing at a faster pace than inflation or than the growth of the economy. It is crucial to reign in these costs through these five priorities to reduce the financial burden on families, businesses, and the state.

Evaluation CriteriaA panel of judges will select one finalist from each of the six Let’s Get Healthy California goals, using the following criteria:

Selected InnovationsSelected innovations will be announced in the fall and showcased at the Let’s Get Healthy California Innovation Conference in early 2016, while data-focused innovations may be highlighted at the statewide Open DataFest. Additional selected innovations will be featured on the Let’s Get Healthy California Web-site. Finalists will have the opportunity to present to thought leaders who collaborated to develop the Let’s Get Healthy California initiative and connect with key health innovators, practitioners and experts.
