Kit de extracción de implantes (KEXIM) 1. DESCRIPCIÓN DEL PRODUCTO Biotechnology Institute, S.L. ha desarrollado este producto para su utilización en la extracción de implantes. El sistema ofrece un método no agresivo para la extracción limpia y sencilla de implantes endóseos de titanio oseo-integrados quirúrgicamente en el hueso maxilar o mandibular. Las piezas utilizan una morfología específica diseñada especialmente para la extracción de una amplia variedad de implantes quirúrgicos con diferentes diámetros y longitudes. 2. INDICACIONES DE USO Los extractores de implantes B.T.I. están diseñados para su uso en cirugía oral implantaria. La indicación de uso del sistema es la extracción de implantes oseointegrados cuando ya no son funcionales o por ser innecesarios en una nueva situación protésica. El sistema es apto tanto para implantes de conexión interna como de externa. Este producto evitará, en la mayoría de los casos, el uso del protocolo tradicional de extracción mediante fresa de trefina. 3. MODO DE USO Nº 0 Nº1 Nº2 Nº3 Nº4 * Los extractores llevan su número correspondiente grabado con láser. Elija el extractor Nº1 en caso de un implante BTI EXTERNA® o TINY®, o Nº3 si se trata de un implante BTI INTERNA®. (Ver figura 1) Para implantes de otros fabricantes, los extractores Nº1 y Nº3 pueden ser eficaces; en caso contrario usar Nº0, Nº2 y Nº4 según criterios: – Nº0: Implantes de diámetro entre 3mm y 3,3mm con métricas pequeñas. – Nº2: Implantes de conexión externa de métrica mayor de 2mm o Implantes de conexión interna de pequeño diámetro. – Nº 4: Implantes de conexión interna de diámetro grande. (Ver referencias en el listado de componentes del KIT) 1) Coloque el extractor de implantes (EXIME, EXIM, INEXIM, INEXIML o EXIMA) en el extensor de la longitud adecuada (ELLCAC, ELLCAL, ELLCAX) y este a su vez en el mango MDPT. Una vez introducidas las primeras espiras del extractor en la rosca interna del implante, en sentido antihorario, retire el pomo y el extensor y monte el extensor en la llave de extracción de implantes LLT200. (Ver figura 2 del gráfico anexo) (Ver advertencias) 2) Introduzca el extractor por la rosca interna del implante asiendo la llave en su marca con los dedos, girando la llave (sentido antihorario) siempre mediante la aplicación exclusiva de torsión sin aplicación de fuerzas laterales. (Ver figura 3 del gráfico anexo) 3) Una vez trabado el extractor en el implante, aplique torsión en sentido antihorario de forma suave y paulatina hasta que el implante quede extraído completamente. (Figura 6 del gráfico anexo) 4) Si la llave de extracción de implantes LLT200 salta (movimiento de giro sobre su articulación) indicando que se ha llegado al torque límite de 200Ncm, se hace imprescindible seguir los pasos siguientes: Rearme la llave (ver sus instrucciones). Vuelva al paso 3 para extraer el implante. Este proceso se reproduciría si fuera necesario hasta tres veces antes de continuar con el siguiente paso. Tras cada salto de la llave, reármela como se indica en su instrucción correspondiente. (Figuras 3, 4 y 5 del anexo gráfico) 5) Si con el paso 4 no ha sido posible extraer el implante, proceda a desinsertar el extractor. Después, frese alrededor del implante con la fresa trefina especial para el sistema adecuada para el diámetro del implante a extraer. La fresa trefina trabajará a una profundidad entre 3 y 5 mm. Tras esto volveremos al paso 2. 4. CONTRAINDICACIONES La utilización de este sistema no produce riesgos adicionales en niños, mujeres embarazadas y lactantes a los ya contemplados en la implantología general. 6. ADVERTENCIAS Los extractores de implantes son de un solo uso. Su reutilización puede producir riesgos de infección y contaminación cruzada entre pacientes además de impedir la correcta extracción del implante. Para la utilización segura y eficaz de los extractores de B.T.I., se recomienda realizar el programa de formación continuada en implantología que organiza regularmente B.T.I. ya que la técnica quirúrgica requerida para la extracción de implantes es altamente especializada y el procedimiento es complejo. Una errónea o inadecuada utilización del procedimiento anterior puede causar el fracaso de la extracción del implante y la necesidad del uso de trefina, provocando la pérdida del hueso que rodea al implante. Sólo en el caso de implantes de diámetro 2,5 mm (IRT25xx) es imprescindible no superar un par de 70 Ncm, y se recomienda el uso de la llave LLMQ multitorque quirúrgica en lugar de la LLT200. Con el extractor EXIME es imprescindible no superar un par de 70 Ncm, y se recomienda el uso de la llave LLMQ multitorque quirúrgica en lugar de la LLT200. BTI no garantiza la eficacia de los extractores de implantes BTI con todos los implantes disponibles en el mercado. 7. PRECAUCIONES Es importante seguir el procedimiento descrito, con el fin de no ejercer palanca (cargas laterales) en el proceso de aplicación de torque, tampoco se debe sobrepasar el torque límite. Si la llave de extracción de implantes LLT200 salta (movimiento de giro sobre su articulación) indicando que se ha llegado al torque límite, nunca se debe forzar la llave o utilizar otro elemento para sobrepasarlo. 8. EMPAQUETAMIENTO Y MANIPULACIÓN Los extractores de implante han sido limpiados meticulosamente, sin embargo, deben ser esterilizados antes de su uso de acuerdo al protocolo de limpieza y esterilización de Instrumental quirúrgico de BTI. La llave de torque y el resto de componentes reutilizables de este sistema, deben limpiarse y esterilizarse antes de cualquiera de sus usos de acuerdo a su protocolo correspondiente. 9. ATENCIÓN Esterilizar antes de su uso. BTI recomienda esterilizar estos productos siguiendo el siguiente protocolo: – Esterilización a vapor con prevacío. – Productos en bolsas termosellables para esterilización. – Temperatura: 132ºC-135ºC; Tiempo de exposición: 3-4 min. Debe permitirse finalizar el ciclo de autoclavado hasta el final, sin interrumpir la fase de secado para evitar riesgos de oxidación. No se deben reutilizar los extractores. La venta de este producto se restringe a odontólogos, estomatólogos y cirujanos maxilofaciales. Implant extraction kit (KEXIM) 1. PRODUCT DESCRIPTION Biotechnology Institute, S.L. has developed this product for use in implant removal. The system offers a non-invasive method for the clean, simple extraction of endosseous titanium implants surgically osseointegrated in the maxillary or mandibular bone. The parts use a specific morphology specially designed for the extraction of a wide range of surgical implants of different diameters and lengths. Wählen Sie Nr. 1, wenn es sich um ein Implantat BTI EXTERNA® oder TINY® handelt bzw. Nr. 3, wenn es sich um ein Implantat BTI INTERNA® handelt. (Siehe Abb. 1) Für Implantate anderer Hersteller können die Extraktoren Nr. 1 und Nr. 3 wirksam sein; verwenden Sie im entgegengesetzten Fall Nr. 0, Nr. 2 und Nr. 4 unter Einhaltung der folgenden Kriterien: – Nr. 0: Implantate mit einem Durchmesser zwischen 3 mm und 3,3 mm mit kleiner Abmessung. – Nr. 2: Implantate mit Außenverbindung, mit einer Abmessung von mehr als 2 mm bzw. Implantate mit Innenverbindung mit kleinem Durchmesser. – Nr. 4: Implantate mit Außenverbindung mit großem Durchmesser. (Siehe Referenzen in der Liste der SET-Komponenten) 1) Den Extraktor (EXIME, EXIM, INEXIM, INEXIML o EXIMA) in den Ratscheneinsatz mit der passenden Länge einsetzen (ELLCAC, ELLCAL, ELLCAX) und diesen wiederum in den MDPT-Handgriff einsetzen. Zunächst die ersten Spiralen des Extraktors in das Innengewinde des Implantats einführen. Dann Handgriff und den Ratscheneinsatz gegen den Uhrzeigersinn drehen und den Ratscheneinsatz an der Drehmomentratsche LLT200 anbringen. (Siehe Abb. 2 im Anhang) (Beachten Sie die Warnhinweise.) 2) Den Extraktor in das Innengewinde des Implantats einfügen. Dabei die Drehmomentratsche an der Markierung mit den Fingern halten und vorsichtig gegen den Uhrzeigersinn drehen. Darauf achten, dass keine seitliche Belastung einwirkt sondern nur die Torsionskraft. (Siehe Abb. 3 im Anhang) 3) Sobald der Extraktor auf dem Implantat aufgesetzt ist, diesen langsam und vorsichtig gegen den Uhrzeigersinn drehen, bis das Implantat vollständig entfernt ist. (Abb. 6 der angehängten Grafik) 4) In Fällen, in denen die Drehmomentratsche LLT200 springt (Drehbewegung im Gelenk), zeigt dies an, dass das max. Drehmoment von 200Ncm erreicht wurde. Befolgen Sie unbedingt die folgenden Schritte: Stellen Sie die Drehmomentratsche in ihre Ausgangsposition zurück (siehe spezifische Anleitung). Führen Sie erneut Schritt 3 zur Extraktion des Implantats aus. Führen Sie diesen Vorgang ggf. maximal 3 Mal aus, bevor Sie den nächsten Schritt einleiten. Stellen Sie die Drehmomentratsche nach jedem Auslösen wie in der spezifischen Anleitung angegeben wieder in ihre Ausgangsposition zurück. (Abb. 3, 4 und 5 im Anhang) 5) Konnte das Implantat mit Schritt 4 dennoch nicht extrahiert werden, nehmen Sie den Extraktor heraus. Wenden Sie einen Trepanbohrer um das Implantat herum an, der speziell auf den Durchmesser des Implantats abgestimmt ist, das extrahiert werden soll. Verwenden Sie den Trepanbohrer über eine Tiefe von 3 bis 5 mm. Kehren Sie anschließend wieder zu Schritt 2 zurück. 4. DAS SET BEINHALTET DIE FOLGENDEN KOMPONENTEN – Kurzer Ratscheneinsatz (ELLCAC) – Zwei Extraktoren für Außenverbindung (EXIM) Nr. 1 – Langer Ratscheneinsatz (ELLCAL) – Zwei Extraktoren für Innenverbindung (INEXIML) Nr. 2 – Extralanger Ratscheneinsatz (ELLCAX) – Zwei Extraktoren (INEXIM) Nr. 3 – Handgriff für Ratscheneinsätze (MDPT) – Zwei Extraktoren (EXIMA) Nr. 4 – Zwei Extraktoren (EXIME) Nr. 0 – Drehmomentratsche (LLT200) 5. GEGENANZEIGEN Die Verwendung dieses Systems stellt kein Risiko für Kinder, Schwangere und stillende Frauen dar, das über die allgemein bekannten Risiken der Implantologie hinausgeht. 6. WARNHINWEISE Die Extraktoren dürfen nur einmal verwendet werden. Bei einer Wiederverwendung besteht das Risiko von Infektionen und Kreuzkontaminationen zwischen Patienten. Außerdem kann sie dazu führen, dass sich das Implantat nicht korrekt extrahieren lässt. Zur sicheren und wirksamen Verwendung der B.T.I.-Extraktoren wird empfohlen, am Fortbildungsprogramm für Implantologie teilzunehmen, dass das B.T.I. in regelmäßigen Abständen organisiert, da die chirurgische Technik für die Explantation hochspezialisiert und das Verfahren komplex ist. Eine fehlerhafte oder nicht sachgemäße Durchführung der o.g. Vorgehensweise kann zu einem Misserfolg der Explantation führen und zur Folge haben, dass ein Trepanbohrer eingesetzt werden muss. Dies führt zu Knochenverlust um das Implantat herum. Nur im Fall der Implantate mit Durchmesser 2,5 mm (IRT25xx) darf auf keinen Fall ein Drehmoment von 70 Ncm überschritten werden. Außerdem wird hier die Verwendung der chirurgischen Drehmomentratsche LLMQ anstelle des Modells LLT200 empfohlen. Bei Verwendung des Extraktors EXIME darf auf keinen Fall ein Drehmoment von 70 Ncm überschritten werden. SICHERHEITSHINWEISE Die beschriebene Vorgehensweise ist unbedingt einzuhalten, um bei der Anwendung des Drehmoments keine Hebelwirkung (seitliche Belastung) anzuwenden. Darüber hinaus ist das angegebene max. Drehmoment nicht zu überschreiten. In Fällen, in denen die Drehmomentratsche LLT200 springt (Drehbewegung im Gelenk), zeigt dies an, dass das max. VERPACKUNG UND HANDHABUNG Die Extraktoren werden unter Anwendung der größten Sorgfalt gereinigt, dennoch müssen sie vor der Verwendung gemäß dem Reinigungs- und Sterilisationsprotokoll für chirurgische Instrumente von BTI sterilisiert werden. Die Drehmomentratsche und alle weiteren wiederverwendbaren Komponenten dieses Systems müssen vor jeder Verwendung unter Einhaltung des geltenden Protokolls gereinigt und sterilisiert werden. 9. ACHTUNG Vor der Verwendung sterilisieren. BTI empfiehlt, diese Produkte unter Einhaltung des folgenden Protokolls zu sterilisieren: – Dampfsterilisation mit Vorvakuum. – Produkte in thermisch verschließbaren Tüten für Sterilisation. – Temperatur: 132ºC-135ºC; Expositionszeit: 3-4 min. Der Autoklaven-Zyklus muss vollständig und ohne Unterbrechung der Trocknungsphase abgeschlossen werden, um Oxidation zu vermeiden. Extraktoren dürfen nicht wiederverwendet werden. INDICATIONS D’UTILISATION Les extracteurs d’implants B.T.I. sont conçus pour être utilisés en chirurgie orale implantaire. Le système est indiqué pour être utilisé dans l’extraction d’implants ostéointégrés dès lors que ceux-ci ne sont plus fonctionnels ou bien rendus inutiles de par un nouveau projet prothétique. Le système est adapté aux implants à connexion interne comme aux implants à connexion externe. Ce produit évitera, dans la plupart des cas, l’usage du protocole traditionnel d’extraction par le biais d’une fraise à trépan. 3. MODE D’UTILISATION Nº 0 Nº1 Nº2 Nº3 Nº4 * Les extracteurs portent le numéro correspondant gravé au laser. Choisissez l’extracteur N°1 pour un implant BTI EXTERNA® ou TINY®, ou N°3 s’il s’agit d’un implant BTI INTERNA®. (See figure 6 of the attached chart) 4) If the implant extraction wrench LLT200 jumps (rotary movement on its articulation) indicating that the maximum torque of 200Ncm has been reached, it is essential to follow the following steps: Reset the wrench (see instructions). Go back to step 3 to extract the implant. This process will be repeated up to three times if necessary before going on to the next step. After each jump of the wrench, reset it as indicated in the corresponding instruction. (Figures 3, 4 and 5 of the attached chart) 5) If it has not been possible to extract the implant with step 4, proceed to remove the extractor. Then, drill around the implant with the trephine drill specially adapted for the diameter of the implant to be extracted. The trephine drill will work to a depth of between 3 and 5 mm. After this, go back to step 2. 4. CONTRAINDICATIONS The use of this system does not cause any further risks in children, pregnant women and lactating women than those already observed in general implantology. 6. WARNINGS Implant extractors are single use only. Reuse may cause risk of infection and cross-contamination between patients as well as preventing the correct extraction of the implant. For safe, effective use of B.T.I. extractors, we recommend you to follow the ongoing training programme in implantology regularly organized by B.T.I., since the surgical technique required for implant extraction is highly specialized and the procedure is complex. Erroneous or unsuitable use of the above procedure may lead to failure of the implant extraction and the need to use a trephine drill, causing loss of bone around the implant.
