Log in using your Student Number (2019xxxxxx) as your username
andyour 5-digit PIN as your password at the Freshman Login link on
theRegistrar’s Corner at the upper left of the homepage.Carefully
follow the instructions provided in the confirmation page.
Confirm your decision to enroll in UPV through online via
crs.upv.edu.ph
IN PERSON. Submit the accomplished UPV Form C1 at the Office of
theUniversity Registrar, Miagao, Iloilo.BY COURIER. Send by
courier the accomplished UPV Form C1 to the: Office of the
University Registrar, U.P. Visayas, Miagao, IloiloBY FAX. Fax the
accomplished UPV Form C1 to the Office of the UniversityRegistrar
through (033) 315-8556
CONFIRMATION AT THE OURAccomplish the UPV Form C1 (downloadable
at the crs.upv.edu.ph) andsubmit to the Office of the University
Registrar through the following:
Confirm your decision to enroll in UP Visayas for the First
Semester 2019-2020 either online through the crs.upv.edu.ph or at
the Office of theUniversity Registrar (OUR) starting Friday, 03 May
2019 until Friday, 24 May 2019.
FAILURE TO CONFIRM WITHIN THE DEADLINE WOULD RESULT IN THE
CANCELLATION OF YOUR SLOT.
NOTE:To all Incoming Freshmen of UP Visayas who are on "Degree
Program withAvailable Slot" status and those who would appeal for
change of degreeprogram, please indicate in the UPV Form C1 OR thru
online confirmationyour choice/s of degree programs to be
waitlisted in.

Please submit the following credentials and documents to the
Office of the University Registrar not later than 24 May 2019 (may
be sent throughcourier) Place all these in a long cream/white
folder and arrange in order:a.) Two (2) identical 2x2 colored
pictures with your name and signature at theback. Scanned photos
are not acceptable.b.) Original High School Card (Form 138) duly
signed by the Principal andbearing the remark: “Graduated and
Eligible for Admission to College.”c.) Original Student Permanent
Record (Grade 7 to 12) or Senior High SchoolTranscript with
certified true copy of Junior High School Transcript withremarks
“Graduated and Eligible for Admission to College” and “Copy for
UPVisayas” (Official request can be downloaded at
crs.upv.edu.ph)d.) Original Certificate of Live Birth issued by
Philippine Statistics Authority
FIRST BATCHENTRANCE CREDENTIALS
Failure to submit the required First Batch of credentials and
documents listed aboveon or before 24 May 2019 would result in the
cancellation of your slot.
Please submit the following documents to the Office of the
University Registrar notlater than 21 June 2019:a.) Medical
Certificate issued by the UPV Health Services Unitb.) Special Power
of Attorney (SPA) designating a Legal Guardian for thosefreshmen
whose parents are living outside Iloilo province and cannot
respondimmediately in case of emergency. Standard Format is
downloadable at thecrs.upv.edu.ph website. c.) Certificate of
Completion of Bridging Program (if applicable)d.) Student Directory
to be signed by the student at the OUR (upon completionof
enrollment data at the crs.upv.edu.ph starting 17 until 21 June
2019)

your personal file and for otheradmission-related purposes before
you submit these to the OUR.
Obtain your ADMISSION SLIP from the Office of the University

Submit yourself to the following: chest x-ray, CBC, fecalysis*
and urinalysis at the UPV HealthServices Unit (Infirmary) Miagao,
Iloilo.
a) Present your UPCAT notice to the UPV HSU staff at Window 2
and get the laboratory and x-rayrequest slips.b) Pay to the cashier
and submit your request slip for laboratory and your specimen to
thelaboratory staff. Kindly observe the following schedules for
laboratory:
PRE-MEDICAL EXAMINATION FOR MIAGAO-BASED PROGRAMS
c) Proceed to the x-ray room and submit your request slip to the
staff.
MEDICAL AND DENTAL EXAMINATIONSa) Submit to Window 2 the
following: one (1) long size white folder without label, original
copy of thelaboratory results, one (1) piece 2x2 colored picture,
fully accomplished Pre-Enrollment MedicalRecord (downloadable at
crs.upv.edu.ph) and a photocopy/screenshot of your UPCAT notice.
Laboratory results should not exceed two (2) weeks by the time you
will have your medical anddental examinations.b) Have your Medical
and Dental Examination at UPV Health Services Unit (Infirmary) at
the MiagaoCampus from 9:00-11:00 a.m. and 2:00-4:00 p.m. during the
following dates:
It is recommended that you follow your assigned schedule for
pre-medical examinations posted at the UPVCRS website for your
convenience.
*You have the option to have your fecalysis done at any
government laboratory .
If your schedule is in the afternoon, please anticipate that
there is a possibility that you cannot finish it within theday.
However, you will be accommodated the following working day to
finish your physical examination.
It will help to hasten your Physical Examination process by
knowing your medical history fully well and being
physically prepared, such as getting enough sleep and water
intake, days prior to your schedule.

ACCOUNTANCY ANDBSBA (MARKETING) PROGRAMS
a) Submit yourself to the following: chest x-ray, CBC, fecalysis
and urinalysis at anygovernment laboratories. Laboratory
results should not exceed two (2) weeks by the timeyou will have
your medical and dental examinations. b) Submit one (1) long size
white folder without label, original copy of the laboratory
results,original official receipt/s of laboratory and X-ray fees,
one (1) piece 2x2 colored picture, fullyaccomplished Pre-Enrollment
Medical Record (downloadable at crs.upv.edu.ph) and
aphotocopy/screenshot of your UPCAT notice. c) Have your Medical
and Dental Examination at UPV Health Services Unit (Clinic), Iloilo

A. FREE TUITIONPlease be informed that UP Visayas is
implementing the Universal Access to Quality TertiaryEducation Act
(RA 10931) and its Implementing Rules and Regulations.
VOLUNTARY OPT-OUTAs part of the Implementing Rules and
Regulations of the RA 10931, a student may voluntarilyforego the
Free Tuition in UP Visayas prior to enrollment and instead, pay the
full tuition andmiscellaneous fees. The Opt-out forms may be
filled-out at your respective Office of theCollege Secretary during
the registration period.
VOLUNTARY CONTRIBUTIONDuring the registration period, the
students of UP Visayas may voluntary contribute anyamount to the
university during the registration period. Please inquire for the
VoluntaryContribution Form at the Office of the University
Registrar.
