All women should be enabled to practise exclusive breastfeeding and all infants should be
fed exclusively on breast milk from birth to 4-6 months of age. Thereafter, breastfeeding,
with appropriate and adequate complementary foods, for up to two years.
Reinforcement of a "breastfeeding culture" and its vigorous defence against incursions of a
"bottle-feeding culture".
Efforts to increase women's confidence in their ability to breastfeed. By removal of
constraints and influences that manipulate perceptions and behaviour towards
breastfeeding.
Measures should be taken to ensure that women are adequately nourished for their
optimal health and that of their families.
All governments should develop national breastfeeding policies and set appropriate
national targets for the 1990s. They should establish a national system for monitoring the
attainment of their targets
National authorities are further urged to integrate their breastfeeding policies into their
overall health and development policies.

BREASTFEEDING ~ Just 10 Steps!
The Baby-Friendly Way The Ten Steps were introduced by WHO/UNICEF jointly in1989
The Innocenti Declaration called upon the world to implement the ten stepsand open Baby Friendly Hospitals all across the world.
PROGRESS ON BFHI AND THE TEN STEPS
Today, more than 20,000 maternity facilities worldwide report ever havingachieved Baby-friendly status.
BFHI has been a key factor contributing to recent declines in childmortality (UNICEF 2009)

1) Breastfeeding can contribute to the Attainment of
No.4- Reducing Child Mortality RatesNo.5- Improving Maternal Health
2) UNICEF reports reduction in child death from 13 million (1990) to 8.8 million(2008)... partly due to such interventions.
3) Implementation of the Ten Steps contributes to increased breastfeedinginitiation at the local, national, global levels.
4) Rates of Exclusive breastfeeding has become stagnated or declined in several
countries because of reduced political support for the BHFI and poor compliancewith the Ten Steps.
5) An estimated 28% of all maternity facilities in the world have at some pointimplemented the Ten Steps which has contributed to an encouraging increase inbreastfeeding rates

Why India needsthe Ten Steps ?
WBFW 2009 theme Are we ready for an Emergency.... year 2010 we haveto reflect back and recon whether we are really ready in case of anemergency???
In India BHFI was started in 1993 which helped in increasing thebreastfeeding but not as much as expected. What is needed is to reinstatethe importance of the BHFI and take it forward.
For taking breastfeeding to the next level in India we need to educate anddevelop the necessary skills in the women working in maternity help system.

Where India Stands?
Breastfeeding Promotion Network of India (BPNI) andInternational Baby Food Action Network (IBFAN) found out froma survey conducted in 2008 that most of the policies andprogrammes in India have lost there relevance in the past threeyears. India scored 69/150 in this index and among the 32 othernation in which this survey was conducted India stands 27th....Report for the year 2008-2009.

Few more Observations
Considerable decline in the Exclusive Breastfeeding rates from the
1st month to the 6th month
Only 20% infants receive exclusive breastfeeding for 6 months
compared to 80% target set in the 10th 5 yearly plan.
In the 6-9 month age group:- infants receiving supplementary food
with breastfeeding is only 55.8% (NFHS-3) compared to DFHS-3 which
reports it to be 23.9%

Summary of the DATA
Except the increase in the rate of breastfeeding within 1st hour from 20% to 40%
no other indices have shown a rise.
Reasons:
Increase in the marketing and promotion of Baby food products Lack of expected support from the family or from workplace to mother.

More than1.4 million children in India die before their 1st birthday and of them
1 million die within the 1st month of their life
Instituting Breastfeeding within the first 1hour of birth alone can bring down
this rate by 22%
Moreover if Exclusive Breastfeeding is done for first 6 month the DISEASE
SPECIFIC MORTALITY by pneumonia can be brought down by 2.5 times and
that by Diarrhoea by 4.6 times.
An issue of Lancent published in 2008 states that less than optimal
Breastfeeding is an important cause of infant deaths by Neonatal Infections asDiarrhoea and Pneumonia
Of the 26-27 million children being born in India 22% are LBW (Low birth
Weight) (NHFS-3). Early breastfeeding can bring this malnutrition factor
because this malnutrition peaks in the first 18 month.

STATUS OF THE BFHI IN INDIA
BFHI started in India in 1993 which continued to function successfully
till 1998 after which the participation of women declined.
In the period (1994-1998) 1372 hospitals were taken under BFHI inwhich the TEN STEPS FOR SUCCESSFUL BREASTFEEDING was launched

Guidelines Suggested for Improving Breastfeeding by BFPNI
Taking the model of hospital centred breastfeeding program to community
level health organization
Keeping the mother and the child together for the maximum possible time
so that the target of Exclusive Breastfeeding for the first six months are
met. Therefore it is proposed to give 6 months of LEAVE WITH PAY to
every working woman.
Health Programmes be integrated with other National Programmes and a
Coordination be established between them.
In order to increase the breastfeeding rates Health and Child Nutrition
Centres be directly connected with the masses.

BABY FRIENDLY COMMUNITY HEALTH INITIATIVE LALITPUR MODEL
Started in 2006 in order to give shape to the guidelines suggested by BFPNIin Lalitpur district of Uttar Pradesh.
Gorakhpur Medical College, Local Administrative body and Unicef jointly
started the program.
Under this 50 girls and boys identified from 6 developmental blocks were trainedas counsellors and teachers.
They trained 2853 village health worker like those from ASHA, Anganbadi workers,Dais and rural women about the Breastfeeding Initiative
These Health Workers in turn are giving routine advice to the village women.
Another 50 trained girls and boys give the team periodic referral report.

RESULTS OF THE INITIATIVE.
EXCLUSIVE BREASTFEEDING rates increased from 6.8% to 50%
Post 6 month supplementation of breast milk with complimentary food increased
From 4.6% to 85%
Infant Mortality Rate brought down by 25-30%
LESSONS TO BE LEARNED FROM LALITPUR MODEL
With help from the trained Human Resource Breastfeeding Rates can be increased.
A Programme be developed for rural areas by which women can be given advice at
Their doorstep.
