What is the PEACH Program?

The PEACH Program is a NSW government funded program. PEACH packages support patients in their wish to die at home by providing additional care which complements existing Local Heath District (LHD) community nursing and palliative care services.Patients and their carers are provided with rapid access to additional care including increased nursing visits to ensure they and their carers are well supported to stay safely and comfortably at home. There are no charges for these services.

Who is the PEACH Program designed to help?

Patients who:• Wish to remain at home for end of life care and are

aware of their diagnosis/prognosis• Are currently registered with a palliative care service

and a community/primary health nursing service• Have a carer living with them• Are assessed as appropriate for a PEACH package

based on set criteria• Require increased level of support due to their

increased care needs.Carers who: • Require additional support to enable the patient to

remain at home.

What services are included?

The services received from the PEACH program are in addition to community nursing, specialist palliative care nursing, GP and specialist medical services that patients have been receiving.Our partner Silver Chain, will provide you with the following additional supports:• Personal care provided by an Assistant in Nursing

during the daytime• Clinical care and support provided by a Registered

Nurse during the evening• Overnight clinical advice and support is available

from a Silver Chain Registered Nurse via the telephone or by using your Silver Chain video device.

Duration of the Program?

The PEACH Program packages are available to patients for up to seven days. This may be extended depending on the patient’s circumstances. Patients can choose to cease the package at any time. This will not affect the provision of LHD community nursing and palliative care services.
