1.0 INTRODUCTION This Monthly Progress Report is submitted in accordance with the ADMINISTRATIVE ORDER ON CONSENT (AOC) FOR REMOVAL ACTION Proceeding Under Sections 104, 106(a), 107, and 122 of the Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA), as amended, 42 U.S.C. SS 9604, 9606(a), 9607, and 9622 (U.S. EPA Docket No.: V-W-'03-C-747) effective July 31, 2003. The next Monthly Progress Report, for the month of July 2005, will be submitted on or before August 15, 2005.
2.0 SIGNIFICANT DEVELOPMENTS IN THIS MONTH • Air monitoring has continued with the excavation activities. Final validated results of the
air-monitoring program for June 2005 are presented in Table 1.1a (polyurethane foam (PUF) samples) and Table 1.1b (total suspended particulate (TSP) 5, 6, 8 and 9 results). The locations of the air monitoring stations in the upstream and downstream areas are presented on Figures 1 and 2.
• Creek restoration construction on Parcels 6 through 11 (east of Bailey Scales Road) is complete. Access roads remain.
• Verification results are presented in Figures 3 through 8 to show progress as of this reporting period. Work completed in the parcels, as presented, is not complete as either additional excavation or statistical evaluation may be required to confirm the cleanup objective has been met. Verification figures for a given parcel are included in the monthly report until all cleanup criteria are met, whereupon the figure is presented with validated data.
• During June 2005, work continued along the stream channel of Parcels 15, 216, and 21 to remove impacted soil between rock bedding planes. During June 2005 a total of 2,739 tons were excavated, stockpiled, characterized, and disposed of at Heritage’s Roachdale Facility. Confirmation sampling was conducted on the excavated parcels as follows:
- Parcel 216 on June 10, 27, and 28, 2005 as presented in Figures 3 and 4; and - Parcel 21 on June 2, 6, 7, 8, 10, 28, and 30, 2005 as presented in Figures 4 and 5.
• Excavation of impacted soil and sediment from Parcels 23, 24, 25, and 28 continued. During June 2005 a total of 986 tons of material were excavated, stockpiled, characterized, and disposed of at Heritage’s Roachdale Facility. Confirmation sampling was conducted on the excavated parcels as follows:
- Parcel 25 on June 22, 2005 as presented in Figure 6.

- Parcel 28 on June 24 and 29, 2005 as presented in Figures 7 and 8. • The summary of PCB waste material disposal for June 2005 is presented in Table 2.1. The
transportation and disposal summary for <50 ppm and >50 ppm PCB waste material are presented in Tables 2.1a and 2.1b, respectively.
• The construction of Grading Areas 1 and 2 were completed on June 13 and 28, 2005, respectively. Stockpiled soil began to be placed in the Grading Area 1 on June 15, 2005. During June 2005, approximately 29,725 tons of soil had been placed into Grading Area 1.
• The estimated total tonnage removed by parcel for June 2005 is presented in Table 3.1. • Water within the remediation areas is collected and treated by either ENTACT
Environmental Services’ (ENTACT’s) or Sevenson Environmental Services’ (SES’s) on-Site water treatment systems. The ENTACT treatment system is being modified to increase retention time in the sedimentation tanks. The treatment system effluent is sampled on a batch basis. When sampling confirms the stored water meets the discharge criteria the water is released to the creek. Once the system is tested, GM will request U.S. EPA approval to resume direct discharge. SES’s water treatment system upgrades are complete. U.S. EPA has approved direct discharge of treated water from the SES’s treatment system. Sample results for June 2005 are provided in Tables 4.1 and 4.2.
• Electrical and mechanical controls for the Site Source Control (SSC) system are being installed. The new Wastewater Treatment Plant for the treatment of water from the stormwater pond was in a position to operate the treatment system (2,000 gallons per minute (gpm)) on an emergency basis on April 30, 2005. The treatment system will not be completed, along with the 300 gpm system designed to treat the site source control water/groundwater, until late September/early October 2005. A successful performance (shakedown) test, including analytical sampling, for the treatment system was conducted on May 19, 2005. Water from the remediation activities is still being sent to one of the temporary treatment systems set up in the Removal Action until final components are received to complete that portion of the new treatment plant.
• CRA contacted Sharon Hall of Indiana Department of Natural Resources (IDNR) to discuss issuance of a Rule 5 Soil Erosion Permit for using Parcel 39 as a borrow source. Ms. Hall requested modifications to the proposed soil erosion controls.
• Conference calls were held on June 2, 9, 15, 28 and 30, 2005 with the U.S. EPA and IDEM, to discuss project progress and items related to the design and construction of the East Plant Interim Measure (IM).
• On-Site construction meetings for the reporting period have been held informally daily and formally weekly. Meetings with SES are held on Wednesdays. SES meetings were held on June 1, 8, 15, 22, and 29, 2005. Meetings with ENTACT are held on Thursdays. ENTACT meetings were held on June 2, 9, 16, 23, and 30, 2005. Meetings were also held with ENTACT and SES to discuss the East Plant Area on June 1, 7, 14, 21, and 28, 2005. Hereafter East Plant meetings will be held on Tuesdays. Minutes of these meetings are attached in Appendix B.

• Units in the Table 1.1a of the March, April, and May 2005 Monthly Progress reports was reported as milligrams per cubic meter (mg/m3) and should have been reported as micrograms per cubic meter (ug/m3). The corrected tables are attached in Appendix C.
3.0 SUMMARIES OF ALL ANTCIPATED PROBLEMS AND PLANNED RESOLUTIONS
• During June 2005, there were days (see Table 1.1B) when TSP concentrations exceeded the
action level. The action level for TSP is defined as 67% above the ambient upwind concentration. − The cause of the exceedances included dust generated during excavation activities in
work zones and hauling activities to and from the staging areas. The contractors have been instructed to continue to implement dust control measures along the haul roads and street sweeping along public roads. Clean, wetted soil tracked onto the public roads is removed using street sweepers.
− The street sweepers cause some additional dust generation. Gravel at entryways is periodically replaced with new material. Truck tires are inspected and washed, if necessary, to prevent tracking of clean material onto public roads.
• GM is evaluating on–Site borrow source and haul routes to minimize truck traffic on public roads.
• GM is looking at what will be needed at Spring_018, once excavation of the creek in the area of Spring_018 is completed.
4.0 PROJECTED WORK FOR THE NEXT REPORTING PERIOD • The following is a list of anticipated work for the next reporting period:
− Restoration on Parcels 5, 6, 7, 8, 10, and 11 is complete. Some additional restoration work has been requested by the Parcel 4 property owner and will be completed during July 2005;
− Restoration on Parcels 12, 13, 15, and 216 will continue during July 2005. − Installation of the SSC components will be completed in July 2005. Piping from wet well
#3 to the permanent SSC water treatment system will be completed in conjunction with the completion of the treatment system. Until that system is complete the water from this area will continue to be pumped to SES’s modutanks and then through the SES treatment system;
− Spring and seep sampling as part of the SSC Work Plan will continue during the next quarter. The 2nd quarter high flow and both 3rd quarter low and high flow sampling will be attempted when the criteria are met;

− Excavation will continue in the Downstream Parcels, upstream of Broomsage Road (SES work area);
− Excavation will continue in the Downstream Parcels, downstream of Broomsage Road (ENTACT work area);
− Road repair work will continue, as needed, through July 2005; − The Supplemental Stormwater Management Plan is under review and will be submitted
to the U.S. EPA and IDEM in the near future; − Coring as part of RFI Work Plan: Addendum No. 9 is anticipated to continue to be
completed during the next reporting period; − Complete the design for the East Plant Area Interim Measure and submit for U.S. EPA
review; − Continue to work with the U.S. EPA and IDEM to gain approval of the East Plant Area
Vault design. Upon receipt of approval, potential contractor bids will be evaluated and the construction award. Construction of the vault liner will begin as soon as possible;
− Submit to the U.S. EPA and IDEM the detailed design for the excavation of the ≥ 50 mg/kg PCB material in the East Plant Area. Work with the U.S. EPA and IDEM to gain approval. Upon receipt of approval, potential contractor bids will be evaluated and the construction award. Excavation of the ≥ 50 mg/kg PCB material from the East Plant Area will commence as soon as the construction of the vault is complete;
− Submit to the U.S. EPA and IDEM the cover system design for the East Plant Area. Work with the U.S. EPA and IDEM to gain approval;
− Continuation of placement of the < 50 mg/kg Removal Action soils in the East Plant Area both as grading fill and as fill in excavations;
− Prepare and distribute Fact Sheet 12 in September 2005; − The next set of Public Meetings was scheduled for September, 2005; and − The next CLP Meeting was scheduled for September, 2005.

5.Calculation of tonnage removed on Parcel 25 includes soil removed for the construction of Diversion Channel 1 which accounts for an inflated estimate. As part of the restoration of this parcel, Diversion Channel 1 will be backfilled. Excavation for Parcel 25 is ongoing.
1. Volumes were calculated using AutoCAD® LandDeveloper2. Excavation volumes were calculated by comparing the pre-excavation survey to the final excavation survey or to an interim survey where excavation is ongoing.3. The tonnage was calculated by the conversion of 1 cubic yard = 1.4 tons.4. Estimate percent complete based on predicted volumes where excavation not complete.
