Synopsis of Lab items for 2015

Anne, you and myself are the management team for the lab. We are required to have periodic

meetings for NELAP compliance, and I am required to submit a report. A few years back, we

decided to change this from quarterly to annually. The following are from the TNI Standard for

NELAP laboratories:

In accordance with a predetermined schedule and procedure, the laboratory's top management shall periodically conduct a review of the laboratory's management system and testing and/or calibration activities to ensure their continuing suitability and effectiveness.

-The management review shall take account of: customer feedback;

-The management review shall take account of: recommendations for improvement;

-The management review shall take account of: other relevant factors, such as quality control activities, resources, and staff training.

NOTE 1: A typical period for conducting a management review is once every 12 months.

NOTE 2: Results should feed into the laboratory planning system and should include the goals, objectives and action plans for the coming year.

NOTE 3: A management review includes consideration of related subjects at regular management meetings.

Findings from management reviews and the actions that arise from them shall be recorded.

The management shall ensure that those actions are carried out within an appropriate and agreed timescale.

Management review shall be completed on an annual basis.

Data Integrity Investigations

All investigations resulting from data integrity issues should be conducted in a confidential manner until they are completed. There were no investigations resulting from data

integrity in 2014.

These investigations shall be documented as well as any notifications made to clients receiving any affected data. Not applicable for this year.

1. Once we meet, I will attach any minutes from that meeting to this memo. 2. I have improved greatly on keeping up with training records and procedure updates. 3. The radiochemistry section of the lab has taken better shape. We have received valuable pieces

of equipment from the DOE and other agencies. James and I completed our training as Radiation Safety Officers in December 2014. I received training in Facility Decommissioning from Argonne National Laboratories through a program with the TCEQ. I need MARSSIM training and one elective to obtain my Decommissioning Certificate. We are receiving small budgets from TCEQ for working on decommissioning uranium mines in Texas, and I expect this to be of larger importance for the lab in 2015.

4. Continuing records destruction in compliance with project, A&M and Tarleton regulations. 5. The amount and type of samples were greatly reduced compared to previous years due to

projects ending and funding not being as great as in the past. 6. Power failures were reduced thanks to better connectivity to the diesel generator. 7. Apparently still doing good on quality control. PT samples generally passing, as do most in-

house QC parameters. We did have a problem with TKN on 2 successive PTs. The problem was a technical one and was resolved.

8. I have been QA Officer and NELAP Technical Director for over a years now. I still have concerns about getting the QA responsibilities done properly. Some improvements in efficiency, reduction in volume of paperwork by going electronic in several areas, and reporting only useful information should help. It would still be good to have another pair of eyes on our system.

9. The issue of accounting of time and allocations has been greatly improved, thanks to Scotty and some changes to ESDMS. This frees up more of my time for other duties.

Summary of 2014 samples

There are 936 sample numbers listed in the ESDMS database. Of those, 659 are actual laboratory

samples received and include grab samples, storm grabs, flow composites, time composites and

sequential bottles collected by autosamplers not composted. The remaining sample numbers are

identities for field data or samples voided for various reasons.

For the ESDMS samples listed, project breakdown was as follows: 335-NBR2, 104-Grace Sabine, 177-

Nolan, 27-PT samples, 73-TCEQ EMRS standards, 1 on State for Tarleton, 143-BL319V2, 14-Elm Sandies,

and 61- Lampasas.

The following were the number of analyses completed and entered in the database:

Corrective Action Reports summary:

There were 111 CARs generated in 2014. 6 of these CARs were field related. 13 CARs were voided when

determined to be duplicate or unnecessary. 91 were related to problems with laboratory analyses or

login/sample receipt, but did not necessarily fail data. 8 were related to laboratory equipment failures

or problems, primarily related to temperatures not being within specifications. 8 CARs were related to

errors or issues at Login or sample receipt. 5 were due to issues with ESDMS or some problem with

computer forms and data entry. 58 CARs were written to document failed quality control measures

during analytical runs. The majority were failed matrix spikes and sample duplicate deviations. 1 CAR

was for PT failure. 1 CAR was for security issues of unlocked doors, which was remedied by ordering

alarms and cameras and by re-keying all door locks. The remainder of CARs were for various other

reasons.

Of the 98 legitimate CARs in 2014, 40 were tier 3 level reports where data or samples were not reported

or reported with a qualifier flag and were due to failed quality control, sampler issues, equipment

problems or other reasons.

Overall, CARs have continued to decrease over the years for various reasons.

Other points of interest for 2014

1. Total solids was removed from our accreditation as it was not being performed except on PT samples

2.

There were 656 changes made to 2014 lab data after entry.

593 by me (Lab Manager):

1. 552 were automatic adjustments by ESDMS for LOD screening 2. 4 were due to unreasonable relationships to other data 3. 2 were data entry errors found at various points of review. 4. The remainder were data added or deleted at the request of the project manager, or deleted or

corrected by order of a Corrective Action Report. In all cases the changes were documented and noted by audit trail.

60 by James Hunter:

1. 57 were automatic adjustments by ESDMS for LOD screening 2. 7 were ESDMS change errors notations where no data were actually changed (noted).

Client Complaints and Customer Service

There were no real complaints from customers in 2014. I solicit feedback in the tag line of all my emails.

We responded to questions and requests in a timely manner, as far I can find in all my emails and notes.

There were none in 2014. We are due for a NELAP audit again at the end of 2015.
