INTRODUCTION Sharecare provides a personalized experience to help effectively manage the health of your population. Engagement is a pivotal factor in ensuring an optimal user experience. This playbook includes the tools and resources we’ve developed to start – and keep – your employees engaged with the program, and motivated to take control of their health.

WHAT IS SHARECARE?

Blue Cross and Blue Shield of Minnesota and Sharecare have partnered to help your employees live a happier, healthier life. Sharecare’s solution is significantly more comprehensive than any other digital platform, coupling the science of Sharecare with the art of Sharecare – bridging clinical programs and content with a personalized experience for our members.

It begins with the RealAge® test, Sharecare’s clinically-validated health risk assessment that shows members the true age of their body. The innate curiosity of how to impact physical age is one of the hooks that draws people into the platform. From there a personalized action plan is created to guide and educate members on the parts of their health that can make the biggest impact on their RealAge. Key insights into each user’s health gleaned from this self-reported data, in addition to smart phone driven data, eligibility and claims data, Social Determinants of Health (SDOH) data and medical record data is then analyzed – with each user’s permission – to recommend personalized resources, tools, information and programs that meet each person where they are in their health journey. Our personalized health profile empowers members to store and access all their health data in one place. Sharecare then provides the resources they

need to manage their health through lifestyle management or disease management coaching.

With RealAge as our baseline, we then motivate users to engage in their health every day through our trackers, the 13 critical health factors that impact an individual’s RealAge the most. Each factor is rated daily on a five-point color scale from green to red, making it easy for individuals to understand their real-time health status. When 8 of the 13 trackers are green on a calendar day, users earn a green day, resulting in a lower RealAge over time. This game-like modality creates an incentive currency that can be linked to almost any health motivation for participants in the platform.

The new Fitness Incentive program counts all your physical activity towards your reward – not just your gym workout. Just meet the required level of activity and earn a marketplace reward, like a gift card or premium merchandise. It’s that simple.

Earning your reward

Track the designated average number of steps for at least 21-days of the month. Prefer yoga or swimming? That counts too. Just track the number of minutes you’re active and convert it into steps using the Step Conversion Chart. Even shoveling snow counts! There are a variety of ways that a member can meet the step requirement, but there are also reasonable alternatives available for members that have a doctor attested medical condition for which they cannot meet the primary step requirements. Members also have the ability to track their steps for up to 7 days prior. The fitness reward program that eliminates the barrier of visiting a gym

Your employees will now experience a whole new way to make their fitness routine pay off.

Fitness Incentive works within the Sharecare platform to take advantage of the step tracker built into the health app on their personal smartphones and/ or link to a member’s fitness device. This encourages those who don’t – or can’t – use the gym to participate and earn rewards for activities that fit within their lifestyle.

In addition to providing a more flexible experience, it: • Provides a configurable solution to meet your

health and wellbeing objectives • Works in tandem with Blue365® fitness center discount • Can be offered to Associate members as an

additional benefit

EASY IMPLEMENTATION

Setting up Fitness Incentive is easy!

Employers can customize the reward amount and choose the reimbursement method. Options include a reward card, health saving account contribution or online shopping experiences. Configurable pieces need to be finalized and provided to Sharecare no later than 60-days prior to launch.

Your BCBSMN Account Manager will work with you to get your Fitness Incentive program configured.

Key points and best practices to keep in mind for a successful launch of the Sharecare platform.

• Keep goals in mind: what are the objectives for the rewards program? Increased awareness, behavior change, etc. How will success will be measured? This will help determine which program would best align to the goals.

• Consider past wellness initiatives: Is this the first incentive program introducing participants to rewards or the Sharecare platform? Have participants been offered programs in the past? If it is a new initiative, keep it simple and start with the RealAge Test. Use your past program data to determine if past programs were successful? Did they result in behavior change, outcomes, desired engagement/completion? If not, why not? What type of feedback did employees have? Take this into account and design a program that fits your unique needs.

• Resources: What level of internal resources are available to support the program? Is it an added task for HR/Benefits or is there a wellness committee or even a dedicated “wellness” employee?

• Incentive: Is it motivating enough to get people to complete the requirements? If incentive budget is small, keep the requirements simple. Consider timeframe and strategy for incentive payouts. Design must allow for activity runout prior to incentive payout.

• Program design and duration: is there enough time for participants to accomplish everything? Programs with a challenge should be ran for a minimum of 3 months, programs with biometric health screenings or claims activities should have a visibility period of at least 1 month.

• React and Iterate: We will work with you to provide aggregate counts throughout your active program to monitor participant enrollment and based on

performance, we will evaluate, react and iterate. 9
