[CLIENT] has recently partnered with Wellworks For You, a corporate wellness management company, to provide a new wellness program for the 2017 benefit year. All benefit-eligible employees will have opportunities to participate in various wellness activities that can reward you with an incentive at the end of the benefit year. Your new program details are outlined throughout this brochure.

Overview Complete the requirements below by their designated deadlines (outlined on page 3) to avoid losing the $30 Wellness Credit in 2018.

STEP 1 – Register on the Wellworks For You Wellness Portal STEP 2 – Complete the Know Your Number Health Assessment STEP 3 – Complete the Annual Preventative Screening STEP 4 – Complete Health Education Coaching Session(s)

Wellness Portal Beginning this year, all participants will be accessing the new Wellworks For You Wellness Portal for the wellness program. Your personal Wellness Portal contains all the tools you need to get started on your path to wellness. Once you register, you will have access to weekly recipes and grocery lists, a fitness and nutrition dashboard, access to all program forms, ability to track your incentive progress, and more! In order for your participation in the program to be tracked, employees are required to have a Wellworks For You Wellness Portal account.

Register on the Wellness Portal 1. Go to www.wellworksforyoulogin.com 2. Click the link to create an account as a New Member 3. Enter your Company ID: XXXXX 4. Create a Username and Password 5. Accept the terms of the Consent Form 6. Enter the required Personal Information

Steps to Earn an Incentive

STEP 1: Register on the Wellness Portal You must register on the Wellworks For You Wellness Portal prior to completing any other activities this year. Follow the instructions on page 2 for registration information.

STEP 2: Know Your Number Assessment Deadline: September 1, 2018 Upon logging into the Wellness Portal, you will be prompted to complete the Know Your Number Assessment. Complete all questions, except for the Health Metrics section. Wellworks will upload your screening results once you complete the Annual Preventative Screening (see step 3 below).

If you see that your metrics are entered, complete all questions and click Finish to submit your responses.

If you see that your metrics are not yet entered, complete all other questions and click Save. The assessment will be completed once your metrics are entered by Wellworks For You.

Once your assessment is completed in its entirety (questionnaire and health metrics), your results report will be generated and uploaded to the Wellness Toolbox under the Health Records section.

STEP 3: Annual Preventative Screening Deadline: September 1, 2018 You must complete at least one (1) of the options below to earn credit for this requirement.

Complete the Onsite Biometric event in February (details to be announced separately)

Visit a LabCorp facility. Print the LabCorp voucher located on the Wellness Portal under My Profile>Wellness Toolbox and follow the instructions to complete the screening.

Complete an Annual Physical Exam with your Primary Care Physician (PCP). Print out the Physician Results Form located on the Wellness Portal under My Profile>Wellness Toolbox and take it with you to your visit. Your physician must fill in all of the required screening results. Submit the completed form to Wellworks For You.

STEP 4: Complete Health Education Coaching Session(s) Deadline: September 1, 2018 Once your Know Your Number assessment is completed in its entirety and a results report is generated, you must complete a certain amount of Health Education Coaching Sessions based on your Know Your Number Risk Tracker Score. You can view your 2017 Know Your Number reports in the Wellness Toolbox under Health Records. Your KYN Risk Tracker Score is located in the Health Risk Summary report. All calls must be completed by September 1, 2018 to receive credit for this requirement. Please Note: All calls must take place at least one week apart. Keep this in mind when scheduling your sessions.

To schedule a coaching session, follow the instructions below: 1. Login to your Wellness Portal and click Coach’s Corner on the menu bar 2. Click the My Coach/Schedule an Appointment tab 3. Choose a Health Coach and click Schedule an Appointment 4. Choose your Time Zone and click on any available appointment time that is convenient with your schedule 5. Fill in the required information and click Confirm Appointment 6. Your Health Coach will reach out to you on the date and time of your appointment

Incentives You must complete the requirements below by their designated deadlines to continue receiving your $30 Wellness Credit. IMPORTANT! If you do not meet the deadlines outlined below, you will NOT receive the $30.00 Wellness Credit for the remainder of the wellness year.

View and Download Documents for Completion

All required forms are located on the Wellness Portal under My Profile>Wellness Toolbox. Participants can download and print all PDF forms for completion.

View Your Participation Wondering what you have completed?

Please allow at least 2 weeks for participation to be updated in the Wellness Portal after completing an activity. If there are any questions about your participation or completion, please contact [NAME] at [EMAIL].

Device/App Technology Users can sync various devices and smartphone applications to the Wellness Portal and track the data in the fitness and nutrition dashboard, WellBalance. Devices and apps include Fitbit, Apple watch, Garmin, Jawbone, Apple Health, Google Fit, MyFitnessPal, and more! Fitbit users can sync their device under Devices/Apps>Fitbit. To view a full list of all additional compatible devices, login to your Wellness Portal and select Devices/Apps>Device/App Connect.

iPhone/Android App The Wellworks For You Portal App includes all of your favorite features from the Portal including Weekly Recipes and Grocery Lists, Pedometer Tracking, and more! Simply search for “Wellworks For You” in the Play Store or App Store.

[Name of wellness program] is a voluntary wellness program available to all employees. The program is administered according to federal rules permitting employer-sponsored wellness programs that seek to improve employee health or prevent disease, including the Americans with Disabilities Act of 1990, the Genetic Information Nondiscrimination Act of 2008, and the Health Insurance Portability and Accountability Act, as applicable, among others. If you choose to participate in the wellness program you will be asked to complete a voluntary health risk assessment or "HRA" that asks a series of questions about your health-related activities and behaviors and whether you have or had certain medical conditions (e.g., cancer, diabetes, or heart disease). You will also be asked to complete a biometric screening, which will include a blood test for [be specific about the conditions for which blood will be tested.] You are not required to complete the HRA or to participate in the blood test or other medical examinations. However, employees who choose to participate in the wellness program will receive an incentive of [indicate the incentive] for [specify criteria]. Although you are not required to complete the HRA or participate in the biometric screening, only employees who do so will receive [the incentive]. Additional incentives of up to [indicate the additional incentives] may be available for employees who participate in certain health-related activities [specify activities, if any] or achieve certain health outcomes [specify particular health outcomes to be achieved, if any]. If you are unable to participate in any of the health-related activities or achieve any of the health outcomes required to earn an incentive, you may be entitled to a reasonable accommodation or an alternative standard. You may request a reasonable accommodation or an alternative standard by contacting Wellworks For You at 800-425-4657. The information from your HRA and the results from your biometric screening will be used to provide you with information to help you understand your current health and potential risks, and may also be used to offer you services through the wellness program, such as [Health Coaching, online Learning Series, and Wellness Challenges]. You also are encouraged to share your results or concerns with your own doctor.

PROTECTIONS FROM DISCLOSURE OF MEDICAL INFORMATION

We are required by law to maintain the privacy and security of your personally identifiable health information. Although the wellness program and [name of employer] may use aggregate information it collects to design a program based on identified health risks in the workplace, Wellworks For You will never disclose any of your personal information either publicly or to the employer, except as necessary to respond to a request from you for a reasonable accommodation needed to participate in the wellness program, or as expressly permitted by law. Medical information that personally identifies you that is provided in connection with the wellness program will not be provided to your supervisors or managers and may never be used to make decisions regarding your employment. Your health information will not be sold, exchanged, transferred, or otherwise disclosed except to the extent permitted by law to carry out specific activities related to the wellness program, and you will not be asked or required to waive the confidentiality of your health information as a condition of participating in the wellness program or receiving an incentive. Anyone who receives your information for purposes of providing you services as part of the wellness program will abide by the same confidentiality requirements. The only individual(s) who will receive your personally identifiable health information are the Wellworks For You team in order to provide you with services under the wellness program. In addition, all medical information obtained through the wellness program will be maintained separate from your personnel records, information stored electronically will be encrypted, and no information you provide as part of the wellness program will be used in making any employment decision. [Specify any other or additional confidentiality protections if applicable.] Appropriate precautions will be taken to avoid any data breach, and in the event a data breach occurs involving information you provide in connection with the wellness program, we will notify you immediately. You may not be discriminated against in employment because of the medical information you provide as part of participating in the wellness program, nor may you be subjected to retaliation if you choose not to participate.
