PURPOsEThe Special Recreation Association of Central Lake County was formed in 1976 as a cooperative effort of Member Agencies with the purpose to provide year round community based recreation services to individuals with disabilities and their families. Member Agencies provide the primary source of operational revenue through a property tax levy available to park districts and municipalities who have entered into a cooperative agreement, or through a general fund.

sRaCLC Board of Directors MeetingsThe public is invited to attend SRACLC Board of Directors meet-ings. Meetings are held the third Wednesday of each month, unless otherwise posted, at 9:00 a.m. at the Libertyville Sports Complex, 1950 N. Hwy. 45, Libertyville. You may call the SRACLC Office if you would like to receive a meeting schedule.

Parent advisory Meeting Parent Advisory Meetings are held biannually with a purpose to voice the interests and needs of those we serve. SRACLC staff will host the meetings and then report to the SRACLC Director. Please watch your emails for meeting invitations or contact Cassie Wodrich at [email protected] with questions.

ADA ComplianceThe Special Recreation Association of Central Lake County intends to comply with the Americans with Disabilities Act (ADA) which prohibits discrimination in the provision and inclusion of programs and services to individuals with disabilities. As stated below, some programs, because of the inherent nature of the activity, are suggested for specific disability groups. If you or any person wishes to participate and is not among the suggested groups, please call SRACLC. Every reasonable effort will be made to include the individual in the program. If you have any questions regarding the ADA or if you feel you have been discriminated against in the inclusion of programs, or the provision of services, please contact John Buckner at (847)816-4866. A procedure is available for you to register your complaint and for its prompt resolution.

Atlanto-Axial InstabilityIndividuals with Down Syndrome are at risk of having a condition known as Atlanto-Axial Instability (AAI). It is an SRACLC policy that any individual with Down Syndrome is required to have a doctor’s written note on file stating the person is free of AAI if that person wishes to participate in any program with a potential to cause undue stress upon the neck. SRACLC will also be offering other programs that are not by the inherent nature of the activity at risk for undue pressure upon the neck. However, the nature of play is active participation. Please take this into consideration when enrolling your child into a program and inform staff if the condition is present.

Dietary RestrictionsWhile SRACLC understands certain participants have food allergies and/or dietary restrictions, we cannot accommodate all needs. Please refer to the program description and make the necessary arrangements for these needs unless otherwise noted in the program write up.

Freedom of Information Act Request methods public may request information and public recordsRequests for information should be directed to the Freedom of Information Act (FOIA), Officer(s) of the Association. All FOIA Officer(s) can be reached at the SRACLC Office, 290 Oakwood Rd., Vernon Hills, IL 60061, Phone (847)816-4866. Requests can be submitted in writing or via e-mail to the address listed above. A request form is also available for your convenience at the SRACLC Office.

For black and white, letter or legal sized copies, the first fifty pages are free and additional pages will cost $0.15 a page. For color copies or abnormal size copies, SRACLC can charge the actual cost of copying. The Association may require the requester to disclose whether the request is for a commercial purpose or to decide whether to grant a fee waiver.

Behavior ExpectationsSRACLC encourages and promotes the concept of fun for everyone. However, certain rules have been established to ensure the safety and enjoyment of all people involved. SRACLC participants are expected to demonstrate appropriate behavior during programs. The Association’s basic behavior code insists that participants shall:• Show respect to each other and staff.• Abstain from using foul or abusive language.• Refrain from inflicting bodily harm to any individual.• Demonstrate respect to equipment and facilities.• Have control of bowel and bladder (diapers and catheters are acceptable).

A caring, positive approach will be utilized, including Handle with Care techniques will be used for any disciplinary methods. Additional or individual behavior management plans may be developed on the advice of parents, guardians, SRACLC staff, or other professionals. SRACLC reserves the right to dismiss a participant if the above noted procedures are not adhered to, and/or, parent/guardian response to a problem situation is not attempted. Each situation will be evaluated individually and on its own merit.

Annual Information FormSRACLC requires all participants to complete a yearly Annual Information Form (AIF). The form provides staff with valuable information which allows SRACLC to conduct its activities in the safest possible manner and to meet the basic needs of participants. If you are new to SRACLC or if the agency receives a registration form and does not have an AIF on file, one will be sent to you to complete and return. If at any time the form needs to be updated (change in medication, behavior, etc.) please call the SRACLC office. All information is kept strictly confidential and enhances successful participation. GENERAL INFORMATIONFinancial aid FundThe commitment of SRACLC and the member agencies is to provide all individuals considered “residents” the opportunity for participation in SRACLC programs. A Financial Aid Fund has been developed to assist individuals who may be experiencing financial difficulty. All personal information required for financial aid procedures is kept strictly confidential. Per season, a maximum of five (5) programs, plus any Special Olympics training programs and Day Camp, are eligible for assistance. Financial aid must be requested in writing each program season. individuals seeking financial assistance should contact John buckner for more information regarding the process and deferred payment options.

Deferred PaymentsDeferred payments are an option only if a financial aid form is filled out and payments are completed by the end of each given season. If these conditions are not met, individuals will not be eligible to register for any programs until the balance is paid in full, and future deferred payments will not be allowed. If you have any questions regarding this policy, please call John Buckner, Director.

inclusionSRACLC understands that not every person with a special need must participate in Special Recreation. SRACLC staff can help with questions regarding any individual’s participation in member agency programs. SRACLC Member Agencies are committed to the provision of recreational services to all residents. If you are interested in participating in programs provided by your local park district or recreation department, please follow their registration procedures/timelines and contact them to state your requests. Always be thorough when completing registration materials and state any special assistance that may be needed. It is important to inform the park district or recreation department staff what your goals and expectations are for your child in the program. By informing the staff as soon as possible regarding special needs, SRACLC staff are better able to provide the necessary assistance in a timely manner. types of assistance that sraclc can provide are:• In-service training for staff, including activity adaptations, behavior management techniques, and disability awareness.• Recruitment of additional staff, if needed.• Consultations or observations at programs.

Residency informationresident: Individuals residing within the legal boundaries of a SRACLC Member Agency (Park Districts of Grayslake, Mundelein, Vernon Hills and Villages of Hawthorn Woods, Lake Zurich, Libertyville, and Lincolnshire) are considered “residents” of the Association and pay the in-district fee. Residents receive priority when registering for all programs through the registration deadline. non-resident: Any individual whose primary residence is outside of a Member Agency’s legal boundary is considered a non-resident.

Non-Resident PoliciesThe Special Recreation Association of Central Lake County has approved and implemented non-resident guidelines and fees for its programs. The fees are listed for all programs in the brochure. All individuals residing outside of the legal boundaries of current SRACLC member agencies will be subject to these guidelines.1. SRACLC will no longer seek funding from townships to off-set non-resident fees for residents of unincorporated areas. Townships that have previously supported the Association have been notified.Non-Residents must:2. For all programs considered “Special Events” pay the indicated fee which has a minimum 25% add on charge to the resident fee.3. All other areas including “Adult Day”, “Day Camp”, “Special Olympics”, and “Weekly” will be subject to:a. Payment of the yearly non-resident administrative fee which will allow full resident access to all programs for one year from the date of the payment. The 2019/20 administrative fee is $2,344.00. Non-residents who paid the administrative fee in 2018/19 will be grandfathered in at $2,018.00. OR b. Pay the indicated fee which has up to a 100% add on charge to the resident fee.4. SRACLC will continue to accept families who own property or a business that pays real estate taxes to a member agency community as residents. Proof of ownership will be required.5. Individuals who were considered as non-residents and benefited from Township funding through SRACLC before April 30, 2011 will not be subject to non-resident fees, except for the 25% charge for “Special Events”. These individuals will be “grandfathered” into programs under the same policies/procedures governing their participation during that time period.

Photography GuidelinesSRACLC occasionally takes photographs or video of participants for promoting/advertising our programs, services, events, activities, and facilities in our brochures, website, or agency social media, and other promotional avenues. By registering for, participating in, or attending SRACLC programs, events, or other activities, the participant (or parent/guardian of a minor) irrevocably agrees to the use and distribution by SRACLC of his or her image (or of his/her minor child/ward) in photographs, video recordings, and any other electronic reproductions of such programs, events, and activities for any purpose without inspection or approval and without compensation, rights to royalties or any other consideration now and in the future.

Medication DispensingSRACLC staff may dispense medication only if the following procedures have been completed.• Permission for Medication Dispensing form must be completed for every program season and signed by parent/ guardian.• All medication must be delivered directly to the SRACLC office and packaged in individual dosage amounts labeled per dosing time and day. • Dosage, time, and potential side effects are clearly noted. • All of the above information must be delivered to the SRACLC Office at least two days prior to the onset of the program. • DO NOT send any medication information to your child’s school with expectations that it be given to SRACLC.• SRACLC staff may under no circumstances perform an injection or any other medically invasive procedure.• SRACLC staff will not administer medications that are not approved through the FDA.• Please call the office; (847) 816-4866; if you have any questions.

Early Drop Off/Late Pick Up Policy• One (1) no-fee written reminder, per season, will be given. Reminders will be documented through a parent or guardian’s signature on the Early Arrival/Late Pick-Up Form.• Five (5) minutes of time will be allotted as a “grace period” prior to and following the advertised programs times. • After one (1) documented reminder, a $5.00 per each cumulated five (5) minutes fee will be assessed. The violating party will be invoiced and the bill must be paid in full prior to future registrations. • To maintain cost effective programs, SRACLC must enforce strict program drop-offs and pick-ups to allow our staff adequate time to adhere to pre and post program responsibilities. Additional time spent supervising participants requires additional paid staff time.

Risk ManagementSRACLC is committed to provide its recreation programs and services in the safest possible manner and to keep the safety of all individuals at the forefront. Participants and parents must realize there is some degree of risk which is inherent in all recreation activities. In an effort to minimize exposure to unsafe conditions, SRACLC conducts an ongoing risk management program and strictly enforces that safety procedures be adhered to by all participants and staff. Please be aware that sraclc does not carry medical or accident insurance for injuries sustained during programs or services. The cost of such would make participant fees prohibitive. Each person registering himself/herself or a dependent in a SRACLC program should review and maintain their own insurance coverage. It must be noted that the absence of individual or family medical insurance coverage does not make SRACLC automatically responsible for the payment of medical expenses. The agency providing liability coverage for SRACLC requires the mandatory execution of a Waiver and Release form for all programs. Thank you for your cooperation.

transportation PolicyThe following procedures are to be followed to maintain safe transportation of sraclc staff and participants.• All passengers must be seated with seat belts fastened immediately upon entering the vehicle. Wheelchairs must be securely belted and fastened. The vehicle will not depart until these procedures have been followed.• SRACLC staff are responsible for determining whether or not an individual may be transported safely due to behavior or health concerns.• SRACLC staff will not leave any person unattended after any program unless written permission has been received from said individual’s parent/guardian.• All wheelchairs must be in proper working condition.• Persons riding Amigo-type wheelchairs must transfer to a van seat.• No object (i.e. hands, head, trash) is to protrude or be thrown from the vehicle at any time. Windows will be opened upon permission from SRACLC staff.• SRACLC does not provide door-to-door transportation but will offer drop off and pick up locations for selected programs.• If three (3) or more people from one community will utilize an additional drop off and pick up location, SRACLC will try to accommodate this request.• SRACLC reserves the right to utilize safety harnesses on the vehicles, if deemed necessary by the Risk Management team. • SRACLC reserves the right to refuse or discontinue transportation services. Please realize that these procedures have been implemented to ensure the safety of all participants and staff. Thank you for your cooperation.• SRACLC vehicles will only wait five (5) minutes after the scheduled time before departing to its next location.

We invite all siblings of children with special needs to have the opportunity to meet with one another in a relaxed, fun, and recreational setting. In addition to an action-packed day of games, we will facilitate discussion to express the joys and concerns of having a sibling with special needs. This will be a safe place for kids to share freely any feelings they may have through bibliotherapy, art, and play. What a great chance to make new friends. special Olympics Reminders*A current Application for Participation must be on file at the SRACLC Office to participate in District and State level competitions.

*An athlete’s Application for Participation is valid for two (2) years and must not expire before State Games in his or her particular sport.

*Athletes must attend and actively participate in 50% of all practices prior to tournament dates to be eligible to compete.

*Appropriate sports’ attire must be worn at practice and games. No jeans or jewelry will be allowed.

*If you are in a team sport and you do not participate in the District or ITRS Competitions, you will not be allowed to compete at State Level Competitions unless absence was due to illness and accompanied by a doctor’s note.

*Any individual or team that makes it to the Sectional or State Level Competitions will be asked to pay an additional fee per athlete. There will be a required tournament fee of $40 per night for coaching, transportation, and supervision. There will also be a lodging fee for those who choose to stay overnight with the team. This fee will be dependent on actual lodging costs and number of athletes.

*SRACLC will be enforcing the maximum numbers of players for team sports. This will ensure that all team players will get adequate playing time during the game. Please be advised that if we do not have enough players to create another team after the minimum/maximum per team has been met, we will go by order of registrations received to create teams (i.e. – late registrations will be the first cut).

*If an athlete is unable to commit to tournaments and competitions, team priority will be given to those athletes who can attend.

*SRACLC will not provide coaches for sectional or state competitions in the event that less than four (4) athletes qualify and attend.

Refund Policies and Procedures• Unless requested or confirmed by the SRACLC office, refunds

will be held as a credit. All credits not requested for immediate reimbursement will be paid out by April 30 of each year. Requested refunds will be processed on the third Wednesday of each month.

• SRACLC reserves the right to cancel a program due to insufficient enrollment. (See minimum and maximum enrollment for each program.)a) If the Association finds it necessary to cancel a program

before the second meeting, a full refund will be given.b) If a program is cancelled by the Association after the

second meeting, a prorated refund will be given on the basis of meetings not held.

• Allrequestsforrefundsmustbesubmittedinwriting.• Medical Reasons: A full or prorated refund will be granted if a

medical condition prohibits participation. • all program refund requests are subject to a minimum

$5.00 per program. day camp and active adult are subject to a $50.00 service charge. The service charge is waived if the refund request is made prior to a given program registration deadline. Any service charge will be applied to an unpaid balance.

• No refund will be given for any special event or program activity that requires a pre-paid purchase by SRACLC unless an alternate can be placed. There can be no exceptions.

• No refund will be given after the registration deadline unless the program is deemed inappropriate for the participant. Please see above for exceptions.

• SRACLC realizes that inappropriate behavior or conduct may occur. However, if the behavior persists and is deemed hazardous to any individual, the SRACLC Director reserves the right to suspend participants with a prorated refund given.

• Refunds will be made available upon request, if you are not able to attend a program due to the observation of a religious holiday.

Registration Process• Full payment is required at the time of registration unless

specific arrangements have been made with the SRACLC Director (i.e. Financial Aid).

• Registrations will not be processed if a fee balance re-mains from a previous season.

The Special Recreation Association of Central Lake County continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for this program/activity must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs.You are solely responsible for determining if you or your minor child/ward are physically fit and/or adequately skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or has recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity.WARNING OF RISKRecreational activities are intended to challenge and engage the physical, mental and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity. All hazards and dangers cannot be foreseen. Depending on the particular activity, certain risks, dangers and injuries may exist due to inclement weather, slips and falls, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and other risks inherent to the particular activity. In this regard, it is impossible for the Special Recreation Association of Central Lake County to guarantee absolute safety.WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISKPlease read this form carefully and be aware that in signing up and participating in this program/activity, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a result of participating in any and all activities connected with and associated with this program/activity (including transportation services and vehicle operations, when provided).I recognize and acknowledge that there are certain risks of physical injury to participants in this program/activity, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation.
