Introduction
Objectif de l’étude
• Évaluer les facteurs de risque de thromboses cérébrales sous L-asparaginase dans les LAL
Présentation de l’étude
• Étude rétrospective
• 424 LAL entre Janvier 2000 et Oct 2009 : service d’Hématologie Clinique de l’Hôpital Aziza Othmana
• 14 thromboses cérébrales : 4%
Signes cliniques au diagnostic
• Crises convulsives généralisées: 9 cas
• Céphalées : 3 cas
• Déficit moteur : 2 cas

âge > 10 ans ( n: 13): 93 % < 10 ans (n: 1): 7%
sexe M ( n: 10): 71 %
F ( n: 4): 29 %

Use of supportive coagulation therapy and clinical outcome: the CAPELAL study
Traitement
• 13 patients ont reçu un traitement anticoagulant en association avec un traitement anticonvulsivant:
• Héparine non fractionnée puis AVK : 8
• Héparine non fractionnée puis HBPM: 2
• HBPM puis relais AVK : 2
• HBPM seul : 1
• 1 patiente a reçu un traitement anticonvulsivant seul : thrombose de l’artère cérébrale moyenne
• La durée moyenne du traitement : 6 mois ( 2 à 9 mois )
• Reperméabilisation vasculaire à l’imagerie de contrôle
• Pas de récidive de la thrombose
• Aucun décès en rapport avec la thrombose
• La L-asparaginase non reprise
Activité anti Xa : 0,5 – 1 UI/ ml
Activité anti Xa : 0,1 – 0,3 UI/ ml
HBPM sera poursuivi 48h après la derniére injection de L-asparaginase E Coli et 2 semamines après l’injection de
asparaginase pegylée
Conclusion • Asparaginase + prednisone ± prédisposition génétique
risque majeur de Thrombose
• Monitoring AT
• Supplémentation en AT si le taux est inférieur < 60% risque de thrombose
meilleure administration de L-aspa et par conséquent une meilleure survie
• La L-Aspa peut être reprise 1 à 3 mois après l’accident thrombotique sous couverture HBPM ± AT
