Dear ESMD Friends,

Changes in the ESMD board bring new spirit and possibilities! Dr. Filip Bonte (B) joined the board. Being experienced in minimally invasive dentistry for many years in his private

office, and being a microscope user since the Vilnius ESMD congress, his enthusiasm and his skills in website management are clearly visible on www.esmd.info. Dr. Eric Meisberger (Nl), being the youngest board member, no doubt helps many more young colleagues find the way to our events and to ESMD membership. He graduated from Mainz State University Germany and works in the University Medical Center Groningen (Nl) as well as in a private endo practice. Our ESMD 2012 Congress committee had its second site visit to Berlin in January, to discover some fascinating options. The Deutsche Gesellschaft für Mikroskopische Zahnheilkunde has become ESMD’s associate partner in the organizing of the 2012 congress. Dr. Nikolas Ganss, DGmikro President, Dr. Johannes Neuschulz, DGmikro Vice-President and Dr. Tomas Lang, DGmikro scientific advisory board, joined the ESMD congress committee. The cooperation works extremely well. The Berlin congress can soon be announced officially with a preliminary program that will inspire and, as we all hope, attract many participants. Besides the micro surgical, micro restorative and

endodontic topics, ESMD2012Berlin ‘Open Eyes, Open Mind’ will emphasize also on the ergonomical aspects of the work and on office management. Teamwork in the dental office as a program topic, brought together on one day, will cause participating dentists to bring their assistants along for a day of training, learning and motivation.A visit to Berlin University Dental School Charité and to the Pfaff Institute has lead to an interesting cooperation for the logistics of the congress. The warm welcome and genuine interest that we experienced there were heartwarming. Many of our partners have already contacted the congress secretariat spontaneously for information on the sponsorship and exhibition conditions. Their enthusiasm for our ESMD meetings encourages us. But much work still has to be done. The Scientific program being under construction, the practical venue negotiations going on and the social program to be decided on, be sure that board and congress committee are e-mailing during a significant part of their time. We look forward to building the big event, ESMD2012 has to be and especially we look forward to your registration on line, be it as an ESMD member with all your privileges, be it as an ESMD member-to-be.

Question: Dentists don’t often acknowledge the loyalty of their staff. A simple thank you or an occasional flower or a few dollars would really make employees feel wanted, needed, and appreciated. How do we change this behavior? Answer: I think it would be fair to say that this is probably one of the biggest frustrations staff experience when working for a dentist. I would like to compliment you on asking how you can change the behavior. The answer is twofold. First, give the doctor the very thing you would enjoy: compliments. Whether handling a difficult patient or delivering a perfectly matched crown, compliments are appreciated by all. Dentists often feel unappreciated by their staff. It’s rather like the chicken and the egg. Someone has to make the first move. Dentists, as a group, are perfectionists. That’s what makes them such good dentists. But perfectionists are very hard on themselves and on the people around them. The dental school environment didn’t expose them to much positive feedback, so many dentists have had little exposure to it. I’m sure that with some open

communication with your doctor, you can both work to become a better team.

Question: Have dentists’ wives become more common and influential in their husbands’ practices? Please address the negative situations that might result. I just left a practice and opportunity to grow in skills due to the tremendous influence of a dentist’s wife. Answer: Having been Practice Administrator for my ex-husband’s practice for 18 years, and my late husband, Dr. David Tom, full time for 6 years, plus I have presented a full day program and a tape series on this subject since 1994, I feel I am somewhat qualified to answer. According to the American Dental Alliance Association (the Association to which dental spouses belong), 62% of all dental practices in this country have the spouse involved in some capacity. Consulting since 1994, and presently with seven consultants in our company, I can categorically express that for 95% of the time the spouse’s involvement in the practice can be one of the most positive aspects of management. There is no denying that for a small minority of occasions (I put it at 5%) the spouse’s involvement does bring more negative than positive forces to bear. It would appear that potentially, you might have experienced one of the 5% situations. You mentioned that there was a “tremendous influence of a dentist’s wife” but failed to mention that this was a positive or negative influence. Often we see the spouse, the wife in your case, often being the stronger force in working with the staff, hence it is not unusual to see an employee feeling this is a negative

influence, whereas in fact, the practice benefits from a strong hand at the helm if the dentist involved has chosen to take a very “hands off” approach to management and staff relations.

Question: One of the employees in our office makes up excuses so the doctor will give her the day off, but she tells co-workers the truth. Should we tell the doctor that she is lying? We don’t want to be tattletales. Answer: While I understand that you do not want to be tattletales, I fear your situation must be causing major morale problems. Some things seem obvious to me. Apparently, the practice doesn’t have clearly defined policies as to when staff members are entitled to a day off. Unfortunately, your co-worker is taking advantage of a kind dentist and lack of policy. This same staff member appears to have no guilt about leaving the practice understaffed while enjoying a day off achieved through dishonest means. I do not believe that discussing the situation with the dentist makes you tattletales. I see it as taking a necessary step to protect the quality of the team and the future of the practice.

Question: The dentist I work with has a habit of sighing when a procedure is complicated or slightly challenging. He sighs loudly while working on patients. How do you feel this problem should be addressed? I’m a new employee. Answer: Dentists have joked about a rule they learn in school: Never say “whoops” in front of a patient. it tends to scare the patient. Sighing definitely falls in the same category as the word

“whoops.” I suggest that the dentist doesn’t realize that he or she is doing it. Yes, it is totally acceptable to address this problem with the dentist. Clinical staff members offer another pair of eyes and ears at the chair side. The professional dentist will appreciate this fact and welcome the feedback. Good luck.

Question: In our practice, we only have staff meetings when there is a problem. As a result, our meetings tend to become gripe sessions. What can we do to make meetings more positive and effective? Answer: Practices, which only schedule a staff meeting when a problem needs resolving, will find they are „managing by crisis. In order to reduce stress and problems, and increase communication and team morale, structured staff meetings need to be scheduled on a consistent basis. We educate our patients to become part of the practice’s continuing care (recall) program. Their participation allows the dentist and hygienist to diagnose and correct small problems inexpensively and conservatively before they increase in size and cost. I feel we need to practice what we preach in our own management style. Procedure personnel or communication problems, however small, when left unaddressed, develop into larger problems. Holding regular monthly meetings allows for fine tuning of the practice rather than waiting until radical surgery is needed.

Question: I work for a wonderful dentist, and the staff works exceptionally well together. Unfortunately, I’m getting paid about

$3 an hour less than most hygienists in my area. I love my job. I doubt I’ll ever find another like it, but the pay is almost insulting for my education and experience. How can I solve this problem? Answer: From your letter, it appears that most of your employee needs have been met. A “wonderful dentist” and a staff that “works exceptionally well together” result in you loving your job. Nationally, employees rate the financial remuneration of their jobs (on a scale of one to 10) at five or six. Everyone in the practice is important. A well thought out incentive and bonus plan will allow you to earn the financial rewards you specifically believe your education and experience deserve, while rewarding all the team players for their extra contribution to meet the needs of the practice and patients.

Question: Our dentist has been going through a divorce for the last 1½ years. He doesn’t have any interest in working. Much of the time, he isn’t here physically or mentally. We want to say something but how? We want to work. Answer: This is a difficult situation. You are quite right in thinking one cannot let the situation continue to drift. If you are conscious of Dr’s “Lack of Interest” in the practice, patients also will feel this, with the potential of the practice’s eventual decline. I feel you could take one of two approaches, one direct and one indirect. The indirect approach would involve asking someone else to talk to your doctor about his need to refocus on his practice; i.e. your local dental society, a dentist in town, the attorney, or a personal friend. The direct approach would be to have a staff member request a meeting with the doctor and share the team’s concerns with him.

“Very interesting presentations and great enthusiasm at DGmikro Symposium 2011”

The utility and versatility of the dental microscope were the theme at this year’s symposium of the German Society for microscopic dentistry in Cologne.

In the great atmosphere of the art design hotel “New Yorker” the microscope enthusiasts met, but also many new users and colleagues who seek training and exchanging opinions. The German Society for Microscopic Dentistry propagates since 2009 the use of the dental microscope in all areas of dentistry and more and more dentists admit that strong magnification and a sharp increase in direct light represent a significant quality improvement, not only in endodontics. Thus, the high quality and often compelling presentations of the speakers presented for so many participating dentists a real eye opener.

After a welcome by Dr. Nikolas Ganss, president of DGmikro, and Dr. John Neuschulz, Vice President, the symposium was opened by Professor Baumann. He clearly emphasized his joy

that it always shows that the “seed falls on fertile ground”, if students get into contact with the dental microscope, already during their dental education. The prelude to the speakers was brought by Dr. Tomas Lang (Essen), with an inspirational speech about the completely new dental opportunities created by the dental microscope. Dr. Lang emphasized that the consistent application of the microscope from diagnosis to therapy transforms minimally invasive dentistry to a micro-invasive dentistry. In impressive video sequences cavities where shown at times with less than one millimeter access opening. Besides this idea of micro-invasive preparation techniques and treatment protocols, he also looked even beyond the confines of dentistry. Much applause was given a lecture

illustrated how the CBCT allows the practitioner to assess situations and treatment options like never before. The symbiosis of both techniques, dental microscope and digital volume tomography (cone beam ct scan) enables new levels of diagnosis and therapy for the benefit of patients. Two lectures were dedicated to endodontic topics. Dr. Vadim Konoval (Aachen) showed that obliterations may be treated without problems under the dental microscope in most cases, by experienced users. Interesting here was, among other things, his techniques for differentiation of Pulpstones and denticles from regular structures such as the endodontic pulpchamber floor to avoid perforations.

Bernd Schneider (Frechen) presented its protocol of the microscope-based apical surgery with retrograde filling. With a variety of case examples, he illustrated, what high quality level of care, even in the mandibular posterior area is now achievable.

by Dr. Jürgen Benz (Bielefeld), who demonstrated a very practical use of the microscope as part of the initial examination of patients. Here it became clear that the microscope can help the patient to see his own mouth through the eyes of the practitioner. Diagnoses and treatment needs are therefore directly perceived and understood by the patient.

The aspect of ergonomics has been taken up by Dr. Richard Hilger (Düsseldorf). Here the different concepts of assistance during treatment under the surgical microscope were shown and evaluated. It was clear that the consistent use of the microscope as a tool not only increases the quality of care but also the quality of life of the practitioner, as back problems are easier to avoid.

Scientifically thoroughly researched was the juxtaposition of the Cone beam CT scan (CBCT) to the microscope by Dr. Marc Semper (Cologne). In his typical style and technically very adept, he

In addition to the scientific presenta-tions, participants had the rare oppor-tunity, in the well-attended exhibition, to try out dental microscopes from six different manufacturers. The possibil-ity of direct comparison was used by many interested parties.

The large and positive feedback from this first Symposium of DGmikro,

included also the participation by the president of the European Society of Microscopic Dentistry (ESMD), Dr. Philippe Van Audenhove. This led to consultation and an enthusiastic partnership of ESMD and DGmikro board for the next congress ESMD2012 Berlin ‘Open Eyes, Open Mind’ on 4-5-6 October 2012.

On 21-22 October 2011 the 4th annual congress of the ProMikro (Czech Association of Microdentistry) took place in one of the most beautiful towns in Europe – Prague. The two days event was focused on using operating microscope and micro – techniques

in conservative dentistry and oral surgery. International but nevertheless Czech and Slovak – it could be best characterised. One speaker from the Czech Republic, one from Slovakia, one from Switzerland (but born in Slovakia), one from Germany (but born in the Czech republic). Thanks to Carl Zeiss company we had the exciting opportunity to see the lecture of dr. med. dent. Peter Velvart. It was a pleasure for me to share experiences from using microsurgery techniques in the field of implantology. Dr. Daniel Jurga showed us a few posibilities to enhance conservative dentistry using the operating microscope. During the

excellent workshop we had the amazing opportunity to share the experiences of dr. Tomas Lang. The congress was completed by a beautiful social

program in the Prague old city. The success of the congress was supported by the unexpected but really cheering guest - president of ESMD, dr. Philippe Van Audenhove. Thanks to him ProMikro will be closer to ESMD and to other national microdentistry organizations.

The Congress Ambassador Program is designed as a partnership between dental professionals and specialists from different countries with the aim of raising the importance of microscope dentistry locally and internationally. ESMD Congress Ambassadors play a key role in sharing knowledge and skills of microscope dentistry through active communication. Every attending country is represented by one or more Congress Ambassadors. The position of Ambassador entitles to represent ESMD 2012 and spread the congress information to potential delegates.

The support and contribution to the organization and promotion of ESMD 2012 is greatly appreciated. Conclusions: Practical organization of the lectures by ESMD is considered excellent to good but the lectures on Friday did only meet the expectations for half of the audience! Obviously many participants were expecting more practical information on the microscope itself. This also reflects in the evaluation of the content. We will catch up with that in Berlin. Our speakers are nevertheless evaluated as good to excellent.

Conclusions: Practical organization of the hands-on courses by ESMD is appreciated with an excellent to good. This is the main work, effort and stress in organizing such event. So we are very happy with this! The Saturday event overall seems to have exceeded the expectations of the majority of the participants. Content and speakers are considered excellent by the majority of participants.

ESMD thanks all participants for the feedback. We will keep in mind the remarks and requests, to serve our members and participants even better in the future!

In a spirit of transparency, we decided to publicize the evaluation of our past autumn school in Leuven. This meeting was organized to give first time users or would be users the basics on microscopes and implementation of microscope use in a practice environment in a two day meeting, consisting of lectures and hands on lessons. There were about 50 attendees. Although the Berlin Congress is of another stature and has other aims, we can learn from this evaluation and try to do better where needed.
