Required Documents

• All required documents must be turned in by 4:30 on last day of Academy session. No exceptions.

•Roster / Attendance (include students’ id’s)

•Lesson Plans •Parent Contact Log•Green Time Sheet – signed by Administrator

•Hand deliver your own documents.

• Student / teacher ratio – minimum of 10 students, no more than 18.

• Classes with less than 10 students will have to be collapsed.
