Several adhesion barriers have been developed in the form of solution or membrane in an attempt to solve the postsurgical problems. However both types of tissue barriers have some limitations in their practical applications. In order to overcome these problems, a tem- perature-sensitive Poloxamer/alginate/CaCl was prepared as an adhesion barrier. The sol-gel transition behavior was measured using a viscometer. And in vitro gel stability test was performed. The anti-adhesion effect was evaluated using a thyroidectomy model. Two week after surgery, the level of adhesion was graded from zero to two using a whole- number system. And also toxicity, inflammation and the adhesion prevention effect were evaluated. The LCST (lower critical solution temperature) of the Poloxamer/Alginate/CaCl was 25 C. The gel stability of Poloxamer was improved by adding cross-linked sodium Alginate/CaCl . The adhesion grade was significantly lower in the experimental group than in the control. From the results, Poloxamer/alginate/CaCl is an excellent candidate material having highly effective anti-adhesion, low inflammatory response, and non-toxicity for clinical applications as a tissue adhesion barrier gel. Key words: Adhesion prevention, Poloxamer, Alginate, Animal model, Thyroidectomy 서 론 착이란 서로 떨어져 있어야 할 장기나 조직들이 달라붙 는 현상이다. 이러한 유착 발생의 원인으로는 외상, 허 혈, 이물질 등이 있으며, 가장 큰 원인이 되는 것 중 하나는 수술이다. 수술 후 유착은 수술이라는 근본적인 침습적 특성으 로 인해 실제로 모든 수술 후에 발생할 수 있으며, 심각한 임 상적 후유증을 초래할 수 있다. 몇 가지 외과적 수술, 즉 골 반 수술 ( 자궁, 난관, 난소 수술), 결장수술 ( 대장 및 소장), 맹 장절제술, 담낭절제술 등이 특히 유착의 형성 경향이 큰 것으 로 알려져 있다. 복부와 골반수술 후 유착발생율은 55-94% 범위에 이르고 이런 수술을 받은 모든 환자 중 평균 80-85% 에서 유착이 형성된다. 1-5) 이러한 유착은 장폐색증 6) , 만성통증, 성기능 장애, 불임 등의 원인이 되며, 비 수술적 치료로 증상 의 호전이 없으면 수술적 치료로 전환을 적극적으로 고려해야 하며 7) 2 차 수술 시 의사와 환자에게 많은 어려움을 준다. 2 차 갑상선 절제술은 간헐적 수술 처치로서 이환율 증가와 관련되 며, 흉터에 의해 발생하는 정상조직면의 손실로 인한 반회후두 신경(recurrent laryngeal nerve) 손상과 부갑상선 기능저하증과 관련된다. 8) 2 차 갑상선 절제술의 주요 적응증은 갑상선 조직 의 제거 후 암의 발병, 갑상선 암의 재발, 단발성 혹은 다발 성 결정성 갑상선종 그리고 갑상선 중독증 등이다. 9) 유착을 방 지하기 위해서 흉쇄유돌근의 안쪽 가장자리를 통한 역행 접근 이 추천된다. 10) 두 손상된 조직 표면이 직접 닿지 않도록 사이에 처치해 주 는 제품을 유착방지제라고 하지만 종종 만족스럽지 못한 결과 를 낳기도 한다. 이는 제품 자체의 유착방지 효과 때문이기도 하지만, 제품의 처치가 용이하지 않은 이유도 있으므로 좀 더 효과적이고 잘 분해되며 적용이 쉬운 제품이 필요하다. 현재까 지 물리적 장벽 역할을 하는 유착방지제로는 용액 혹은 막 * 책임연락저자: [email protected]유

This study was conducted to compare the effectiveness of Poloxamer/alginate/CaCl2 for reducing adhesion after thy-roidectomy in a rat model. Tissue adhesion is one of the major causes of well known postsurgical complications,including patient's pain, functional obstruction and difficult preoperative surgery. Several adhesion barriers have beendeveloped in the form of solution or membrane in an attempt to solve the postsurgical problems. However both typesof tissue barriers have some limitations in their practical applications. In order to overcome these problems, a tem-perature-sensitive Poloxamer/alginate/CaCl2 was prepared as an adhesion barrier. The sol-gel transition behavior wasmeasured using a viscometer. And in vitro gel stability test was performed. The anti-adhesion effect was evaluated usinga thyroidectomy model. Two week after surgery, the level of adhesion was graded from zero to two using a whole-number system. And also toxicity, inflammation and the adhesion prevention effect were evaluated. The LCST (lowercritical solution temperature) of the Poloxamer/Alginate/CaCl2 was 25oC. The gel stability of Poloxamer was improvedby adding cross-linked sodium Alginate/CaCl2. The adhesion grade was significantly lower in the experimental groupthan in the control. From the results, Poloxamer/alginate/CaCl2 is an excellent candidate material having highly effectiveanti-adhesion, low inflammatory response, and non-toxicity for clinical applications as a tissue adhesion barrier gel.
Gentamycin (Daesung Microorganism Institue, Korea)을 수
술 후 주사하였다. 마취가 되면 목 부위의 털을 제거한 뒤,
에탄올과 포비돈 용액으로 소독해 주었다. 사지를 결박하고, 마
취된 쥐의 목 부위를 약 2cm 가량 중심선과 수직 방향으로
절개한 후, 악하선과 근육이 손상되지 않도록 조심하면서 오
른쪽 갑상선을 노출시킨 뒤, 혈관겸자를 이용해 혈관을 묶어
두고 갑상선의 대부분을 절제 한 다음, 혈관을 결찰해 주었다
. 이때 과다 출혈이 발생할 경우 흡인기와 전기소작기를 이용
하여 지혈하였다. 이후 대조군에는 아무것도 처치하지 않고 봉
합하였으며, 실험군에는 근육과 갑상선 조직 사이에
Poloxamer/alginate/CaCl2 유착방지제를 넣어 물리적 장벽 역할
을 해 주도록 하였다 (Figure. 1). 수술이 끝난 동물은 2주간
사육 후 희생시켜 유착정도를 측정하였으며, 각 군에서 각각 5
마리씩 간, 비장, 신장 등을 적출하여 H&E 염색을 시행한 후

조직학적 소견을 관찰하였다. 유착정도는 0에서 2까지 분류하
였으며, 0 = 유착이 없는 경우, 1 = 유착이 얇게 형성되어
무딘 도구로도 쉽게 분리되는 경우, 2 = 유착이 두껍고 강하
게 형성되어 쉽게 분리되지 않고 분리하기 위해서는 예리한
도구가 필요한 경우로 분류하였다.11) 각 군에서 유착의 정도
를 앞에서의 기준에 따라 구분하여 평가하였으며 평가 시에는
평가자가 실험군을 알 수 없도록 맹검을 실시하여 객관성을 높
일 수 있도록 하였다. 유착정도의 측정값은 평균값과 이의 표
준편차(Mean±Standard Deviation)로 표시하였다. 측정값에 대
한 통계처리는 Student T-test를 이용하였고, p-value는 0.05
이하를 유의성 있는 것으로 보았다.
결과 및 고찰
Poloxamer/alginate/CaCl2 유착방지제의 제조
생체 적합성 고분자인 Poloxamer에 sodium alginate와
CaCl2를 가교시켜 안정성을 가지며 주사주입 가능한 유착방지
제를 제조하였다.
현재까지 물리적 장벽 역할을 하는 유착방지제로는 용액 혹은
막(membrane) 형태로 제조되어 임상에서 쓰이고 있다. 용액형
유착 방지제는 수용성 생체 고분자 용액을 재료로 하여 제조되
고 있으며, dextran,12) heparin,13) carboxy methylcellulose
(CMC),14) hyaluronic acid(HA),15) chondroitin sulphate16) 등
포, 장간막 세포(mesenterical cells), 섬유모세포(fibroblasts) 등
다양한 세포의 증식과 이동을 포함한다. 또한 세포외 기질의
합성과 침착 등도 포함된다. 위의 유착기전에 근거하여 유착을

방지하는 방법으로 몇 가지 방법이 제안되었다. 그 하나는 수
술시 손상을 최소화하는 것이다. 두 번째로는 스테로이드 및
소염제를 투여하여 염증을 감소시키는 방법이 있다. 그리고 세
번째로는 tPA(tissue plasminogen activator) 활성을 증가시키는
방법이 있으며, 마지막으로 최근에는 상처 면을 주변조직과 분
리시키기 위한 물리적 장벽을 사용하는 것이다. 장벽용으로 사
용하는 경우 조직의 상처가 치유되는 동안 물리적인 장벽으로
작용하며, 인접한 조직 사이에 유착이 형성되는 것을 막아주고
일정기간의 상처 치유가 끝난 후에는 분해 혹은 흡수되어 제
거되어야 한다. Poloxamer/alginate/CaCl2 유착방지제가 창상
조직을 precoating 하고 물리적 장벽으로써 작용하여 유착형성
을 방지한 것으로 판단된다. 따라서 Poloxamer/alginate/CaCl2유착방지제는 유착방지 효과에 있어 유효하다고 판단할 수 있
다. 또한 이 유착방지제는 체내에 투여하기 전에는 졸 상태로
되어 있어 쉽게 주사주입을 할 수 있었으며, 체내에 투여 된
후 젤이 되는 확인할 수 있었다.
수술 부위에 대한 심각한 염증이나 이상반응은 관찰되지 않
았으며, 주요 장기에 대한 안전성 및 독성검사를 위해서 각 군
마다 간, 비장, 콩팥의 조직학적 관찰을 하였으나, 대조군과 마
찬가지로 실험군에서 염증세포의 침윤이나 세포 또는 조직 괴
사 등의 이상반응이 없었다(Figure 5). 염증도 유착 형성에 중

요한 원인이 되고 있다. 자궁유착의 경우 자궁내 상처부위에
염증이 일어나면 자궁내막이 얇아지거나 없어지면서 서로 붙게
된다. 본 연구에서는 수술부위에 대한 육안 관찰 결과 대조군
과 실험군의 염증 정도의 차이는 없었으며, 유착 정도는 실험
군이 낮게 나왔다. 결과적으로 Poloxamer/alginate/CaCl2 유착
방지제는 각각의 기관에 대해 손상을 줄 수 있는 독성의 여부
는 없는 것으로 확인 되었으며, 염증이나 이상반응이 없어 유
착방지제로서 더욱 적합하리라 생각된다.
결 론
본 연구에서는 25oC 부근에서 졸-젤 전이되는 Poloxamer/
alginate/CaCl2 유착방지 젤을 제조하였으며, 이 젤에 약하게
가교된 alginate를 첨가함으로서 젤의 안정성이 향상됨을 확인
하였다. 동물실험 결과 갑상선절제술 후 유착방지 효과에 있어
유효한 결과를 확인할 수 있었으며, 주요 장기들의 조직 염색
결과 안전성에 문제가 없음을 확인하였다. 따라서 제조된
Poloxamer/alginate/CaCl2 젤이 유착방지용 장벽으로서 우수한
후보 물질임을 알 수 있었다.
