1. Introduction

These procedures have been designed for use on quarries and sand plants. This includes Lorry Owner Driver’s (LOD), contractors, visitors, service providers and Boral employees’.

1.1 Emergency Definition

An emergency may be described as an abnormal, dangerous or life threatening situation requiring timely action to protect people, property and the environment from harm.

1.2 Aim

The aim of these procedures is to provide a system and resources to ensure that all personnel are capable of responding correctly and safely to any emergency situation.

1.3 Objective

To enable sites to manage an emergency and minimise the risk of injury and/or damage to site personnel, members of the public, plant and the environment.

1.4 Instructions to Employees, Contractors and Tradespeople

All employees, contractors and tradespeople who are employed / engaged on the site must be given adequate instructions in the actions they must take in the event of an emergency.

This instruction must cover the following:

The method of raising the alarm and warning others when confronted by an emergency. The action to be taken by each employee/contractor when he/she hears the alarm. The need to search and account for persons when evacuating premises. The pathways of escape from the premises and the location of the Evacuation Assembly

Areas. The position, type and method of operation of the fire fighting equipment installed. The ways to deal with specific emergencies likely to occur at the site.

All visitors to the site are to be accompanied at all times in operational areas on site by suitable site personnel and must be made aware of restricted work areas and the emergency assembly point(s). Refer to the “Visitor Induction Form” OHS 5.2.I.F / ACM_EMS_IND02. Where a visitor cannot be accompanied at all times in operational areas then they must be considered a contractor and inducted accordingly.

2. Emergencies on site The emergency procedures outlined are designed to cover the types of emergencies listed below, where a full or partial evacuation may be required.

These procedures cover the immediate actions and control actions for each emergency listed.

Moving persons from immediate danger to safety is always the first action (if safe to do so).

Control actions from then on must ensure all persons are safe and accounted for.

Automatic and immediate actions depend on clear communications occurring amongst the control staff, from the alarm signal, hand held radios and internal phone system.

2.1 Fire, Smoke or Explosion Personnel should only attempt to fight small fires, which can be safely put out with extinguishers or fire hoses.

All other fire fighting must be left to the Fire Service.

In particular, no attempt should be made to fight LPG fires, for which the automatic shut down system provides the primary response.

Do not collect personal effects following instructions to evacuate.

2.2 Urgent Medical Emergency Treat the person as quickly as possible. The first person at the scene may not be trained in first aid, and every effort must be made to summon qualified personnel. Attempts to move a victim should be left to ambulance/first aid personnel unless the victim is in danger. Ambulance should be called through a direct “000” call Person Discovering Notify Area Warden/Chief Warden – get assistance

If trained and qualified in fist aid techniques initiate first aid / CPR If not first aid trained stay with patient until the site first aider /

medical assistance arrives

2.3 Vehicle / Mobile Plant Rollover Cease work immediately in the area. Person Discovering Move all personnel away from the area

Check condition of person and follow “Urgent Medical Emergency” procedures outlined above in section 2.2 (if safe to do so)

Shut down any machinery that may be running (if safe to do so) Advise Area Warden/Chief Warden

2.6.1 Bomb Threats – Telephones

Do not use mobile phones, radios, pagers or any other radio transmitting device.

Use line telephones to communicate.

accents, etc. that may give a clue to the age, sex and location of the caller

2.6.2 Suspect Mail

Do not use mobile phones, radios, pagers or any other radio transmitting device. Use line telephones to communicate. Suspect Letter and Parcel Recognition Points: All persons handling mail should be conscious of the following suspicious features:

Foreign mail, air mail, special delivery Restrictive marking – confidential, personal Excessive postage Handwritten – poorly typed addresses Unusual shape or odour Misspelling of common words Oily stains or discolouration Excessive weight Rigid, lopsided or uneven envelope Protruding wires/tinfoil Excessive securing materials such as masking tape/string.

IF SUSPECT MAIL ITEM IS RECEIVED: DO NOT TOUCH IT! DO NOT MOVE IT! DO NOT COVER IT! DO NOT DISTURB IT!

2.6.3 Armed Intrusion – Hold Up

Notify any suspicious persons to your Area Warden/Chief Warden NO AMOUNT OF MONEY OR INFORMATION IS WORTH A LIFE Person Confronted Remain calm

Make no sudden movements or take any action that may irritate or excite intruder(s)

Be courteous and answer all questions Obey all instructions Hand over cash and any personal valuables on request Evaluate intruder(s): Take note of appearance and other characteristics, mannerisms,

2.7 Cyclone (Refer to Site Cyclone/Tsunami Plan, if one developed for site). 2.8 Drowning Personal Floatation Device (PFD) must be worn by ALL rescue personnel. Ambulance should be called through a direct “000” call Person Discovering Raise Alarm - Notify Area Warden/Chief Warden – get assistance

Throw a buoyant object / rope to the person Launch the rescue boat (if safe to do so) If trained and qualified in fist aid techniques initiate CPR If not trained stay with victim until medical assistance arrives

2.9 Flooding Avoid entering flood waters. Stay away from drains, culverts and water over “30cm deep”. Act under instructions of person-in-charge / emergency services Person Discovering Move personnel away from the area

2.10 Bush Fires Act under instructions of person-in-charge / emergency services

2.11 Misfire The appointed shotfirer should notify the Senior Site Executive (SSE) immediately and telephone for the company, explosive supplier and explosive inspectorate for assistance whenever there is a misfire. (Refer to the “Quarry Blast Management Procedures”, section 4.9 “Dealing with Misfires”)

2.13 Major Spill Onsite ( Includes distillate, oil or chemical spillage) Personnel operating distillate, oil and chemical dispensing facilities must be trained in the safe handling of these materials, and must understand the importance of immediately containing a spill, to reduce any fire threat and to prevent contamination of the stormwater system. Distillate and oil/chemical absorbent materials and appropriate personal protective equipment (PPE) must be located near and be readily accessible to all dispensing and storage locations. Appropriate signage should be erected to identify the positioning of these absorbent materials. Person Discovering Shut down dispenser – prevent further release of spill

Site Emergency Response Plan - Gold Coast Quarry Site OHSQ:12.1 ER:F6 2.14 Offsite Light Vehicle/Mobile Plant Emergency Includes light vehicle and mobile plant rollover, fire in a vehicle, damage estimated at more than $1000, incident where an employee is injured.

2.15 Vehicle or Plant Contact with Overhead or Underground Power

lines When it is suspected that the vehicle has come into contact with an overhead or underground power line, ALWAYS ASSUME IT IS LIVE. The driver and any other occupants should remain inside the vehicle until assistance arrives. If it is essential to leave the cabin because of fire or some other life threatening situation, then it is essential the following is done to avoid electrocution:

Jump clear of the vehicle. Avoid touching the vehicle and the ground at the same time Try to land on both feet simultaneously Avoid touching the ground with your hands Shuffle slowly away for a distance of at least 10 meters from the vehicle Wait for assistance to arrive.

and not to touch any part of the vehicle Notify Supervisor/Manager immediately

Supervisor Request ambulance assistance if needed (Call “000” direct) and

other relevant services i.e. Energex Advise Site Manager

Person Discovering Stop and make the scene safe, if you are able, Contact emergency services through direct call “000”. Move personnel away from the area. Keep passers by clear and

wait for fire brigade to attend Check the condition of the person, but do not attempt to move

them. It should be left to the ambulance / first aid personnel unless the person is in immediate danger and only if safe to do so

Shut down any machinery that may be running Advise Supervisor or Manager For tankers, stop the flow of product, disconnect hoses and close

all valves, hatches and caps, again only if safe to do so Follow the directions of the emergency services once in attendance Never admit or suggest liability to anyone outside the company Obtain as much information as possible at the scene

Supervisor Confirm all persons have been removed to safety Advise Site Manager / Production Manager immediately

3. Responsibilities

Key Personnel The key personnel in the emergency control organisation are the Chief Warden (e.g. Manager/ Supervisor / Batcher in Charge (BIC)), Communications Officer (Weighbridge Operator / Receptionist, Loader Operator), Area Wardens / Wardens (plant and mobile plant operators, truck drivers and Wardens (any other personnel available) (see Notes below). The Emergency Executive is the Site / Plant Manager and, depending on the size of the site, possibly other senior managers. Other specialist support staff such as qualified First Aiders and Site Access Controller should also be included. The responsibilities of the emergency personnel are to: -

Chief Warden Take control of the emergency situation and active evacuation procedures if required. Ensure any injured personnel receive immediate treatment as needed and all other personnel are accounted for and safe at assembly area.

Emergency Executive Assist Chief Warden / Area Warden(s) where possible and to fill in as the Chief Warden/ Area Warden(s) in their absence.

Area Warden (s) / Warden (s) Assist Chief Warden where possible and to fill in as the Chief Warden in their absence.

Communication Officer Communicate information to all site personnel and emergency services as required.

Site Access Controller (s) To restrict access to site and only allow emergency services or approved Boral staff access.

First Caller – person reporting emergency

Provide accurate information about the emergency and any first aid or medical injuries.

Call Receiver – person receiving the report

Relay information to emergency services and Boral staff as accurately as possible, to enable correct response.

First Aiders Provide first aid if safe to do so to any injured persons.

Emergency Response Team members Ensure site is secured safely until emergency situation is rectified.

6. Emergency communications Emergency information must be passed quickly so people can respond to the situation as it develops. The emergency communications set out advise all site personnel of relevant alarm signals to notify of an emergency and the various channels people can use to report an emergency and emergency response personnel can use to talk to one another.

7. Evacuation procedure The emergency evacuation advises all site personnel of relevant procedure to evacuate the site in the event of an emergency.

Evacuation routes and Assembly Areas for this site are shown on the site plan – see Pg 24 of this document.

If possible avoid crossing a road, and move by the safest route to reach an Evacuation Assembly Area, or site Emergency Exit Point.

Alarm Wait for the evacuation alarm to be sounded Signal for evacuation is 4 long repeating blasts on the aerosol horn, car/truck horn or loud hailer.

Preparation

Prepare to evacuate. Remain calm and do not return for personal belongings.

Order to evacuate

All personnel evacuate to emergency assembly areas in a safe controlled fashion do not run or rush if no emanate danger exists.

Use tracks and roads to assembly area or emergency exit point (refer Pg 24 of this document).

Assembly areas All personnel to assemble at the allocated assembly points for site Block access to site entrance, only allowing emergency services and approved Boral staff to enter the site.

The designated Assembly Area is at the site entrance / exit point at Old Coach Rd. UBD Map Ref. 59 G19. In the event of bush-fire – there are three (3) alternative exit points – please refer to Pg 24 of this document.

People with special needs Assist anyone that requires assistance to the assembly areas.

Accounting for everyone

The Chief Warden must take the visitors book to the assembly area and account for all staff and visitors. It is the responsibility of the Chief Warden to ensure all personnel are accounted for and to return to the site if safe to do so to locate any unaccounted for personnel.

All clear Receive all clear from Emergency Services (if attending) or Chief Warden/Emergency Executive (if no Emergency Services attending) before allowing entry back into the site

Returning to work The Chief Warden/ Emergency Executive will confirm when it is safe to return to work.

8. Lock down procedure There may be an external situation which requires the lock-down of one or more building.

Lockdown is designed to protect occupants from an external threat by excluding or isolating the threat (e.g. intruder with a gun). The Chief Warden/Emergency Executive will assess the risk and scope of the required lock-down.

External threats which require lockdown procedures to be activated are deemed remote for operations but in the event of a threat, call “000” Emergency Services immediately (only if safe to do so).

The directions of the Chief Warden and/or Emergency Executive should be followed until arrival of the Emergency Services.

Alarm If an external threat occurs, the Chief Warden and/or Emergency Executive will advise you either via the UHF radio or in person of the situation and what is required (if safe to do so).

Securing the work area

Ensure doors and windows in your area are not chocked open Secure your own area of work and wait for further instruction

Taking shelter

Do not place yourself or others at risk

Stay out of sight and wait for further instructions

Communicating

Details of the threat should be announced over the UHF radio or in person (only if safe to do so)

9. Carrying out drills Selection and training of site personnel in their specialised roles is to be provided, followed by provision of general training to all personnel on what immediate actions are required of them in the case of an emergency. Most commonly, they will need to know how to raise the alarm, the position of fire fighting and spill equipment, and the location of evacuation assembly areas. The effectiveness of these procedures must be evaluated with simulation exercises, carried out under the direction of the Chief Warden. These training drills should occur at intervals not exceeding 6 months and records must be kept of each training event. The evacuation checklist must be completed (see section 14.1). Following each training event the Chief Warden must hold a debriefing session with the emergency control staff, to critically evaluate the effectiveness of these procedures and, where necessary, recommend changes. New employees must be given instructions in these emergency procedures as soon as practical after commencement, usually as part of their induction programme and normally on the first day of employment on the site. The intent is that this instruction should be immediate.

10. Debriefing and counselling When an emergency is over a debriefing must be held with all those involved to find out how the emergency happened and to see what remains to be done to resume operations. An “Emergency Evacuation Checklist OHSQ.12.1 ER (pictured below) should be completed after every emergency (refer to section 14.1). The debriefing should also identify the need for staff counselling and make sure the families of injured personnel have been notified.
