Warum ist der therapeutische Erfolg variabel?Resistenzmechanismen:
1) Pharmakokinetisch:Zytostatikum kommt nicht hin z.B. ZNS ⇒ Relaps der ALL aus Zellen jenseits der Blut-Hirnschrankeoder Enzym der Biotransformation (CYP) zur Aktivierungfehlt

2) Zellkinetisch:Tumoren, in denen der überwiegende Teil der Zellennicht aktiv proliferiert ⇒ resistent
je größer ein solider Tumor, desto geringer Anteil der aktivproliferierenden Zellen ⇒ schlechtere Ansprechrate (⇒ daherFrüherkennung von Tumoren wichtig!!)rasch wachsende Tumoren = empfindlicher:z.B. Verdoppelungszeiten der Tumormasse:ca. 1-2 d: Chorionkarzinom, kindl. ALLca. 3-6 d: Hodgkin-Lymphom, Hodenkrebsca: 70-100d: Colon-, Bronchialkarzinom

Slamon et al. (2001) Use of chemotherapy plus a mono-clonal antibody against HER2 for metastatic breast cancer that overexpresses HER2. N Engl J Med 344:783-792
The HER2 gene, which encodes the growth factor receptor HER2, is ampli-fied and HER2 is overexpressed in 25 to 30 % of breast cancers, increasing the aggressiveness of the tumor. METHODS: We evaluated the efficacy and safety of trastuzumab, a recom-binant monoclonal antibody against HER2, in women with metastatic breast cancer that overexpressed HER2…. We randomly assigned 234 patients to receive standard chemotherapy alone and 235 patients to receive standard chemotherapy plus trastuzumab. RESULTS: The addition of trastuzumab to chemotherapy was associated with a longer time to disease progression (median, 7.4 vs. 4.6 months; p<0.001), a higher rate of objective response (50 % vs. 32 %, p<0.001), a longer duration of response (median, 9.1 vs. 6.1 months; P<0.001), a lower rate of death at 1 year (22 % vs. 33 %, P=0.008), longer survival (median survival, 25.1 vs. 20.3 months; p=0.01), and a 20 % reduction in the risk of death. The most important adverse event was cardiac dysfunction of New York Heart Association class III or IV.

A. Levitzki (1997)The Future of Signal Transduction Therapy:
• "The complexity and heterogeneity of cancers ... suggest that it may be necessary to develop a large number of drugs against a large number of target proteins.
• Furthermore, it is likely that for each tumour, a unique cocktail of signal interceptors will have to be used.
• It is likely that once signal interceptors enter the clinic they will do so in combination with cytotoxic drugs in current use.
• Progress in this direction is inevitably slow ... but steady..."
