USG dan CT scan pada Pielonefritis

(A) Renal Doppler ultrasonography (DUS) shows mild swelling and a wedge-shaped hypoechoic focus (arrow) of the right kidney related to acute pyelonephritis. (B) The color flow DUS image demonstrates diminished flow through the involved area (arrow).

USGUltrasound is insensitive to the changes of acute pyelonephritis, with most patients having 'normal' scan, and abnormalities only identified in ~25% of cases 1. Possible features include:particulate matter in the collecting systemreduced areas of of cortical vascularity by usingpower Dopplergas bubbles (emphysematous pyelonephritis)abnormal echogenicity of the renal parenchyma 1focal/segmental hypoechoic regionsmass like changeUltrasound is however useful in assessing for local complications such as hydronephrosis, renal abscess formation, renal infarction, perinephric collections, and thus guiding management.CT Scan

Contrast enhanced CT scan in patient with fever and flank pain shows ill defined linear enhancement abnoramlity in the left mid pole arteriorly (arrows) known as "striated nephrogram" which is a finding seen in acute pyelonephritis. Importantly, no complications of pyelonephritis are noted (e.g., no renal abscess).
