¿Qué es el cough assist?

El cough assist T70 es un dispositivo que aplica presión mecánica para provocar tos y eliminar las flemas (secreciones) de los pulmones. La máquina aplica presión positiva gradual en las vías aéreas que rápidamente cambia a presión negativa. La rápida caída en la presión hace que salga el aire de los pulmones, imitando a la tos natural.

¿Cómo se conecta el dispositivo a la cánula de traqueostomía?

El cough assist se puede conectar directamente a la cánula de traqueostomía o tubo endotraqueal (trach) con un adaptador en forma de L (figura A) o al catéter de aspiración (succión) en línea (figura B).

Aspiración superior

La aspiración (o succión) en línea permite eliminar fácilmente las secreciones de la cánula. Cuando use la sonda de aspiración en línea con el cough assist, solo aspire la parte superior de la cánula. Usando el cough assist, solo aspire durante la exhalación para que la máquina provea toda respiración hacia los pulmones. Ver foto abajo.

Cómo agregar oxígeno al cough assist

El doctor puede agregar oxígeno al cough assist. Verifique las instrucciones para calibrar el flujo de oxígeno durante el tratamiento. El adaptador de oxígeno se ubica entre el filtro y el tubo (ver abajo).

Sólo con cánula de traqueostomía: Antes de realizar el tratamiento, sóplele oxígeno con la bolsa de reanimación ambú o aumente el oxígeno en el collarín humidificador.

Con respirador: Aumente el nivel de oxígeno antes del tratamiento con cough assist.

Cómo usar cough assist

Preparación: 1. Reúna los suministros para la aspiración.

2. Trate de elevar la cabecera o que el paciente se siente lo más erguido posible.

3. Si tiene una cánula de traqueostomía con globo (manguito), ínflelo durante el tratamiento con el cough assist.

Inicio del tratamiento 1. Encienda el cough assist y verifique la configuración y las presiones. Abra el oxígeno hacia el cough assist. Siga las instrucciones del respirador (R) o traqueotomía (T) a continuación.

Con respirador: desconecte el circuito del respirador desde el conector L del tubo de aspiración en línea en plena inspiración (cuando los pulmones están llenos de aire), póngalo a un lado, pero donde lo alcance fácilmente. Conecte el tubo del cough assist al conector L de la línea de aspiración e inmediatamente inicie el tratamiento presionando el botón “Therapy”. El cough assist seguirá automáticamente el ciclo de inhalación (positivo, entra aire) y exhalación (negativo, sale aire). Al final de la expiración, la máquina se pausará antes de la siguiente respiración. El doctor puede ordenar unas gotas de solución salina en la cánula para desprender las flemas. En ese caso, eche 3 o 4 gotas de solución salina directamente en la cánula si es necesario.

2. Aspiración

Aspiración simple: si requiere aspiración durante el tratamiento, desconecte el tubo del cough assist al final de la exhalación y aspire si es necesario. Deje que recupere el aliento y reinicie el tratamiento si es necesario. Si necesita una sonda simple de aspiración, es posible que le indiquen que use la bolsa de reanimación ambú y oxígeno entre los ciclos de cough assist.

Aspiración en línea: aspire sólo en la parte superior de la cánula si ve secreciones durante el tratamiento con cough assist durante el proceso de exhalación.

3. Después de 5 ciclos de inhalación y exhalación con cough assist: • Ponga el cough assist en standby (espera). • Desconecte el tubo de cough assist de la cánula de traqueostomía.

Solo con cánula de traqueostomía: Aspire la cánula de traqueostomía y espere a que la respiración vuelva a la normalidad. Conecte el humidificador de aire cálido en el cuello o al dispositivo que usa para humidificación.

Con respirador: Conecte el respirador. Aspire la cánula de traqueostomía con la sonda en línea y espere que la respiración vuelva a la normalidad.

4. Repita los pasos anteriores 3 a 5 veces más, según sea necesario para eliminar las secreciones. Que no quede conectado al cough assist por más tiempo del indicado.

5. Después de terminar el tratamiento: • Cierre el oxígeno hacia el cough assist. • Ponga el oxígeno en configuración normal en el respirador o en el

humidificador. • Tire los suministros de aspiración usados. Apague el cough assist.

What is a cough assist device?

The Cough Assist T70 device removes mucus (secretions) from your child’s lungs through the use of mechanically applied pressure. The machine applies a positive pressure to the airway, then rapidly shifts to a negative pressure. The rapid drop in pressure forces air from the lungs, imitating a natural cough.

How do you connect the device to the trach?

The cough assist can be attached directly to the tracheostomy tube with a trach elbow adapter (see A below) or to an inline suction catheter (see B below).

How to do a top suction

Using an inline suction allows for easy removal of secretions from the tracheostomy tube. When using an inline suction catheter, only suction at the top of the trach during the cough assist treatment. Only apply suction during cough assist treatment during the exhalation process to ensure the full breath is pushed into the lungs by the machine. See photo below.

How to add oxygen to the cough assist

Your child’s doctor may order oxygen to be added during cough assist. Check the instructions for how high to set the oxygen liter flow during the treatment. The oxygen adapter is added between the filter and the tubing (see below).

For child with trach only: Before doing treatment, give blow-by O2 with resuscitator bag or increase the oxygen on the mist collar.

For child on a ventilator: Increase the oxygen setting before the cough assist treatment as directed.

Steps to using cough assist

Get your child ready: 1. Gather suctioning supplies. 2. Place your child in an upright sitting position as tolerated. 3. If your child has a cuffed tracheostomy tube, inflate the cuff during the

cough assist treatment.

Start the treatment: 1. Turn on the cough assist and check the settings and pressures. Turn on the oxygen to the cough assist device. Follow instructions for vent (V) or trach (T) below.

For child on a ventilator: Disconnect the ventilator circuit from the elbow of the inline suction at full inspiration (when lungs are full of air) and set aside in easy reach. Connect the cough assist tubing to the to the elbow of the inline suction and immediately start the treatment by pressing Therapy. The cough assist device will cycle automatically from inhalation (positive, pushing air in) to exhalation (negative, sucking air out). At the end of exhalation, the machine will pause before the next breath. Sometimes the doctor will order a few drops of saline in trach to help move mucus. If so, put 3 to 4 drops of normal saline directly into the trach when necessary.

2. Suction

Simple suction: If your child needs to be suctioned during the treatment, disconnect the cough assist tubing at the end of exhalation and suction as needed. Allow your child time to catch their breath and then resume the treatment if needed. If your child requires a simple catheter, you may be told to use the resuscitation bag with O2 between cough assist cycles.

Inline suction: Suction just at the top of the tracheostomy tube if you see secretions during the cough assist treatment during the exhalation phase.

3. After 5 in/out breath cycles on the cough assist: • Switch the cough assist device to standby. • Disconnect the cough assist tubing from the trach.

For child with trach only: Suction the trach and allow your child to resume normal breathing. Place your child back on the heated mist collar or current humidification device.

For child on a ventilator: Place your child back on the ventilator. Suction the trach with the inline catheter and allow child to resume normal breathing.

4. Repeat the steps above 3 to 5 more times as needed to clear secretions. Do not keep your child connected to the cough assist device longer than the prescribed amount of time.

5. After complete treatment: • Turn off the O2 source to the cough assist. • Return the oxygen setting back to the normal setting on the ventilator or

mist collar. • Discard used suction supplies. Turn off the cough assist.

Information about cough assist settings

Settings The Cough Assist T70 has the option to add up to 3 different settings for your child’s needs. This will be on your home orders. To change the setting, press settings on the front and select Preset 1, 2 or 3.

Cough Trak Cough Trak – If your child can easily trigger breaths on the cough assist, the Cough Trak setting will synchronize the breaths from the cough assist to your child as they take a breath.
