Thermon: 5 Years Injury Free Skyline: Scaffold builders @ B-2700 completed a scaffolding wrap around T-611

without any injuries or incidents. Brand: Alex Bazaldua intervened with a person working without gloves & without

proper eye protection. Coached the employee and gave employee proper PPE to perform the job safely.

Brand: Done an outstanding job on D20 & D25 T/A and performed quality craftsmanship and excellent safety standards.

STS: Site Oxy TA crew @ OC-5 completed the TA without incidents or injuries. Brand: Scaffold Builder & Painters @ Chlorine 4 performed a job in a sensitive

area without any incidents & completed the job safely. Brand: While building a scaffold around the HRSG @ Power 9, Willie Rivas

inquired if the catwalk was stable enough to support the weight of the scaffolding. The question was escalated & it was determined that the catwalk was not built to support the weight of the scaffolding.

Brand: Jack Bowling was recognized for a job well done for handling the paint jobs @ A-3600 PMDI. He done a good job communicating & had a great attitude while performing the job safely.

Testronics: William Whitten escalated a safety concern about a staircase over @ A-800 pipe rack that was a incredible safety risk. Steps were fixed.

Brand: Frank Carrier done an outstanding job in the block painting rails. He performed the job without any incidents and completed the job safely.

STS: B-7200 TA Crew completed the TA working long hours without any injuries or incidents.

HOUSTON BUSINESS ROUNDTABLE Brazos M&E: Received the “Best in Class” Safety Excellence Award from the

Houston Business Roundtable (HBR). Miken: Received the “Best in Class” Safety Excellence Award from the Houston

Business Roundtable (HBR). SPX: Received the “Best in Class” Safety Excellence Award from the Houston

Business Roundtable (HBR). Thermon: Received the “Best in Class” Safety Excellence Award from the

Houston Business Roundtable (HBR).

REMINDERS EXEMPT PERSONNEL: •Safety Contacts, Supervision, certain workers, (depending on their tasks) are NOT signing into the Block, and if they do sign in, (they are NOT waiting for Operations to authorize them entry). •Operations must sign in all exempt personnel. This is done to ensure they are made aware of any Hazards, spills, releases for example. It is a step in Operations communicating to visitors in their Block. This communication is also to verify that the

Exempt work will NOT require a SWP.
