Central Referral System

The Central Referral System is being phased

out and replaced by HMIS.

▪ No new applications will be accepted on the CRS

▪ Information will not be transferred from the

CRS to HMIS

▪ This means people not connected to a housing

path must be assessed and entered into HMIS to

be matched to a housing intervention as capacity

allows

the path people take to access your programs.

It will change how partners collaborate to

successfully move people into housing.

▪ Skilled Assessors will complete standardized housing

assessments on HMIS with applicants

▪ Your project may have Skilled Assessors or this may be a

partnership with other projects including Access Points

◻ Access Points are places people can be referred to if they

have not yet completed an assessment and are listed on

our website under Access:

Assess - HMIS

Applicants only need to be assessed once to connect to a

housing partner

▪ This is visible to you on HMIS if someone in your project

has been assessed by viewing their entry into the

Coordinated Entry System Skilled Assessor project

After a person has been matched to a housing provider, if s/he

falls back into homelessness another assessment will be

required at that time.

Assessment Updates - HMIS

Skilled Assessors:

Updates can be made to the Standardized Housing

Assessment by any Skilled Assessors as information

changes.

Housing Providers:

Any Housing Provider can update contact information or

a change in household size in a newly created

Standardized Housing Assessment - New Contact

Information.

Updated data is shared with the CES matching team.

The Shriver Center has an online Help Hub for resources

and updates specific to Medicaid enrollment.

▪ Unless you circle your name on the sign-in sheet we

will share your e-mail so that you will be invited to

register for this online resource.

More information to follow on how to help people look up

their Managed Care Organization to explore

healthcare options.

Match

The One List

▪ HMIS uses data shared by all providers to create the One List, a By

Name List of all individuals and households currently experiencing

homelessness

▪ This list includes people considered to be active, meaning enrolled in a

homeless project or exited from one within the last 90 days

The Match

▪ Catholic Charities uses this One List to match people to housing

programs based on availability, eligibility, and prioritization

◻ This process includes notifying providers working with the household

via email

◻ Housing Providers will receive an individual and/or household’s

history of program Entries recorded in HMIS

▪ Sharing HMIS entries allows housing partners to verify

homelessness for applicants using your entry and exit data

Importance of accurate data:

▪Your accurate and timely reporting is the foundation of the

Coordinated Entry System

▪ Example: If a person states he has experienced homelessness for

one month in his assessment and has been entered in your

homeless project for 12 months, the housing provider will have

your HMIS entry for homeless verification

Navigate

Shelters and Drop-in Centers

▪Case managers listed on the Standardized Housing Assessment

will be visible to the housing provider for follow up along with

HMIS entries and exits

Outreach Professionals

▪For those connected to an outreach team, the outreach worker

will assist with navigation into housing

Housing System Navigators

▪Starting in August, we will have Housing System Navigators for

families and people facing chronic homelessness

▪Navigators assist with the process of connecting applicants to the

housing they are matched to on HMIS

Documentation

▪Documents can be uploaded by Skilled Assessors in a way

that allows the housing providers to have access to the

documentation to expedite the process of confirming

eligibility

▪Shelters serving the applicant will appear for housing

partners to reach out to regarding documentation
