This directory contains a list of providers that are included in Tower Health’s Level 1 benefit Tier. While great care is taken to make the directory complete and accurate, the listing of participating providers is dynamic and may change from time to time. Because the directories and the information are updated, it is important for you to check them periodically.

It is extremely important for you to verify your provider’s tier status as higher out‐of‐pocket expenses will be incurred for procedures performed at a Level 2 or Out‐of‐Network Hospital or facility. Before receiving services, members should always verify with Capital Blue Cross if the provider, Hospital or facility is participating at Level 1 benefits. The hospital affiliation key denotes the specific Tower Health Hospital(s) where the provider has privileges and can perform procedures, if applicable.

Allied Health professionals, including PA‐Cs and CRNPs, working in the office of a Level 1 physician are included at Level 1 benefits.
