The Healthy Arizona Worksite Award seeks to recognize and award businesses that are making efforts to positively affect the health and well-being of their employees, their families and their community through evidenced-based worksite health initiatives and promising programs. Organizations that meet the Healthy Arizona Worksite Award criteria will be recognized by the Arizona Department of Health Services for their efforts. Award winners will be recognized in statewide communications, at a recognition event, and on the Healthy Arizona Worksite Program website. There are three levels of award recognition based on program maturity and effectiveness: Copper Level: This recognizes a business that has taken the initial steps to receive worksite health training, gain leadership support, begin organizational assessment, create infrastructure supports and develop an introductory worksite health improvement plan. Silver Level: This award level indicates that a businesses’ worksite program has the minimum components needed for a comprehensive worksite health program. This is a program that has senior level support, an active wellness team, a program budget, and has conducted biometric screenings. A Silver award program will also have a basic implementation plan and strategy in place and is beginning to track outcomes. Gold Level: A Gold Level program is a mature worksite health program. These businesses are tracking and documenting outcomes and behavior change. Gold level programs have demonstrated institutional support and have an integrated worksite health program with business policies and benefits. Their programming is data and outcomes driven. These are programs that may have been in existence for five or more years. How can you start the process of applying for the Healthy Arizona Worksite Award? The first step in applying for the Healthy Arizona Worksite Award is to attend a Healthy Arizona Worksites Program (HAWP) training session. The HAWP training has been developed to help organizations learn how to implement evidence-based worksite health initiatives that can improve business and employee health. Additional Data Requirements:• Employee interest data• Health risk appraisal (must have completed at least two)• Biometric screening or collected biometric data (must have completed at least two)

Minimum 50% participation rate in health risk appraisal and biometric screeningMeet the Gold requirements listed in the Supportive Policies and Environments section
