Addendum # 1 The following questions have been submitted by the Move Coordinator RFP respondees, and MassDOT has responded.
Questions from Fort Point Project Management
1. Q. Please identify the additional sites / scope / number of staff / major equipment also relocating to the new Hopkinton site.
A. The Pavement Management Group, a staff of eight, currently located in Concord will be moving to the new Lab. The PM Server equipment has been identified on the drawings. However beyond the specialty vehicles, the garage equipment has not been catalogued. The move coordinator’s work to support this portion of the move will be based on the fee schedule provided by the bidder in RFP item V.B.1.
2. Q. Please identify the vendor and process for the relocation of hazardous materials. B. MassDOT currently has a vendor on state contract to handle hazardous
materials, but the vendor for this work has not been determined at this time; the move coordinator will assist in developing a scope of services for this service.
3. Q. Are key equipment service providers engaged re; specialized equipment relocation and recalibration?
A. The move coordinator will inventory existing equipment, work with key MassDOT personnel to identify existing service contracts and any specific relocation requirements.
4. Q. Please identify the logistical constraints associated with the relocation of the curing concrete cylinders (ie moisture control).
A. The cylinders will need to be kept in a temperature and moisture controlled environment.
5. Q. Pricing Proposal – lists component pricing a – d, currently does not include move execution, post move, etc, or is that intended to be covered in point 1?
A. The fee schedule provided by the bidder in RFP item V.B.1 will be the basis of payment for these activities.

6. Q. Is there a deadline for final questions before the proposal due date (2:00pm, 10/23)?
A. Tuesday October 20, 2015 at noon. 7. Q. Should Scope of Move be amended to include a Statement of Work for
environmental (chemical) movers, and if so, should pricing be separate or should it be included in part 2a of the pricing.
A. Do not include in part 2a, this may be required and if so the fee schedule provided by the bidder in RFP item V.B.1 will be the basis of payment for these activities.
8. Q. Re: “preparation of equipment placement plans”, approximately what percent of significant equipment is already sited on the plans. Because a lot of equipment is shown on the floorplans attached as part of the RFP, it is not clear if the scope is to site or confirm the placement of equipment.
A. Assume 95% of the equipment has been sited on the plans. The scope is to confirm the placement of equipment and identify all necessary support systems (water, power, data, gas, venting) have been identified; at this time all known equipment has been sited.
9. Q. Will all vendors (i.e., movers, riggers, manufacturers’ reps, etc.) work directly for and be paid by the Commonwealth?
A. The mover will contract directly with the Commonwealth; other vendors may be subs to the mover or contract directly with the Commonwealth. It is not anticipated that the Move Coordinator will employ subs.
10. Q. At what additional locations are people and/or equipment located (to be moved to Hopkinton)?
A. The Pavement Management Group will be relocated from Concord to Hopkinton. See the response to Question 1.
11. Q. Will the move coordinator be expected to provide welcome/informational packets for those moving to Hopkinton?
A. The move coordinator may be asked to provide these packets; if so the fee schedule provided by the bidder in RFP item V.B.1 will be the basis of payment for these activities.
12. Q. Will the move coordinator (vs the architects) be responsible for file management – managing the transition/conversion from existing filing cabinets to new (or a mix of existing and new) file cabinets
A. Yes. 13. Q. Is there in-house support for disconnecting equipment (i.e., equipment that is hard-
wired, vented, etc.) or will the move coordinator be responsible for sourcing contractors?
A. Yes there are MassDOT in-house trades and vendors on contract that can assist in disconnecting equipment. The move coordinator will coordinate these activities.
14. Q. Should the price proposal be amended to provide estimate of hours and fee for the move itself (multiple move days)?
A. It is not necessary as the fee schedule provided by the bidder in RFP item V.B.1 will be the basis of payment for these activities.
15. Q. Will the move coordinator be responsible for any aspect of decommissioning the D Street labs?
A. Yes the move coordinator may play a role, the fee schedule provided by the bidder in RFP item V.B.1 will be the basis of payment for these activities.

16. Q. The pricing is made up of 4 sections: Develop Statement of Work / Equipment Inventory /Equipment Placement Plans / Equipment Utilities Conformation. The scope and pricing do not reference the relocation i.e. Relocation planning, vendor onboarding, specialty vendor coordination, move day coordination, move communication, post move. Is this scope included? How should the costs associated with this be indicated?
A. The fee schedule provided by the bidder in RFP item V.B.1 will be the basis of payment for these activities.
17. Q. Are the file cabinets being reused? A. No.
18. Q. If new file cabinets are being purchased, then is the move coordinator responsible for file mapping and planning?
A. Yes. 19. Q. Is the move coordinator responsible for hiring and coordinating the chemical vendor?
A. MassDOT will provide a list of state approved vendors; the move coordinator will not be contracting with the vendor, but will coordinate with.
20. Q. III – second bullet – what is being cataloged? A. Office files will be catalogued.
21. Q. Are other locations being relocated? If so what and from where? A. The Pavement Management Group will be relocated from Concord to
Hopkinton. See the response to Question 1. 22. Q. Is move coordinator responsible for the coordination of new equipment being
purchased? A. The move coordinator will coordinate the delivery and installation of new

NOTICE OF REQUEST FOR PROPOSALS The Massachusetts Department of Transportation (MassDOT) requests proposals from qualified firms to provide Move Coordinator Services for MassDOT’s Central Research and Materials Laboratory construction project. Proposals should be submitted to MassDOT in care of Stephanie LeBlanc, MassDOT, Ten Park Plaza, Suite 7520, Boston, MA 02110. Each Respondent shall submit in a sealed package two (2) hard copies of its proposal and one (1) electronic copies in PDF format on disks by 2:00pm on Friday October 23, 2015. The package shall be clearly marked on the outside, "Move Coordinator Services for The MassDOT Central Research and Materials Laboratory," and shall clearly identify the Respondent's name and business address, and the name, email address and telephone number for the contact person. MassDOT reserves the right to reject any or all proposals submitted in response to this Request for Proposals (“RFP”) or to take any other action, as MassDOT may deem to be in its best interest. There will be a Pre- Proposal site visit to the existing Laboratory at 400 D Street, South Boston, on Friday October 9, 2015 at 10am. MassDOT is soliciting proposals, per M.G.L. c30B sec 4, for Move Coordinator services to assist in the relocation of the Research and Material Laboratory currently located in South Boston on D Street. MassDOT is constructing a new Central Research and Materials Laboratory in Hopkinton, located adjacent to the I-90 westbound Westborough service area. The property is bound by the MassPike, Route 135 and Service Rd (the service plaza access road). Project Description The building as designed, is a one and a half story building of approximately 47,210 gsf, of which 40,860 sf are occupied office and laboratory spaces, a mechanical penthouse of 2,400 sf, a six-bay exterior loading dock of 770 sf, and a six-bay 2,540 sf garage. The office portion is two full stories, each 14’-0” in height, with two interconnecting stairs and one elevator. The laboratory is a single 16’-0” tall floor, with the mechanical penthouse over the west-center end portion. The elevator will access the mechanical penthouse, and there is an access stair from the second office floor. At the lab, building materials are tested to ensure the contracts meet the required specifications. The Laboratory Unit is comprised of the following laboratories:
• Cement Concrete • Soils and Aggregates (Gravel, Embankment Materials, Loam, Coarse and Fine, etc.) • Asphalt Binder (Emulsions) • Hot Mix Asphalt (Core Extractions)

• Chemical (Bridge Paint, Pavement Markings) The heavy laboratory equipment will be located in the lab wing on the first floor. The current equipment list is included as Attachment A. Floor layouts are included in Attachment B. The majority of office furniture will be new and delivered by the furniture vendor. Some of the lab equipment will have unique power, telephone, data, air, water, drainage or gas needs. Hoisting services will be required. The project team currently consists of: Construction Manager – Barr & Barr Builders
Architect – Elkus Manfredi Architects Owner Project Manager – City Point Partners
II OVERVIEW OF MOVE COORDINATOR SERVICES The Move Coordinator services will include all activities necessary or appropriate to efficiently manage the relocation of the MassDOT Laboratory to the point of fully functional and the disposition of surplus materials. The Move Coordinator will work with MassDOT and with any representatives designated by MassDOT, in performing services in connection with the Project. In anticipation of additional move services required to clear out the existing lab and surplus materials the contract will extend to March 31, 2017, with a one year extension possible. III SCOPE OF MOVE COORDINATOR SERVICES
• The Move Coordinator will develop the Statement of Work,(SOW) meeting the requirements of the statewide contract FAC78 Moving &State Surplus Disposal Services Statewide contract, included as Attachment C. This SOW, including Worthless Furniture Removal and Scrap Removal will be the basis of bids for the moving contractor.
• The Move Coordinator will include the services of a technician(s) to assist in cataloguing, labeling, tagging and tracking.
• Confirmation and inventorying /tagging of all equipment that will be relocated from the existing facility:
o Include “offsite” equipment (from other laboratories) o Inventory to include equipment utility requirements o Identification of any equipment requiring “manufacturer handling” (e.g. warranty
would be voided if moved by unauthorized party, etc.) (inventory can exclude small handheld equipment)
• Listing of new equipment, that will be purchased by the DOT, for placement in the new facility at the time of move-in
• Preparation of equipment placement plans for the new facility • Verification of appropriate utility connections to support equipment placement in the new
facility (this is especially important if the move-in locations vary from the architectural plan location)

The MassDOT Central Research and Materials Laboratory is currently under construction and is scheduled to be substantially complete by late August 2016. The services of the Move Coordinator will begin upon contract execution, fall of 2015; the new facility will achieve full occupancy in September 2016. V. PROPOSAL SUBMISSION REQUIREMENTS
A. NON PRICE PROPOSAL: Each proposal must contain all of the information and documentation described below: 1. Description of the firm's background. 2. Description of the firm's experience, with particular attention to identifying and describing
projects in which similar services, particularly relocating testing laboratories. 3. A resume and references for each Project team member. The personnel assigned to the
project shall remain for the duration and not be replaced without the MassDOT’s consent. Please note that MassDOT requires that the key team members identified in the proposal be the individuals who render services under the contract, and such individuals must be fully available to devote the time and attention necessary to ensure timely and complete delivery of required services.
4. Submit a list 3 similar projects completed. Provide a list of references with email and phone contact information.
5. Identification of any firms or individuals not part of the responding firm that will be collaborating on the Project. For each such firm, provide a detailed description of its role in the Project, and a complete resume and description of the length and substance of its experience as it relates to the Project.
6. List and discuss how many years your firm has been providing Move Coordinator services. 7. List any terminations or legal proceedings in the last 5 years which was concluded adversely
against your firm. 8. Executed copies of each of the following documents in the form attached as Section VI.,
Proposal Contract Forms.
B. PRICE PROPOSAL 1) Provide a complete fee schedule for services; include hourly rates for each position. 2) Provide an order of magnitude pricing for the listed tasks, based on the estimated number

Contract Summary PO-14-1080-OSD01-OSD10-00000001296 This contract covers Commercial Moving Services and State Surplus Disposal Services for moves of all sizes both intrastate and interstate. Contract Services include but are not limited to providing estimates, consultation service and scrap office furniture removals. Emergency storage during a move is also available. Labor is covered under Commonwealth Prevailing Wage Rates except for “school room furniture” moving (Topical Outline of Massachusetts Prevailing Wage Law 3/6/12 page 51).
Benefits and Cost Savings An RFQ process is required from 2 or more vendors that supports Best Value
Vendors respond to requests for free written cost estimates within 72-hours of site visit
Vendors offer special inside office cranes to lift and move file cabinets without damage
Site Supervisors are available for each move supporting consulting, pre-move meetings and full move
Consultation services include planning and relocation services for smooth execution of each move
Vendors obtain all required permits and notify purchaser of their costs within the total quoted cost
Vendors will pack, remove and transport Surplus Furniture within the total quoted cost

A purchaser is required to obtain quotes from multiple (two or more) Statewide Contractors prior to issuing a Purchase Order. Contractors are listed on the MBPO - click the “Vendor” Tab, then click the “Distributors” Tab.
All orders placed under this contract must include the following statement: “This order is placed under Statewide Contract FAC78” Otherwise, the order will not be considered “use of a Statewide Contract” for procurement purposes. In addition it is recommended that the following language be included: “All of the terms and conditions of the Statewide Contract FAC78 are incorporated herein and made a part hereof. Turnaround time for quotes will be no more than 72 hours from site visit. Before a Purchase Order is generated the cost estimate must be agreed upon. This contract has been set up as a distributor model (solicitation enabled) Master Blanket Purchase Order (MBPO) in COMMBUYS. A Solicitation Enabled Distributor Model MBPO is a single MBPO that has multiple vendors listed as distributors. This allows you to solicit quotes within COMMBUYS, then award the best value bidder and place the order through COMMBUYS. To see contractors are listed on the MBPO - click the “Vendor” Tab, then click the “Distributors” Tab. Specific steps to follow to create the bid for quoting through COMMBUYS: 1. Start with a New Requisition - on the General Tab, fill in all required information and make sure to check off the Solicitation Enabled check box. 2. Within the requisition, Click on the Items tab, click ‘Search Items’; then click the plus sign next to Advanced Search to do an advanced search. Use FAC78 in the Description field; or enter the MBPO number in the Contract / PO# field. 3. Once item is added to the requisition, COMMBUYS will bring you to the Items tab and have an error message saying that the line item needs to have a value greater than 0. Click on Enter Info under the Catalog Price/ Unit Cost and enter in the estimated cost. This can be hidden from the bid before sending it to the vendors. 4. Click on the “Vendor” Tab, then click the “Distributors” Tab, then select the vendors from whom you will solicit quotes. 5. Attach your Statement of Work and any other pertinent documents in the Attachment tab, making sure to check the box that says Show to Vendor. 6. The Reminders tab is for internal reminders visible to those with Basic Purchaser access to COMMBUYS. 7. Review the Summary Tab, and then Submit for Approval.

8. Once Approved, the Requisition will turn into a bid ready for purchasing, once it is in this status, you will need to click Convert to Bid. 9. Once it is converted to a Bid, go through all the tabs and enter in the required information. On the Items tab you can delete the cost that was previously entered on the requisition so no estimated cost shows to the vendor, or if you leave the previously entered cost this will show to the vendor as an estimated cost. 10. Once all information is reviewed submit the Bid for approval. Once approved, send it to the vendors. 11. After quotes are received and evaluations of them are completed, awards and orders should be placed through COMMBUYS from the Bid. Prevailing Wage Rates: Vendors must pay Prevailing Wages for all moves except for “school room furniture” moving. While the Prevailing Wage rate schedule is posted as an attachment to the SWC, each purchaser is required to request their own Prevailing Wage rate for each moving job. Prevailing Wage rate information and forms can be found at Department of Standards. Purchasers must receive a certified payroll record from the vendor prior to paying any invoices. The Purchaser (or, for the purposes of the statute, “awarding authority”) has a legal obligation to request a prevailing wage schedule from the Department of Labor Standards (DLS) at www.mass.gov/dols. Any Purchaser that uses the contract will be considered the “awarding authority” and must request a prevailing wage schedule at the time of the engagement of the contractor for specific services or projects. In addition, contractors must agree to comply with the Prevailing Wage Law, as administered by the DLS. Questions regarding the Prevailing Wage Law may be answered by accessing the DLS Website at www.mass.gov/dols or by calling the DLS Prevailing Wage Program at (617) 626-6953. Travel Expenses: Travel can be billed at a two (2) hour maximum. Purchasers are not required to pay for additional travel time over 2 hours. Other Expenses: No meals, commuting expenses, fuel surcharges, lodging, incidental expenses or other expenses can be billed to the purchaser.

Identify whether or not the location(s) have freight elevator access and if so are there any special procedures for scheduling it. Older buildings do not necessarily have either a passenger or a freight elevator. If it is a leased facility, the freight elevator is generally under the control of the landlord for scheduling purposes.
Provide vendors with complete detailed lists of items to be moved, or approve lists drawn up by vendors on site visits, including dates/times that items that can be moved, special handling requirements, on site contact person and contact numbers/email, and any other special instructions.
Purchasers cannot hold vendors responsible for items not specified on the purchaser approved list.

The transportation and emergency storage during a move of confidential material and records should be identified with clear and specific security procedures in place. The use of locked and secure vehicles should be identified with continuous oversight, security and control specified. Specific requirements and/or protocols should be detailed to ensure confidentiality
Vehicles may be inspected by purchasers to ensure that all vehicles used in these services will allow for property transported being in a closed van/truck with protection from weather elements.
Padding will be used for all furniture and equipment and boxes used will be easily identified. Shrink wrap will be used for any open carts, unlockable cabinets or equipment with loose or movable parts.
Waste removal and recycling services should be clearly stated along with same day cleanup of all areas involved in the move.
Clearly identify the equipment, materials and tools required to conduct a commercial and office move, which will also become evident during a site visit.
The number of vendor employees will be identified in advance and be required to wear identification displaying the moving company’s name (such as identification badge, company uniform, T-shirt).
Include specific dates and times for the move to occur and if there is a need for emergency in-transit storage or additional pick-ups that must be identified in advance.
Many moves require the use of special heavy duty equipment to relocate cabinets, empty and/or filled which need to be identified during the site visit. Such special equipment has included inside office cranes that can lift and move file cabinets without damage.
Identify the location of all special electronic equipment, fragile equipment, items of special value and requiring special professional handling. Proper packing and moving equipment for computer and computer related moves need to be clearly stated. The purchaser may request the vendor to identify their experience moving computer and computer related equipment. The purchaser will identify when they will disconnect and unplug all computer cables and plugs to all computer and computer related equipment prior to the move.
Separate agreements are suggested covering specialty moving services: antiques, large musical instruments (such as harps or pianos), museums, libraries, rigging, and reassembly/reinstallation by a manufacturer certified technician either as prime movers or through sub-contracting of movers who specialize in these services. Sub-vendors will be held to the same rates as the prime movers.
Services that include emptying and packing contents of bookcases, storage cabinets and racks at the point of origin with reversing the process at the final destination must be clearly stated and identified. This should also include costs for disassembling shelving and related cabinets with all hardware securely attached without damage to the surface of the shelving or cabinets.
Identify if it is a requirement for moving location numbers to be assigned and attached, without surface damage, to all items being moved that aligns with destination building floor layouts.
Be clear as to the responsibilities for Permit and Regulatory Requirements with vendor certifying that they will obtain the necessary permits and licenses related to moving services and operations, including any special permits for moving on Saturdays, Sundays, or Holidays, if necessary. Applicable costs for each required permit will be identified within the total quote provided for the service.
The purchaser should specify that vendors secure permits, and identify any and all proprietary regulations and restrictions, such as, but not limited to, individual Building Work Permits for all State buildings.
Clearly identify the Risk of Loss and Insurance with necessary precautions and safeguards to prevent personal injury and property damage while performing services under the contract.
Emphasize that it is the vendor's responsibility to ensure that their operations are conducted in a safe and secure manner at all times. The vendor will be responsible to replace/repair, at the purchaser’s sole option, any property damage(s) made during contract performance.
Specify that the Commonwealth of Massachusetts is self-insured and is not itself purchasing additional insurance under the name of the purchaser or the Commonwealth. Departments are prohibited from insuring Commonwealth property under M.G.L. c. 29, § 30 absent specific legislative authorization. This

means that a department is prohibited from insuring its property or equipment from damage or loss. In addition to being prohibited from insuring its own property, a department may not insure property it does not own. In the event that a purchaser needs additional insurance coverage than what is provided by a Vendor and in order to adequately insure the value of the assets being moved (for example computer servers), then the purchaser is authorized to pay a higher premium for the VENDOR TO INSURE the assets for loss and replacement and NOT for the Commonwealth to purchase a separate policy in the name of the Commonwealth. Vendors must offer this additional higher premium coverage and provide a certificate of insurance to the purchaser.
State in the SOW that an itemized Bill of Lading is to be prepared for every shipment transported, this will be the same as shown on the order for service. Worthless (Scrap) Furniture Removal All state agencies must first submit a list of their worthless equipment to the Commonwealth of Massachusetts State Surplus Property Office (SSPO), Operational Services Division, One Ashburton Place, Room 1017, Boston, MA 02108, Phone: 617-720-3300 Fax: 617-727-4527 for permission to dispose and follow all SSPO policies and procedures before identifying surplus furniture as scrap. Agencies are asked to first complete the Scrap Furniture Disposal Form and provide the vendor with a copy of this form prior to receiving a written estimate from the vendor. Local municipalities or other non-state entities do not have to write to SSPO prior to the removal of their worthless items by a service. Additional Statement of Work (SOW) Considerations for Scrap Removal • Details of the responsibilities of the services to be rendered for Worthless (Scrap) Furniture Removal should be included within a Statement of Work (SOW) as identified above for all moving services for specific services, identification of items, site visit, cost estimates, etc. • Additional details will include a written report explaining where and how the surplus furniture will be disposed of (i.e. recycled, sold-off, or destroyed). It is desirable that as much of the scrap as possible be recycled. • Be sure that the total quoted cost includes specific language that the vendor will remove, pack, and incur all freight costs with the transport of all scrap office furniture. • To avoid potential additional costs such as overtime, it is suggested that the purchaser specify that the removal of scrap office furniture will take place during the facility’s regular work hours. • Clearly state that the vendor will take all office furniture listed on the Scrap Furniture Disposal Form. The Vendor will not be allowed to remove only the furniture that looks desirable and leave the remainder. Recycling A purchaser may work with their vendors to secure recycling under the Statewide Contract (SWC) for Solid Waste and Recycling Services currently identified as FAC33 or a successor SWC.

Purpose of Form A person who is required to file an information return with the IRS must get your correct taxpayer identification number (TIN) to report, for example, income paid to you, real estate transactions, mortgage interest you paid, acquisition or debt, or contributions you made to an IRA. Use Form W-9 only if you are a U.S. person (including a resident alien), to give your correct TIN to the person requesting it (the requester) and , when applicable, to: 1. Certify the TIN you are giving is correct (or
you are waiting for a number to be issued). 2. Certify you are not subject to backup
withholding If you are a foreign person, use the appropriate Form W-8. See Pub 515, Withholding of Tax on Nonresident Aliens and Foreign Corporations. What is backup withholding? Persons making certain payments to you must withhold a designated percentage, currently 28% and pay to the IRS of such payments under certain
conditions. This is called “backup withholding.” Payments that may be subject to backup withholding include interest, dividends, broker and barter exchange transactions, rents, royalties, nonemployee pay, and certain payments from fishing boat operators. Real estate transactions are not subject to backup withholding. Penalties Failure to furnish TIN. If you do not have a TIN, write “Applied For” in the space for the TIN, sign and date the form, and give it to the requester. For interest and dividend payments, and certain payments made with respect to readily tradable instruments, generally you will have 60 days to get a TIN and give it to the requester before you are subject to backup withholding on payments.
